RESUMEN

OBJECTIVE@#To investigate the effects of the B7-H4 gene rs10754339 and miR-125a gene rs12976445 on cancer susceptibility through a case-control study and meta-analysis.@*METHODS@#A total of 1,490 cancer patients (lung/gastric/liver/: 550/460/480) and 800 controls were recruited in this case-control study. The meta-analysis was performed by pooling the data from previous related studies and the present study.@*RESULTS@#The results of this study showed that in the Hubei Han Chinese population, the rs10754339 gene was significantly associated with the risk of lung and gastric cancer but not liver cancer, and the rs12976445 gene was significantly associated with the risk of lung cancer but not liver or gastric cancer. The meta-analysis results indicated that rs10754339 and rs12976445 contributed to cancer susceptibility in the Chinese population and also revealed a significant association between rs10754339 and breast cancer risk, as well as between rs12976445 and lung cancer risk.@*CONCLUSION@#The B7-H4 gene rs10754339 and miR-125a gene rs12976445 may be the potential genetic markers for cancer susceptibility in the Chinese population, which should be validated in future studies with larger sample sizes in other ethnic populations.

RESUMEN

The differences of the active ingredients in Dendrobium huoshanense of different growth years and their protective effects on acute liver injury were studied to provide evidence for optimizing harvest time. The contents of polysaccharides, total flavonoids and total alkaloids in D. huoshanense of different growth years were determined by UV spectrophotometry, and the contents of gigantol in D. huoshanense were determined by HPLC. C57 BL/6 mice were randomly divided into blank control group(saline), modeling group(saline), high-dose(7.5 g·kg~(-1)) and low-dose(1.25 g·kg~(-1)) groups of D. huoshanense of different growth years. Each group was intragastrically administered every day for 2 weeks. 500 mg·kg~(-1) paracetamol was injected intraperitoneally 2 h after last treatment except the control group. After 12 hours, the serum and liver tissues were collected to detect the activities of ALT and AST, and the levels of SOD and MDA. The hepatic histopathological examination was performed. The results showed that the chemical constituents of D. huo-shanense of different growth years were significantly different(P<0.05). The contents of polysaccharide and gigantol of D. huoshanense of 2 growth years were the highest. The contents of flavonoids and alkaloids of D. huoshanense of 3 growth years were the hig-hest, followed by the D. huoshanense of 2 growth years, and the lowest were that of 1 growth year. Compared with the modeling group, D. huoshanense of different growth years could decrease the activities of ALT and AST in serum. Meanwhile, the levels of MDA reduced significantly, while those of SOD increased markedly. Histopathological results suggested that all D. huoshanense samples were effective in the reduction of the necrosis of hepatocytes in different degrees. The results of the multi-component SPSS paired tests showed that polysaccharide and gigantol probably played a leading role in the liver protection effects, while D. huoshanense of 2 growth years showed the best efficacy. The optimal harvesting time of D. huoshanense is 2 growth years.

Heavy metals and other harmful elements in traditional Chinese medicines inflict serious damage on public health. Therefore, risk assessment of Chinese raw materials has gained increasing attention. To date, few reports have been published on the health risk assessment of heavy metals and harmful elements in Chinese patent medicines. To gain a comprehensive understanding of heavy metals and other harmful elements in Chinese patent medicines and to establish proper limits, residual Pb, Cd, As, Hg, Cu and Cr in 15 054 samples of 295 drugs was analyzed with regard to distribution and variation between elements and dosage forms. In addition, in accord with procedures including hazard identification, hazard characterization, exposure assessment and risk characterization, basic procedures and specific parameters for risk assessment of heavy metals and harmful elements in Chinese patent medicines were clarified based on the health risk assessment of 14 787 samples and 276 drugs. A method and equation for establishing residual limits is proposed. The results show that content and target hazard quotients (THQs) of the investigated elements in all samples showed a skewed distribution approaching 0. Content of Pb, As, Cu, Hg, Cd or Cr in the samples exceeded 100 mg·kg-1 and the content of Pb, As, or Cu in individual samples exceeded 1 000 mg·kg-1. THQs of 586 samples and four drugs were above 1. We believe that the health risk of Hg, Pb and As in Chinese patent medicines with dosage forms of pill, capsule, tablet and powder, especially those in raw powder preparations, warrant concern.

