RESUMEN

Mechanical ventilation is used for some infants in neonatal intensive care units [NICU] due to many physiological and clinical causes. Since these patients have endotracheal tubes, cleaning and keeping the airways open through suctioning should be done to increase oxygenation. This study aimed to evaluate effect of open and closed suctioning methods on respiratory parameters of infants undergoing mechanical ventilation. In this crossover clinical trial, 44 infants were selected among those undergone mechanical ventilation in NICU of Isfahan's Al-Zahra Hospital using convenience sampling method. The subjects were randomly divided into two groups. In the first group, open suctioning was carried out and after three hours of cleaning, closed suctioning was done. In the second group, closed suctioning was firstly done and following three hours of cleaning, open suctioning was implemented. Respiratory rate [RR] and percentage of arterial blood oxygen saturation was measured before, during and after each type of suctioning. Data were analyzed using repeated measures ANOVA and independent student's t-test. There was a significant difference between mean respiratory rate and arterial blood oxygen saturation in infants before, during and after the closed and open suctioning. The percentage of arterial blood oxygen saturation had a significant reduction in open method compared to closed method during suctioning and immediately after it. RR three minutes after suctioning showed a significant reduction in both steps in open method compared to closed method. Close method caused fewer changes in hemodynamic status of infants. Therefore, in order to prevent respiratory complications in infants, nurses are recommended to perform the endotracheal tube suctioning by closed method.

RESUMEN

Acute myeloid leukemia [AML], the most common form of acute leukemia, is treated by remission induction and post-remission therapy. Remission induction is usually achieved by administration of cytarabine along with an anthracycline such as Daunorubicin [DAU] or Idarubicin [IDA]. Our objective was see the benefits if any of IDA over DAU in AML therapy. Eighty adult AML patients were enrolled in this study, where 40 received DAU and 40 were treated with IDA. Remission status in each subject was studied and response to therapy was subsequently analyzed using SPSS. Complete remission, partial remission and no responsive status were 15, 19, and 14 respectively for patients on DAU and 14, 18, and 11 for patients on IDA protocol. No significant benefit was detected for IDA compared to DAU in response to therapy. We found no benefit in using IDA over DAU in induction therapy for AML patients treated in northwest of Iran

RESUMEN

Introduction: Teenage period is the most important and susceptible stage of human growth and development. Teenagers get responsible for taking care of their parents but can be disturbed by chronic disease. These patients are very hospital dependent and need various types of treatment and this puts them in more danger than the other teenagers at their age. Therefore, investigation of physical experiences of these teenagers can be applied in providing them with a better care by nurses


Methods: This is a qualitative phenomenological survey. The population studied was composed of teenagers with major thalassemia referring to Imam Reza clinic in Isfahan Sayed Shohada hospital. The data were collected during two months by deep interview. The data were analyzed by Collaizzi seven-stage method


Results: The findings of the research were classified in 46 codes and three categories as: 1] physical effects related to disease process, 2] physical effects related to cure process, 3] physical effects related to complications


Discussion: The thalassemic teenagers tolerate a lot of physical problems that can be treated to make them more comfortable with a better care
