RESUMEN

OBJECTIVES@#The excretion of urinary vitamin D-binding protein (uVDBP) is related to the occurrence and development of early-stage renal damage in patients with Type 2 diabetes (T2DM). This study aims to explore the significance of detecting uVDBP in T2DM patients and its relationship with renal tubules, and to provide a new direction for the early diagnosis of T2DM renal damage.@*METHODS@#A total of 105 patients with T2DM, who met the inclusion criteria, were included as a patient group, and recruited 30 individuals as a normal control group. The general information and blood and urine biochemical indicators of all subjects were collected; the levels of uVDBP, and a marker of tubular injury [urine kidney injury molecule 1 (uKIM-1), urine neutrophil gelatinase-associated lipocalin (uNGAL) and urine retinol-binding protein (uRBP)] were detected by enzyme-linked immunosorbent assay. The results were corrected by urinary creatinine (Cr) to uVDBP/Cr, uKIM-1/Cr, uNGAL/Cr and uRBP/Cr. The Pearson's and Spearman's correlation tests were used to analyze the correlation between uVDBP/Cr and urine albumin-to-creatinine ratio (UACR), estimated glomerular filtration rate (eGFR) and markers of tubular injury, and multivariate linear regression and receiver operating characteristic curve were used to analyze the correlation between uVDBP/Cr and UACR or eGFR.@*RESULTS@#Compared with the normal control group, the uVDBP/Cr level in the patient group was increased (P<0.05), and which was positively correlated with UACR (r=0.774, P<0.01), and negatively correlated with eGFR (r=-0.397, P<0.01). There were differences in the levels of uKIM-1/Cr, uNGAL/Cr, and uRBP/Cr between the 2 groups (all P<0.01). The uVDBP/Cr was positively correlated with uKIM-1/Cr (r=0.752, P<0.01), uNGAL/Cr (r=0.644, P<0.01) and uRBP/Cr (r=0.812, P<0.01). The sensitivity was 90.0% and the specificity was 82.9% (UACR>30 mg/g) for evaluation of uVDBP/Cr on T2DM patients with early-stage renal damage, while the sensitivity was 75.0% and the specificity was 72.6% for evaluation of eGFR on T2DM patients with early-stage renal damage.@*CONCLUSIONS@#The uVDBP/Cr can be used as a biomarker in early-stage renal damage in T2DM patients.

OBJECTIVE To analyze the influencing factors for the metabolism of voriconazole in adult patients, and to provide reference for the rational use of voriconazole in clinic. METHODS The clinical data of adult patients admitted in our hospital receiving voriconazole and therapeutic drug monitoring from April 2021 to March 2022 were collected. The trough concentration of voriconazole (c0) and plasma concentration of voriconazole-N-oxide concentration (cN) were determined, and voriconazole-to-voriconazole N-oxide concentration ratio (c0/cN) was calculated. Pearson’s correlation analysis was used to analyze the influencing factors for c0 and c0/cN of voriconazole. Multiple linear regression models were used to analyze the independent influencing factors for c0 and c0/cN of voriconazole. RESULTS The underlying diseases of the patients were mainly pneumonia, kidney disease and leukemia. The detected fungi were mainly Aspergillus, Candida and yeast-like fungi. Voriconazole was mainly administered by intravenous drip, especially in patients who used proton pump inhibitor in combination. The levels of C-reactive protein (CRP), total bilirubin (TBIL), direct bilirubin (DBIL) and indirect bilirubin (IBIL) were positively correlated with c0 of voriconazole, while platelet count and albumin levels were negatively correlated with voriconazole c0. The levels of CRP, TBIL and DBIL were positively correlated with c0/cN, while albumin levels were negatively correlated with c0/cN. Multiple linear regression analysis showed that the independent influencing factors of voriconazole c0 included the levels of CRP and IBIL, route of administration and dose of voriconazole, and the independent influencing factors of voriconazole c0/cN were the levels of CRP and DBIL and age. CONCLUSIONS The levels of CRP and IBIL, route of administration and dose of voriconazole are independent influencing factors of voriconazole c0; the levels of CRP and DBIL and age are independent influencing factors of voriconazole c0/cN. The influence of above indexes on the metabolism of voriconazole should be considered when using voriconazole clinically; and the route of administration and dose of voriconazole should be adjusted reasonably.

RESUMEN

Starting from the perspective of meridian theory, this article briefly analyzes the meridian pathophysiology of snoring and the relationship between snoring and meridian theory. It proposes that acupuncture treatment for snoring should focus on regulating qi from the shaoyang meridians, harmonizing the spirit by the governor vessel, resolving phlegm through the three yang meridians, and harmonizing qi and blood from the yangming meridians. Additionally, attention is placed on both the root cause and the symptoms, the theory of "four seas". The ultimate goal is to promote the flow of meridian and qi-blood, improve symptoms such as nighttime snoring, poor sleep quality, and daytime sleepiness, and achieve the desired outcome of stopping snoring and ensuring restful sleep.