Objective To explore the effect of epidural labor analgesia on the prolactin (PRL) levels of parturient.Methods Sixty primipara who delivered in the First Affiliated Hospital of Xinxiang Medical University from October 2016 to May 2017 were selected as research object.The parturient were divided into observation group and control group,with 30 cases in each group.The parturient in observation group were given ropivacaine combined with sufentanil for epidural block labor analgesia;the parturient in control group took natural production method without any analgesia method.The pain score,duration of labor,milk yield,serum PRL level before analgesia and 24,48 h postpartum and the number of initial lactation at 30 rmin,24 h,48 h postpartum of maternal were compared between the two groups.Results The active period of the first stage of labor of parturient in the observation group was significantly shorter than that in the control group (P < 0.05);there was no statistic difference in the latent period of the first stage of labor,the second stage of labor and the third stage of labor of parturient between the observation group and the control group(P < 0.05).There was no statistic difference in the pain score of parturient between the two groups before analgesia(P < 0.05);the pain score of parturient in observation group was significantly lower than that in the control group at the time point of uterine orifice open 3 cm,uterine orifice full dilatation and fetal disengagement(P< 0.05).There was no statistic difference in the PRL levels of parturient between the two groups before analgesia (P < 0.05);the PRL level of parturient in observation group was significantly higher than that in the control group at 24,48 h postpartum(P <0.05).The condition of initial lactation of maternal in the observation group was significantly better than that in the control group(P < 0.05).The milk yield of parturient in observation group was significantly more than that in the control group at 24,48 h postpartum (P < 0.05).Conclusion Epidural analgesia can reduce the pain of parturient,shorten the active stage of the first stage of labor,increase the level of PRL and increase the milk yield of the postpartum.

Early enteral nutrition is more compatible with physiological status,is beneficial to cell and organ function recovery and helpful to reduce the incidence of infection and to improve the prognosis.However,due to facts like intra-abdominal hypertension,hemodynamic instability,intestinal dysfunction,early enteral nutrition is usually difficult to successfully implemented in surgical critically ill patients.This paper reviews the progress of early enteral nutrition implementation in surgical ritically ill patients.

Objective The study was carried out to investigate the effects of different dietary Ca levels on bone metabolism, based on the osteoprotegerin ( OPG)-receptor activator of NF?κB ligand ( RANKL)?receptor activator of NF?κB ( RANK) system in growing WHBE rabbits. Methods Twenty one weaned male WHBE rabbits at the age of 42 days were divided into 3 groups (I, II and III) according to dietary Ca levels (0. 95%, 1. 10%, and 1. 30%, respectively) for a 42?d feeding trial. The above three diets had similar phosphor (P) (about 0. 64 g/kg), digestible energy (9. 50 MJ/kg), crude protein (about 19. 70%) and crude fibre (13. 57%) contents. When the feeding trial finished, the serum in?dices of bone metabolism (Ca, PTH and BALP) were detected, and the OPG?RANK?RANKL system in bone tissue was analyzed by real?time fluorescence quantitative PCR assay and immunohistochemistry, respectively. At last, the relationships between bone metabolism and dietary Ca were evaluated according to RANKL/OPG ratio. Results All the contents of serum Ca, PTH and BALP had no significant differences in the groups I, II and III (P>0. 05). Both the RANKL mR?NA and RANKL/OPG mRNA ratio were lowest in the group II and had significant differences with group I and III ( P<0. 05). Dietary Ca levels had significant effects on the protein expression of OPG?RANK?RANKL in bone tissues (P<0. 01). The positive index of OPG in the groups II and III was significantly higher than that in the group I(P<0. 01), while the positive index of RANK in the group II was lower than those of the group I and III(P<0. 01). The protein ex?pression positive index ratio of RANKL to OPG was also lowest in the group II, showing a significant difference with group I(P<0. 01). Furthermore, both the gene transcription ratio of RANKL to OPG and the protein expression positive index ratio of RANKL to OPG had significant correlations (with quadratic curve) to the dietary Ca levels (R2 =0. 4068, 0. 8433;P<0. 05,P<0. 001). Conclusions In summary, the bone metabolism of WHBE rabbits during growing periods has sig?nificant correlation with dietary Ca levels. An optimal bone metabolism status can be obtain at 1. 10% dietary Ca level as demonstrated in this study.

