RESUMEN

Objective:To analyze the epidemiological and clinical features of patients with visceral leishmaniasis (VL), and to improve the understanding of the disease and standardize the diagnosis and treatment.Methods:The clinical data and diagnosis and treatment process of 62 patients with VL admitted to The First Hospital of Lanzhou University from January 2010 to December 2020 were retrospectively analyzed. The clinical symptoms and laboratory data, including aspartate aminotransferase (AST), lactate dehydrogenase (LDH) and procalcitonin (PCT) were compared between children and adults. Chi-square test was used for statistical analysis.Results:Of the 62 patients, 31(50.0%) patients were males, 38(61.3%) patients were children and 24(38.7%) were adults. There were 32 cases (51.6%) living in Longnan City, and 11 cases (17.7%) living in Gannan Tibetan Autonomous Prefecture. Forty-seven cases (75.8%) had a delay over 30 days from onset to diagnosis. All patients had fever and chills, 34 cases (54.8%) had fatigue and poor appetite, 30(48.4%) had cough and expectoration, 10 cases (16.1%) had headache and dizziness, 45 cases (72.6%) had splenomegaly, 40 cases (64.5%) had anemia, and 29 cases (46.8%) had hepatomegaly. There were 32 cases (51.6%) accompanied with respiratory infection, 11 cases (17.7%) accompanied with hemophagocytic lymphohistiocytosis and 10 cases (16.1%) accompanied with abnormal liver function. Of the 60 patients who received treatment, 14 were recurrent cases who had received at least one course of pentavalent antimony monotherapy before admission. Forty-six patients received standard antimony monotherapy, 14 patients received standard antimony combined with amphotericin B therapy. Thirteen patients discontinued amphotericin B due to impaired renal function, and 50 patients were followed up for half a year without recurrence. The proportions of splenomegaly and hepatomegaly in children were 86.8%(33/38) and 65.8%(25/38), respectively, and the corresponding numbers in adults were 50.0%(12/24) and 16.7%(4/24), respectively. The differences were statistically significant ( χ2=10.03 and 14.26, respectively, both P<0.050). Thirty-three cases (86.8%) in children and 14 cases (58.3%) in adults had a delay over 30 days from onset to diagnosis. The difference was statistically significant ( χ2=6.52, P=0.011). The proportions of patients who had elevated AST, LDH and PCT were 76.3%(29/38), 94.3%(33/35) and 73.9%(17/23) in children, respectively, while 45.8%(11/24), 71.4%(15/21) and 5/17 in adults, respectively, and the differences were all statistically significant ( χ2=5.97, 3.89 and 7.82, respectively, all P<0.050). Conclusions:Patients with VL are tend to have complications and be severe. Early diagnosis and standardized treatment are important measures to improve the prognosis. Patients who have failed antimony treatment can be treated with amphotericin B or combination therapy, but kidney injury and other adverse reactions should be closely monitored.

Objective:To analyze the situation of free cervical cancer screening from 2018 to 2020 in Lucheng District of Changzhi City, Shanxi Province.Methods:The cervical liquid-based thin-layer cytology (TCT) results of 13 043 women who underwent free cervical cancer screening at the Lucheng District Maternal and Child Health Family Planning Service Center of Changzhi City in Shanxi Province from 2018 to 2020 were retrospectively analyzed. The women with positive TCT results underwent colposcopy and biopsy.Results:Among 13 043 women, 800 women were positive for TCT, with a positive rate of 61.3/100 000. The positive rates of cervical cancer screening TCT in 2018, 2019 and 2020 were 4.82% (145/3 006), 5.63% (170/3 017) and 6.91% (485/7 020), which showed an upward trend year by year (contingency coefficient = 0.037). In 2018, 2019 and 2020, the positive rates of TCT among women aged 35-44 years old were 2.95% (31/1 052), 3.78% (42/1 112) and 4.22% (82/1 969), among women aged 45-54 years old were 5.48% (68/1 241), 6.50% (90/1 385) and 7.72% (2 258/3 341), and among women aged 55-64 years old were 6.45% (46/713), 7.31% (38/520) and 8.42% (144/1 710); the positive rates of cervical cancer screening TCT among different age groups in the same year, same age group in different years and overall different age groups in 3 years were statistically significant (all P < 0.05), and the positive rates of cervical cancer screening TCT in women aged 45-54 and 55-64 years old were higher than those in women aged 35-44 years old (all P < 0.05). Among 800 cases of positive cervical cancer screening TCT, 615 cases (76.9%) were diagnosed with cervical intraepithelial neoplasia (CIN), and the incidence rate of CIN was 4.72% (615/13 043); there was statistical differences in the incidence of CIN among different years ( P < 0.05). Conclusions:The positive rates of cervical cancer screening increase year by year from 2018 to 2020 in Lucheng District of Changzhi City, Shanxi Province. Early cervical cancer screening can detect cervical lesions in time.

