RESUMEN

The present study was undertaken to compare the homocysteine levels in patients of ischemic stroke with controls. Our study included 117 patients of ischemic stroke and 101 controls. The mean homocysteine levels in patients with ischemic stroke were 16.80±6.71 umol/L while in controls it was 12.30±4.68 umol/L, the difference being statistically significant (P<0.01). The increased homocysteine levels in patients with ischemic stroke are independent of diabetes mellitus, age and sex. The homocysteine levels were higher in hypertensive subjects than non-hypertensive (P<0.05).

Low antioxidant levels and oxidative stress due to airway inflammation may be determinant of asthma severity. The study was conducted to find the extent of lipid peroxidation and change in the levels of plasma iron in asthmatic patients with severity of disease. Study included 155 asthmatic and 156 healthy volunteers of the age groups 18-45 of either sex. Asthmatic patients were grouped into mild, moderate and severe groups on the basis of Forced Expiratory Volume in first second percent (FEV1%). Level of plasma malondialdhyde (MDA) was used as index of lipid peroxidation. A significant increase (P<0.01) in plasma MDA and plasma iron levels was found in asthmatic subjects as compared to controls. There was maximum increase in plasma MDA and iron levels in moderate asthmatic group. A positive correlation between plasma MDA and iron (r = +0.3) has been found in asthmatic patients. An increased plasma iron levels in asthmatics may contribute to aggravate lipid peroxidation.

Injury to the myocardial tissue due to ischemia and reperfusion occurs because of imbalance between the formation of oxidants and available antioxidants in the heart. Levels of vitamin C (ascorbic acid) and vitamin E (alpha--tocopherol) were evaluated in 52 patients of acute myocardial infarction (AMI) treated by streptokinase. They were further divided into reperfused group (39 patients) and non-reperfused group (13 patients). Twenty normal healthy subjects served as controls. Vitamin C and vitamin E were estimated in study group before and after thrombolytic therapy and in controls. Vitamin C levels were low in AMI cases as compared to controls (8.74 +/- 1.87 and 10.63 +/- 3.26 mg/L, respectively, P < 0.001). Trend of fall in vitamin C levels in the two study groups was not statistically significant. Vitamin E levels declined from 12.19 +/- 6.71 to 9.96 +/- 6.50 mg/L by 4 hours which was significant (P < 0.01) in the reperfused group, but the change in non-reperfused group (9.28 +/- 6.37 to 9.35 +/- 6.07 mg/dL by 4 hours) was non-significant. This is because of increased consumption of this antioxidant in suppressing the oxidative stress which occurs with reperfusion. Vitamin E can be proposed as a valid marker for reperfusion.

RESUMEN

The aim of the study was to evaluate the role of ascitic fluid fibronectin in the diagnosis of malignant ascites. Fibronectin is a glycoprotein which plays an important role in regulating the organisation of the cell cytoskeleton and cell morphology. Ascitic fluid samples from 35 patients, 20 with portal hypertension (Group-I) and 15 with malignant ascites (Group-II) were analysed for proteins, cell counts, fibronectin levels and malignant cell cytology. Mean ascitic fluid fibronectin level was found to be significantly higher in malignant ascites as compared to portal hypertension (p < 0.001). At a cut off value of 94.67 microg/ml, the sensitivity, specificity, positive accuracy, negative accuracy and overall diagnostic accuracy was found to be 100%, 95%, 93.8%, 100% and 97.1% respectively.

RESUMEN

Sixty samples of smoking and chewing tobacco from subjects in and around Ludhiana, Punjab were tested for arsenic content. No significant difference between the two groups could be detected. The probable role of arsenicosis in neuropathy and liver disease in the Indian population is discussed.

RESUMEN

In a series of 4,366 patients screened at Ludhiana, Punjab, the incidence of hypothyroidism was of the order of about 20 per cent and of hyperthyroidism about 15 per cent in both sexes. The female-to-male ratio was a little over 3:1. These statistics conform broadly to conventional Western figures. The relevance of the thyroid function tests in detecting subclinical thyroid disorders in relation to other systems cannot be overemphasised.

RESUMEN

The study was undertaken to estimate plasma copper and zinc in thirty-five depressed patients. Two blood samples were drawn from each patient, one before starting treatment and the second after recovery from depression. The results were compared with the thirty-five normal healthy individuals. The mean plasma copper in controls, depressed patients and in patients after recovery were 106.82, 122.14 and 104.22 micrograms/dl, respectively. The copper levels in patients when depressed were significantly higher as compared to controls and after recovery from depression. The mean plasma zinc levels in controls, depressed patients and after recovery were 115.80, 107.62 and 125.68 micrograms/dl, respectively. No significant difference could be obtained between control and depressed patients. However, the values were significantly higher in recovered patients compared to patients with depression.

The present study was an attempt to assess the cause of persistent pain in lower limbs among children from Kashmir. The study was conducted on one hundred children attending Paediatric out-patient department of Sher-i-Kashmir Institute of Medical Sciences, Srinagar. All the children were in the age group of 5 to 14 years. They showed markedly raised levels of serum alkaline phosphatase, whereas serum phosphorus, serum calcium levels and antistreptolycin O-titres were normal in 93% cases. None of them had any rheumatic or rheumatoid pathology. Among 15 suspected clinical rickets only three were established radiologically. Dietary and socio-economic history revealed deficient vitamin D intake and less exposure to sun. It was hypothesized that sub-clinical vitamin D deficiency could be a major cause of persistent pain in lower limbs and raised serum alkaline phosphatase could be the earliest marker of vitamin D deficiency. It was confirmed by injecting single dose of vitamin D (3 lac I. U.) which relieved bone pain and lowered the levels of serum alkaline phosphatase to normal within 14 weeks of initiation of therapy.

Drugs are chiefly metabolised in the liver usually in two phases, viz. oxidation and conjugation. The present study was undertaken to investigate the effect of protein-calorie malnutrition (PCM), rehabilitation and effect of phenobarbitone on the hepatic drug metabolising enzymes in weanling albino rats, fed on a semisynthetic diet containing 18% or 0.5% protein. The two representative enzymes of oxidation and conjugation employed were aminopyrine N-demethylase and bilirubin UDP-glucuronyl transferase, respectively. The study revealed that PCM severely impairs oxidative drug metabolising enzyme but less so in conjugation stage. On refeeding 18% protein diet, drug metabolising enzymes returned to normal within 2-3 weeks. Phenobarbitone administration increased the activities of drug metabolising enzymes.

RESUMEN

Activity of the enzyme gamma-glutamyl transpeptidase (GGT) was measured in 21 patients of BL or LL disease and 10 age matched healthy controls. None of the patients had any systemic or hepatic diseases. Mean values in the patients (53.67 +/- 9.67 U/L) were significantly higher (p less than 0.05) compared to that of controls (34.2 +/- 5.69 U/L).
