We report a case of an 81-year-old woman with extensive pelvic lymphadenopathy that caused severe stenosis and occlusion of the right common and external iliac veins and proximal common femoral vein. Pelvic lymphadenopathy resulted from the recurrence of a previous right ovarian epithelial tumor. The patient had severe right lower extremity edema, consistent with severe venous insufficiency.She was treated with high-pressure balloon angioplasty (12-14 mm in diameter) and four self-expanding stents (14-10 mm diameter, 80-40 mm length). The postoperative response was dramatic to a near-complete resolution of the edema. The venous clinical severity scores were 10 and 2 at presentation and 6 months after the follow-up, respectively. Balloon angioplasty and stenting are safe and effective methods for providing symptomatic relief for lower extremity venous insufficiency in patients with extensive and unresectable pelvic masses.

RESUMEN
Endovenous Laser Therapy [EVLT] for Greater Saphenous vein [GSV] insufficiency is a newly established method of treatment only recently made available in Iran. The present study seeks to describe the results of the first 20 patients treated with EVLT at Shohada-e Tajrish Medical Center, Tehran, Iran. 20 patients [16 male, 4 female] with the mean age of 38.9 and an average length of symptoms of 5.9 years, were treated with a 980-nm laser diode under local anesthesia. CEAP classification [Clinical Severity, Etiology, Anatomy, Pathophysiology] and AVSS scores [Aberdeen Varicose Vein Symptom Severity Score] were used to determine disease severity and symptoms before and after the procedure. Outcome was measured by the rate of recurrence as shown in Doppler ultrasonography evaluation. The mean procedure time was 49 minutes, and the mean admission time was 1.1 days. A success rate of 85% percent was recorded at 6-12 months of follow up. The patients showed a significant reduction in AVSS and CEAP scores [PV=0.0001], Pain [PV=0.00001], Parasthesia and Edema [PV=0.001]. EVLT seems promising as a novel method of treatment for GSV insufficiency in the Iranian population with many advantages, including higher success rates in comparison with conventional methods of treatment

We are presenting a case of abdominal tuberculosis who had peritonitis and two large enteroliths were removed from the small intestine during laparotomy. Following a full course of medical treatment, she developed small bowel obstruction and treated by laparotomy and enterolysis but showed no evidence of stricture. We had a diagnostic and management challenge which will be discussed along with a review of the literature