A serious leaf disease caused by Colletotrichum dematium was found during the cultivationof Sarcandra glabra in Jingxi, Rong’an, and Donglan Counties in Guangxi Province, whichinflicted huge losses to plant productivity. Biological control gradually became an effectivecontrol method for plant pathogens. Many studies showed that the application of actinomycetesin biological control has been effective. Therefore, it may be of great significance tostudy the application of actinomycetes on controlling the diseases caused by S. glabra.Strains of antifungal actinomycetes capable of inhibiting C. dematium were identified, isolatedand screened from healthy plants tissues and the rhizospheres in soils containing S.glabra. In this study, 15 actinomycetes strains were isolated and among these, strains JT-2F,DT-3F, and JJ-3F, appeared to show antagonistic effects against anthracnose of S. glabra.The strains JT-2F and DT-3F were isolated from soil, while JJ-3F was isolated from plantstems. The antagonism rate of strain JT-2F was 86.75%, which was the highest value amongthe three strains. Additionally, the JT-2F strain also had the strongest antagonistic activitywhen the antagonistic activities were tested against seven plant pathogens. Strain JT-2F isable to produce proteases and cellulase to degrade the protein and cellulose componentsof cell walls of C. dematium, respectively. This results in mycelia damage which leads toinhibition of the growth of C. dematium. Strain JT-2F was identified as Streptomyces tsukiyonensisbased on morphological traits and 16S rDNA sequence analysis.

OBJECTIVE@#To study the effects of electroacupuncture (EA) in chronic constrictive injury (CCI) rat model and the expression of N-methyl-D-aspartate receptor type 2B (NR2B) in ipsilateral spinal dorsal horn in rats to explore the analgesic mechanisms of EA.@*METHODS@#According to the random number table, totally 180 rats were evenly divided into a sham group, a CCI group, and an EA group. CCI model was conducted with four 4-0 chromic gut ligatures loosely ligated around the left sciatic nerve 1 cm above the trifurcation. Rats in the EA group received 2 Hz EA therapy bilaterally at acupoints of Zusanli (ST 36) and Sanyinjiao (SP 6) once daily (30 min/d) for 30 days after surgery. Paw withdrawal thresholds (PWTs) were measured on 0 (baseline), 1, 3, 7, 15, 30 days after surgery. Rats were sacrificed on 0, 1, 3, 7, 15 and 30 days after surgery, and the L4-5 segments of spinal cord were removed to detect the expression of NR2B by immunohistochemistry and quantitative polymerase chain reaction.@*RESULTS@#PWTs in the CCI group were significantly lower than the sham group at Day 1-30 after surgery, and reached its lowest at Day 1 (P<0.01). After EA treatment, the PWTs recovered rapidly and were significantly higher than those in the CCI group on 3, 7, 15 and 30 days after surgery (P<0.01). The numbers of NR2B-immunoreactive cells of the CCI group significantly increased after CCI surgery compared with the sham group (P<0.01). Compared with the CCI group, stimulation of EA markedly decreased the numbers of NR2B-immunoreactive cells at Day 3, 7, 15 and 30 (P<0.05). In the sham group, NR2B mRNA was expressed at a low level. It increased after CCI surgery, which increased rapidly at Day 7 (P<0.01) and reached its peak value at Day 15 (P<0.01). After EA stimulation, relative quantity of NR2B mRNA expression was less than that in the CCI group at Day 15 and 30 (P<0.05).@*CONCLUSIONS@#Low frequency of EA had antinociceptive effect in CCI rat model. The analgesic effects of EA might be through the inhibition of NR2B.

