RESUMEN

Purpose@#This study was to evaluate several tasks performed at a high intensity in terms of their ability to elicit EMG activity in the serratus anterior by comparing the EMG activities of the serratus anterior, upper trapezius, and lower trapezius muscles during six tasks combined shoulder flexion with rotation. @*Methods@#Fifteen healthy males were recruited to this study. Each subject was instructed to assume a sitting position without back support and asked to flex (90° or 120°) the right shoulder and protract the scapula in the sagittal plane with maximal external rotation; to assume a neutral position; or to internally rotate the glenohumeral joint. The EMG data were collected from the serratus anterior (SA), upper trapezius (UT), and lower trapezius (LT) muscles were normalized to maximum voluntary isometric contraction. The UT/LT and UT/SA muscle activity ratios in each task were assessed by calculating the surface EMG. Data were analyzed by two-way repeated-measures analysis of variance, with the level of significance set at p<0.05. @*Results@#The results of this study, shoulder flexion with external rotation resulted in low upper trapezius/serratus anterior and upper trapezius/lower trapezius ratios and a relatively high level of serratus anterior activation. @*Conclusion@#Shoulder flexion with external rotation used herein may be considered as important for clinical interventions aimed at selectively increasing SA strengthen and clinical selection of exercises for improving glenohumeral joint and scapulothoracic control.

RESUMEN

One of the predominant methods for preserving the internal iliac artery (IIA) in aortoiliac aneurysms is to use an iliac bifurcated device (IBD). However, there are a few limitations to the use of IBD in Korea. Our study aims to present the technical aspects of these devices, and to provide a mid-term analysis of IBD. Since 2013, 4 IBDs were used in 4 patients, with a mean follow-up of 14 months (range, 6-22 months). A 100% technical success rate was achieved without mortality and morbidity in our cases. The mean procedural time was 176 min (range, 145-240 min), and the mean contrast dose used was 184 mL (range, 135-220 mL). Type I or III endoleaks and postoperative expansion of the aneurysms weren't observed. Our cases demonstrates that IBD is a relatively safe repair method of aortoiliac aneurysms with preservation of the IIAs. However, a longer follow-up is needed to review the midterm results.

RESUMEN

Heart failure is the most common cause of death of infective endocarditis. The contributing factors of heart failure include valve destruction, myocarditis, coronary artery emboli with myocardial infarction and abscess. Recently, we experienced a thiry nine year-old man who was hospitalized at Inha University Hospital because of fever, chill and dyspnea (NYHA functional class I-II). The grade IV/Vi systolic murmur was heard at the right upper sternal border and the apex and the grade III/VI diastolic murmur was heard at Erb's area. No crackles were heard. Blook cultures grew Streptococcus viridans. Chest X-ray showed mild cardiomegaly without pulmonary congestion sign. Echocardiogram showed aortic valve vegetations, abscess and grade II/IV aortic regurgitation. Aortic valve replacement and abscess removal were performed. Findings included henegg sized abscess which reduced 70% of cross sectional area of left ventricular outflow tract and located between posterior wall of left ventricle and right and left coronary rings.

A 5 year old girl was visited with the chief complaint of abnormal genitalia from birth. Physical examination revealed enlarged clitoris, urogenital sinus formation, increased urinary 17-ketosteroid, Jailer's test positive and sex chromatin positive. Diagnosis was made congenital adrenogenital syndrome.
