COMVC-19: A Program to protect healthcare workers' mental health during the COVID-19 Pandemic. What we have learned

OBJECTIVE: In 2020, the COVID-19 pandemic brought a work and stress overload to healthcare workers, increasing their vulnerability to mental health impairments. In response, the authors created the COMVC-19 program. The program offered preventive actions and mental health treatment for the 22,000 workers of The Hospital das Clinicas da Faculdade de Medicina da Universidade de São Paulo (HCFMUSP). This paper aims to describe its implementation and share what we have learned from this experience. METHODS: Workers were able to easily access the program through a 24/7 hotline. Additionally, a mobile phone app that screened for signs and symptoms of emotional distress and offered psychoeducation and/or referral to treatment was made available. Data from both these sources as well as any subsequent psychiatric evaluations were collected. RESULTS: The first 20 weeks of our project revealed that most participants were female, and part of the nursing staff working directly with COVID-19 patients. The most frequently reported symptoms were: anxiety, depression and sleep disturbances. The most common diagnoses were Adjustment, Anxiety, and Mood disorders. CONCLUSIONS: Implementing a mental health program in a multimodal intervention was feasible in a major quaternary public hospital. Our data also suggests that preventive actions should primarily be aimed at anxiety and depression symptoms, with a particular focus on the nursing staff.

Psicanálise a céu aberto / Psychoanalysis under the open sky / Psicoanálisis a cielo abierto / Psychanalyse à ciel ouvert

A proposta dos autores é pensar em práticas psicanalíticas "a céu aberto" como possibilidade de escuta analítica voltada àqueles comumente hostilizados ou até ignorados em sua invisibilidade, provocada por um sistema que exclui pessoas. Ao discutir o tema intolerância, fazem uma relação com o conceito de abjeto. Destacam a importância de um dispositivo nas instituições psicanalíticas que ponha o profissional em contato com o contexto em que vivem.

The authors propose a reflection on psychoanalytic practices under the open sky. According to this paper, those practices may enable psychoanalysts (and their psychoanalytic listening) to reach those individuals who are usually marginalized, or even ignored in their invisibility - an invisibility which arises from a social and economic system that excludes people. The authors show a relation between intolerance and the concept of abjection as they discuss the issue of intolerance. This paper emphasizes the importance of an apparatus, in the psychoanalytic institution, that puts psychoanalysts in touch with the reality in which they live.

La propuesta de los autores es pensar en prácticas psicoanalíticas "a cielo abierto" como posibilidad de escucha analítica direccionada hacia aquellos generalmente hostilizados o, incluso, ignorados por su invisibilidad que es producto de un sistema que excluye personas. Al discutir el tema de la intolerancia hacen una relación con el concepto de abyecto. Destacan la importancia de que las instituciones psicoanalíticas posean un dispositivo que coloque a los profesionales en contacto con la realidad en la cual viven.

La proposition des auteurs est de penser aux pratiques psychanalytique "à ciel ouvert", comme une possibilité d'écoute analytique destinée à ceux qui sont souvent harcelés ou même ignorés dans son invisibilité, causée par un système qui exclut les gens. Lors de la discussion du sujet de l'intolérance, ils font une association avec le concept d'abjection. Ils mettent en évidence l'importance d'un dispositif dans les institutions psychanalytiques qui place le professionnel en contact avec la réalité dans laquelle ils vivent.

O trabalho apresenta uma atividade desenvolvida pela equipe editorial do Jornal de Psicanálise na interface entre cinema e psicanálise. Um grupo de analistas se interessou pelo convite da equipe para conversar sobre o filme Festa de Família, do diretor Thomas Vinterberg. A conversa foi gravada e transcrita. Pensamos ser importante explorar as maneiras pelas quais a psicanálise pode ser difundida. Esta atividade demonstrou que um filme pode servir como estímulo para uma agradável e instigante conversa psicanalítica e trouxe à tona toda a riqueza deste tipo de encontro onde a possibilidade de trocar experiências de forma espontânea, tal qual pensamos possa ocorrer no próprio trabalho analítico, pode ter lugar. Foi possível ter a dimensão de um trabalho implicado, no qual a psicanálise esteve presente de forma viva e atuante, encarnada na fala de cada colega...

The paper presents an activity developed by the editorial staff of the Jornal de Psicanálise, in the interface between cinema and psychoanalysis. A group of analysts interested in the team invitation chose to talk about The Celebration, a film directed by Thomas Vinterberg. The conversation was recorded and then transcribed. This activity demonstrated that a film can work as a stimulus for an enjoyable and thought-provoking psychoanalytic conversation and brought up all the wealth of this kind of meeting, in which the possibility of exchanging experiences spontaneously took place, as it is thought to occur in the analytic work itself. It was possible to realize the magnitude of an involving job in which psychoanalysis was presented in an active and living form, incarnate in the speech of each colleague...

Este trabajo presenta una actividad desarrollada por el equipo editorial del Jornal de Psicanálise en la interface entre cine y psicoanálisis, que consistió en un encuentro de analistas para conversar sobre la película La Celebración [Festen] del director Thomas Vinterberg. La discusión fue grabada y transcripta. Pensamos que es importante ampliar las maneras por las cuales el psicoanálisis puede ser difundido. Esta actividad mostró que una película puede servir de gatillo para una agradable y estimulante charla psicoanalítica, y quedó evidente la enorme riqueza de este tipo de reunión, donde la posibilidad de intercambiar experiencias de manera espontánea, como nos parece que ocurre en el propio trabajo analítico, tiene lugar. Hubo un trabajo implicado, en el cual el psicoanálisis estuvo presente de forma viva y actuante, encarnado en el discurso de cada colega...

Considerações sobre uma experiência peculiar: Psicoterapia na Faculdade de Medicina da Universidade de São Paulo e no Instituto de Psiquiatria do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo / Afterthoughts on a peculiar experience: Psychotherapy in the School of Medicine of the Universidade de São Paulo and the Institute of Psychiatry at the Hospital de Clínicas of the School of Medicine of the Universidade de São Paulo

O artigo descreve a história do ensino da psicoterapia na Faculdade de Medicina da Universidade de São Paulo e no Instituto de Psiquiatria do Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo, desde o Serviço de Psicoterapia, criado em 1963, até o estabelecimento de espaços de ensino da teoria psicanalítica e de outras formas de psicoterapia dentro do atual programa de residência em psiquiatria, ao lado das neurociências e da psicofarmacologia. A modificação da psiquiatria, a partir da evolução da perspectiva biológica e da mudança do espaço físico das Unidades de Internação Psiquiátricas nas últimas décadas do século XX, o consequente afastamento dos aspectos subjetivos nesse período e as tentativas de integração dos diferentes referenciais no momento atual são descritos e discutidos. Por fim, o formato atual do programa de residência, os núcleos de formação desenvolvidos por ex-alunos e a inclusão da pesquisa em psicoterapia no programa de pós-graduação são apresentados.(AU)

The article presents the story of psychotherapy teaching in the Department and Institute of Psychiatry - University of São Paulo, from the establishment of the Psychotherapy Service in 1963 to the appearance of spaces for the teaching of Psychoanalytic theory and other forms of psychotherapy within the current program of psychiatry residency along with neurosciences and psychopharmacology. The modification of Psychiatry from the biologic perspective and change in the physical space of the Psychiatric Hospitalization Units in the last decades of the 20th Century, the resulting withdrawal from subjective aspects and the attempts to integrate different theoretical references nowadays are described and discussed. Moreover, the current format of the residency program, the training centers developed by former students and the inclusion of research in psychotherapy in the post-graduation program are presented.(AU)
