RESUMEN

Objectives: To compare the global indices obtained by Swedish Interactive Threshold Algorithm [SITA]-Standard strategy with those obtained by SITA-Fast strategy using Humphrey Visual Field Analyzer, in patients of glaucoma


Study Design: Cross Sectional/Validative Study


Place And Duration: Armed Forces Institute of Opthalmology, between March and September 2010


Methodology: SITA Standard and SITA Fast visual field analysis of 184 patients were carried out on Humphrey Visual Field Analyzer [HFA-II] using white on white perimetry with 30-2 program on consecutive patients that fit the inclusion criteria, presenting to Armed Forces Institute of Ophthalmology [Rwp] between March 2010 and September 2010. Value of Pattern Standard Deviation [PSD], Mean Deviation [MD] and time taken to complete the examination were noted.. A p-value of <0.05 was considered significant


Results: The Mean Deviation [MD] as calculated by the HFA-II, using the SITA Standard algorithm was -14.03 +/- 6.31 dB, MD calculated by SITA Fast algorithm was -13.8 +/- 6.2 dB, with no significant difference between the two, p>0.05 The mean PSD as calculated by the HFA-II, using the SITA Standard algorithm was 7.7 +/- 4.4 dB, PSD using the SITA Fast algorithm was 7.5 +/- 4.2 dB, with no significant difference between them, p < 0.05. It took 6:34 +/-2.1 minutes to complete the examination with the Fast strategy and 9.3+/-2.6 minutes with the Standard Strategy. The results were significant


Conclusion: SITA Fast strategy takes significantly less time to complete an examination with the same reliability as SITA Standard strategy

RESUMEN

To determine the diagnostic accuracy of fine needle aspiration cytology [FNAC] in cervical lymphadenopathy using histopathologic examination of the excised lymph node as gold standard. Cross sectional study. Combined Military Hospital Rawalpindi in collaboration with Armed Forces Institute of Pathology [AFIP] Rawalpindi, from January 2007 to January 2008. A total of 70 patients suffering from cervical lymphadenopathy were selected. After obtaining informed consent FNAC was obtained from the lymph node. Afterwards same lymph node was removed under local anesthesia for histopathology. All specimens were sent to AFIP for histopathology reporting. Sensitivity, specificity, positive and negative predictive values and diagnostic efficacy for both the modalities were calculated. Data was analyzed using SPSS version 10. Mean and standard deviation for age and gender were calculated. Mean age of the patients was 38.14 +/- 16.88 year. FNAC findings showed a sensitivity of 79.5%, specificity 47.61%, positive predictive value 78%, negative predictive value 50% and diagnostic efficacy 70%. FNAC appears well established method of diagnosis, as its results compared favorably in many respects with those obtained from traditional surgical biopsy.

RESUMEN

To compare the intraocular pressure [IOP] lowering effect of Timolol/Dorzolamide combined preparation to that of Latanoprost in eyes with primary open angle glaucoma. Randomized control trial [RCT]. The Eye department, Military Hospital, Rawalpindi, over six months, from 9th June 2008 to 8th December 2008. Eighty Six patients included were divided into two equal groups. Initial IOP measurement was taken for every patient using Goldmann Applanation Tonometer with Fluorescein. Topical Antiglaucoma therapy was started in patients of either group. Group A patients were administered timolol/dorzolamide combined preparation where as latanoprost was administered to group B patients. Follow up IOP measurements were recorded at 4 week and 8 week interval for every patient and findings were endorsed on a pre-designed proforma. Data were analyzed using SPSS version 11.0. Eight week follow up IOP of Group A [14.51 +/- 2.54] was significantly lower than the initial IOP [19.37 +/- 2.49] [p<0.05]. For Group B, difference between initial IOP [19.53 +/- 2.69] and 8 week follow up IOP [14.09 +/- 2.23] was also statistically significant. The difference between mean IOP of the two groups at 4 week [p=0.284] and at 8 week [p=0.419] follow up was not statistically significant. Frequency of cases with >/= 4 mm Hg drop at 4 week follow up was 26 [60.46%] in group A as against 29 [67.44%] in group B [p=0.50], whereas at 8 week follow up it was 31 [72.09%] for group A and 34 [79.07%] in group B [p=0.451]. The results suggest that latanoprost monotherapy and timolol/dorzolamide combined therapy is equally effective in controlling IOP in patients with POAG.

RESUMEN

To compare quality of scar after subcuticular stitches between Polypropylene [Prolene], a non-absorbable suture, with Polyglactin 910 [Vicryl], an absorbable suture after thyroid surgery. Randomized controlled trail [RCT]. Department of Surgery Combined Military Hospital and Military Hospital Rawalpindi from 1 Jan 2007 to 31 Dec 2007. Sixty patients undergoing elective thyroid surgery were included and randomly divided into two groups using random number tables. Informed written consent was obtained from each patient. The neck wound was closed by subcuticular suturing technique and the suture material was used according to the sampling technique. The patients were followed up in OPD on 10[th] post operative day and twelve weeks after surgery for the quality of scar. After 10 days, 1 patient in each of the Polypropylene group [3.3%] and Polyglactin 910 group [3.3%] had a bad scar [p = 1.000]. After twelve weeks of surgery, 2 patients in the Polypropylene group [6.7%] and 3 patients in the Polyglactin 910 group [10.0%] had bad scars [p = 1.000]. There is no significant difference between the frequency of bad scar formation in the Polypropylene and Polyglactin 910 group. It is thus recommended that absorbable Polyglactin 910 sutures should be used for wound closure after thyroid surgery as it does not require suture removal and thus prevents patient anxiety and discomfort and also saves surgeon and staff's time

A 25 year old male had presented with history of abdominal distension of 20 days duration and on examination he had prominent abdominal vein and a venous hum with Ascites. He had no history of altered sensorium, melena. The clinical scenario was suggestive of Cruveilhier Baumgarten syndrome
