RESUMEN

Purpose@#Melioidosis is a zoonosis that can infect humans or animals. The disease is caused by the bacteria Burkholderia pseudomallei, which is found in contaminated soil and water. We investigated the epidemiological aspects of melioidosis cases among oversea travelers in Korea and Japan during 2011 to 2020. @*Methods@#Raw data were obtained from the website of melioidosis cases from the Korea Diseases Control and Prevention Agency, and the National Institute of infectiousDiseases in Japan, 2011−2020. @*Results@#There were 26 cases of melioidosis cases in Korea and 14 cases in Japan between 2011 and 2020. Cumulative incidence rate per 1,000,000 oversea travelers (OTs) of Korea (0.14) did not substantially differ that of Japan (0.09), respectively.The incidence of melioidosis in males (96.2% of total 26 cases) was much more common than in females (3.8%) in OTs of Korea (P<0.01), while there were significant differences level between males (85.7% of total 14 cases) and females (14.3%) in OTs of Japan (P<0.01). On the other hand, the distribution by adjusted-age groups for melioidosis cases were statistically similar distribution between Korean and Japanese that total cases occurred in the over 40-years old age, clearly showing a more infected of melioidosis (P<0.05). @*Conclusion@#This study demonstrates that there is a similar pattern of imported melioidosis cases in Korea and Japan. Therefore, to prevent melioidosis infections, greater attention should be paid to individuals who are planning to travel to the presumptive regions of melioidosis.

Purpose@#Zika virus (ZIKV) infection is a mosquito-borne Flavivirus disease. we investigated the epidemiological aspects and status of imported ZIKV infection cases in Korea (Republic of) and Japan during 2016–2020. @*Methods@#Raw data of ZIKV infection cases from the Korea Diseases Control and Prevention Agency, and the National Institute of infectious Diseases in Japan, 2016– 2020. @*Results@#There were 33 cases of ZIKV infection in Korea and 21 cases in Japan between 2016 and 2020. Cumulative incidence rate per 1,000,000 overseas travelers (OTs) of Korea (0.30) did not substantially differ that of Japan (0.27), respectively.We observed the ZIKV infected cases of male (69.7% of total cases) were much more than that for female (30.3%) in OTs of Korea (P<0.05). However, while there was none significant differences level between males (52.4%) and females (47.6%) in OTs of Japan. On the other hands, the distribution by adjusted-age groups for ZIKV infections were similar between Korean and Japanese that total cases occurred in the over 20 to 59-years old age, clearly more showing a higher infected of ZIKV (P<0.01). The presumptive origin imported ZIKV infection cases in Korean were as follows; Southeast Asia (81.8% of total cases) and Central & South Americas (18.2%), and those in Japan, these were Southeast Asia (42.9%) and Central & South Americas (42.9%) and Oceania (4.7%), respectively. @*Conclusion@#This study demonstrates that there is a similar pattern of imported ZIKV disease cases in Korea and Japan. Therefore, to prevent ZIKV infections, greater attention should be paid to individuals who are planning to travel to the presumptive regions of ZIKV.

Purpose@#The purpose of this paper is to study the epidemiologic series of Tsutsugamushi disease (TD), also known as scrub typhus in the Republic of Korea (Korea) and Japan. @*Methods@#We investigated raw data of TD outbreaks from the National Notifiable Disease Surveillance System of the Korea Diseases Control and Prevention Agency in Korea, and the National Institute of Infectious Diseases in Japan, 2016 to 2020. @*Results@#There were 36,785 cases of TD with a cumulative infectious rate (CIR) of 14.3 per 100,000 populations in Korea from 2016 to 2020. During the same period in Japan, there were 2,350 cases with a CIR of 0.4. When compared, Korea was much higher than that in Japan (P<0.01). In Korea, more females (60.2% of total cases) were infected than males (39.8%), while there was a significant difference between males (58.8%) and females (41.2%) in Japan, respectively (P<0.01). @*Conclusion@#These differences in TD risk factors reflect differences of vectors/hosts, climate, and geographical and cultural characteristics between the two countries. The surveillance of TD, primarily a zoonotic disease, should be continued in order to obtain a better understanding of its current status. Moreover, guidelines for the prevention of TD, and its control measures should be more established.

