RESUMEN

Objective:To investigate the clinicopathological features and prognosis of medullary thyroid carcinoma (MTC) co-existent with Hashimoto's thyroiditis (HT).Methods:Seven cases of MTC co-existent with HT were collected from Fujian Cancer Hospital between Jan 2007 and May 2021. Its clinicopathological features were analyzed.Results:All patients were adult female with a median age of 49 years, and 6 of them were unilateral MTC. The tumor diameter ranged from 0.4 to 7.0 cm. The thyroid tissue surrounding the tumor showed HT. The follow-up time was 16 to 152 months, with a median time of 61 months, 5 patients (tumor diameter <1 cm) were biochemically cured, 1 patient (tumor diameter >1 cm) was anatomically cured. The another patient died due to tumor progression 30 months after surgery, the tumor cells in which were highly atypia and more mitoses (5~8/10HPF) necrosis and vascular invasion were also observed.Conclusion:MTC with HT are mostly sporadic cases, and most of them have a fair prognosis.

RESUMEN

Objective@#To investigate the clinical presentation, clinicopathologic features, diagnosis and differential diagnosis of adult T cell leukemia/lymphoma (ATLL).@*Methods@#Four cases of ATLL from Fujian Cancer Hospital between October 2017 and May 2018 were analyzed using hematoxylin-eosin and immunohistochemical stains and polymerase chain reaction (PCR) for HTLV-1 provirus genes. The relevant literature was reviewed.@*Results@#There were two males and two females, age range 38-80 years. All patients were from coastal cities of Fujian province. Clinical presentations including lymphadenopathy, hepatomegaly and splenomegaly were detected in most patients; skin lesion, hypercalcemia and lymphocytosis were also commonly detected.Histologically, there was diffuse effacement of the normal architecture by tumor cells infiltration. The inflammatory background is usually sparse, with scanty eosinophils. The atypical lymphoid cells were typically medium to large sized with pronounced nuclear pleomorphism, irregular nuclei, chromatin clumping and prominent nucleoli. Blast-like cells with transformed nuclei were present in variable proportions. Giant cells with convoluted or cerebriform nuclear contours may be present. Rare cases may be composed predominantly of anaplastic tumor cells. Characteristic "flower cells" with large multi-lobated nuclei can be seen. The tumor cells were strongly positive for CD2, CD3, CD5, CD4 and CD25, but negative for CD7, CD8 and cytotoxic molecules (including TIA-1, Granzyme B and perforin). In three cases, the large transformed cells were positive for CD30. In one case, the anaplastic large cells were diffusely and strongly positive for CD30. All cases were negative for EBER, but positive for HTLV-1 provirus.@*Conclusions@#ATLL is a rare type of T cell lymphoma with unique clinical and pathological features, and should be distinguished from peripheral T cell lymphoma, NOS, ALK negative anaplastic large cell lymphoma and mycosis fungoides. Hypercalcemia, systemic disease, characteristic "flower cells" and specific immunophenotypic profile of CD3+, CD4+, CD25+, and CD7- are highly suggestive. However, ATLL can only be confirmed if the presence of HTLV-1 provirus.

RESUMEN

Objective To study the protective effect and mechanism of maslinic acid (MA) on acute liver injury (ALI) in rats.Methods Fifty BALB/c mice were randomly divided into control group,model group,and low (12.5 mg/kg),medium (25.0 mg/kg) and high doses (50.0 mg/kg) of MA,with 10 rats in each group.The control group was intraperitoneally injected with normal saline.The other groups were intraperitoneally injected with lipopolysaccharide (LPS) (50 mg/kg) and D-Gal N (500 mg/kg) to prepare mouse AL[model.The MA groups were administered with 12.5,25.0,50.0 mg/kg MA 1 h before model establishment,respectively.All the mice were sacrificed 6 h after model establishment,and serum and liver tissues were collected.Serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels were measured.Hematoxylin-eosin staining was used to observe the pathological changes of liver tissue.Thiobarbituric acid method was used to determine malondialdehyde (MDA).H2O2 reaction product colorimetric was used to determine the content of myeloperoxidase (MPO).The tumor necrosis factor alpha (TNF-α) and interleukin-6 (IL-6) in serum and liver tissue was detected by enzyme-linked immunosorbent assay,respectively.Western Blot was conducted to detect the expression of nuclear factor E2 related factor 2 (Nrf2),heme oxygenase-1 (HO-1) and the activation of nuclear factor-kappa B (NF-κB) pathway.Results Compared with the model group,the liver histopathology in the low,medium and high doses MA groups was significantly improved.The serum ALT and AST levels were decreased,and the differences were statistically significant (all P<0.05).The contents of MDA and MPO in liver tissues were decreased,and the differences were statistically significant (all P<0.05).The protein contents of Nrf2 and HO-1 were increased,the differences were statistically significant (all P<0.05).The NF-κB pathway was inhibited,and the differences were statistically significant (all P<0.05).The levels of TNF-α and IL-6 in serum and liver tissues were decreased,and the differences were statistically significant (all P<0.05).Conclusions MA has a protective effect on LPS/D-Gal N-induced ALI,and its mechanism is related to inhibition of NF-κB pathway and activation of Nrf2/HO-1 pathway.

