RESUMEN

Congenital diaphragmatic hernia (CDH) associated with pulmonary hypoplasia usually presents in early neonatal life. Despite recent advances in perioperative intensive care and neonatal pharmacology, there is still a 53-62% mortality rate. CDH presenting outside of the neonatal period is called CDH delayed presentation, and it is uncommon and the diagnosis is difficult due to misleading clinical signs and symptoms. The prognosis in delayed presentation is determined by complications as strangulation or incarceration of the herniated bowel or stomach, mediastinal shift. A 3 month-old male infant was visited to pediatric department because of irritability. Breath sound were decreased over the left hemithorax and blood gas examination showed hypoxemia. Plain chest X-rays demonstrated intestinal bowel gas shadow in the left hemithorax and had done revealed CDH. An immediate operation was performed. The post-operative course was uneventful and the postoperative chest X-ray returned to normal. She was discharged on the 8th post-opertive day.

RESUMEN

The purpose of a conculation is to seek the opinions and advice of those whom we recognize to be more expert in a particular field of medicine than we are. Most surgeons regard the anesthesiolgist as a conculatant. Thus we analyzed 308 cases of interdepartmental consulatation submitted to our department from June 1991 to Novermber 1991. All case were elective ones and emergent cases were excluded. The analysis were as follow. 1) The overall conculation rate were 11.5%. 2) The highest rate of consultation were requested from orthopedic surgery department(29.9%) followed by ophthalmology department(25.0%), neurosurgery department(24.6%) and urology department(13.4%). 3) Male was 42.8% and female was 57.2%. 4) Most requestered problem was hepatic diseases including elevated transaminase, hepatitis and liver cirrhosis(35.1%) and then cardiovascular problem was 20.5%. 5) Because of our opinion, 22 cases were postponed. 6) Consultation rate increased according to advancing of age.

RESUMEN

The effect of general anesthesia for the cesarean section varies according to anesthetic type, drug dosage and concentration of the anesthetics. Thus, the authors studied the effects of general anesthesia for twenties healthy parturients according to anesthetic agents. The method consisted of administration of halothane(Group I, N=IO) or a combination of fentanyl and droperidol(GroupII, N=10). We checked the hemoglonin value, rbc number on the pre-operative day, immediate post-operative, post-operative seocond and post-operative fifth day. The results were as follows. In Group I, hemoglobin value and rbc number significantly decreased on the post-operative secone and fifth day(p<0.025). But in Group II, there are no differences in hemoglonin value and RBC number.