Purpose@#Hepatitis E outbreaks are a serious public health concern in the world. The pilots or air traffic controllers can be incapacitated by severe hepatitis symptoms. If the symptoms of acute hepatitis do not improve, it is not suitable for aviation duties.In this study, we compare of incidence status of hepatitis E virus (HEV) infections, and epidemic aspects between South Korea (from here on, ‘Korea’) and Japan. @*Methods@#The raw data on confirmed of HEV infection cases were obtained from the National Notified Disease Surveillance System of Korea Diseases Control and Prevention Agency, the Ministry of Health and Welfare in Korea, and the National Epidemiological surveillance of Infectious Diseases surveillance system, administered by the National Institute of Infectious Diseases in Japan. Statistically significant differences between the epidemiological aspects and risk factors were determined using the Pearson’s chi-squared test or paired t-test. All data analyses were performed in Microsoft Excel 2010 (Microsoft, Redmond, WA, USA). @*Results@#There were 191 cases of HEV infected cases with a prevalence rate (PR) of 0.37 per 100,000 populations of Korea in 2020. During the same year in Japan, there were 454 cases with a PR of 0.36. When compared, there is statistically none significantly differences. Moreover, both PR per 100,000 populations of HEV infections in habitat of two countries were compared, in the provinces (0.39) of Korea were much higher than that in capital city of Seoul (0.27; P<0.01), but that in Japan were capital city of Tokyo (0.84) was higher than that of the provinces (0.30; P<0.01). Male to female morbidity ratio was 1.65 and 3.78 in Korea and Japan, respectively. The distribution by the age adjusted groups were similar in Korea and Japan that the total cases occurred in the over 40-years-old age bracket, clearly showing a higher incidence in the elderly. @*Conclusion@#We believed that the HEV are a zoonotic virus, and human occurs mainly through the fecal contamination of water and consumption of contaminated meat from infected animals. In addition, HEV infection can be a problem for pilots and air traffic controllers as well, requiring further investigation and research.

Background@#Obesity is a major public health problem that is causally related to serious medical conditions. In this study, the public health implications of obesitybased on body mass index (BMI) with anthropometric measures among adults in theRepublic of Korea and Japan in 2019 were compared. @*Methods@#A simple cross-section, nationally representative of the raw data from the National Health and Nutrition Examination Survey in 2019 between Korea and Japanwere used. We analyzed the data of those categorized as obese with a BMI of ≥25 kg/m 2 according to the World Health Organization Expert Consultation. @*Results@#The average values of BMI based on body height and body weight amongmales and females in Korea were 24.6±0.08 and 23.3±0.09, respectively; those in Japan were 23.9±0.08 and 22.6±0.08, respectively. The measured anthropometric values were obviously higher in Korean than in Japan (P<0.01). The prevalence rates(PRs) of obesity by sex in Korea were 41.4% among males and 27.3% females; thosein Japan were 32.5% among males and 22.0% among females. The statistically significant results showed that the total obesity rate was higher in males than in females in both countries (P<0.01). The PRs of obesity in Korea were 41.4% in malesand 27.3% in females; those in Japan were 32.5% in males and 22.0% in females. Thestatistically significantly PR of total obesity among Korean adults was greater than that among Japanese adults (P<0.01). @*Conclusion@#The PR of obesity in Korea relative to that in Japan showed a gradually increasing trend. Obesity is a major problem, especially in the pilot group. Reducing the prevalence of obesity among pilots is important for reducing in-flight medical incapacitation and ensuring flight safety. Obesity management is necessary to prevent obesity-related diseases and promote pilot health.