BACKGROUND@#The tyrosine kinase inhibitors (TKI) treatment of non-small cell lung cancer (NSCLC) with epidermal growth factor receptor (EGFR) mutation may have a positive effect, but most patients may develop drug resistance, therefore, the detection of the developing time in drug resistance and the research of the mechanism of drug resistance are need to be solved. While the emerge of next generation sequencing (NGS) have make it possible. The aim of this study is to monitor the efficacy of targeted therapy by studying the variation of circulating tumor DNA (ctDNA) mutation frequency and gene mutation spectrum through the targeted therapy.@*METHODS@#Our center enrolled 22 patients with EGFR mutation detected by tissue or peripheral blood, and collect 8 mL of peripheral blood of the patients for ctDNA sequencing in different phases, before systematic prior treatment, followed-up by 2 months and disease progression after TKI administration.@*RESULTS@#Patients with EGFR gene mutation may acquire a longer median survival time after receiving targeted drug therapy, due to the drop of mutation abundances, while the therapy may have a minor effect in patients which their mutation abundances have slightly decreased compared to the statistics before the cession (P=0.015,3). The significantly reduced group median progression was associated with a longer survival [progression free survival (PFS)=390 d]. At the same time, we found out that when related to TP-53 gene mutation, the effect of targeted drug therapy for EGFR-sensitive mutation was unsatisfactory (the median PFS was 120 d compared with 630 d, P=0.000,2).@*CONCLUSIONS@#Patients who has lower mutation abundance with EGFR sensitive mutations after TKI treatment may have a longer survival period (P<0.05), and the mutation abundance were not significantly dropping or accompanied by other mutations may indicating TKI resistance.

Objective To observe the changes in tear quantity and tear break-up time(BUT) before and after wearing overnight orthokeratology (OK),and investigate the influence of OK on tear film in young adolescents.Methods Prospective clinic study was conducted in 60 adolescents,aged 8-16 years,who met the inclusion criteria during November 2016 and May 2017 in the Second Hospital of Dalian Medical University were chosen and fitted with OK (OK group).At the same time,60 adolescents who met the same requirement were fitted with spectacle (spectacle group).The two groups were both divided into three subgroups according to the age (subgroup of 8-10 years old,11-13 years old,and 14-16 years old).The BUT,Schirmer' s I test score,and corneal fluorescent (FL) were measured and recorded before wearing and 1 week,1 month,3 months,6 months and 12 months after wearing.Results Totally 57 patients in the OK group (114 eyes) and 59 patients in the spectacle group (118 eyes) completed the follow-up study.No significant difference in Schirmer' s Ⅰ test scores was observed between the OK group and spectacle group (all P>0.05).The BUT in OK group was significantly lower than that in the spectacle group at all the time points (all P < 0.05).Of all subgroups,the subgroup aged from 8-10 years exhibited the sharpest and most rapid reduction in BUT.Conclusion Based on our findings,long-term OK wearing has no effect on tear volume but reduces the stability of tear film.The results suggest that OK should be considered to be a good alternative for children and adolescents with spectacle;however,it may influence the quality of tear film.In addition,special attention should be paid to younger adolescents,and regular follow-up is needed.

BACKGROUND: With the development of 3D printing technology, organ and tissue construction can be achieved by constructing a three-dimensional scaffold that is conducive to cell growth. OBJECTIVE: To solve the scaffold over-accumulation during 3D printing.METHODS: Fluent, a finite element analysis software developed by ANSYS Company in the United States, was used to analyze the extrusion process of print heads and to obtain suitable viscosity and extrusion pressure of materials for the 3D printing of cellulose gel composites. We then compared simulation results with experimental results. RESULTS AND CONCLUSION: The error between simulation results and experimental results was less than 5%. The simulated values at a kinetic viscosity of 45 and a pressure of 0.10-0.12 MPa solved the phenomenon of over-accumulation of cellulose gel composites during the 3D printing process, ensuring enough space for the 3D printed scaffold.