Objective To use the cognitive reinforcement comprehensive intervention program constructed by our team to intervene in patients with spinal cord injury and evaluate its clinical application effect.Methods A non-randomized trial design was adopted to select 97 patients with spinal cord injury from November 2021 to September 2022.Forty-four patients from March to September 2022 in a Grade A hospital in Hefei City were included in the experimental group,and 53 patients from November 2021 to February 2022 were included in the control group.The cognitive reinforcement comprehensive intervention program was used to intervene in the experimental group,and the conventional rehabilitation nursing was used to intervene in the control group.The intervention lasted for 12 weeks in both groups.The Changsha Montreal Scale,Social Support Rating Scale,Rehabilitation Exercise Self-efficacy Scale,Spinal Cord Injury Independence Rating Scale and Hamilton Anxiety Scale were used to measure the two groups before intervention,1 month after intervention and 3 months after intervention.Results 40 cases in the experimental group and 48 cases in the control group completed the study.Repeated measurement ANOVA showed that the temporal,interactive and intergroup effects of cognitive function scores and anxiety scores were statistically significant(P<0.05).The time effect and interaction effect of the subjective support dimension score,coping self-efficacy dimension score of the two groups were statistically significant(P<0.05).One month after the intervention,the cognitive function scores of test group were higher than before intervention and control group,and the anxiety scores were lower than before intervention and control group(P<0.05).Three months after the intervention,the scores of cognitive function,subjective support dimension and coping self-efficacy dimension of experimental group were higher than those before intervention and control group,and the scores of anxiety level were lower than those before intervention and control group(P<0.05).Conclusion Comprehensive intervention of cognitive reinforcement can improve the cognitive function of patients with spinal cord injury,delay the process of cognitive impairment,enhance self-confidence,relieve anxiety,and promote physical and mental rehabilitation of patients.

Objective:To observe the clinical effect of vitrectomy, inner limiting membrane (ILM) peeling, subretinal injection of compound electrolyte intraocular irrigation solution (CEIIS) and conbercept in the treatment of diabetic macular edema (DME) with hard exudate (HE) (DME-HE).Methods:A prospective clinical study. Thirty-three patients with DME-HE diagnosed by examination in Weifang Eye Hospital from June 2020 to February 2022 were included in the study. Among them, there were 15 males (16 eyes) and 18 females(20 eyes), with the mean age of (62.00±6.54) years. All patients underwent the examinations of best corrected visual acuity (BCVA), scanning laser ophthalmoscope, optical coherence tomography (OCT), and multifocal electroretinography (mf-ERG). Snellen visual acuity chart was used for BCVA examination, which was converted into logarithm of the minimum angle of resolution (logMAR) BCVA for statistic analysis. Macular foveal retinal thickness (CMT) and macular volume (MV) were measured by OCT. The 1 ring P1 wave amplitude density was measured by mf-ERG. The patients were randomly divided into group A and group B, with 17 patients (18 eyes) and 16 patients (18 eyes), respectively. There were no significant differences in age, logMAR BCVA, HE area, CMT, MV, and 1 ring P1 wave amplitude density between the two groups ( t=0.403, 0.972, 0.291, 0.023, -0.268, -0.206; P>0.05). Group A was treated with vitrectomy, ILM peeling, and subretinal injection of CEIIS and conbercept (combined therapy). Group B was treated with intravitreal injection of conbercept (IVC). Follow-up was 12 months after treatment. The changes of BCVA, HE area, CMT, MV, 1 ring P1 wave amplitude density were compared between groups and groups after treatment. The times of injection and complications after treatment were observed. Independent sample t test was used for comparison between the two groups. Results:At 12 months after treatment, compared to before treatment, there were significant differences in logMAR BCVA ( F=14.837), HE area ( χ2=94.522), CMT ( χ2=199.212), MV ( χ2=81.914) and 1 ring P1 wave amplitude density ( F=8.933) in group A ( P<0.05); there were significant differences in CMT ( F=5.540) and MV ( F=7.836) in group B ( P<0.05). Compared between the two groups, logMAR BCVA: 1 week and 6 and 12 months after treatment, the difference was statistically significant ( t=2.231, -2.122, -3.196; P<0.05); HE area: except 1 week after treatment, there were statistically significant differences at other times after treatment ( t=-2.422, -3.107, -3.540, -4.119; P<0.05). CMT, MV, 1 ring P1 wave amplitude density: 12 months after treatment, the differences were statistically significant ( t=-2.653,-2.455, 2.204; P<0.05). During the follow-up period, the injection times of group A and group B were (3.06±1.89) and (5.56±2.04), respectively, and the difference was statistically significant ( t=-3.815, P<0.05). Macular hole and vitreous hematoma were found in 1 eye in group A and 1 eye in group B. Conclusion:Vitrectomy, ILM peeling, subretinal injection of CEIIS and conbercept to treat DME-HE can effectively remove HE, alleviate macular edema, improve BCVA, and reduce CMT and MV. Combination therapy can reduce the number of IVC re-treatments.