BACKGROUND/AIMS: The prevalence of hepatitis C virus (HCV) infection in Busan, Gyeongnam, and Jeonnam Provinces in Korea is more than twice the national average. This study aimed to examine whether demographic and lifestyle characteristics are associated with HCV infection in these areas. METHODS: A case control study was performed at three study hospitals. HCV cases were matched with two controls for sex and age. Patient controls were selected from non-HCV patients at the same hospital. Healthy controls were subjects participating in medical checkups. Conditional logistic regression models were used. RESULTS: A total of 234 matched-case and patient- and healthy-control pairs were analyzed. The significant risk factors for both controls were sharing razors (adjusted odds ratio [aOR], 2.39 and 3.29, respectively) and having more than four lifetime sexual partners (aOR, 2.15 and 6.89, respectively). Contact dockworkers (aOR, 1.91) and tattoos (aOR, 2.20) were significant risk factors for the patient controls. Transfusion (aOR, 5.38), a bloody operation (aOR, 5.02), acupuncture (aOR, 2.08), and piercing (aOR, 5.95) were significant risk factors for the healthy controls. Needle stick injuries and intravenous drug abuse were significant in the univariate analysis. CONCLUSIONS: More education concerning the dangers of sharing razors, tattoos and piercings is required to prevent HCV infection. More attention should be paid to needle stick injuries in hospitals and the community.

BACKGROUND/AIMS: This study compared the prevalence of hepatitis C virus (HCV) infection in the Republic of Korea and estimated the high-risk regions and towns. METHODS: National Health Insurance Service data for 8 years from 2005 to 2012 were used. The subjects of the study had visited medical facilities and been diagnosed with or received treatment for acute or chronic HCV as a primary or secondary disease according to ICD-10 codes of B17.1 or B18.2, respectively. Any patient who received treatment for the same disease multiple times during 1 year was counted as one patient in that year. To correct for the effect of the age structure of the population by year and region, the age-adjusted prevalence was calculated using the direct method based on the registered population in 2010. RESULTS: The overall prevalence of HCV infection among Korean adults (>20 years old) increased from 0.14% in 2005 to 0.18% in 2012. The sex-, age-, and region-adjusted prevalence in 2012 was 0.18%. The prevalence was highest in Busan, Jeonnam, and Gyeongnam, and there were towns with noticeably higher prevalences within these regions: Jindo (0.97%) in Jeonnam, Namhae (0.90%) in Gyeongnam, and Seo-gu (0.86%) in Busan. CONCLUSIONS: The prevalence of HCV infection differs by regions as well as towns in the Republic of Korea, and is highest in Busan, Jeonnam, and Gyeongnam. The reasons for the high prevalence in these specific regions should be identified, since this could help prevent HCV infections in the future. In addition, active surveillance and treatment policies should be introduced to stop any further spread of infection in these high-prevalence regions.

OBJECTIVES: To compare the self-rated health in chronic disease patients with depression, chronic disease patients, and depression patients, and to observe the related factors to the self-rated health of people age 65 and older. METHODS: The subjects were 2,549 elderly people, over 65 years old in Busan Metropolitan City who participated in 2009 community health survey. Association between self-rated health and general characteristics, life style and disease status were observed. Depressive symptoms were measured using the Centre for Epidemiologic Studies Depression Scale (CES-D). Analysis of complex sample was done with SAS (ver. 9.2), using chi2-test and multiple logistic regression. RESULTS: Among total 2,549, there were 740 normal people (29.8%), 50 people with depression (1.8%), 1,495 people with other chronic diseases (58.2%), and 264 people with the comobidity of depression and other chronic diseases (10.1%). Good self-related health accounted for 20.3% for the whole, 33.4% for normal, 16.7% for chronic disease, 16.1% for depression, and 3.2% for chronic diseases with depression. Disease, gender, education, income, alcohol drinking, regular exercise and regular walking were independent factors associated with the good self-related health. CONCLUSIONS: It is suggested that when the chronic disease control program for the elderly is developed, depression care should be considered along with the program. This program should be given priority to the women and the vulnerable classes and should also be related to the regular walking.