Objective:To investigate the clinical significance of the changes of T lymphocyte subsets in the peripheral blood of patients with gastric cancer. Methods:The lymphocyte subsets in 67 patients with gastric cancer were analyzed with flow cytometry. CEA and CA199 from all patients were also detected. Results were statistically analyzed, and the patients were followed up. Results:The aver-age values of Tc (CD8+CD28+) in patients with stagesⅢandⅣgastric cancer were 12.3±5.1 and 9.9±4.6, respectively, with significant difference (P=0.043). The average values of Tc (CD8+CD28+) in patients with elevated CEA and with normal CEA were 9.3±3.6 and 12.1± 5.6, respectively (P=0.025). Similarly, the average values of Tc (CD8+CD28+) in patients with elevated CA199 and with normal CEA were 9.3±4.2 and 12.1±5.3, respectively (P=0.029). The overall survival of patients with decreased Tc (CD8+CD28+) was lower than that of patients with normal Tc (CD8+CD28+) (8.1 vs. 15.1 months, P=0.008). Conclusion:The cellular immune function of patients with gastric cancer gradually decreased with the progression of TNM. Higher CA199 and CEA values indicated lower average values of Tc (CD8+CD28+). The overall survival of patients with decreased Tc (CD8+CD28+) was lower.

Objective To explore any effect of proprioceptive neuromuscular facilitation (PNF) on the proprioception and balance of patients with knee osteoarthritis (KOA).Methods Forty patients with KOA were randomly divided into an experimental group (EG) and a control group (CG),each of 20.The PNF techniques of isotonic combined contraction,and rhythmic stable and dynamic reversal were applied in the EG,while the CG received quadriceps muscle strength training.Knee proprioception was evaluated using knee angle reconstruction experiments,and balance ability was measured using the one leg standing test (OLS) and the five times sit to stand test (FTSST).Results The errors in active and passive knee angle reconstruction at 30°,60° and 110° all improved significantly in the EG,but not in the CG.After the treatment,the OLS and FTSST results improved significantly in the EG,but only the OLS results improved significantly in the CG,not the FTSST times.Even so,the average OLS time in the EG was significantly longer than that of the CG after the training.Conclusion Proprioceptive neuromuscular facilitation can improve the proprioception and balance of persons with knee osteoarthritis.

It has been confirmed that the body′s response to hepatitis C virus (HCV) is not only associated with virus, but also associated with some cytokines and their gene polymorphisms. In this paper, the current research on some cytokines associated with HCV and their gene polymorphisms is reviewed. And it is shown that interleukin-28B is closely associated with the course and prognosis of chronic hepatitis C (CHC). Therefore, it is of great significance for the clinical diagnosis and treatment of CHC to investigate host cytokines and their gene polymorphisms.

ObjectiveTo explore the influencing factors for chronic hepatitis C (CHC) with thyroid dysfunction (TD) in untreated Chinese patients and provide evidence for clinical individualized treatment. MethodsOne thousand and twelve untreated CHC patients were collected nationwide in China. Thyroid function and associated influencing factors (region, age, gender, and hepatitis C virus (HCV) RNA replication level) in the patients were investigated. The relationships between the influencing factors and CHC with TD were analyzed. Between-group comparison of categorical data was performed by χ2 test and Fisher′s exact test. ResultsThere were geographical differences between different types of CHC with TD. Across different regions, the incidence of TD was highest in north and northwest China, i.e., 28.3% and 26.5%, respectively. Subclinical hypothyroidism was the most common type of TD, accounting for 58.8% of the total TD cases. Middle-aged patients were most common among the cases of CHC with TD (44.0%), who had a significantly higher incidence of hypothyroidism than other age groups (χ2=1010、617, P=0.001、0013). Females with CHC had a significantly higher incidence of TD than male patients (58.9% vs. 41.1%, χ2=13.1, P=0.00). Although a high HCV RNA replication level was most common in Chinese patients with CHC, this factor had little influence on TD. ConclusionIn China, CHC with TD is influenced by geographic distribution, gender, and age, but less associated with HCV RNA replication level.

