RESUMEN

Objective To investigate the effects of postoperative analgesia with Dexmedetomidine combined with Sufentanil on pulmonary infection complications and immune function in patients with lung cancer.Methods A total of 200 patients with lung cancer who underwent radical lung cancer in our hospital from July 2014 to June 2018 were randomly divided into the group A (n=100,receiving Sufentanil analgesia)and group B (n=100,receiving Sufentanil combined with Dexmedetomidine analgesia)according to the random number table method.The analgesic effect,pulmonary complication incidence and immune function were compared between the two groups.Results The analgesic and sedative effects were better in the group B than in the group A at 4,8,12 and 24 hours after surgery,respectively(P < 0.05).The visual analogue scale (VAS) and Ramesay score had statistically significant differences between the two groups at 4,8,12 and 24 hours after surgery,respectively(VAS:t =4.137,2.575,3.869,12.676 and 9.945,all P <0.05;Ramesay score:t=6.771,5.647,16.763,21.154 and 6.556,all P<0.05).The pulmonary infection,acute lung injury,focal lung infiltration,atelectasis and respiratory failure occurred in 1,0,1,1 and 0 case in the group B and 3,2,3,1 and 1 case in the group A,respectively.The incidence of pulmonary complications was lower in the group B than in the group A(3% or 3/100 vs.10% or 10/100,X2 =4.031,P=0.045).There were no significant difference in the levels of CD3+,CD4+,CD8+ and CD4+/CD8+ cells before surgery between the two groups(P>0.05).The levels of CD3+,CD4+,CD8+ and CD4+ /CD8+ cells were higher in group B than in group A at 1 day after surgery(t=7.419,7.867,11.968 and 8.755,P=0.000).Conclusions Postoperative Dexmedetomidine combined with Sufentanil analgesia not only helps to reduce the incidence of pulmonary complications,but also improves the analgesic effect and immune function in patients with lung cancer undergoing radical surgery.

RESUMEN

Stature is one of the important criteria to individual recognition. And it is always a focus in forensic anthropology. There are some new progresses studies in stature estimation in recent years. We review some studies in stature estimation published in domestic and foreign in the past six years, and find some progresses in stature estimation: (1) As the increasing average human stature, the stature estimation equation is needed to be corrected; (2) Gender, age, race and territory have a great influence to stature estimation. Thus when the study protocol designed, these influences should be considered; (3)The medical imaging technology, especially Computed Tomography, is becoming a new tool to infer stature estimation in forensic anthropology.

Objective To analyze the misdiagnosis reasons of lung lesions detected by 18 F-FDG PET-CT .Methods Twenty-seven cases of misdiagnosed lung lesions were retrospectively analyzed ,including 14 cases of benign lesions misdiagnosed as lung cancer and 13 cases of lung cancer misdiagnosed as benign lesion ,which were analyzed from the aspects of the CT signs ,lesion size , SUVmax value ,SUVmax value at two-phase PET and clinicopathology .Results The CT signs in the false positive group were mainly manifested as clear border ,uniform density and so on ,while which in the false negative group were mainly manifested as lob-ulation sign ,spicule sign ,pleural indentation sign ,and so on .The mean maximum lesion diameter in the false positive group was (3 .23 ± 0 .59)cm ,which in the false negative group was (1 .67 ± 0 .27)cm .The SUVmax mean value in the false positive group was 10 .49 ,which in the false negative group was 5 .49 .The mean increasing rate of the SUVmax by dual-phase PET-CT in the false positive group was 38 .4% ,which in the false negative group was 17 .9% .The clinicopathologic inflammatory lesion was easy to be misdiagnosed as lung cancer ,high differentiated lung cancer and carcinoid were easy to be misdiagnosed as benign lesion .Conclusion Judging the malignant or benign lung lesions should consult in the patient′s clinical data and strengthen the comprehensice appli-cation of CT signs ,lesion size and SUVmax value .

RESUMEN

Objective To verify the relationship between personality traits and clinical nurse competence in five third-A level hospitals in Shenyang. Methods A total of 1559 clinical nurses from five tertiary-level hospitals in Shenyang were recruited into this study. Two self-report questionnaires, the Nurse Competence Scale (NCS) and the Tridimensional Personality Questionnaire (TPQ), were used to collect data. Results The average score of NCS was 71.97 ± 13.25.Nursing competence increased with the increase of working years (β=0.272, P<0.01) and RD (β=0.134, P<0.01). Practicing nurses with the lower HA appear to have a higher level of competence (β=-0.117, P<0.01). Conclusions The overall level of nurses' competencies good in third-A level hospitals in Shenyang. The predicting factors of nursing competence are work experience, HA and RD. More psychological courses should be provided in fundamental nursing education and continuing nursing education for helping nurses shape their good personality traits.

