RESUMEN

The intervention and prevention of perioperative hypothermia is not only reflected in the technical level, but also reveals the important role of humanistic care in the whole intervention work. If perioperative patients have hypothermia, it is likely to cause a series of complications such as postoperative shivering, which seriously threatens the life safety of patients. Prevention and intervention was based on a comprehensive understanding of the causes and hazards of hypothermia, especially the impact on the lives of the elderly. Effective supervision was implemented in the whole process of operation, such as dynamic monitoring of vital signs including body temperature, followed by room temperature regulation, body temperature protection and preoperative and postoperative psychological nursing. At this time, the sense of responsibility, good humanistic care of medical staff are of positive significance to effectively prevent and reduce the probability of perioperative hypothermia and accelerate the postoperative rehabilitation of patients.

RESUMEN

Objective To optimize the supercritical CO2 extraction conditions of volatile oil from Wenjing Huoxue cataplasm. Methods On the basis of single factor investigation on the comprehensive score of extraction yield , osthole content and isoimperatorin, the effects of extraction temperature, pressure and time on the comprehensive score of extracted volatile oil were optimized by orthogonal design. Results In the single factor experiment, the factors that had a great influence on the comprehensive score of the extracted volatile oil were extraction temperature, extraction pressure and extraction time. The orthogonal experiment results showed that the extraction temperature and extraction pressure had a significant influence on the comprehensive score of volatile oil. The optimized extraction process was as follows: extraction temperature at 55 ℃, extraction pressure as 30 MPa, and extraction time as 2 h. Conclusion The extraction process optimized in this experiment is stable and feasible, which could be used for the extraction and preparation of the volatile oil.

Objective:To investigate the clinical value of Omniview combined with volume contrast imaging (VCI) in measuring the posterior angle of fetal near-field lateral ventricle.Methods:Fifty fetuses whose lateral ventricles were widened, corpus callosum was poorly developed, and who had hydrocephalus as indicated by routine ultrasonography performed between October 1, 2018 and December 31, 2019 in Hangzhou Obstetrics & Gynecology Hospital were included in this study. The width of the posterior angle of fetal near-field lateral ventricle was measured using routine ultrasonography, Omniview-VCI technique, and magnetic resonance imaging (MRI) in each fetus and compared among the three techniques.Results:The width of the posterior angle of fetal near-field lateral ventricle measured by Omniview-VCI and MRI techniques was (1.17 ± 0.15) cm and (1.20 ± 0.12) cm, respectively, which was significantly greater than that measured by routine ultrasonography [(1.11 ± 0.13) cm, t = 2.137, 3.597, both P < 0.05]. There was no significant difference in the width of the posterior angle of fetal near-field lateral ventricle measured by Omniview-VCI technique versus MRI technique ( t = 1.104, P > 0.05). The mean examination cost and waiting time of Omniview-VCI technique were (222.15 ± 3.20) yuan and (0.24 ± 0.04) days, which were less or shorter than those of MRI technique [(597.23 ± 11.02) yuan, (1.02 ± 0.10) days, t = 213.126, 51.210, both P < 0.05]. Conclusion:Omniview-VCI technique can replace MRI and accurately measure the posterior angle of fetal near-field lateral ventricle. It provides a more simple, fast and effective method for evaluating fetal near-field lateral ventricle, and can become a conventional application technique.

Objective To explore the management of human genetic resources in our university,propose relevant suggestions for promoting the appropriate protection and scientific management of human genetic resources.Methods Based on the existing regulations and policies,discuss the potential problems relevant to how to strengthen management of human genetic resources in China at institute level.Results Main problems identified including the management scope of human genetic resources is not clear,the inconsistence of application materials,the informed consent and its process involved in biobank and lack of the management of whole process of human genetic resources exploration.Conclusions With the rapid development of biotechnology,the economic value and strategic significance of human genetic resources have become increasingly prominent.Enhancement of training and whole process management,as well as the improvement of informed consent for biobank play important roles in effectively improve the protection of human genetic resources.

