The authors report two cases of congenital synostosis between the lunate and the triquetral bones, occurring in two different patients. One is a 26 year-old Caucasian female and the other is a 14 year-old Black male. The first complained of a painful wrist and the second is completely asymptomatic, except for a distal radius fracture, when the synostosis was discovered. Wrist arthroscopy and lunate-triquetral arthrodesis were performed in the first patient. The second patient was surgically treated only for the distal radius fracture. The authors review the literature about congenital synostosis between the lunate and the triquetral bones and discuss the indications for lunate-triquetral arthrodesis and wrist arthroscopy. They conclude this is an alteration that is rarely discovered in the carpus, with good indication for wrist arthroscopy and lunate-triquetral arthrodesis, when symptomatic
