RESUMEN

Sixty nine patients with fracture femur constituted the material for a prospective randomized trial over a two-year period, done in Ismailia General Hospital to study the relation between early versus delayed stabilization of fracture femur on the development of pulmonary complications. The patients were randomly assigned to four groups according to the timing of stabilization [48 hours were taken as a cut off point] and presence or absence of multiple injuries. Measures for outcome were the development of pulmonary complications and hospital stay. It was found that the incidence of pulmonary complications was higher and hospital stay was longer when stabilization of the fractures was delayed, as compared with those who had early stabilization

RESUMEN

Thirteen patients [15 hips] with slipped capital femoral epiphysis were treated by in situ fixation. In 7 patients three knowles pins [4 mm diameter] were used, in 6 patients a single cancellous short threaded [6.5 mm diameter] screw was used for stabilization of the slipped femoral epiphysis. The disease occurred more in males with a ratio of 2.3: 1, and the age at onset of the disease ranged in males from 11 to 17 years and in females from 10 to 14 years. In 3 cases the condition was acute, 4 cases acute on chronic and in 6 cases the slipping was chronic. The results of treatment were 88.9% excellent and good with multiple pinning, and 100% excellent and good in the group treated with a single screw stabilization. No change occurred in the head shaft angles measured immediately postoperatively and during the follow-up period. Epiphyseal fusion occurred in 86.7% of cases [88.9% in multiple pinning and 83.3% in single screw stabilization]. A complication in form of chondrolysis occurred in one patient of the multiple pinning group. It was concluded that a single screw fixation has a success rate that is comparable with that of multiple pinning and is safer and simple