<p><b>OBJECTIVE</b>To investigate the distribution of HCV genotypes in Chinese Han population with chronic hepatitis C (CHC).</p><p><b>METHODS</b>This randomized multicenter study included 1 014 CHC patients from 28 hospitals in different regions of China. SPSS 20.0 was applied to analyze the relationship among region, HCV genotype, gender and the replication level of HCV-RNA.</p><p><b>RESULTS</b>HCV 1 genotype (56.80%) was the most common genotype. The majority of CHC patients were of genotype 1, 2, 3, 6 in the order of frequency, except those in southwestern, southern and central China. HCV 1, 2, 3, 6 genotypes were most common among male patients in southern China; among female patients in northern China; among male patients in northern and northwestern China and among male patients in northwestern China, respectively (all P <0.05). There was no statistical significance between different genders in other regions. The high viral load was more common than the low viral load among HCV 1, 2, 3, 6 genotype-infected patients.</p><p><b>CONCLUSION</b>There are different distributions of HCV genotypes among the different regions. In addition, HCV genotypes are correlated with gender and HCV-RNA load.</p>

With homology cloning approaches coupling with RACE (rapid-amplification of cDNA ends) techniques, the full-length coding sequence of pathogenesis-related protein PR10-1 with differential expression was cloned from the total RNA of the root of Panax notoginseng, and its function was explored furtherly. As a result, the longest 465 bp ORF (named as PnPR10-1 with the Accession No. KJ741402 in GenBank) was detected from the cloned sequence with full-length of cDNA of 863 bp. The corresponding peptide encoded consisted of 155 amino acids, contained some domains such as Bet-v-I, and showed high similarity with that from Panax ginseng by analysis of phylogenetic trees created from the alignments. Real-time quantitative PCR showed that the expression of PnPR10-1 gene was constitutive in different tissues of 1-3 year old plant, suggesting that it might be involved in growth, development, and secondary metabolism; yet it was up-regulated significantly with the infection of Fusarium oxysporum in root, suggesting that it might be involved in defense against many diseases including root rot in P. notoginseng.

Objective To discuss the active role of health education on control of diabetes mdlitus and prevention of its complications.Methods 96 cases of hospitalized patients with diabetes mellitus were randomly divided into the control group and the observation group with 48 patients in each group from June 2009 to June 2010.The control group received conventional diabetes treatment and distribution of health education brochures,based on this,the observation group was given health education,including cognitive,nutrition,behavioral intervention.The general prevention condition of diabetes and its complications as well as the education effect were compared before the education and six months after education.Results The fasting blood glucose,2-hour postprandial blood glucose,glycosylated hemoglobin,body mass index and incidence of complications were greatly improved in the observation group than those of the control group.Conclusions Strengthening health education can improve self-care ability of patients,effectively improve the overall control level of diabetes,reduce acute and chronic complications,disability,death rate.

OBJETIVO: O cúbito varo é uma doença muito comum em crianças e adultos, ocasionada por fratura supracondilar. Existem vários procedimentos cirúrgicos e fixações internas para correção do cúbito varo, com diferentes desfechos, embora a fixação interna com placa e parafusos seja o mais comum. Contudo, o impacto do posicionamento da placa sobre a cirurgia raramente foi estudado até agora. MÉTODO: Em nosso estudo, 12 pacientes com cúbito varo foram divididos em dois grupos, operados pelo método de osteotomias em cunha com fechamento lateral e fixações internas com placa e parafusos. Em um grupo, as placas foram colocadas no lado póstero-lateral; no outro, as placas foram colocadas no lado lateral do úmero. RESULTADO: O período de acompanhamento foi 4,5 meses (faixa de 2 a 7 meses). Houve cinco resultados excelentes (83,3 por cento) e um bom (16,7 por cento) em cada grupo. Em todos os casos, a aparência é muito semelhante ao lado oposto; não há diferenças de amplitude de movimento (AM) no cotovelo depois da cirurgia. Um paciente no grupo B teve paralisia nervosa transitória; não houve infecções nem osteomielite. CONCLUSÃO: A posição da placa de fixação interna não tem impacto sobre a osteotomia em cunha de fechamento lateral. Nivel de Evidência II, Prospectivo Comparativo.


