RESUMEN

Low-viscosity composite resins may produce better sealed margins than stiffer compositions (Kemp-Scholte and Davidson, 1988; Crim, 1989). Flowable composites have been recommended for use in Class V cavities but it is also controversial because of its high rates of shrinkage. On the other hand, in the study comparing elastic moduli and leakage, the microfill had the least leakage (Rundle et al. 1997). Furthermore, in the 1996 survey of the Reality Editorial Team, microfills were the clear choice for abfraction lesions. The purpose of this study was to evaluate the microleakage of 6 compostite resins (2 hybrids, 2 microfills, and 2 flowable composites) with and without load cycling. Notch-shaped Class V cavities were prepared on buccal surface of 180 extracted human upper premolars on cementum margin. The teeth were randomly divided into non-load cycling group (group 1) and load cycling group (group 2) of 90 teeth each. The experimental teeth of each group were randomly divided into 6 subgroups of 15 samples. All preparations were etched, and Single bond was applied. Preparations were restored with the following materials (n=15): hybrid composite resin [Z250(3M Dental Products Inc. St.Paul, USA), Denfil(Vericom, Ahnyang, Korea)], microfill [Heliomolar RO(Vivadent, Schaan, Liechtenstein), Micronew(Bisco Inc. Schaumburg, IL, USA)], and flowable composite [AeliteFlo(Bisco Inc. Schaumburg, IL, USA), Revolution(Kerr Corp. Orange, CA, USA)]. Teeth of group 2 were subjected to occlusal load (100N for 50,000 cycles) using chewing simulator(MTS 858 Mini Bionix II system, MTS Systems Corp., Minn. USA). All samples were coated with nail polish 1mm short of the restoration, placed in 2% methylene blue for 24 hours, and sectioned with a diamond wheel. Enamel and dentin/cementum margins were analyzed for microleakage on a sclale of 0 (no leakage) to 3 (3/3 of wall). Results were statistically analyzed by Kruscal-Wallis One way analysis, Mann-Whitney U-test, and Student-Newmann-Keuls method. (p=0.05) RESULTS: 1. There was significantly less microleage in enamel margins than dentinal margins of all groups. (pA=H=M>D>Z. But there was no significant differences. 6. In enamel margins, load cycling did not affect the marginal microleakage in significant degree. 7. In dentin margins, load cycling did affect the marginal microleakage only in Revolution. (p<0.05)

RESUMEN

Antibiotic-acrylic bone cement composites demonstrated antibacterial activity as a result of diffusion of antibiotic from bone cement into surrounding enviroment, accordingly, the use of antibiotic incorporation to bone cement for the prevention and treatment of infection after joint surgery replacement has been adovocated. Early report of clinical success by using antibiotic-acrylic bone cement composites are encouraging. Our authors applied this concept to the treatment of chronic osteomyelitis. Antibiotic contained with bone cement was chosen by sensitivity test on pus culture finding. Seven cases of chronic osteomyelitia treated by using antibiotic-acrylic bone eement composites were .studied in the Department of Orthopaedic Surgery, Kyung Hee University Hospital from July 1979 to March 1981. The follow-up period was from 3 months tc 22 months. The results were as follow: 1. Six cases showed no evidence of recurrence except one by using antibiotic acrylic bone cement composites. 2. The impregantion of antibiotic in acrylic bone cement shall be regarded to be effective in increasing the local antibacterial activity by leaching the antibiotic into surrounding tissue over a period continiously in treatment of chronic osteomyelitis. 3. The use of antibiotic-acrylic bone cement compositea into dead space by saucerization are consid- ered an one such approach to the decrease of dead space. 4. The impregnation of antibiotic in acrylic bone cement is regarded a new concept in treatment of chronic osteomyelitis.

RESUMEN

The authors reviewed 73 cases of ligament injuries in 70 patients from October, 1972 to June, 1978 who were admitted and treated in the Orthopedic Department at Kyung Hee University Hospital. Of 70 patients, the sex distribution was 59 patients (84%) in male and 11 patients (16%) in female. The high incidence was in age group from 21 to 40 years, that was 46 patients (55%). Traffic accident was most common cause of ligament injuries that was 61 patients (87%). Methods of treatment were direct repair, reconstruction and conservative treatment. Degree of angle and distance were measured with stress radiogram. Degree of angle and distance were also compared with normal and abnormal knee joints. Eleven cases were treated conservatively, and 62 cases, operatively, of whlch direct repair performed in 56 cases and reconstructive surgery, in 6 casea, The summary were as follows: 1. The result was better in the group with conservative treatment, as compared with operative treatment. And conservative treatment was applied only in single ligament injuries. 2. In normal knee joint, degree of angle was distributed evenly in the range of 0° to 10°, and distance was in the range of 0 to 10 mm. 3. In stress radiograrn, the significance of ligament injuries was present in more than 10° and 10 mm. 4. The measurement of distance seemed to be more meaningful than degree of angle. 5. It is helpful to measure the degree of angle and distance in diagnosis of ligament injuries, though not absolute.

RESUMEN

Extraskeletal Neoplasm Resembling Ewings Sarcoma was firstly reported with comprehensive review of 39 cases by L. Angervall and F.M. Enzinger in 1975. The tumors chiefly affected young adults and most commonly involved the soft tissue of lower extremity. Microscopically, they were consisted of solidly packed small, round or ovoid cells arranged in sheets or lobules, which were indistinguishable from those of the Ewings sarcoma of bone. The authors recently experienced a soft tissue sarcoma occurring in deltoid area of young male patient, which was diagnosed extraskeletal neoplasm resembling Ewings sarcoma. The case review is presented.

RESUMEN

The goals of hip arthroplasty have been to eliminate pain, decrease deformity, increase morbility and obtain stabiIity. Currently total hip replacement is the best avaliable procedure to accomplish these goals. A prospective study of total hip arthroplasty was begun at the orthopedic dept of Kyung Hee Medical College in 1974. Between May, 1974 and June, 1977. 69 total hip replacements of the Charnley type, the Trapesoidal-28 type and the Muller type were peformed by the authors. Of the 60 Patients involved in this study, 9 had bilateral operations. The Charnley type Prosthetic device was used in 56 cases, the Trapesoidal-28 type in 7 cases and the Muller type in 6 cases. Complications included wire breakage, dislocation, loosening, femur shaft perforation, nonunion of greater-trochanter, acetabular protrusion, transient femoral nerve palsy, femur shaft fracture, infection, socket lateralization, ectopic bone formation and death. In 59 patients of the 60 patients, 53 patients felt that they were definitely improved, 5 patients felt their condition was unchanged and 1 patient felt her condition was worse.
