RESUMEN

SUMMARY OBJECTIVE: Beta-thalassemia minor is a blood disease caused by a hereditary decrease in beta-globin synthesis, frequently leading to hypochromic microcytic anemia. Formerly called endothelial cell-specific molecule 1, endocan is a proteoglycan released by vascular endothelial cells in many organs. Our aim was to investigate the relationship between the beta-thalassemia minor patients and the healthy control group in terms of serum endocan level. METHODS: The study was performed in a total of 80 subjects. They were divided into two groups, the beta-thalassemia minor group (n=40) and the healthy control group (n=40). Serum endocan levels, age, sex, body mass index value, and tobacco use data of these groups were compared. RESULTS: No statistically significant difference was detected between the two groups in terms of age, sex, and body mass index values (p>0.05). Endocan levels were measured to be 206.85±88.1 pg/mL in the beta-thalassemia minor group and 236.1±162.8 pg/mL in the control group with no significant difference between the groups in terms of serum endocan levels (p>0.05). CONCLUSIONS: In our study, there was no change in endocan level in beta-thalassemia minor. This might be because serum endocan levels are affected by multi-factorial reasons. Serum endocan levels may be altered secondarily to decreased beta-globin chain, increased sympathetic activity due to anemia, or platelet dysfunction induced by oxidative stress in beta-thalassemia minor. Further multicenter studies involving more patients are necessary to demonstrate this.

RESUMEN

Objective:To investigate the Effect of Pine muscle nursing training and external application of loose tendon ointment on muscle strength and muscle tension in patients with disused knee joint contracture.Method:From August 2018 to June 2019, 80 subjects were selected in the hospital, divided into three groups using the random number table method, and arranged in different wards. The routine rehabilitation group performs routine hospital rehabilitation (good limb placement, massage, active and passive exercise training), routine rehabilitation + pine muscle nursing training group is to implement rehabilitation nursing training on the basis of conventional rehabilitation treatment in hospital (relaxation training, hand micro-vibration therapy, pasteurization training), routine rehabilitation + pine muscle nursing training + medication group is based on the conventional rehabilitation treatment and pine muscle nursing training of hospitals to implement external application of loose tendon ointment. After 8 weeks of intervention, the muscle strength of the affected quadriceps (knee extensor) and the biceps femoris (Knee flexor) were compared. Results:Before the intervention, the patients in the three groups all experienced a decrease in the strength of the affected quadriceps muscle, and the flexion contracture of the knee joint caused by the hypertonic biceps muscle tension (minimum value ≥5uV) ( P>0.05). After 8 weeks of intervention, the muscular tone of the affected biceps femoris in the three groups was decreased ( F=10.623, P<0.001), and the muscle strength of the quadriceps femoris was increased ( F=10.888, P<0.001).The pairwise comparison results showed that compared with the conventional rehabilitation group, the difference between the conventional rehabilitation + pine muscle nursing training group was significant ( P<0.05), and the difference between the conventional rehabilitation + pine muscle nursing training + medication group was significant ( P<0.001). Compared with the routine rehabilitation + pine muscle training group, the difference between the conventional rehabilitation group and the routine rehabilitation + pine muscle nursing training + medication group was significant (both P<0.05). Conclusion:Patients with disused knee flexion contracture treated with Pine muscle nursing training combined with external application of loose tendon ointment can loosen the contraction and stiffness of the joint tissue, significantly reduce the muscle tension of the flexor knee muscle, increase the strength of the extensor knee muscle, restore and improve The rhythmic contraction and relaxation ability of the flexion and extension knee muscles makes their muscle strength/muscle tension ratio tend to balance, enhance the coordination control ability between muscle groups, and effectively reduce the degree of knee flexion contracture.

