RESUMEN

BACKGROUND@#Primary dysmenorrhea (PD) is the most common complaint associated with menstruation and affects up to three-quarters of women at some stage of their reproductive life. In Chinese medicine, navel therapy, treatment provided at Shenque (CV 8), is used as a treatment option for PD.@*OBJECTIVE@#To evaluate the effect of navel therapy on pain relief and quality of life in women with PD, compared with Western medicine (WM).@*METHODS@#China National Knowledge Infrastructure (CNKI), Chinese Scientific Journal Database (VIP), SinoMed and Wanfang Database, MEDLINE, the Cochrane Library, Embase, Web of Science, and the International Clinical Trial Registry of the U.S. National Institutes of Health were searched from their inceptions to April 1, 2021. Randomized controlled trials (RCTs) assessing therapeutic effects of navel therapy on PD were eligible for inclusion. RevMan 5.4 software was used for data analyses. The certainty of the evidence was assessed using the online GRADEpro tool.@*RESULTS@#Totally 24 RCTs involving 2,614 participants were identified. Interventions applied to acupuncture point CV 8 included: herbal patching, moxibustion or combined navel therapy (using at least 2 types of stimulation). Compared to placebo, there was a significant effect in favor of navel therapy on reducing overall menstrual symptom scores at the end of treatment [mean difference: -0.82, 95% confidence interval (CI): -1.00 to -0.64, n=90; 1 RCT]. As compared with Western medicine, navel therapy had a superior effect on pain intensity as assessed by Visual Analogue Scale at the end of treatment [standardized mean difference (SMD): -0.64, 95% CI: -1.22 to -0.06, I2=80%, n=262; 3 RCTs]; on symptom resolution rate at 3-month follow-up (risk ratio: 1.94, 95% CI: 1.47 to 2.56, n=1527, I2=38%; 13 RCTs); and on global menstrual symptoms score at the end of treatment (SMD: -0.67, 95% CI: -0.90 to -0.45, I2=63%, n=990; 12 RCTs). Subgroup analyses showed either a better or an equivalent effect comparing navel therapy with Western medicine. No major adverse events were reported. The methodological quality of included trials was poor overall.@*CONCLUSIONS@#Navel therapy appears to be more effective than Western medicine in decreasing menstrual pain and improving overall symptoms of PD. However, these findings need to be confirmed by well-designed clinical trials with adequate sample size (Systematic review registration at PROSPERO, No. CRD42021240350).

RESUMEN

BACKGROUND@#High on-clopidogrel platelet reactivity could be partially explained by loss-of-function alleles of CYP2C19, the enzyme that converts clopidogrel into its active form. Shexiang Tongxin Dropping Pill (STDP) is a traditional Chinese medicine to treat angina pectoris. STDP has been shown to improve blood flow in patients with slow coronary flow and attenuate atherosclerosis in apolipoprotein E-deficient mice. However, whether STDP can affect platelet function remains unknown.@*OBJECTIVE@#The purpose of this study is to examine the potential effects of STDP on platelet function in patients undergoing percutaneous coronary intervention (PCI) for unstable angina. The interaction between the effects of STDP with polymorphisms of CYP2C19 was also investigated.@*DESIGN, PARTICIPANTS AND INTERVENTION@#This was a single-center, randomized controlled trial in patients undergoing elective PCI for unstable angina. Eligible subjects were randomized to receive STDP (210 mg per day) plus dual antiplatelet therapy (DAPT) with clopidogrel and aspirin or DAPT alone.@*MAIN OUTCOME MEASURES@#The primary outcome was platelet function, reflected by adenosine diphosphate (ADP)-induced platelet aggregation and platelet microparticles (PMPs). The secondary outcomes were major adverse cardiovascular events (MACEs) including recurrent ischemia or myocardial infarction, repeat PCI and cardiac death; blood biomarkers for myocardial injury including creatine kinase-MB isoenzyme (CK-MB) and high-sensitive troponin I (hsTnI); and biomarkers for inflammation including intercellular cell adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1), monocyte chemoattractant protein-1 (MCP-1) and galectin-3.@*RESULTS@#A total of 118 subjects (mean age: [66.8 ± 8.9] years; male: 59.8%) were included into analysis: 58 in the control group and 60 in the STDP group. CYP2C19 genotype distribution was comparable between the 2 groups. In comparison to the control group, the STDP group had significantly lower CK-MB (P < 0.05) but similar hsTnI (P > 0.05) at 24 h after PCI, lower ICAM-1, VCAM-1, MCP-1 and galectin-3 at 3 months (all P < 0.05) but not at 7 days after PCI (P > 0.05). At 3 months, the STDP group had lower PMP number ([42.9 ± 37.3] vs. [67.8 ± 53.1] counts/μL in the control group, P = 0.05). Subgroup analysis showed that STDP increased percentage inhibition of ADP-induced platelet aggregation only in slow metabolizers (66.0% ± 20.8% in STDP group vs. 36.0% ± 28.1% in the control group, P < 0.05), but not in intermediate or fast metabolizers. The rate of MACEs during the 3-month follow-up did not differ between the two groups.@*CONCLUSION@#STDP produced antiplatelet, anti-inflammatory and cardioprotective effects. Subgroup analysis indicated that STDP inhibited residual platelet reactivity in slow metabolizers only.@*TRIAL REGISTRATION@#This study was registered on www.chictr.org.cn: ChiCTR-IPR-16009785.