Objective To study related factors of the duration of untreated psychosis (DUP) in Tibetan patients with schizophrenia in Qinghai Province. Methods The related factors on DUP were investigated in totally 188 Tibetan pa?tients with schizophrenia using questionnaires of mental health services and symptom onset for schizophrenia. All the Ti?betan patients were provided with the National Continuing Management and Intervention Program for Psychoses (686 Pro?gram). Results The median (low quartile, upper quartile) of DUP in Tibetan patients with schizophrenia was 375 days (4 days, 1661 days). The patients were divided into short DUP group (DUP≤375days, 90 patients) and long DUP group (DUP>375 days, 98 patients). There were significant differences in mode of onset,marital status, educational level, family type, place of residence between short DUP group and long DUP group (P<0.05). Logistic regression analysis found that lack of family structure (OR=2.340, 95%CI:1.130~4.847, P=0.022), chronic onset (OR=2.136, 95%CI:1.172~3.891, P=0.013) and living in pastoral areas (OR=2.239, 95%CI:1.097~4.571, P=0.027) were risk factors of DUP. Conclusion Ti?betan patients with schizophrenia have a longer DUP and related risk factories of DUP are lack of family structure, chron?ic onset and living in pastoral areas.

RESUMEN

Generally, P-wave is the wave of low-frequency and low-amplitude, and it could be affected by baseline drift, electromyography (EMG) interference and other noises easily. Not every heart beat contains the P-wave, and it is also a major problem to determine the P-wave exist or not in a heart beat. In order to solve the limitation of suiting the diverse morphological P-wave using wavelet-amplitude-transform algorithm and the limitation of selecting the pseudo-P-wave sample using the wavelet transform and neural network, we presented new P-wave detecting method based on wave-amplitude threshold and using the multi-feature as the input of neural networks. Firstly, we removed the noise of ECG through the wavelet transform, then determined the position of the candidate P-wave by calculating modulus maxima of the wavelet transform, and then determine the P-wave exist or not by wave-amplitude threshold method initially. Finally we determined whether the P-wave existed or not by the neural networks. The method is validated based on the QT database which is supplied with manual labels made by physicians. We compared the detection effect of ECG P-waves, which was obtained with the method developed in the study, with the algorithm of wavelet threshold value and the method based on "wavelet-amplitude-slope", and verified the feasibility of the proposed algorithm. The detected ECG signal, which is recorded in the hospital ECG division, was consistent with the doctor's labels. Furthermore, after detecting the 13 sets of ECG which were 15 min long, the detection rate for the correct P-wave is 99.911%.

RESUMEN

<p><b>OBJECTIVE</b>To study the Ri plasmid transformation and metabolism of ginseng products on HepG2 cells.</p><p><b>METHOD</b>The inhibitory effect of ginseng saponin on hepatoma HepG2 cells was studied. The hairy root-induced conditions were screened by orthogonal experimental design. The culture conditions were determined through hairy root biomass accumulation and saponin content. The effect of ginsenoside on HepG2 cells was determined by MTT assay.</p><p><b>RESULT AND CONCLUSION</b>The optimal ginseng hairy root inducing conditions: A = 0.6, infection time of 10 min, pre-incubation time for the 3 d. The best culture conditions: MS medium, pH 6.1, 24 degrees C. At those conditions the hairy root bio-accumulation and saponin content were higher. The results of ginseng saponins on the inhibitory effect of HepG2 cells showed that inhibition of ginseng saponins on HepG2 was the concentration positively related.</p>

RESUMEN

The association of the serum phospholipid fatty acid components with high-sensitivity C reactive protein in patients with metabolic syndrome and type 2 diabetes mellitus was investigated.Saturated fatty acid in patients with metabolic syndrome and type 2 diabetes mellitus was increased (45.98±1.55 vs 42.05±2.04,P<0.01),while polyunsaturated fatty acid was decreased(42.38±1.93 vs 46.60±2.06,P<0.01),and high sensitivity C reactive protein was significantly increased[(6.36± 1.28 vs 0.79±0.45) mg/L,P<0.01)].