Objective: To observe the effect of Yi Jin Jing (Sinew-transforming Qigong Exercises) on the muscle strength in senile sarcopenia. Methods: Sixty-five old people with sarcopenia were randomized into Yi Jin Jing group and a blank control group. Thirty-three patients in Yi Jin Jing group practiced Yi Jin Jing (Sinew-transforming Qigong Exercises), while 32 patients in the blank control group didn't receive any interventions. The muscle strength was measured before and after 12-week training. Results: During the study, each group had 1 dropout. The muscle strength was improved after 12-week training in Yi Jin Jing group, and the difference was statistically significant (P<0.05); there was no significant difference in the blank control group (P>0.05). After the intervention, there was a significant difference between Yi Jin Jing group and the blank control group in comparing the muscle strength (P<0.05). Conclusion: Constant Yi Jin Jing (Sinew-transforming Qigong Exercises) training can notably improve skeletal muscle strength in senile sarcopenia.

AIM:To investigate the effects of losartan on lipopolysaccharide (LPS)-induced glial fibrillary acidic protein (GFAP) expression,and to determine whether adenosine 5'-monophosphate (AMP)-activated protein kinase (AMPK) activation is involved in the mechanism.METHODS:Adult male KM mice were divided into control group,LPS model group,losartan treatment group,and losartan and Compound C co-treatment group.To establish a model of central nervous system inflammation,the mice received daily intracerebroventricular injection of LPS (24 μg/d) for 2 d.Daily losartan administration (0.5,1 or 5 mg · kg-1 · d-1,ip) initiated at 14 d prior to LPS injection.Compound C (10 mg/kg,ip),a selective AMPK inhibitor,started to be injected daily at 2 d prior to LPS injection.The hippocampal tissues in each group were isolated at 3 d after the last LPS injection,and then the protein levels of GFAP,AMPK,p-AMPK,mammalian target of rapamycin (mTOR) and p-mTOR were determined by Western blot.RESULTS:Twice LPS injections significantly increased the expression of GFAP in the hippocampus (P < 0.01).Losartan inhibited LPS-induced GFAP expression in a concentration-dependent way,and losartan at 5 mg· kg-1 · d-1 significantly inhibited GFAP expression and AMPK activation (P < 0.05),but it had no obvious effect on mTOR activation.Furthermore,Compound C significantly reversed the effect of losartan treatment on LPS-induced GFAP expression and AMPK phosphorylation (P < 0.05).CONCLUSION:Losartan inhibits LPS-induced GFAP expression in the mouse hippocampus,and AMPK activation but not mTOR,is involved in the mechanism.

Objective To analyze the regional management policies for the configuration and utilization of medical facilities before the issuing of Regulations for the Supervision and Administration of Medical Devices in May 2017, so as to provide reference for relevant policy-making in the future. Methods Regional policies in 15 provincial-level administrative regions such as Zhejiang, Shanghai and Hunan were collected and analyses were conducted on regulations of the configuration management, application and discard of large-scale medical facilities. Results While regional policies played a crucial role in the configuration and administration of medical facilities, problems still existed, and proposals were made on future policy-making accordingly. Conclusion Policies for the configuration and administration of medical facilities require to be improved.

Objective:This study aimed to present the time trend of suicide rate in people aged 5 years and over in China from 2002 to 2015,and to describe the rural and urban distribution,and population characteristics of suicide rate in recent 13 years.Methods:The data of suicide rate were based on the China Health Statistics Yearbook published by the National Health and Family Planning Commission (formerly as the Ministry of Health) of the Peoples Republic of China.Poisson regression models were used to test the time trend of the general suicide rate,and region-,gender-and age-specific suicide rates.Results:Since 2002,the suicide rates in urban population,rural population,male and female have decreased significantly.Urban population had higher decreasing trend than rural population,and female had higher decreasing trend than male.Suicide rates in women of reproductive age (15 -49 years) both in rural and urban areas had significantly decreasing trends,and were lower than male with same ages in recent years.There was no statistical difference of the decreasing trend in rural population aged 85 years and over,or young male population in rural area.The annual average suicide rate from 2012 to 2015 was 6.75/100 000.Rural population,male and the older persons had higher suicide rate.Conclusion:The overall suicide rate decreased over the period of 2001-2015.However,it is still important to carry out suicide prevention program,especially in rural area,and in older persons and young male.