RESUMEN

Objective:To study the protective effect and mechanism of Ranae Oviductus protein hydrolysate (ROPH) on the expression of pathway-related proteins in ethanol-induced L-02 cell injury. Method:The ROPH was prepared by compound enzymatic hydrolysis. L-02 cell injury model was induced with 400 mmol·L-1 ethanol. Cell viability was detected by cell counting kit-8 (CCK-8) assay. Cell cycle and apoptosis were examined by flow cytometry. JC-1/Hochest staining was employed for qualitative investigation. The expression of related proteins in apoptosis, mitogen-activated protein kinase (MAPK) signaling pathway, and pyroptosis in L-02 cells was detected by Western blot. Result:The results of the CCK-8 assay showed that 400 mmol·L-1 ethanol could induce L-02 cell injury within 12 hours. Compared with the blank group, the model group showed decreased viability of L-02 cells (P<0.01), elevated percentage of the cell cycle in the G0/G1 phase (P<0.01), increased total cell apoptosis rate (P<0.01), reduced mitochondrial membrane potential (P<0.01), up-regulated expression of apoptosis-related proteins [B-cell lymphoma-2 (Bcl-2)-associated X protein (Bax), Cytochrome C (Cyt C), and cysteine-dependent aspartate specific protease-3 (Caspase-3)] (P<0.05, P<0.01) and MAPK signaling pathway-related proteins [C-Jun amino-terminal kinase (JNK) and p38 MAPK] (P<0.05, P<0.01), and potentiated expression of pyrolysis-related proteins Caspase-1 and interleukin-1β (IL-1β) (P<0.05). Compared with the model group, the ROPH treatment group exhibited improved cell cycle arrest (P<0.05, P<0.01), diminished total cell apoptosis rate (P<0.01), elevated mitochondrial membrane potential in a dose-dependent manner, down-regulated expression of Bax, Cyt C, and Caspase-3 proteins (P<0.05, P<0.01), up-regulated expression of Bcl-2 protein (P<0.05, P<0.01), and a downward trend in expression of proteins related to MAPK signaling pathway and pyrolysis (P<0.05, P<0.01). Conclusion:ROPH could inhibit oxidative stress-triggered liver injury in ethanol-induced cells by improving mitochondrial membrane potential, reducing the expression of proteins in the mitochondria-mediated apoptosis pathway, and inhibiting the expression of proteins related to the MAPK signaling pathway and pyrolysis pathway to reduce the mitochondrial dysfunction and inflammatory response in ethanol-induced L-02 liver cells and inhibit oxidative stress, thereby exerting a therapeutic role in alcoholic liver injury.

RESUMEN

Erigeron breviscapus, a species within the genus of Erigeron, is mainly distributed in Southwest China. It is cold in property, slightly bitter in taste, and has the effect of dispersing cold table, removing wind and dehumidification, promoting blood circulation and removing blood stasis, relieving pain and inflammation. Breviscapine is the extract of E. breviscapus. It is mainly consisted of flavonoids, lignans, coumarins, terpenes, phytosterols, etc. As the major components of breviscapine, the content of breviscapine b (4′-hydroxybaicalin-7-O-glucuronide) and breviscapine a (apigenin-7-O-glucuronide) is greater than 90%. Modern pharmacological studies have shown that breviscapine has a wide range of pharmacological effects, including anti-oxidation, anti-fibrosis, anti-inflammation, anti-aging, anti-platelet aggregation, lowering blood lipid, increasing blood flow, improving microcirculation, preventing and treating tumors, and resisting brain injury. In clinical, breviscapine has been widely used in the treatment of diabetes, cerebral insufficiency, sequelae caused by cerebral hemorrhage, hypermucolipemia, cerebral thrombosis, kidney disease, liver disease, Alzheimer's disease, and some other complex diseases. Specially, in the treatment of diabetes and its chronic complications, such as diabetic nephropathy, diabetic cardiomyopathy, diabetic foot, diabetic retinopathy, breviscapine has showed significant efficacy. In addition, studies have demonstrated that the combined application of breviscapine, mecobalamine, and micopol can improve the therapeutic effect. In this work, the application of breviscapine in the treatment of chronic complications of diabetes mellitus and its related combination drugs were reviewed, by which we attempted to provide some valuable clues for the clinical application of breviscapine in the treatment of diabetes mellitus and its chronic complications.

