Lichen simplex chronicus on the scalp: exuberant clinical, dermoscopic, and histopathological findings

Abstract: Lichen simplex chronicus is a skin disease that mostly affects female patients, with a peak incidence between ages 35 and 50 years. On the scalp, it appears as a single or multiple oval lesions, showing scaling and hair shaft loss or breakage. An important dermoscopic feature of the disease are the "broom fibers." Histopathology reveals the "hamburger" and the "gear wheel" signs. The aim of this report is to demonstrate a case of lichen simplex chronicus on the scalp with typical and exuberant clinical, dermoscopic, and histopathological findings.

Granuloma annulare treated with narrowband UVB phototherapy

Abstract Granuloma annulare is a benign cutaneous inflammatory disease, whose lesions have spontaneous improvement in two years in 50% of cases, but there is recurrence in 40% of patients. Treatment may be topical, intralesional or systemic. The use of phototherapy with narrowband UVB is highlighted, whose mechanism of action in this disease is still unclear, probably related to the inhibition of T lymphocytes. Herein, a case of a disseminated granuloma annulare of difficult therapeutic management is described. It was treated with narrowband UVB phototherapy twice a week for six months, with good clinical improvement, being a good low-risk therapeutic option and that, in this case, provided quick and satisfactory response.

Nevo sebáceo de Jadassohn no couro cabeludo - reconstrução com retalho de rotação bilateral / Nevus sebaceous of Jadassohn of the scalp - reconstruction with bilateral rotational flap

O nevo sebáceo de Jadassohn é hamartoma benigno congênito da pele, mais incidente no couro cabeludo, podendo apresentar-se na face. Devido ao potencial de malignização, sua excisão muitas vezes se faz necessária. Lesões situadas no couro cabeludo constituem desafio ao cirurgião devido às características anatômicas do local, com escassez e inelasticidade da pele adjacente. Neste relato, apresentamos um caso de excisão de nevo sebáceo de Jadassohn no couro cabeludo, com fechamento da ferida operatória por retalho de rotação bilateral, técnica muito simples e versátil, excelente para topografias de difícil reconstrução, como o couro cabeludo.

The nevus sebaceous of Jadassohn is a congenital benign hamartoma of the skin that occurs more frequently in the scalp, but can also arise in the face. Due to its potential for malignization, exeresis is often required. Lesions located in the scalp are challenging to surgeons because of the anatomical characteristics of this body site, namely scarcity and inelasticity of the adjacent skin. In the present report, the authors describe a case of excision of a nevus sebaceous of Jadassohn located in the scalp, using a bilateral rotation flap ­ a very straightforward and versatile technique, excellent for reconstructing difficult topographies such as the scalp ­ for the closure of the surgical wound.