Objective:To analyze related factors to the informed consent competency of patients with schizophrenia.Methods:Subjects were divided into two groups,namely one group including 100 patients with schizophrenia and the other group including 28 heathy controls.Patients with schizophrenia were administrated from community rehabilitation units and The Sixth Hospital of Peking University,diagnosed with the Diagnostic and Statistical Manual of mental disorder,Fourth Edition (DSM-Ⅳ).Informed consent competency of subjects were evaluated with the Chinese Mandarin Version of MacArthur Competence Assessment Tool for Clinical Research.Psychiatric symptoms,intelligence quotient and severity of disease were accessed individually with the Positive and Negative Symptom Scale (PANSS),Wechsler Abbreviated Scale of Intelligence (WASI) and Clinical General ImpressionSeverity Scale (CGI-S).Logistic regression analysis model was used to analyze the risk factors of impaired informed consent competency in schizophrenia.Retsrts:The rates of absent,impaired and adequate informed consent competency were 26.0% (26/100),47.0% (47/100) and 27.0% (27/100) in schizophrenia group,and 3.6% (1/28),57.1% (16/28) and 39.3 % (11/28) in control group.Rate of absent informed consent competency was higher in schizophrenia group than that in control group (26.0% vs.3.6%,P <0.01).There were no statistic significant differences in rates of impaired and adequate informed consent competency between two groups (both P > 0.05).Results of logistic regression analysis showed that schizophrenic patients with higher scores of positive subscale (OR =1.15),negative subscale (OR =1.23) and CGI-S (OR =1.57) had more tendency to impaired capacity.Higher education level (OR =0.73) and higher scores of IQ (OR =0.92) had less risks for that.Conclusion:Competency of informed consent in schizophrenia may be worse than that in health control.Patients with schizophrenia with impaired informed consent competency may have more serious of positive,negative psychiatric symptoms and higher severity of disease.Higher level of education and IQ scores may reduce the risk of impaired informed consent competency.

Objective To investigate the protective effect of curcumin (Cur) on mice cornea against ultraviolet A (UVA)-induced damage.Methods Together 30 C57BL/6 mice were randomly divided into 3 groups (10 mice in each group):UVA irradiation group (UVA group),UVA irradiation with curcumin treatment group (UVA + Cur group) and control group.Mice in the UVA group was exposed to UVA for 24 h,with the wavelength of 320-400 nm and the irradiation intensity of 0.05 W · cm-2.In UVA + Cur group,mice were given 30 mg · kg-1 curcumin intraperitoneally 3 days before irradiation and lasted until 48 h after irradiation.And the control group was left untreated.Then corneal opacity was evaluated and scored through slit-lamp biomicroscopy.After irradiation for 48 h,corneal tissue was collected for the detection of the expression of NF-κB by electrophoretic mobility shift assay.Results After irradiation for 48 h,the score of corneal opacity in the UVA group (3.10 ± 0.74) was higher than that of the control group (0) and UVA + Cur group (0.35 ± 0.24) (P < 0.05).The expression of NF-κB in cornea was detectable at a low level in the control group (1.25 ± 0.13),but was remarkably increased in UVA group (3.98 ± 0.58) and suppressed by curcumin in UVA +Cur group (1.58 ± 0.34) (P < 0.05).There was no significant difference in corneal opacity score and NF-κB expression between control and UVA + Cur group (P > 0.05),but there was significant difference between control and UVA group and UVA + Cur and UVA group (all P < 0.05).Conclusion Curcumin can protect cornea against UVA-induced damage via inhibiting NF-κB activation.