RESUMEN

To investigate the effect of scutellarin (Scu) on diabetic cardiomyopathy in mice, type 2 diabetes mellitus was induced by intraperitoneal injection of 50 mg∙kg-1 streptozotocin (STZ) into a high-fat diet. Scu was injected intraperitoneally. After 8 weeks, fasting blood glucose and serum biochemical parameters were measured. Masson staining was performed on myocardial tissue. The expression levels of Nrf2, NFκB, AKT and p-AKT in myocardium of mice were observed by Western blot. All the procedures were approved by the Laboratory Animal Ethics Committee of the Peking Union Medical College. The results showed that Scu significantly decreased the heart-body ratio, increased myocardial contractile function, decreased the level of myocardial fibrosis and the expression of collagen I and collagen III in myocardium of diabetic mice. Scu can effectively reduce the levels of lactate dehydrogenase (LDH), creatine kinase isoenzyme (CK-MB), malondialdehyde (MDA) in serum of diabetic mice, increase the level of antioxidant enzymes in serum, and inhibit the release of inflammatory factors. Further studies showed that Scu significantly increased Nrf2 nuclear translocation, inhibited NFκB nuclear translocation and increased AKT phosphorylation. It indicates that Scu has significant effect on diabetic cardiomyopathy in mice.

RESUMEN

INTRODUCTION@#There is an increasing reliance on informal caregivers to continue the care of patients after discharge. This is a huge responsibility for caregivers and some may feel unprepared for the role. Without adequate support and understanding regarding their needs, patient care may be impeded. This study aimed to identify the needs valued by caregivers and if there was agreement between acute care nurses and caregivers in the perception of whether caregiver needs were being met.@*METHODS@#We conducted face-to-face interviews with 100 pairs of acute care nurses and caregivers. Participants were recruited from inpatient wards through convenience sampling. Questionnaires included demographic data of nurses and caregivers, patients' activities of daily living, and perception of caregiver needs being met in six domains of care. Independent t-test was used to compare mean values in each domain, and intraclass correlation coefficient was used to compare agreement in perception.@*RESULTS@#Caregivers valued reassurance the most. Three domains of care needs showed significant differences in perception of caregiver needs being met:reassurance (p = 0.002), honesty and timeliness (p = 0.008), and kindness and genuine care (p = 0.026). There was poor agreement in all six domains of caregiver needs being met between nurses and caregivers.@*CONCLUSION@#Although caregivers valued reassurance the most, there was poor agreement between acute care nurses and caregivers in the perception of caregiver needs being met. Hence, more attention should be paid to the caregiver's needs. Further studies can examine reasons for unmet caregiver needs and interventions to improve support for them.

RESUMEN

OBJECTIVE@#To compare the serum glycerophospholipid levels in the inflammatory subtypes of asthma by using targeted metabolomic analysis.@*METHODS@#Demographic and clinical data were collected from 51 patients with asthma between January 2015 and December 2015. Routine blood and sputum induction tests were performed. Eosinophilic asthma was defined as induced sputum containing ⪖ 3% eosinophils, and neutrophilic asthma, as induced sputum containing ⪖ 71% neutrophils. Serum metabolic glycerophospholipid profile was determined by liquid chromatography-mass spectrometry. Differences in glycerophospholipid levels between eosinophilic and non-eosinophilic asthma and between neutrophilic and non-neutrophilic asthma were analyzed using partial least squares discriminant analysis.@*RESULTS@#The serum lysophosphatidylglycerol level was significantly higher in the group with ⪖ 3% eosinophils in sputum than in the group with < 3% eosinophils in sputum. The area under the receiver-operating characteristic curve was ⪖ 70%. There was no significant difference in the serum metabolic glycerophospholipid profile between the group with sputum neutrophils ⪖ 71% and the group with sputum neutrophils < 71%.@*CONCLUSION@#Serum lysophosphatidylglycerol is produced abundantly in eosinophilic asthma and may be a biomarker of eosinophilic asthma. This information is helpful for identifying and tailoring treatment for the common asthma subtypes.