Objective:To analyze the correlation between saliva glucose and blood glucose level by measuring the concentration of saliva with high-precision ion chromatograph, and further to provide the clinical data for saliva glucose as an auxiliary index of blood glucose monitoring.Methods:A total of 268 subjects with normal glucose metabolism (NGT) and diabetes mellitus (DM) were enrolled and fasting venous blood and saliva samples were collected at the same time. The levels of saliva glucose, blood glucose and glycosylated hemoglobin (HbA1c) were measured by ion chromatograph, automatic biochemical analyzer and glycosylated hemoglobin analyzer, respectively. Methods of Kruskal-Wallis H test was used for comparison between groups, and the Spearman correlation was used for correlation analysis. Results:The mean values of blood glucose, saliva glucose and HbA1c in the DM group are all higher than those in the NGT group, and the differences are all statistically significant ( P<0.05). Saliva glucose cut-off points are set at 10, 15, 20 and 25 mg/L, respectively. When the saliva glucose concentration is greater than or equal to the above cut-off points, the saliva glucose level are positively correlated with the blood glucose level ( r=0.321, 0.379, 0.509 and 0.428, respectively, P<0.05) . Conclusion:The level of saliva glucose in DM is significantly higher than that in NGT. When the concentration of saliva glucose is greater than 20 mg/ L, there is a significant positive correlation between saliva glucose and blood glucose, and the max correlation coefficient r is 0.509.

OBJECTIVE To study the pharmacokinetic characters of Baihe -zhimu decoction (BZD) and its single herbs . METHODS A liquid chromatography -tandem mass spectrometry method was developed to simultaneously determine the blood concentrations of 7 effective components (neomangiferin,mangiferin,regaloside A ,regaloside Ⅰ,timosaponin B Ⅱ,timosaponin E and timosaponin A Ⅲ)in rats after oral administration of BZD extract ,single herb extract of Lilium brownii and Anemarrhena asphodeloides. The pharmacokinetic parameters were calculated . RESULTS The linear range of 7 effective components as neomangiferin were 1-1 000,1-1 000,0.1-100,0.4-400,1-1 000,0.1-100 and 0.5-500 ng/mL(all r>0.998),respectively. The accuracy of the method ranged from 87.50% to 115.00%,and the RSDs of intra -day and inter -day precision were 0.62%-14.35%. RSDs of matrix factor were 2.68%-14.03%,and deviation of stability were within ±15%. Compared with L. brownii and A. asphodeloides,AUC0-24h,AUC0-∞ and cmax of 7 effective components in BZD were increased significantly (P<0.05),while CL z/Fof 6 effective components (except timosaponin B Ⅱ)decreased significantly (P<0.05). CONCLUSIONS BZD can increase the absorption of effective ingredients in rats ,slow down their elimination and prolong their retention time ,indicating pharmacokinetic behaviors of effective components in BZD possess more advantages over those of single herbs .

Objective To investigate the relationship between cystatin C level and the plasma trough concentration of teicoplanin, so as to provide a reference for the rational application of teicoplanin in clinical practice. Methods The clinical data of the patients receiving teicoplanin, who admitted to our hospital from October 2017 to July 2020 were retrospectively analyzed. The distribution of teicoplanin concentration, the difference of teicoplanin concentration under different cystatin C level, and influence factors for teicoplanin concentration (<15 µg/ml) were analyzed. Results A total of 98 patients including 65 males and 33 females, aged 19 to 94 (52.2±16.2) years old, with 141 trough concentrations were enrolled. The trough concentration of teicoplanin was 11.51 (8.35, 19.07) µg/ml, and the range was 3.57-41.93 µg/ml. 95 cases (67.38%) had teicoplanin concentration <15 µg/ml. When the concentration of cystatin C was >1.05 mg/L, the trough concentration of teicoplanin were 11.37 (8.96, 20.52) µg/ml, significantly higher than those when the concentration of cystatin C was in normal [8.68 (6.34, 11.79) µg/ml, Z=−2.636, P<0.05]. Logistic regression analysis showed that cystatin C level was the influencing factor for teicoplanin trough concentration does not meet the standard (OR=1.529, 95%CI=1.001-2.336, P<0.05). Conclusion The concentration of teicoplanin is significantly increased when the cystatin C level is higher than the normal. Cystatin C level is the influence factor for teicoplanin trough concentration not meeting the standard. The cystatin C level may be considered as a reference for teicoplanin dosage adjustment in clinical practice.