Objective: To investigate the effects of silencing zinc finger E-box binding homeoboxl (ZEB1) gene on the expressions of mesenchymal markers and cell migration in the glioma U87 cells, and to clarify the effect of ZEB1 on the epithelial to mesenchymal transition (EMT) in the glioma cells. Methods: The constructed ZEB1 shRNA interfering plasmid and control plasmid (shCtrl) were transfected into the glioma U87 cells and the interfering effects were detected by Western blotting method. The glioma U87 cells were divided into control group (the glioma U87 cells were transfected with shCtrl), EMT group (EMT was induced by TGF-fil in the glioma U87 cells transfected with shCtrl) and ZEB1 silence group (EMT was induced by TGF-fil in the glioma U87 cells transfected with ZEB1 shRNAs plasmid). The protein expression levels of mesenchymal markers (N-cadherin, Vimentin), and matrix metalloproteinase-9 (MMP-9) in the glioma U87 cells were measured by Western blotting method. The scratch-healing assay was performed to examine the migration ability of glioma cells. Results: The Western blotting results showed that the expression levels of ZEB1 in the glioma U87 cells transfected with shZEBl # 1 and shZEBl # 2 were significantly lower than that in the cells transfected with shCtrl (P<0. 05 or P< 0. 01), and the inhibitory effect of shZEBl #2 on the ZEB1 expression was more obvious, indicating that ZEB1 was stably transfected into the U87 cells. Compared with control group, the expression levels of mesenchymal markers N-cadherin, Vimentin, and MMP-9 in EMT group were significantly increased (P<0. 05 or P<0. 01). Compared with EMT group, the expression levels of the above proteins in ZEB1 silencing group were markedly reduced (P< 0. 05 or P<0. 01). The cell migration rate in EMT group was obviously elevated compared with control group (P< 0. 01), and the cell migration rate of the glioma U87 cells in ZEB1 silence group was significantly lower than that in EMT group (P<0. 01). Conclusion: Silencing ZEB1 gene expression can inhibit the EMT in the glioma U87 cells and reduce the cell migration abilities, suggesting ZEB1 as an important therapeutic target of invasive glioma.

Objective Summarize and share the working practice of Investigator Research Ethics and Research Integrity Training conducted at Peking University Human Research Protection Program (PKU HRPP) ,to further explore continuing quality improvement of investigator ethical training at university level .Methods Conduct systematic review of the archiving files of PKU HRPP investigator training activities during 2012-2018 ,summarize feedback information from investigators to i-dentify possible experiences for sharing and space for improvement .Results There are some positive experiences for sharing a-bout the training mechanism and practices at PKU HRPP .Conclusions Based on the previous ethical training work and experi-ences at PKU HRPP ,possible proposals for continuing quality improvement may including :strengthening the requirements of ethical training of investigators ,encouraging and recognizing ethical training conducted by research teams ,emphasizing training Quality and the promotion of sharing and mutual recognition mechanisms for ethical training .

Objective: To investigate the stem cell-like properties of glioma U87 cells undergoing epithelial to mesenchymal transition (EMT), and to clarify the inhibitory effect of resveratrol (Res) on their stem cell-like properties. Methods; The glioma U87 cells were treated with transforming growth factor-β1 (TGF-β1) to induce EMT, and the morphology of U87 cells was observed under inverted microscope. Western blotting method was used to detect the expression levels of mesenchymal markers (N-cadherin, Vimentin) and EMT-inducing transcription factors (Snail, Slug, Zebl, Twistl). After the EMT of glioma cells was confirmed, the experiment was divided into control group (U87 cells didn't receive any treatment), EMT group (the glioma cells presented EMT induced by TGF-β1) and Res treatment group (the glioma cells with EMT were treated with Res). The number of secondgeneration gliospheres and the expression levels of stem cell markers (Bmil and Sox2) in glioma U87 cells in various groups were detected. Results: The glioma U87 cells treated with TGF-β1 for 48 h showed dispersed, elongated, and similar to the appearance of fibroblasts. The expression levels of N-cadherin, Vimentin, Snail, Slug, Zebl and Twistl in glioma cells in different concentrations of TGF-β1 groups were significantly increased, suggesting the presence of EMT in glioma U87 cells. Compared with control group, the number of second-generation gliospheres in the U87 cells in EMT group was increased (P<0. 01), and the expression levels of stem cell markers (Bmil and Sox2) in the glioma U87 cells in EMT group were markedly increased (P<0. 05 or P<0. 01). Compared with EMT group, the number of second-generation gliospheres in the glioma U87 cells in Res treatment group was significantly decreased (P<0. 01), and the expression levels of Bmil and Sox2 in the glioma U87 cells in Res treatment group were decreased (P < 0. 01). Conclusion: The glioma U87 cells undergoing EMT exhibit the stem cell-like properties, and Res can inhibit their stem-like properties.

