RESUMEN

In adults, although malignant fibrous histiocytoma (MFH) is the most frequent malignant soft tissue tumor. Primary malignant fibrous histiocytoma of the round ligament of the uterus is comparatively very rare. A patient presented with severe lower abdominal pain and a palpable pelvic mass in the lower abdomen. Emergency laparotomy revealed a 15 X 15 cm tumor located in the left round ligament and adhered with uterus and intestine. The patient underwent total abdominal hysterectomy and bilateral salpingectomy. The final diagnosis was based on the pathological report of the surgical specimen.

RESUMEN

OBJECTIVE: Conization is used for diagnosis and treatment of cervical neoplasia. Our purpose of this investigation is to determine the efficacy of loop conization for the treatment of cervical dysplasia and the significance of the clinical and histological factors used to predict residual dysplasia after loop conization. METHODS: We reviewed the charts of patients who were received conization and subsequently total hysterectomy at Kangnam St Mary Hospital during 1989 and 2000. Logistic regression and Chi-square test were used for analysis. RESULTS: Total 257 patients were included. The mean age of the patients was 44.7 years. Of these patients, 87 (33.8%) had residual disease in the hysterectomy specimens. Age, the involvement of cut surface of endocervix and exocervix, severity and extent of the lesion, and scattering lesion were associated with the residual disease. But preoperative HPV infection, which is known as the cause of cervical neoplasia was not associated with the residual disease. CONCLUSION: When the high risk factors are present, the validity of conservative treatment should be considered and more careful follow up with pap smear, HPV test and colposcopy is necessary. Because about one thirds of patient has residual disease after conization, LEEP conization should be used for diagnosis of cervical neoplasia rather than treatment.

RESUMEN

OBJECTIVE: The aim of this study is to evaluate the clinicopathologic characteristics of tuberculous peritontitis from calcinomatosis peritonei. METHODS: From July 1994 to May 2002, fifteen women who were diagnosed tuberculous peritonitis in Kangnam St. Mary's Hospital were analyzed. We reviewed their medical records. RESULTS: A total of 15 patients eventually diagnosed by pathological or bacteriological method were reveiwed. Five patients were taken explorative lapartomy and biopsy. Eight patiets were taken laparoscopy and biopsy. One patient shows M. tubcerculous only cutured in ascites. One patient with cervical lyhphadenopathy shows chronic granuloma on cervical lymph node. CONCLUSION: Tuberculous peritonitis needs differential diagnosis with ovarian cancer because of its vague symptoms as ascites, abdominal pain and fever. Since there is no specific and sensitive method, at present, an explorative laparotomy or pelviscopic surgery can be alternatives for differetial diagnosis between tuberculous peritonitis and ovarian cancer.
