RESUMEN

Objective:To evaluate the detection accuracy of hepatitis B virus (HBV) serological markers among grassroots medical institutions in the demonstration areas of infectious diseases.Methods:A fixed sampling method was used among the followed-up hepatitis B cohort in general population of six infectious disease demonstration areas nationwide. Blood samples of chosen objects were collected, in which HBsAg and anti-HBs were tested by grassroots medical institutions and were also parallely tested by the third-party platform. The test results were compared between the two groups. Statistical analyses were conducted by SAS 9.4 software.Results:A total of 5 756 and 5 263 samples of HBsAg and anti-HBs were successfully tested, respectively. Comparing the results of HBsAg and anti-HBs from grassroots medical institutions with the results from the third platform, the agreement of HBsAg and anti-HBs was 97.13% and 77.33%, respectively. The Kappa value was 0.56 (95% CI 0.50-0.62) for HBsAg and 0.54 (95% CI 0.52-0.56) for anti-HBs, respectively; and the McNemar tests indicated the difference between the results (all P<0.01). There were also significant differences in agreement of testing results with the third platformin among different regions ( P<0.05 or <0.01). The Kappa values indicated that Jiangsu province and Guangdong province had high accordance rates of HBsAg (0.87 and 0.81, respectively), and Gansu province and Guangdong province had high accordance rates of anti-HBs (Both were 0.74). Regarding the results from the third platform as the standard, the sensitivity of HBsAg testing in grassroots medical institutions was moderate (40.51%) and the specificity was well (99.96%). The sensitivity of anti-HBs testing was substantial (73.18%) and the specificity was well (84.31%). Guangdong province (Youden index: 0.69) and Jiangsu province (Youden index: 0.80) had high identification ability for HBsAg, and for indicator of anti-HBs, Gansu province (Youden index: 0.78) and Guangdong province (Youden index: 0.76) had high identification ability. Conclusion:There are certain differences in results of HBV serological markers tests between the grassroots medical institutions in the demonstration areas of infectious diseases and the third platform. Current testing strategies in grassroots medical institutions are suitable for identifying people without hepatitis B infection, while it is necessary to pay attention to the situation of potential false negative error.

RESUMEN

The application progress of machine learning in research of acupuncture and moxibustion was reviewed from three aspects: mining of acupuncture and moxibustion prescription and indications, acupuncture efficacy prediction and its influencing factors, acupoint specificity and acupuncture manipulation research, and the existing problems in current research and future research trends were discussed. It is believed that the appropriate machine learning algorithm should be selected to build the model according to the research purpose and data characteristics in the future research; attention should be paid to feature design, feature selection and feature cleaning; sample data collection should be a priority, and data sharing platform and standardized data collection should be developed to improve the data quality.

Objective@#To investigate the influencing factors of hyperuricemia in patients with IgA nephropathy (IgAN).@*Methods@#A retrospective study was performed in patients with renal biopsy diagnosed as IgAN in the Department of Nephrology, Provincial Hospital of Anhui Medical University from January 2016 to October 2018. According to the blood uric acid level, they were divided into two groups: patients with hyperuricemia and patients without hyperuricemia. The general clinical indicators and renal pathological data were compared between the two groups. Logistic regression model was used to analyze the influencing factors of hyperuricemia in IgAN patients.@*Results@#A total of 125 IgAN patients with age of (35.70±11.16) years old were enrolled, including 63 males and 62 females. The morbidity of hyperuricemia was 44.0%(55/125). Compared with the normal blood uric acid group, the blood urea nitrogen, serum creatinine and the proportion of chronic kidney disease (CKD) stage 3-5, small arterial wall thickening, fibrous crescents/globules, renal interstitial fibrosis, renal tubular atrophy, glomerular sclerosis and inflammatory cell infiltration in the hyperuric acid group were higher, while the level of estimated glomerular filtration rate (eGFR) was lower. And the differences were statistically significant (all P<0.05). Multivariate logistic regression analysis showed that the level of serum creatinine was an independent related factor of hyperuricemia in IgAN patients (OR=1.034, 95%CI 1.005-1.064, P=0.021).@*Conclusions@#IgAN patients with hyperuricemia presented more severe glomerular, tubular and interstitial lesions, and the level of serum creatinine is an independent related factor of hyperuricemia in IgAN patients. High uric acid level may have an important influence on the progression of IgAN, so good control of serum uric acid may improve the prognosis of patients with IgAN.