RESUMEN

OBJECTIVE:To investigate clinical efficacy and safety of montelukast combined with budesonide in the treatment of bronchial asthma(BA),and it effects on clinical symptom,inflammatory factor and immune function of BA children. METHODS:To-tally 106 BA children selected from pediatrics department of our hospital during Jan. 2014-Dec. 2015 were divided into observation group and control group according to random number table,with 53 cases in each group. Based on routine treatment,control group was given Budesonide suspension for inhalation 400μg,bid. Observation group was additionally given Montelukast sodium tablets oral-ly,4 mg for under 5 years old,qd,5 mg for 5 years old or older,qd,on the basis of control group. Treatment courses of 2 groups last-ed for 8 weeks. Clinical efficacies of 2 groups were compared as well as clinical symptom scores,inflammatory factor and immune fac-tor levels before and after treatment. The occurrence of ADR was also compared between 2 groups. RESULTS:The total response rate of observation group(94.34%)was significantly higher than that of control group(77.36%);the symptom relief time and lung signs disappearance time were significantly shorter than control group,with statistical significance(P0.05). After treatment,daytime and nighttime symptom scores,the levels of IL-6,IL-8,TNF-αand IgE in 2 groups were significantly decreased, while the levels of IgA and IgG were increased significantly;the observation group was significantly better than control group,with sta-tistical significance(P<0.05). There was no statistical significance in the incidence of ADR between 2 groups(5.66% vs. 7.55%) (P<0.05). CONCLUSIONS:Montelukast combined with budesonide help to reduce the level of serum inflammatory cytokines in BA children,improve immune function and clinical symptoms with good safety.

RESUMEN

Objective To prepare polyclonal antibody of transactivated protein 5 of hepatitis C virus nonstructural 5A (NA5ATP5) and to explore its expression in the liver tissues. Methods In Escherichia coil BL21, the prokaryotic expression vector pET32a(+)-NS5ATP5 was induced by isopropyl-β-D-thiogalactoside (IPTG), and it was analyzed with sodium dodecyl sulfate-polyaerylamide gel electrophoresis (SDS-PAGE) and Western blotting. And the purified protein was used to immunize the rabbit to prepare polyelonai antibody, with which we studied the function of NSSATP5 by determining the different liver tissues with the streptavidin-perosidase (SP) immunohistochemistry method. Results Recombinant NS5ATP5 (molecular weight: 65 kD) and polyclonal antibody were successfully prepared. NS5ATP5 expression in the liver of patients with chronic HCV infection was much higher than that of a normal person, and it was detected mainly in the cytoplasm. Conclusion The findings of the expression difference between HCV patients and normal people led to a novel diagnostic marker to detect HCV infection.

Objective:To evaluate the clinical efficacy of shenxiong glucose injection in the treatment of chronic re- nal failure(CRF).Method:78 patients with CRF were divided into a treatment group(n=40)and a controlled gronp(n= 38).The treatment group received shenxiong glucose injection 200ml daily,14 days for one course of treatment.The con- trolled group received ligustrazine injection 80mg with 5% glucose.The changes of serum BUN and SCr were observed.Re- sult:The serum BUN and SCr were significantly decreased in the two groups after the treatment(P

RESUMEN

BACKGROUND:With the increasing morbidity of type 2 diabetes, people should pay more attention to the early intervention of cardiovascular complications in order to reduce the incidence of its complications. OBJECTIVE:To observe the risk factors of type 2 diabetes combining coronary heart disease(CHD) and the images of coronary angiography. DESIGN:A cross-sectional observational study. SETTING:Internal medicine department of a municipal hospital. PARTICIPANTS:The study was completed in the Department of Endocrine Harbin First Hospital from May 1998 to May 2003.Inclusive criteria:Type 2 diabetic patients aged above 30 years old and combined with CHD with either sex, who met the diagnostic standard of diabetes set by ADA in 1997 and Naming and Diagnostic standard of ischemic heart disease set by WHO in 1979. Exclusive criteria:Patients suffering from severe heart,liver,kidney disease and serious infection,rheumatoid heart disease,pulmonary heart disease and so on.There were 98 inpatients, who met the above mentioned inclusive criteria, were set as the diabetic group with 56 males and 42 females,aged from 35 to 70 years old;and 85 CHD patients without combining diabetes admitted to hospital at the same time were chosen as the control group with 53 males and 32 females, aged from 40 to 75 years old. INTERVENTIONS:The blood glucose was observed with the glucose oxidase method,fasting insulin and C peptide with radioimmunoassay (RIA),glycosylated hemoglobin with affinity chromatography, triglyceride(TG),total cholesterol(TC),high density lipoprotein-cholesterol(HDL-C) and low density lipoprotein-cholesterol (LDL-C) with automatic biochemical analysis,and selective right coronary angiography was applied by JUDKINS method. Of coronary arteries. RESULTS:The comparison of the coronary angiographic images between the two groups showed that there was no significant difference in the lesion of left main trunk and circumflex artery between the diabetic group and control group(P >0.05);While there were more cases with lesion of anterior descending artery and right coronary artery in the diabetic group than in the control group(P< 0.05). The pathological changes in the diabetic group mainly manifested with lesion of three branches and single branch in which represented diffusive changes.Lesion of three branches was the main pathological change of coronary artery in type 2 diabetic patients with diffusive changes. CONCLUSION:With the extension of course of type 2 diabetes in patients,the incidence of CHD will gradually increase with extensive pathological changes and much severe conditions.
