RESUMEN

Objective: Anxiety disorders are highly prevalent and the efficacy of the available anxiolytic drugs is less than desired. Adverse effects also compromise patient quality of life and adherence to treatment. Accumulating evidence shows that the pathophysiology of anxiety and related disorders is multifactorial, involving oxidative stress, neuroinflammation, and glutamatergic dysfunction. The aim of this review was to evaluate data from animal studies and clinical trials showing the anxiolytic effects of agents whose mechanisms of action target these multiple domains. Methods: The PubMed database was searched for multitarget agents that had been evaluated in animal models of anxiety, as well as randomized double-blind placebo-controlled clinical trials of anxiety and/or anxiety related disorders. Results: The main multitarget agents that have shown consistent anxiolytic effects in various animal models of anxiety, as well in clinical trials, are agomelatine, N-acetylcysteine (NAC), and omega-3 fatty acids. Data from clinical trials are preliminary at best, but reveal good safety profiles and tolerance to adverse effects. Conclusion: Agomelatine, NAC and omega-3 fatty acids show beneficial effects in clinical conditions where mainstream treatments are ineffective. These three multitarget agents are considered promising candidates for innovative, effective, and better-tolerated anxiolytics.

ABSTRACT Cynara scolymus L., Asteraceae, are traditionally used to treat dyspepsia. This study evaluated the hypolipidemic and antiatherogenic effects of an aqueous extract prepared from the leaves of C. scolymus in rat's model. Hypercholesterolemic rats (1% cholesterol and 0.5% cholic acid for 15 days) were treated (0.5 ml/200 g) with extract of C. scolymus (150, 300, or 600 mg/kg p.o.; n = 6) or simvastatin (4 mg/kg p.o.; n = 6) once per day for 30 days along with hypercaloric diet. A control group (C) was given water (0.5 ml/200 g; n = 6). A high-cholesterol diet was maintained throughout the treatment period. Rats treated with extract of C. scolymus (150, 300, or 600 mg/kg) and simvastatin showed significant decreases in serum levels of total cholesterol (−46.9%, −51.9%, −44%, and −41.9%, respectively) and low-density lipoprotein-cholesterol (LDL-C; −52.1%, −54.8%, −51.9%, and −46.7%, respectively), compared with group C (p < 0.005). Biochemical analyses revealed significant decrease in the concentration of IL-1, IL-6, TNF-α, IFN-γ, C-reactive protein, oxidized-LDL, and antioxidized-LDL in rats treated with extract of C. scolymus (150, 300, or 600 mg/kg). There were no differences in serum ALT enzyme activity between the groups. Our results suggest that hypolipidemic and antiatherogenic effects could be related with the presence of polar substances present in aqueous extract of C. scolymus.

A interação medicamentosa é um assunto relevante para prática odontológica. O número de interações possíveis no dia a dia do cirurgião-dentista é imenso. O levantamento adequado dos fármacos contribui para que sejam diminuídas as possibilidades de interação que causem efeitos deletérios ao paciente. Dessa forma, o objetivo dessa pesquisa foi determinar as principais interações medicamentosas relatadas na literatura na prática odontológica utilizando uma ferramenta on-line (UpToDate®). Cabe ao profissional realizar uma adequada anamnese e avaliar as potenciais interações. Ferramentas on-line e aplicativos para dispositivos móveis podem auxiliar a análise das interações medicamentosas e proporcionar mais segurança ao profissional e ao paciente.


Drug interaction is a relevant issue in dental practice. The number of possible interactions in the everyday dentist is enormous. The adequate research of drugs contributes to diminish possibilities of interactions that cause deleterious effects to the patient. Therefore, the objective of this research was to determine major drug interactions reported in the literature in dental practice using an online tool (UpTo- Date®). The professional needs to conduct a proper interview and assess potential interactions. Online tools and applications for mobile devices can facilitate the analysis of drug interactions and provide more security for the dental professional and the patient.