RESUMEN

Legg-Calve-Perthes (LCP) disease is characterized by idiopathic avascular osteonecrosis of the epiphysis of the femur head. The main factor that plays a role in the etiology of the disease is decreased blood flow to the epiphysis. Many predisposing factors have been suggested in the etiology of LCP disease, and most have varying degrees of effects. Here we present the case of a boy aged 4 years and 10 months with complaints of short stature and a diagnosis of multiple hypophyseal hormone deficiency, in whom LCP disease and difficult birth-related pituitary stalk interruption syndrome were identified by anamnesis. The present case revealed that LCP disease and hypophyseal hormone deficiency could be secondary to difficult birth and that LCP disease could be secondary to insulin-like growth factor 1 deficiency. Additionally, to the best of our knowledge there is no published case on the relation between LCP disease and insulin-like growth factor 1 deficiency. Therefore, we believe that this case is worthy of presentation.

RESUMEN

Since congenital hypothyroidism [CH] is the most important cause of preventable mental retardation, its screening is important In this present study, it was aimed to evaluate congenital hypothyroidism cases before and after the initiation of screening program in year 2007 in our country. A total of 400 patients diagnosed with CH at our outpatient clinic were retrospectively evaluated. Age of diagnosis, complaint, clinical signs, and etiological distribution were detected and changes in those parameters were evaluated before and after year 2007, which was the initiation date of our national screening program. After year 2007, 70.6% of patients were diagnosed in the first month; 21.2% in 1-3 months; 6.5% in 3-6 months, and no patient was diagnosed after 9 months. Before initiation of the screening program, 48.4% of cases were diagnosed in the first month, the percentage was increased to 62.8% after the program; the increase was significant While mental retardation was detected in 13.3% of patients before the screening, it was decreased to 4% after initiation of the program. An interesting finding was that age of treatment onset in girls was significantly higher than in boys before the screening; there was no difference between them after initiation of the screening. In this present study, it was observed that ages of diagnosis and treatment as well as mental retardation rates were significantly decreased in girls after the screening program, but ideal results have not been reached yet, and is expected to be reached in the future

RESUMEN

Bone maintenance theory considers that the external load is the direct stimulating source of the bone remodeling. In this article, the method of experimental observation of self-repairing process of the bone defect and related results are introduced. Firstly, a hole was drilled in the rabbit thighbone so that the continuity of the bone was changed. Then bone defect model was established, and the thighbone data were obtained by using CT scanning, and the self-repairing process of bone defects caused by growth factor were observed and analyzed by MIMICS software. Finally, the relationship between volume changes of sclerotin was established, and scientific bases were provided for introducing the bionic topology optimization method to the remodeling process. The experimental results showed that the self-repairing of the each layer sclerotin of the young rabbits was faster than that of the adult ones under the same condition. In addition, the volume always changes contrarily between the spongy bone and enamel bone during the self-repairing process of bone defect.

RESUMEN

OBJECTIVETo prospectively compare of no drain or with drain after unilateral total knee arthroplasty (TKA).

METHODSA randomized prospective study was conducted between February 2006 and February 2007. The study included 100 patients who were divided into 2 groups: group A (n = 50), which included patients without drain release, and group B (n = 50), which included patients who had a drain release. The true total blood loss was calculated depending on the height, body mass and pre-and post-operative Hct, and hidden blood loss was gotten by subtracting the visible blood loss from total loss. The recovery after surgery, the correlative complications and transfusion amounts were compared and analyzed.

RESULTSAll 100 patients undergoing TKA were involved in the result analysis. In patients without a drain, the total blood loss was (535 +/- 295) ml and the hidden blood loss was (513 +/- 290) ml, in patients with a drain, the total blood loss was (853 +/- 331) ml and the hidden blood loss (689 +/- 324) ml. The total and hidden blood loss, blood transfusion amount between drain-group and without drain-group was significantly different (P < 0.05). The difference in the incidence of infection and post-op recovery and correlative complications did not reach statistical significance.

CONCLUSIONSIn patients having primary TKA, using a drain, compared with no use of a drain, does not show us a clear advantage that make us accept it. Without a drain release could present an effective and economic method for reducing blood loss and preventing blood transfusion in patients undergoing unilateral TKA.