RESUMEN

The C677T mutation in the methylene tetrahydrofolate reductase (MTHFR) gene results in elevated homocysteine levels and, presumably, in increased cardiovascular risk. Moreover, elevated homocysteine levels are reportedly associated with high serum uric acid levels. We evaluated the MTHFR genotype and a panel of biochemical, hematological variables, and lifestyle characteristics in 327 elderly Korean men (age range 40-81 yr; mean, 51.87). This study shows that mutation of the MTHFR gene may be a risk for hyperuricemia. The mean uric acid levels for the C/C, C/T and T/T genotypes were 5.54, 5.91 and 6.33 mg/dL, respectively (p=0.000). The T/T genotype was significantly more frequent in subjects with high uric acid levels (p=0.003). Thus, this mutation of the MTHFR gene is implied by the study results to be a risk factor of hyperuricemia in elderly Korean men. However, the relationship between the MTHFR mutation and uric acid metabolism remains unclear. Therefore, further studies are necessary to explain the associated between the MTHFR mutation and elevated uric acid levels, and to examine potential relationships between it and conventional cardiovascular risk factors.

To evaluate the interactive effect of methylenetetrahydrofolate reductase (MTHFR) genotype and dietary factors on the development of breast cancer, a hospital based case-control study was conducted in South Korean study population consisting of 189 histologically confirmed incident breast cancer cases and their 189 age-matched controls without present or previous history of cancer. A PCR-RFLP method was used for the genotyping of MTHFR (C677T) and statistical evaluations were performed by unconditional logistic regression analysis. Consumption of some dietary factors, such as green vegetables (OR=0.3, 95% CI: 0.2-0.6), white vegetables (OR=0.3, 95% CI: 0.1-0.7) mushrooms (OR=0.4, 95% CI: 0.3-0.7), and meats (OR=1.7, 95% CI: 1.1-2.8) significantly decreased or increased the risk of breast cancer. Although the breast cancer risk was 1.7-fold (95% CI: 0.8-3.2) increased in women with MTHFR TT genotype, the association was not statistically significant. Women with MTHFR TT genotype and low green vegetable intake increased 5.6-fold (95% CI: 1.2-26.3) risk of breast cancer compared to high green vegetable intake group containing MTHFR CC/CT genotype. However, the interaction was not significant (p for interaction=0.96). Our findings suggest that MTHFR polymorphism did not influence individual susceptibility to breast cancer. However MTHFR (C667T) genotype and green vegetable intakes appeared to have the interactive effect in breast cancer development.

PURPOSE: After delivery, eating brown seaweed soup is a typical Korean tranditional habit. But, excessive intake of iodine transiently inhibits biosynthesis and secretion of thyroid hormones if a certain threshold of iodine is reached in the serum. The aim of our study was to demonstrate whether the observed the elevations of TSH level in the breast fed neonatal infants was caused by mother's eating brown seaweed soup or not. METHODS: We performed neonatal TSH test by enzyme linked immunosorbent assay(FRELISA Screenig TSH) and cheked TSH level for 178 newborns with a appropriate gestational age. We divided the study subject into 3 groups, the group used breast feeding, mother had taken brown seaweed soup and blood sampled at 6th day after birth was categorized Group A, formular feeding, blood sampled at the 4th days after birth was categorized Group B, breast feeding and had taken brown seaweed soup and blood sampled at the 19th day after birth was categorized Group C. RESULTS: 1) The mean TSH level was 1.5+/-1.3 uU/ml in Group A, 3.3+/-2.5uU/ml in Group B, 1.7+/-l.3uU/ml in Group C(Group A vs Group B: p<0.05, Group A vs Group C: p<0.05). 2) There was no statistical significances between the TSH levels and the amount of brown seaweed soup which mother had one day, the duration which mother had brown seaweed soup and duration of breast feeding. CONCLUSIONS: Mother had taken brown seaweed soup about 3 times a day in korea at present days, There was no significant changes of TSH levels in the beast fed newborn infants after the korean mother had taken brown seaweed soup.