RESUMEN

Objective To investigate the relationship between serum omega-3 polyunsaturated fatty acid (ω-3PUFA) and insulin resistance(IR) in patients with type 2 diabetes mellitus and non-alcoholic fatty liver disease (NAFLD). Methods This trial involved 51 patients of type 2 diabetes mellitus with NAFLD (G4 group), 50 patients of type 2 diabetes alone(G3 group), 45 patients of NAFLD alone (G2 group) and 42 healthy control subjects (G1 group). Serum ω-3PUFA profile was analyzed with capillary gas chromatography. Insulin resistance was assessed by homeostasis model assessment (HOMA-IR). ALT, AST, γ-glutamyltransferase(GGT) and serum lipids were measured. Results The levels of HOMA-IR were higher in G4 group than those in G3, G2 and G1 group(4. 90 ± 2. 54 vs 2. 38 ± 1.23, 2. 20 ± 1.15, 1.13± 0.42;P<0.05). The level of ALT, AST, GGT, TC, TG, LDL-C were higher in G4 group than those in G3, G2 and G1 group(P < 0. 05). The level of ω-3PUFA was signifieantly lower in G4 group than those in G3, G2 and G1 group(5.68 ±2.02 vs 7. 17 ±2. 38, 6.97±2.32, 10.08±2.76;P <0.05). ω-3PUFA cocentration was negatively correlated with HOMA-IR, TC, TG and LDL-C (r = -0.491, -0. 376, - 0. 462, - 0. 408, P < 0. 05). Conclusions Serum ω-3 PUFA is significantly decreased in patients with type 2 diabetes mellitus and NAFLD. Serum ω-3PUFA is negatively correlated with insulin resistance. ω-3PUEA plays a very important role in the development of diabetes mellitus and NAFLD.

RESUMEN

OBJECTIVE To study the antimicrobial sensitivity of pathogens isolated from diabetic foot patients. METHODS Totally 102 diabetic foot patients were enrolled from Jun 2000 to Dec 2007 in our hospital.Specimens such as pus and wound exudate were collected for culture.Pathogenic spectrum and antimicrobial sensitivities were investigated. RESULTS From 70 cases 109 strains of pathogenic bacteria were isolated,of which 61 were Gram-positive bacteria,43 Gram-negative bacteria and 5 fungi.Thirty-seven patients were with single microbial infection and 33 patients with polymicrobial infection and 28 of 33 patients were with Wagner′s grade 3 and upwards.According to susceptibility test,multi-drug resistance was found.Gram-negative bacteria were sensitive to imipenem and ?-lactamases inhibitor,and Gram-positive bacteria were sensitive to vancomycin,chloramphenicol,and cephalosporin. CONCLUSIONS The pathogenic bacteria in diabetic foot infection distribute extensively and some of them are multi-drug resistant.The key to the treatment of diabetic foot infection is early combination application of sensitive antimicrobial agents.

Objective:To examine the role of dopamine D4 receptor gene(DRD4) in attention deficit hyperactivity disorder(ADHD) with/without disruptive behavior disorder(DBD) by focusing on a-521C/T SNP within the promoter region of this gene.Methods:A total of 401 DSM-Ⅳ ADHD children(including 284 trios) of Chinese Han descent were genotyped.Chi-square test and the transmission disequilibrium test(TDT) were used to test for associations in ADHD with and without DBD respectively.Results:In the comparison of ADHD with(n= 143) and without(n= 258) DBD,the-521T allele(?2 = 6.778,P= 0.009,OR= 1.485) and the TT genotype(?2 = 6.292,P= 0.012,OR= 1.729) showed higher frequency in children with ADHD and DBD simultaneously.For family based analysis,T allele of the-521C/T polymorphism was preferentially transmitted to ADHD children with comorbid DBD(n= 100,?2 = 3.868,P= 0.049),whereas no significant distortion was found in the transmission of the tested variant for ADHD without DBD(n= 184,?2 = 0.223,P= 0.637).Conclusion:Our findings suggest that the-521C/T SNP of DRD4 may contribute to the predisposition to ADHD with comorbid DBD.This study supports for the hypothesis that ADHD with comorbid DBD may be influenced by greater genetic effect compared to ADHD alone.

RESUMEN

The intima-media thickness (IMT) of common carotid artery was measured using B-mode ultrasonography in 55 patients with type 2 diabetes mellitus (DM) and 25 normal controls. The serum levels of soluble intercellular adhesion molecule-1 ( sICAM-1) and high-sensitivity C-reactive protein ( hs-CRP) were measured by ELISA and high-sensitivity particle enhanced immunonephelometry respectively. The results suggest that sICAM-1 and hs-CRP were risk factors of thickened IMT in type 2 DM.