Methods: The glioma U87 cells were divided into control group (no treatment), EMT group (EMT was induced with TGF-β1) and Res treatment group (EMT was treated with Res). The expression levels of mesenchymal markers (N-cadherin, β-catenin, Vimentin), EMT-inducing transcription factors (Snail, Slug, Twistl), matrix metalloproteinase (MMP)-2 and MMP-9 in the glioma U87 cells were measured by Western blotting method. Scratch-healing assay and Trans well assay were performed to examine the cell migration and invasion abilities. Results: Compared with control group, the protein expression levels of mesenchymal markers (N-cadherin, β-catenin, Vimentin), EMT-inducing transcription factors (Snail, Slug, Twistl), and MMP-2 and MMP-9 in EMT group were significantly increased (P<0. 05). Compared with EMT group, the expression levels of above proteins in Res treatment group were markedly down-regulated (P<0. 05 or P <0.01); the effect in 40 μmol · L1 Res treatment group was more obvious than that in 20 μmol · L-1 Res treatment group. The migration and invasion rates of the U87 cells in EMT group were obviously elevated compared with control group (P<0. 01), and the migration and invasion rates of the glioma U87 cells in Res treatment group were significantly lower than those in EMT group (P<0. 01). Conclusion: Res can inhibit EMT of the glioma U87 cells and reduce the migration and invasion abilities of the cells.

The operating room is the main place of surgery,but there exist many ethical problems when developing quality nursing service in operating rooms.These problems mainly related to the imperfect preoperative and postoperative visit systems,the lack of effective communication and protection of patients privacy during the operations,and the lack of self-discipline spirit.To solve these problems,we should improve the preoperative and postoperative visit system,strengthen the psychological counseling for patients,cultivate the nurses' self-discipline spirit,respect and protect the privacy of patients,so as to improve the quality of nursing service in the operation rooms,and better service for patients.

Objective To evaluate the efficacy of ultrasound-guided continuous brachial plexus block for analgesia after fixation of upper extremity fractures in the pediatric patients.Methods Sixty American Society of Anesthesiologists physical status Ⅰ pediatric patients of both sexes,aged 3-10 yr,weighing 13-46 kg,with body height of 97-152 cm,scheduled for open reduction and internal fixation of distal humeral fractures,were selected and divided into 2 groups (n =30 each) using a random number table:intravenous analgesia group (group Ⅴ) and continuous brachial plexus block group (group B).Surgery was completed under brachial plexus block combined with general anesthesia in the two groups.At the end of surgery,patient-controlled intravenous analgesia was used in group Ⅴ,and continuous brachial plexus block was performed with 0.1% ropivacaine 250 ml (background infusion 0.1 ml · kg-1 · h-1,bolus dose 0.2 ml/kg,lockout interval 30 min) in group B.Tramadol was given as rescue analgesic when necessary.Ramsay sedation scores were assessed and recorded at 2,4,8,12,24,36 and 48 h after surgery,and the development of over-sedation was recorded.The require,nent for rescue analgesics and development of respiratory depression,dizziness,nausea and vomiting,pruritus and urinary retention during analgesia were recorded.The development of adverse reactions such as vascular and nerve injury,local hematoma and pneumothorax was recorded in group B.Family's satisfaction with analgesia was assessed and scored.Results Compared with group Ⅴ,Ramsay sedation scores at 2-12 h after surgery,the incidence of oversedation,nausea and dizziness and requirement for tramadol were significantly decreased (P<0.05),and no significant change was found in the degree of family's satisfaction with analgesia in group B (P> 0.05).The adverse reactions such as vascular and nerve injury,local hematoma or pneumothorax were not found in group B.Conclusion Ultrasound-guided continuous brachial plexus block can be safely and effectively used for analgesia after fixation of upper extremity fractures in the pediatric patients.

Objective To explore nursing care of the ex-utero intrapartum treatment (EXIT) in fetal neck mass oppressing airway. Methods A case of EXIT in which a fetal neck mass was diagnosed on ultrasound was fully prepared supplies and carried out. Nursing care was well implemented and vital signs of the puerpera and fetus were observed in preoperative and intraoperative surgery. Results The EXIT procedure was successful performed and vital signs of the puerpera and fetus were steadied. The score of Apgar was seven to eight. Conclusions Adequate equipment, multi-department collaboration, preoperative exercise and close cooperation are the important guarantee for the success of ex-utero intrapartum treatment.