Objective To evaluate the success rate,vascular patency time and their influencing factors of percutaneous transluminal angioplasty (PTA) in treating swollen hand syndrome in hemodialysis patients.Methods The clinical data of 16 hemodialysis patients with swollen hand syndrome,who were admitted to authors' hospital during the period from May 2015 to March 2017 to receive PTA,were retrospectively analyzed.The technical success rate,the follow-up primary vascular patency time and primary patency rate were calculated,and the factors influencing technical success rate and vascular patency time were analyzed.Results Venography with DSA revealed that a total of 16 segments of venous stenosis or occlusion were found in 16 patients,including 6 stenotic lesions and 10 occlusive lesions.Successful PTA was obtained in 14 patients,including one patient whose angiography performed immediately after PTA with balloon dilatation showed that the stenosis was still over 50%,and stent implantation had to be carried out.The technical success rate was 87.5%,in 2 patients PTA failed as the guide wire could not pass through the long segment of vascular occlusion.The 14 patients were followed up for 3-24 months,and the median patency time was 10.5 months.The 3-,6-and 12-month primary patency rates were 71.4% (10/14),57.1% (8/14) and 42.9% (6/14) respectively.Univariate analysis indicated that the length of occlusive segment and the balloon pressure required for angioplasty were the potential factors that affected the postoperative vascular patency time.Conclusion For the treatment of swollen hand syndrome in hemodialysis patients,PTA is safe and effective,although long-term vascular patency rate needs to be further improved.

Migraine is a kind of nerve vascular disease, characterized by hemialgia, moderate and severe, pulsating pain. Acupuncture as a safe and effective analgesic therapy, has been widely used in more than 160 countries and regions around the globe. Treating migraine with acupuncture has certain good features as effective, non-toxic side effects and untoward effect.But its mechanism is still not very clear.The cerebral blood flow study ( CBF study) is currently the most credibility of migraine pathogenesis theory.Now based on the theory of CBF study, this paper discusses the acupuncture treatment of migraine which can provide the basis for clinical treats.

Compartments of soil microorganism and enzymes between stereoscopic cultivation (three storeys) and field cultivation (CK) of Panax notoginseng were carried out, and the effects on P. notoginseng agronomic characters were also studied. Results show that concentration of soil microorganism of stereoscopic cultivation was lower than field cultivation; the activity of soil urea enzyme, saccharase and neutral phosphatase increased from lower storey to upper storey; the activity of soil urea enzyme and saccharase of lower and upper storeys were significantly lower than CK; agronomic characters of stereoscopic cultivated P. notoginsengin were inferior to field cultivation, the middle storey with the best agronomic characters among the three storeys. The correlation analysis showed that fungi, actinomycetes and neutral phosphatase were significantly correlated with P. notoginseng agronomic characters; concentration of soil fungi and bacteria were significantly correlated with the soil relative water content; actinomycete and neutral phosphatase were significantly correlated with soil pH and relative water content, respectively; the activities of soil urea enzyme and saccharase were significantly correlated with the soil daily maximum temperature difference. Inconclusion, The current research shows that the imbalance of soil microorganism and the acutely changing of soil enzyme activity were the main reasons that caused the agronomic characters of stereoscopic cultivated P. notoginseng were worse than field cultivation. Thus improves the concentration of soil microorganism and enzyme activity near to field soil by improving the structure of stereoscopic cultivation is very important. And it was the direction which we are endeavoring that built better soil ecological environment for P. notoginseng of stereoscopic cultivation.

Light intensity, gas temperature, soil temperature and gas exchange parameters were determined of three years old Panax notoginseng planted on different layers seedbed and different location (left, middle, right) of the same layer in greenhouse. Result show that diurnal variation of light intensity, gas temperature and soil temperature showed that upper layer > middle layer > lower layer; different locations of the same layer showed that light intensity of upper layer was not different among different locations; light intensity of middle and lower layer in right and left were the same, and significantly higher than those in the middle position; the gas temperature of each layer all with less different of each location; soil temperature of 12 cm depth is the lowest, and was gradually increased to the upper and lower surface; net photosynthetic efficiency of P. notoginseng showed that upper layer > middle layer > lower layer; there were significant correlation between soil temperature, stomatal conductance, intercellular CO2 concentration and photosynthetic rate were correlated with light intensity significantly; transpiration rates had notable correlation with light intensity and gas temperature. All above indicated that net photosynthesis rate of P. notoginseng was affected by light intensity directly, gas temperature and soil temperature indirectly. Inconclusion, stereoscopic cultivation of P. notoginseng was practicable in present study. The planting quality of P. notoginseng under stereoscopic cultivation could be improved by ameliorate the structure of seedbed to enhance the light intensity of middle and lower layer. Increase the thickness of the seedbed to decrease the temperature difference of soil. Further the management of ventilation facilities of greenhouse to control the gas temperature.