Objective To explore and assess the reasonable application of integrated collaborative service mode in obstetrics and gynecology clinical service.Methods 152 pregnant women admitted in obstetric outpatient from March 2016 to May 2016 were randomly assigned into observation group and control group,76 cases in each group.The control group received obstetric traditional outpatient treatment and hospitalization mode,while the observation group implemented the integrated collaborative service mode.It was a integrated mode based on the traditional mode and multidisciplinary professionals integrated into groups for synergistic services,from the beginning of pregnancy to discharge.Results The number of cases who were knowledged maternal diseases,intraoperative coordination,reasonable diet and breastfeeding of the observation group (69,70,69,66) were superior to those of the control group (51,40,54,50) (x2=17.56,52.58,12.44,11.11,all P<0.001).The incidence rate of maternal and neonatal complications of the observation group were significantly lower than those of the control group(t=12.04,6.49,2.87,9.26,-7.14,all P<0.005).And patients satisfaction survey was improved,the number of satisfaction and complaint in the observation group was 70 persons,1 person,which of the control group was 70 persons,6 persons (x2=7.65,P<0.01).Conclusion Integrated collaborative service mode can improve the quality of medical service and patients' satisfaction,while reduce cesarean section rate,postpartum hemorrhage and maternal complications.Integrated collaborative service mode is an effective method to promote the development of perinatal medicine.

OBJECTIVE@#To observe the effects of sevoflurane treatment on lung inflammation in rats with lipopoIysaccharide-induced acute lung injury (ALI).@*METHODS@#The rat model of ALI was established by intratracheal instillation of lipopolysaccharide (LPS). 45 infantile SD rats [body weight (272±15) g] were randomly divided into 3 groups (n=15): control group, LPS group, sevoflurane group. NS (1 mL/kg) was instillated in rats' airways of control group; LPS (5 mg/kg) was instillated in rats' airways of LPS group. Sevoflurane group rats received sevoflurane (2.4%) inhalation for a hour after LPS was instillated in rats' airways. Six hours after NS or LPS instillation, all rats were exsanguinated. Lung tissues were examined by HE staining. Expressions of TNF-α and ICAM1 mRNA were detected by semiquantitative RT-PCR techniques. The protein level of TNF-α and ICAM1 were assessed by western blot techniques.@*RESULTS@#In LPS group the permeability of lung tissues increased, organizational structure severely damaged and the alveolar wall tumed thick, with interstitial edema and Europhiles infiltrated increasingly. The LPS group had higher mRNA expressions of TNF-α and ICAM1 than control group and sevoflurane group (P<0.05), and LPS group had higher protein level of TNF-α and ICAM1 than control group and sevoflurane group (P<0.05).@*CONCLUSIONS@#Sevoflurane treatment can attenuate lung inflammation in rats with lipopolysaccharide-induced acute lung injury.

<p><b>OBJECTIVE</b>To establish a retinal vein occlusion (RVO) animal model and observe the therapeutic effect of a Chinese herbal composition (Fufang Xueshuantong Capsule, , FXC)inischemicinischemic) in ischemic retinal disease.</p><p><b>METHODS</b>Fifteen adult male Sprague-Dawley rats underwent laser photothrombosis to induce RVO on their right eyes and were subsequently randomized to receive FXC (the intervention group, n=7) or placebo treatment (the control group, n=8). Fundus fluorescein angiography was performed after 2, 4 and 8 weeks of treatment. Real-time reverse transcription-PCR was used to quantify the mRNA expression of vascular endothelial growth factor (VEGF) and stromal cell-derived factor-1 (SDF-1). The main outcomes were the mRNA copies of VEGF and SDF-1 and the counts of RVO signs.</p><p><b>RESULTS</b>Laser photothrombosis procedure induced typical lesions of RVO, including hemorrhage, leakage, retinal detachment, capillary non-perfusion, filling defect of retinal vessels, and lateral circulation/dilation of small vessels. The retinal lesions were associated with an increased expression of VEGF (P<0.05). No significant change of SDF-1 expression was noticed. Compared with the control group, the intervention group had numerically fewer RVO lesions at week 2 (1.71±0.76 vs. 3.50±1.51, t=-2.82, P<0.05). The benefit of intervention remained at weeks 4 and 8.</p><p><b>CONCLUSIONS</b>A rat model of laser photothrombosis-induced RVO was established and an increase in the VEGF expression was observed in the retinal lesion. The FXC had therapeutic benefit in improving retinal lesions in the rat model of RVO.</p>