RESUMEN

The purpose of this study was to describe growth changes of soft tissue profile in koreans with normal occlusion and to get differences between male and female. The biennial serial cephalometric radiographs of 26 samples (15 males, 11 females) with normal occlusion from 8.5 years to 18.5 years of age were used in this study. The following results were obtained : 1. Both sexes had lower facial parts more protruded at 18.5 years of age than 8.5 years of age : in total facial convexity angle, male showed a decrease(P0.05). 2. In the growth changes of soft tissue thickness, male outgrew female in the areas of nose and upper and lower lips, and female outgrew male in the pogonion area 3. In the growth changes of upper facial height and upper part of lower facial height, male's was higher than female's growth (P0.05). 5. Male showed a sustained change into older age compared with female in soft tissue profiles

It is. commonly assumed that nasorespiratory function can exert a dramatic effect upon the development of the dentofacial complex. Specially, it has been stated that chronic nasal obstruction leads to mouth breathing, which causes altered tongue and mandibular positions. If this occurs during a period of active growth, :the .outcome is development of the "adenoid facies". Such patients characteristically: manifest a vertically long lower third facial height, narrow alar bases, lip incompetence, a long and narrow maxillary arch and a greater than normal mandibular plane angle. But several authors have reported that so-called adenoid facies is not always associated with adenoids and mouth breathing, and that a particular type of dentition is not alwarys found in mouth breathers with or without adenoids. Some authors have believed adenoids lead to mouth breathing in cases with particular facial characteristics and types of dentition. We assumed that the ability to adapt to individual's neuromuscular complex is various. So, we compared the difference of influence of mouth breathing between childrens who have different facial types. This study included 60 patients and they were divided into three groups by Rickett's facial type. Their dentition and tongue position were compared. The results are as follows. 1. There is a significant difference in arch width of upper molars between different facial types. Especially dolichofacial type patients have narrowest arch width. 2. There is a significant difference in tongue position between different facial types. Especially dolichofacial type patients have lowest positioned tongue.

We obtained successful functional and esthetic results by grafting of iliac marrow-cancellous bone in 2 cases of alveolar-palatal cleft patients. Bone graft of alveolar-palatal clefts provide bony support to adjacent teeth of cleft area, prevented from relapse of orthodontic arch expansion, closure of oroantral fistula and improvement of speech problem. 1. In one case, extraction of upper right central incisor that was little bone support, alignment of rotated teeth and expansion of collapsed arch segment were done with pre-orthodontic treatment. The other case. Bone grafting was done after removal of prosthesis with no pre-orthodontic treatment. 2. After mucoperiosteal incision in cleft area, the mucosal flap of labial area, palate and nose were separation and the raised nasal mucosa was sutured for closure of oroantral fistula. Then, the iliac marrow-cancellous bones were grafted to cleft site. 3. After 6 months of operation, we had seen the new bone deposition to cleft site in dental radiograph and prosthetic treatment of missing teeth were done.

RESUMEN

This is a case report of correction of malunioned maxilla after traffic accident by Le Fort I osteotomy and posterior segmental osteotomy. By this procedure, authors obtained the following results. 1. The malunioned maxilla after traffic accident which had anterior crossbite, posterior open bite and scissor's bite were corrected by Le Fort 1 osteotomy and posterior segmental osteotomy. 2. No postoperative infection and specific complication were seen in this case. 3. Postoperative intermaxillary fixation was maintained for 8 weeks. And then, the patient could open his mouth in normal range after a week of intermaxillary fixation removal. 4. For rigid fixation and reducing relapse, the osteotomized maxilla was fixed with miniplates.