The output and agronomic characters of 3-year-old Panax notoginseng cultured under stereo structure (upper, middle and down layers) were investigated, and the annual change of N, P and K of its planting soil were also studied. Results showed that, compared with field cultured Panax notoginseng, growth vigour and output of stereo-cultivation were significantly lower. But the total production of the 3 layers was 1.6 times of field. The growth vigor and production of P. notoginseng was in the order of upper layer > middle layer > down layer. The content of ginsenoside in rhizome, root tuber and hair root of P. notoginseng was in the order of upper layer > field > middle layer > down layer. Organic matter content and pH of stereo-cultivation soil decreased with the prolonging of planting time, which with the same trend of yield. Organic matter content of stereo-cultivation soil was significantly higher than field, but the pH was significantly lower. Contents of total and available N, P and K in stereo-cultivation soil and field decreased with the prolonging of planting time. The content of N and P were in the order of upper layer > middle layer > yield > down layer, the content of K was in the order of upper layer > middle layer > down layer > yield. Compared with field, the proportion of N and P in the organ of underground (rhizome, root tuber and hair root) of upper layer were increased, while decreased in middle and down layers. Proportion of K in underground decreased significantly of the 3 layers. In conclusion, the agronomic characters and production of stereo-cultivation were significantly lower than that of yield. But the total production of the 3 layers were significantly higher than field of unit area. And the aim of improving land utilization efficiency was achieved. Nutritions in the soil of stereo-cultivation were enough to support the development of P. notoginseng, which was not the cause of weak growth and low production. The absorbing ability of P. notoginseng to N, P and K nutrients was decreased by stereo-cultivation mode. So, improve the growth vigour of P. notoginseng from the perspective of adjusting the stereo-cultivation mode so as to improve the nutrient absorption capacity is the future direction.

By searching the registry information of systematic review regarding acupuncture included before March 31, 2014 in PROSPERO systematic review registry platform, information in the project plan such as care gories of diseases, interventions, research team and the completion status was analyzed to make a comprehensive understanding on registry status of acupuncture systematic review in this platform. As a result, a total of 52 project plans was finally included. The health problem concerned was mainly painrelated diseases, and the interventions were mostly simple acupuncture or combination of acupuncture and moxibustion. The registered plan participated with Chinese team appeared comparatively late, which was featured with fewer independent projects and concentrated research organization, so its scientific research advantage in acupuncture did not present. In conclusion, it is believed that the consciousness on systematic review registry in domestic researchers needs to be improved, and researchers might take good advantages of the PROSPERO platform in the future.

RESUMEN

<p><b>OBJECTIVE</b>Through analysis of oversea published literatures on central effect mechanism of acupuncture in recent years, the general tendency in this filed is discussed so as to provide references for later studies.</p><p><b>METHODS</b>Pubmed database from January 2009 to May 2012 is retrieved with the key words of acupuncture and fMRI. As a result, 38 articles are included.</p><p><b>RESULTS AND CONCLUSIONS</b>Healthy subjects are adopted for most of researches. And studies are carried out through three aspects of response characteristics of acupuncture analgesia, response characteristics of acupuncture specificity and duration of acupuncture effect. However, the brain activities of healthy subjects are obviously different from that of patients. Moreover, acupuncture treatment is also different from modern therapies. Therefore, it is held that patients should be adopted as study subjects in the future studies, and various bias in research designation and implementation course should be avoided.</p>

RESUMEN

To identify acupuncture resources in six databases of Cochrane Library (CL) with computer retrieve. Seventy-two literatures were identified in Cochrane Database of Systematic Reviews (CDSR). Among them, 12 Cochrane systematic review (CSR) verified the effectiveness of acupuncture, 29 concerning the indeterminacy of the efficacy of acupuncture with 1 didn't support acupuncture for epilepsy and 31 remained as protocols; 121 literatures were found in Database of Abstracts of Reviews of Effects (DARE) with more types of diseases or symptoms and rich modality comparing to CSR; 4218 randomized controlled trials and clinical controlled trials were identified in Cochrane Central Register of Controlled Trials (CCRCT); 43 literatures in Cochrane Methodology Register Database (CMRD) which focused on blindness study, quality assessment of methodology of research and publication bias and so on; 25 literatures in Health Technology Assessment Database (HTAD) and 18 in NHS Economic Evaluation Database (NHS EED) which were centered on acupuncture analgesia. Consequently, acupuncture literatures in 6 databases of CL do provide good resources for acupuncture researchers due to its abundant content, concrete classification and high quality evidence.