Objective To observe the high-flow nasal cannulae can reduce indoor postoperative intensive care patients with tracheal intubation in offline acute respiratory failure after extubation reintubation rates. Methods 53 cases of postoperative acute hypoxia type patients with respiratory failurein the ICU in offline after extubation were divided into two groups, control group of 24 patients, 29 cases of observation group patients. Control group patients in the event of a failure after using non-invasive mechanical ventilation (NIMV) and observation group of patients using HFNC. All other patients with same treatment and nursing. Compare two groups of patients reintubation rates. Results Observation group of reintubation rate was 20.69%(6/29), the control group was 45.83%(11/24), reintubation rate difference of two groups of patients were statistically significant (χ2=3.81, P < 0.05). Conclusions HFNC can reduce postoperative extubation after weaning reintubation rates in patients with acute respiratory failure.

OBJECTIVE:To establish the quality standard for Yiwei granule. METHODS:TLC was conducted for the qualitative identification of Coptis chinensis,Corydalis yanhusuo and Schisandra chinensis;HPLC was conduced for the content determination of paeoniflorin. The column was XDB C18 with mobile phase of methanol-water(25:75,V/V)at flow rate of 1.0 ml/min,detection wave-length was 230 nm and injection volume was 20μl. RESULTS:The TLC showed clear spots and good separation. The linear range of paeoniflorin was 7.575-60.6 μg/ml(r=0.999 9);RSDs of precision,stability and reproducibility tests were lower than 2.0%,recov-ery was 96.97%-102.51%(RSD=1.92%,n=9). CONCLUSIONS:The method is simple and reproducible,and can be used for the quality control of Yiwei granule.

Objective:To explore the effective way of cultivating the operating room nurses′humanistic quality by using the morning shift and to improve operating room nurses′comprehensive quality .Methods:The operating room nurses were divided into five specialist groups according to the nurses′levels, helping each group to determine the training topic of humanistic quality training , via taking a variety of training methods to promote operating room nurses′humanistic quality by making good use of the morning shift time .Results: Through taking a variety of training methods to promote the operating room nurses′humanistic quality by making good use of the morning shift time, the operating room nurses′humanistic care ability , theoretical knowledge , professional skills were improved significantly(P<0.05),and the operating rooms doctors, surgical patients′satisfaction of operating room nurses′service were improved ( P<0 .05 ) .Conclusion:Making good use of the morning shift time to improve the nurses′humanistic quality , at the same time , the nursing service quality and patients satisfaction were also improved .

Objective To investigate the expression of blood CD +4 CD +25 Treg GITR,CD +4 T cell GITRL in children with asthma,and the role of them in asthmatic inflammation.Methods 50 cases of severe asthma were selected,and were controlled with thirty two healthy children.The venous blood was collected both in the period of acute episode and clinic remission.The mean fluorescence intensity of CD +4 CD +25 Treg GITR and CD +4 T cell GITRL was detected by flow cytometry.Results The expression of CD +4 CD +25 Treg GITR in the asthma acute period group was (24.2 ±8.2)MFI,which was significantly lower than (28.5 ±6.0)MFI in the control group(t =2.5,P 0.05).Moreover,the expression of CD +4 CD +25 Treg GITR in the asthma in remission group after treatment was (29.5 ±8.3)MFI,which was significantly higher than that in acute period group before treatment(t =-9.9,P 0.05).Furthermore,there was no significant correlation between levels of CD +4 CD +25 Treg GITR and CD +4 T cell GITRL.Conclusion The level of CD +4 CD +25 Treg GITR in acute period asthmatic patients was decreased,but it was increased in remission,but no changes of CD +4 T cell GITRL expression were observed.GITR/GITRL signal system might be involved in the asthmatic inflammation procession.

Objective To evaluate effect of preventing ventilator associated pneumoia (VAP) of patients with oral cavity endotracheal intubation by air impulse clearing away secretion drainage. Methods 348 patients with oral trachea cannula and mechanical ventilation (MV) in intensive care unit (ICU) were divided into control group (n=174) and observational group (n=174). The control group adopted conventional airway management methods without the subglottic secretion drainage while the observational group adopted conventional airway management methods on the basis of combining air impact method remove stranded on airbags. Other treatment and care for all of patients were basically the same. The MV time, duration in ICU and incident rate on VAP were compared between the two groups. Results The average of MV time in control group was (5.25±1.18) days, (5.62±3.20) days in ICU and the rate of VAP was 10.34%(18/174). In observational, it was (8.96 ±5.43) days, (10.43 ±4.96 ) days and 34.48%(60/174), respectively. The MV time, duration in ICU and incident rate on VAP were significantly different (P<0.05), with statistical significance. Conclusions The air impact method to remove airbags retentate could shorten time of MV and during time in ICU, and could reduce the rate of VAP.
