RESUMEN

Objective To study the risk factors and prognosis of very/extremely low birth weight preterm (VLBW/ELBW) infants with bronchopulmonary dysplasia (BPD) during the first three years of life.Method From January 1st to December 31st,2012,a retrospective study was conducted on the VLBW/ELBW preterm infants with > 28 days of hospitalization in the NICU of our hospital. The infants were assigned into BPD group (FiO2 > 21%) and non-BPD group depending on the oxygen requirement on the 28th day after birth. The incidences of bronchitis, pneumonia, wheezing and re-hospitalization during the first three years of life were analyzed. The pulmonary function tests were performed at one-year-age.Independent-sapmles t test,Kruskal-wallis test and x2 test were used to compare the results between the two groups.Result A total of 72 patients were enrolled into this study.34 patients in the BPD group and 38 in the non-BPD group. The gestational age, birth weight and the use of INSURE technique of BPD group were significantly lower than non-BPD group, while the duration of hospital stays were longer than non-BPD group (P <0.05).The ratio of male, neonatal respiratory distress syndrome (RDS),patent ductus arteriosus (PDA),intrauterine infection, mechanical ventilation, nosocomial infection and the inhalation of high concentrations of oxygen were significantly higher in BPD group than non-BPD group (P < 0.05).Incidences of bronchitis and wheezing during 0 ~ 1,1 ~ 2 and 2 ~ 3 years of age in the BPD were significantly higher than non-BPD group, separately. No significant differences existed in the incidences of pneumonia and re-hospitalization between the two groups.Pulmonary function test showed that the respiratory rate (RR) and peak tidal expiratory flow in the BPD group were significantly higher than non-BPD group, while the tidal volume, peak expiratory time and peak expiratory volume were significantly lower in BPD group than non-BPD group (P < 0.05).Conclusion BPD often occurs in preterm infants with gestational age less than 32 weeks. It may cause impaired pulmonary function, characterized by obstruction in small airway, and increases the risk of bronchitis and wheezing during the first three year of life.

Objective To investigate the image quality and radiation dosage of the prospective ECG-triggered 256 multi-slice computer tomography (MSCT) angiography in accessing coronary artery (CA) on children. Methods Coronary arteries of ninety-three children were evaluated using prospective ECG-triggered 256-MSCT with the same system setting.Seventy-four patients had the heart rate records and a four-point scoring system was applied to study the capability of MSCT in detecting CA.The signal, noise and contrast-to-noise ratio (CNR) were analyzed to investigate the association of image quality with age (n =74).Then volumetric CT dose index (CTDIvol),dose-length product (DLP) and effective dose (E) were utilized to study the association of radiation dosage with age (n =93).Results On 74 patients with heart records, the detection rate for original, proximal, middle, distal and all 11 segments of CA was 100%,97%,92%,78% and 91%,respectively.No influence of age was found on their detection (all P>0.05).E of group A,B,C and D were 1.8,1.3,0.8 and 0.7 mSv,respectively.A strong negative correlation was found between E and age (r =-0.803,P < 0.01).Higher DLP was found on elder patients, though no correlation was found between ages vs.DLP(r =0.124,P > 0.05).No correlation was found between noise vs.age (r =0.041,P > 0.05).Significantly larger signals,CNR and E(t =3.386,2.073,3.825,P < 0.05) were found on younger patients (age < 3 years). Conclusions Prospective ECG-triggered 256-MSCT has considerable performance for the evaluation of coronary arteries on children. However, children under the age of 3 are not recommended for this examinary, but for examination on children ≥ 8-year old,the tube current could be further needed.

RESUMEN

In this paper, we present a virtual endoscopy (VE) for diagnosis of heart diseases, which is proved efficient and affordable, easy to popularize for viewing the interior of the heart. The dual source CT (DSCT) data were used as primary data in our system. The 3D structure of virtual heart was reconstructed with 3D texture mapping technology based on graphics processing unit (GPU), and could be displayed dynamically in real time. When we displayed it in real time, we could not only observe the inside of the chambers of heart but also examine from the new angle of view by the 3D data which were already clipped according to doctor's desire. In the pattern of observation, we used both mutual interactive mode and auto mode. In the auto mode, we used Dijkstra Algorithm which treated the 3D Euler distance as weighting factor to find out the view path quickly, and, used view path to calculate the four chamber plane.

RESUMEN

ObjectiveTo evaluate the feasibility and accuracy of velocity vector imaging (VVI) in assessing ventricular function in children with single ventricle after cavopulmonary connection. Methods Thirty children with single ventricle after cavopulmonary connection were enrolled in this study,30 agematched normal children were served as control group. The systolic peak velocity, displacement, strain and strain rate in this two groups measured in 6 segments by VVI were compared.dp/dt of single ventricle was estimated by atrioventricular regurgitation using simplified Bernoulli equation. Results Strain and strain rate were significantly lower in all 6 segments in children with single ventricle after cavopulmonary connection compared with values in normal children( P <0.05,respectively),strain rate of the basal segment at the rudimentary chamber correlated best with dp/dt (r =0.72,P <0.01).Conclusions Segmental ventricular dysfunction was observed in children with single ventricle after cavopulmonary connection, and could be assessed accurately using velocity vector imaging.