OBJECTIVE: To study the effects of low intensity ultrasound irradiation applied on the spinal cord, in the regeneration of the rat's sciatic nerve after a controlled crush injury, evaluating the functional results of the sciatic functional index as measured on video recorded images of the foot sole. METHODS: Eighteen rats were submitted to a controlled crush injury of the right sciatic nerve, and divided into two groups according to the treatment: Group 1 (n=9), simulated irradiation; Group 2 (n=9), effective irradiation. Low-intensity ultrasound irradiation was started on the 7th postoperative day and applied daily for 6 weeks. Images of the animals´ foot soles were video recorded on a transparenttreadmill belt at weekly intervals until the 6th week of irradiation, and the corresponding sciatic functional index (SFI) was measured usingspecific software. RESULTS: The SFI during the first and last week of treatment was -59.12 and -12.55 in Group 1, -53.31 and -1.32 in Group 2, indicating improvements of 79 percent and 97 percent, respectively, but differences between the groups were only significant (p<0.05) during the third week of treatment. CONCLUSION: The authors conclude that low intensity therapeutic ultrasound enhances nerve regeneration, with significance during the 3rd week of treatment. RESUMEN

Objective To study the treatment of neonate-infants after open-heart surgery during perioperafive period.Methods The clinical data of 1619 neonate-infants during perioperative period were analyzed.Results 68 cases died,mortality 4.2%.Postoperative complication 190 cases(11.7%).Conclusion The management of components and quantities of the infusion,admistration of cardiovascular drugs,the management of respiratory tracts and the protection of heart function,ale main points of perioperafive therapy of neonate-infants after open-heart surgery.

RESUMEN

<p><b>BACKGROUND</b>The down-regulation of constitutive nitric oxide synthase (cNOS) and up-regulation of inducible nitric oxide synthase (iNOS) are associated with the allergen-provocated airway hyperresponsiveness (AHR). This study aimed to determine whether their alteration also plays an important role in the AHR induced by lipopolysaccharide (LPS).</p><p><b>METHODS</b>Hartley male guinea pigs, weighing between 250 g and 350 g, were injected with LPS at a dose of 1 mg/kg every 24 hours for three days. A non-selective NOS inhibitor, N(omega)-nitro-L-arginine methyl ester (L-NAME), or a selective inducible NOS inhibitor, aminoguanidine (AG), were used thirty minutes before each injection of LPS. Airway reactions, nitric oxide (NO) production and inflammatory changes were detected 24 hours after the last dose of LPS.</p><p><b>RESULTS</b>AG significantly decreased the NO production in the bronchoalveolar lavage fluid (BALF) and sharply reduced the intensity of bronchoconstriction to histamine challenge. L-NAME also significantly decreased the NO production in the BALF, but had no effect on airway reactions or, perhaps, a tendency to enhance the intensity of AHR.</p><p><b>CONCLUSIONS</b>The data suggest that inducible NOS contributes to the AHR induced by repetitive intraperitoneal LPS, and constitutive NOS was also involved.</p>

The aim of this study was to set up a new method for 5, 10-Methylenetetrahydrofolate reductase (MTHFR) single nucleotide polymorphisms (SNP) genotyping, and to investigate the hereditary susceptibility of hematological malignancy. Prepared an aimed gene microarray based on cDNA microarray theory, dual-color fluorescence hybridization was used to detect SNP loci, and DNA sequencing was performed to confirm the results. The MTHFR C677T SNP loci of 157 controls and 127 patients with hematological malignancies (30 multiple myeloma, 28 non-Hodgkin's lymphoma, 22 acute lymphoblastic leukemia, 40 acute myeloid leukemia, 7 chronic myeloid leukemia) from Jiangsu province were detected. The results showed that after overlapping, homozygous wild type, heterozygote type and homozygous mutant type yielded green, yellow and red fluorescence, respectively. DNA sequencing validated these results. The allele frequency of 677C and 677T in patients and controls were 58.7% and 66.9%, 41.3% and 33.1% respectively, showing statistically significant difference (chi2 = 4.077, P = 0.043). 677TT genotype showed a significantly higher risk of MM (OR = 4.21; 95% CI = 1.50 - 11.83; P = 0.006). It is concluded that this microarray-based method is accurate, high-throughput and inexpensive, suitable for SNP genotyping in a large number of individuals. C677T polymorphisms influence the risk of hematological malignancies. 677TT genotype is susceptive to MM.