RESUMEN

Combining the functional metabolic imaging and anatomical imaging,PET-CT stands for the highest level of current nuclear medical imaging techniques,and has been applying increasingly in acupuncture studies.However,owning to complexity of the brain function and sensitivity of brain metabolism,the lower reproducibility of cerebral function imaging even may reverse many results.This has been provoked more and more attention by researchers.Its main cause is intimately related to poor experimental methodology.For this reason,the present paper raises the quality control of the research on mechanisms of acupuncture in the process of application of PET-CT techniques from 1) the included standards of participants,2) the preparation of participants during the scanning process of PET-CT,3) the setting of the scanning parameters,4) the used machine and the imaging reagent,5) the analysis of dada,6) the standardization of acupuncture manipulation,and 7) the designs of the acupuncture operation,needling opportunity and scanning opportunity,hoping to offer some beneficial information for the coming researches.

Orf 9b was amplified by PCR from SARS-CoV genome and cloned into the Nco I and Bam HI sites of the pET32c expression vector, and then recombinant plasmid pET32c-Orf 9b was constructed. The recombinant fusion protein Orf 9b was expressed by IPTG induction and purifed. After being cleaved by rEK, Orf 9b protein with MW 11 kD was separated and collected. It was demonstrated by ELISA that the purified Orf 9b protein could react with sera of SARS rehabilitaion patients but not with sera from healthy donors. CD and Infrared spectroscopy were used to predict the secondary structure of the purified Orf 9b protein. The distribution percentages for the the secondary structures of alpha-helix, beta-sheet, and random coil in the Orf 9b protein estimated by CD were 12.5%, 40%, and 47.5%, respectively, while the same parameters estimated by Infrared spectroscopy were 13.7%, 47.5%, and 37.9%, respectively. The results obtained by the two methods were substantially identical and showed that the structure of the Orf 9b protein consisted mostly of beta-sheet, and comprised only few alpha-helix. The acquisition of purified protein and the structural information presented herein may provide foundation for further functional study.

OBJECTIVE To identify 134 fungus strains and to detect their drug susceptibility in order to instruct clinical treatment.METHODS The colored culture medium isolated fungus was identified by ID 32C.The resistance to antifungal agents,such as amphotericin B,fluconazole,itraconazole and voriconazole,was detected by ATB Fungus 3 automatic analysis system.RESULTS The isolation rate of C.albicans was the highest from 134 fungus strains,the next was C.tropicalis,the non-C.albicans infection assumed an escalation trend.From the result of drug susceptibility of 134 fungus strains,the susceptibilities to amphotricin B,voriconazole and 5-fluorocystine were the highest,the lowest to fluconazole.CONCLUSIONS As a result of the widespread application of hormons,the immunity inhibitors and the antibiotics,the change of fungal infection is increased,It is necessary to make the fungus identification and the drug sensitive experiment in order to instruct correct clinical uses of the antifungal agents.

Establishing the ideal animal model to study its cause and pathogenesis of this disease is very important. The ideal animal model should have the same manifestation with human inflammatory bowel disease on clinical and pathologic feature etc. In this article, the method, the pathologic character istics and concerning pathogenesis, of a few common useful experiment animal models are discussed. AIM: To investigate DNA-binding activity of nuclear factor ka ppa B (NF-?B) and pyrrolidine dithiocarbamate (PDTC) anti-inflammation effect and mechanism in trinitrobenzene sulfonic acid (TNBS)-induced acute colitis in rats. METHODS: Acute colitis was induced by instilling TNBS/ ethanol into the lumen of the rat colon. Rats were randomized into PDTC-treat g roup (including group P10,P25,P50,P100), TNBS/ethanol group and normal control g roup. The rats in PDTC-treated group were injected intraperitoneally with PDTC at the dosages of 10, 25, 50, and 100 mg?kg -1 , respectively. Rats wer e killed at 4 h after enema. Colonic inflammation, the ratio of wet/dry weig ht and MPO, SOD, and MDA activity were detected. DNA-binding activity of NF-? B was assessed by EMSA. RESULTS: NF-?B activity in colon homog enate was increased markedly at acute colitis in rats. PDTC attenuates the devel opment of rat colonic inflammation but not by reducing the NF-?B activity. CONCLUSION: NF-?B is activated in rats with acute colitis, which may be a mechanism of self-protection. PDTC can attenuate the development of in flammation.