Objective Few studies are reported about the values of the levels of plasma lipoprotein related phospholipase A2 (LP-PLA2) and serum resistin in predicting the prognosis of atherosclerotic cerebral infarction (ACI).This article aims to evaluate the predictive values of LP-PLA2 and serum resistin in the prognosis of ACI.Methods This study included 136 cases of ACI diagnosed and treated in Huaihe Hosptial from September 2013 to September 2014.The patients were followed up for 2 years, during which 48 were found with adverse outcomes (the poor prognosis group) 76 without disease progression (the good prognosis group). We analyzed the influencing factors on prognosis using the Cox proportional hazard model and evaluated the sensitivity and specificity of these factors in predicting the prognostic risks of the patients by ROC curve analysis. Results The rate of poor prognosis was 38.71% among the included patients.Analysis with the Cox proportional hazard model showed significant impacts of LP-PLA2 (OR =2.105,95% CI:1.878-2.413) and serum resistin (OR=1.784,95% CI:1.509-2.213) on the prognosis of the patients.Compared with the good prognosis group, the poor prognosis group exhibited markedly higher levels of LP-PLA2 ([128.78±76.22] vs [268.65±89.02] mg/L,P<0.01)and serum resistin ([20.71±6.15] vs [24.36±4.87] mg/L,P<0.01).The sensitivity and specificity of LP-PLA2 combined with serum resistin were 81.35% and 78.26%,respectively.Conclusion The combination of LP-PLA2 with serum resistin has a good predictive value for the prognosis of atherosclerotic cerebral infarction and is expected to be widely applied as a routine index in clinical practice.

RESUMEN

@#AIM: To explore whether there was a difference in clinical efficacy of toric design orthokeratology and regular spherical orthokeratology in myopia patients with high difference in corneal surface. <p>METHODS: A retrospective analysis of 48 moderate myopia patients(94 eyes), who went to the optometry center of Ruikang Hospital Affiliated to Guangxi University of Chinese Medical form July 2016 to December 2017. The height difference between the horizontal axis and the vertical axis away from the central cornea at 4mm was ≥30μm calculated in all patients with corneal topography. In Group A, 25 cases(48 eyes)with toric design orthokeratology, Group B(23 cases, 46 eyes)with regular spherical. After wearing orthokeratology 1wk, 1, 3mo, the uncorrected visual acuity(UCVA), the changes of corneal astigmatism in 3mm corneal center, the eccentricity of corneal treatment area were observed. <p>RESULTS: The UCVA was improved of both groups after operation, while was not significantly different between the two groups<i>(P</i>>0.05). The corneal regular astigmatism in 3mm corneal center were decreased after wearing orthokeratology 1wk, 1, 3mo, the changes were more obvious in Group A than Group B(<i>P</i><0.05). the eccentricity of corneal treatment area in Group A were less than Group B(<i>P</i><0.05). <p>CONCLUSION: For myopic patients with height difference between the horizontal axis and the vertical axis from the central cornea, the toric design orthokeratology is superior to regular spherical design orthokeratology in reducing astigmatism and stability of treatment area.

RESUMEN

As traditional Chinese medicinal herbs, Physalis plants have a variety of pharmacological activities, such as anti-inflammatory, anti-oxidant, and anti-cancer effects, and have been used for the treatment of malaria, rheumatism, hepatitis, asthma, and cancer. In addition to the medicinal value, many Physalis species are also the high-grade nutrition health care fruits, can be made canned and candied etc. In the study, the application progress of DNA molecular marker technologies in medicinal Physalis plants in recent years was reviewed, in order to provide an important molecular technical basis for the identification, classification and rational development and protection of medicinal Physalis resources.

RESUMEN

A rapid Liquid chromatography-tandem mass spectrometry method was developed for the simultaneous determination of six neurotransmitters in rat plasma, adrenal gland and hypothalamus, including γ-aminobutyric acid (GABA),serotonin(5-HT), tyrosine(L-Tyr), dopamine(DA), norepinephrine(NE) and epinephrine(E). This method was used for the neurotransmitters detection in rat adrenal, blood and hypothalamus. The samples were pre-column derivatized with dansyl chloride, and 5-HTP and CA were used as internal standards. A Thermo C18column (50 mm×3 mm,2.7 μm) was used for sample separation with 0.1% aqueous formic acid solution as phase A and methanol as phase B at a flow rate of 0.2 mL/min, and the injection volume was set at 2 μL. The detection was carried out with multi-reaction monitoring(MRM) in electron spray ionization (ESI) positive mode. The linear ranges for detection of GABA,5-HT, L-Tyr, DA, NE and E were 0.26-620.80 μmol/L,0.03-11.20 μmol/L,1.20-88.00 μmol/L,0.03-41.02 μmol/L, 0.01-47.20 μmol/L, 0. 01-90. 24 μmol/L, respectively, with recoveries varying from 91. 16% to 116.20%. This method can detect the neurotransmitters rapidly and accurately, providing a platform for the determination of neurotransmitters in rat plasma, adrenal gland and hypothalamus in pharmacological experiments.