RESUMEN

<p><b>OBJECTIVE</b>To study the primary effects of Niuhuang Qingwei wan on the gastrointestinal function in aninmal for justifying its efficacies in clinic.</p><p><b>METHOD</b>Mice were twice administered with Niuhuang Qingwei wan (0.83, 1.67, 3.33 g x kg(-1), ig) and rats were twice administered with Niuhuang Qingwei wan (0.59, 1.18, 2.36 g x kg(-1), ig). The effects on the stomach function were evaluated by the gastric emptying test in mice and the gastric analysis in rats. The effect on the intestinal function were evaluated by the propulsive motility of the total gastrointestinal tract test in mice by recording the time and frequency of excreting carbo medicinalis. Its analgesia was explored by using the abdominal constriction test induced by acetic acid.</p><p><b>RESULT</b>Niuhuang Qingwei wan decreased the activity and secretion of pepsin in a dose-dependent manner (P < 0.01, P < 0.05), the gastric juice volume at middle and high doses (P <0.01, P <0.05), and the gastric acid volume at high dose (P <0.05); However, it had no significant effects on the gastric emptying in normal mice and the acidity in gastric juice. It shortened the excreting time of feces and increased the frequency of defecation (P < 0.01, P < 0.05). It also inhibited abdominal constriction responses at high dose, and the inhibition rate was 40.0% (P <0.01).</p><p><b>CONCLUSION</b>Niuhuang Qingwei wan can promote gastrointestinal motility, decrease gastric acid volume and activity of pepsin and show certain analgesia effect. Those findings are consistent with its treating stomach heat in clinic.</p>

<p><b>OBJECTIVE</b>To perform a preliminary study on the dysfunction of spermatogonial stem cells (SSCs) induced by cyclophosphamide.</p><p><b>METHODS</b>According to development stage of spermatogenesis in Wistar rat, 72 rats were divided into three groups, 1, 3 and 9 weeks groups, respectively. Then 24 rats per group were randomly divided into experimental and control groups, and 100 mg/kg cyclophosphamide was injected by intraperitoneal injection in experimental groups, with the same volume of normal saline in the control. Apoptosis of spermatogenic cells were detected after 24 hours by TUNEL. Then 60 one-week rats, whose germ cells were only SSCs, were randomly divided into experimental and control group. We detected c-Kit by immunohistochemistry and cell cycle by flow cytometry at 24 hours, 3 and 9 weeks.</p><p><b>RESULTS</b>There was no significant difference in apoptosis of germ cells in 1 week between experimental group and control group ( P > 0.05); however, there were significant differences among other groups. The ratio of S stage and c-Kit expression of spermatogonial cells were decreased in one-week rats at 24 hours, 3 and 9 weeks in experimental group compared with the control, respectively (P < 0.01).</p><p><b>CONCLUSION</b>Cyclophosphamide does not significantly induce SSCs apoptosis. It may be more important to interfere the proliferation and differentiation of SSCs.</p>

<p><b>AIM</b>To investigate the anti-inflammatory mechanism of esculentoside A (EsA) and to observe the effects of EsA on cellular adhesion between human umbilical vein endothelial cell (VEC304) and human neutrophil and to further observe the mRNA expression of intercellular adhesion molecule-1 (ICAM-1) and cluster of differentiation 18(CD18).</p><p><b>METHODS</b>The hemocyte counting method was used for assaying the adhesion rate between VEC304 and neutrophil. The RT-PCR method was used for measuring the mRNA expression of ICAM-1 and CD18.</p><p><b>RESULTS</b>The adhesion rate between VEC304 and neutrophil was increased with treatment of lipopolysaccharide(LPS). EsA (3 - 12 x 10(-6) mumol.L-1) was shown to inhibit the high cellular adhesion induced by LPS. A further investigation of adhesion molecules mRNA expression was undertaken using semi-quantitative reverse transcribed polymerase chain reaction (RT-PCR). The results of RT-PCR from VEC304 and human neutrophil treating with LPS showed that ICAM-1 and CD18 mRNA expressions were higher than those of normal cells, while this increased expression of ICAM-1 and CD18 mRNA was remarkably attenuated by the addition of EsA.</p><p><b>CONCLUSION</b>EsA was found to inhibit the increased adhesion rate induced by LPS. Moreover, LPS induced high expression of ICAM-1 and CD18 was inhibited with treatment of EsA. It might be involved in the mechanisms of anti-inflammation of EsA.</p>
