Posterior sagittal anorectoplasty for anorectal anomalies

In the last 5 years, 51 patients, [34 females and 17 males] underwent posterior sagittal anorectoplasty [PSARP] as a definitive procedure for anorectal malformations. Their ages ranged between 5 months and 8 years. Thirty-seven patients had an initial left iliac colostomy; of these, three had a cloacal anomaly. Fourteen females with rectovestibular anus had a one-stage PSARP. The rectal pouch was too high to be delivered through a sacral approach in only two patients where an abdominosacral pull-through was performed. Postoperative complications were minimal. Oral feeding was started promptly. Four patients [8%] had superficial wound sepsis and two had recurrent fistulae [4%] Five patients [9.8%] had a tight anal ring that responded to dilatation in four but the last one needed refashioning. Continence was complete in all cases with rectovestibular anus [14 cases]. Among the 37 cases with high anomalies, 32 closed their colostomies. Of the latter, 26 patients were continent [81%]. The remaining five cases were assessed and three had occasional soiling, whereas the remaining two patients were under the age of 3 years and it was still too early to make a judgement about continence. One patient died after closure of the colostomy