Objective:To establish an analytical method for measuring the concentration of glucose in saliva by ion chromatography.Methods:The proteins in saliva were removed by thermal denaturation method, CarboPac PA20 (3×30 mm) was used as a protective column and CarboPac PA20 (3×150 mm) was used as an analytical column for ion chromatography analysis. Gradient elution was carried out with A: ultra-pure water, B: 250 mmol/L NaOH solution and C: 500 mmol/L NaAc solution. Pulsed ampere detector was used for detection.Results:This method had a good linear relationship in the range of 0.04 to 0.12 mg/L, with a linear relation coefficient of 0.9967. The detection limit of glucose was 2 μg/L, the mean value of the relative standard deviation (RSD) of the repeatability measurement was 0.75%, and the average spike recovery was 103.07%.Conclusion:This method is simple, sensitive, accurate and stable, and can be used for the determination of glucose concentration in saliva.

Objective:To explore the association between urinary stone disease (USD) and peripheral arterial disease (PAD).Methods:The study was based on the cross-sectional chronic diseases survey performed in Pinggu district, Beijing from March to May, 2014. All subjects completed a questionnaire, physical examination, renal ultrasound examination to detect USD, ankle-brachial index (ABI) examination to detect PAD (defined as ABI<0.9 on either side of the body), and brachial-ankle pulse wave velocity (baPWV) measurement to estimate arterial stiffness. Blood and first morning urine sample were detected for serum creatinine, blood glucose and so on.Results:There were 10 281 participants included in this study. Among these participants, the prevalences of USD and PAD were 5.66% and 3.95%, respectively. Compared with non-stone participants, the persistent USD formers had a higher prevalence of PAD (8.26% vs 3.90%, P<0.001) and baPWV [(16.3±3.5) m/s vs (15.5±3.2) m/s, P<0.001]. Even after adjusting the confounding factors, the persistent USD formers also had a 2.066-fold increased risk of PAD ( OR=2.066, 95% CI 1.276-3.343, P=0.003). In the subgroup analysis, persistent USD patients in older participants who were≥60 years old, women, chronic kidney disease, and central obesity had a significantly increased risk of PAD. Conclusions:In the present population, persistent USD is positively associated with a high risk of PAD and increased arterial stiffness. Patients with persistent USD should be screened for vascular diseases.

ObjectiveThe development of new endovascular imaging techniques has optimized surgical treatment strategies. In this paper, we investigated the effect of rotational atherectomy (RA) guided by intravascular ultrasound (IVUS) on long-term postoperative follow-ups.MethodsClinical data of 211 patients who underwent RA in the Department of Cardiology, Nanjing Drum Tower Hospital from November 2011 to December 2018 were retrospectively analyzed, and they were divided into IVUS Group (116 patients) and Non-IVUS Group (95 patients) according to whether they underwent the guidance of intravascular ultrasound or not. Basic information of all patients, coronary artery lesions and details of operation and other clinical data were collected. The long-term prognosis of the patients was collected and compared by telephone or outpatient follow-ups.ResultsThe head diameter, average stent diameter and total hospitalization expenses of the IVUS Group were significantly higher than those of the Non-IVUS Group, and the differences were statistically significant [(1.53±0.19) mm vs (1.46±0.14) mm, P=0.001; (3.09±0.48) mm vs (2.87±0.30) mm, P0.05). Multivariate COX regression analysis showed that the cardiogenic mortality was significantly reduced in the IVUS Group (HR=0.10, 95%CI: 0.02~0.63, P=0.014), but there was no statistically significant difference between the two groups in the incidence of all-cause death and long-term MACE (P>0.05).ConclusionCompared with the Non-IVUS Group, IVUS-guided RA can significantly reduce the incidence of long-term cardiogenic death and total hospitalization expenses.

OBJECTIVE:To provide reference for clinical treatment of Acinetobacter baumannii infection and rational use of antibiotics. METHODS :By retrospective analysis ,64 500 strains of bacteria were isolated from the inpatients of our hospital during Jan. 2015 to Dec. 2018. WHONET 5.6 software was used to analyze the detection rate ,specimen type ,departments of A. baumannii. The resistance of A. baumannii to 18 commonly used antibiotics in 4 years was analyzed by RxC table χ 2 test. RESULTS:A total of 2 072,2 040,2 017 and 2 143 strains of A. baumannii were isolated during 2015-2018,accounting for 12.85%,13.38%,13.60%,11.71% of positive specimens. The main specimen types of 8 272 strains of A. baumannii were sputum(4 368 strains,52.81%),pus(1 106 strains,13.37%),ascites(804 strains,9.72%). The main departments were burn department(1 605 strains,19.40%),hepatobiliary department (1 200 strains,14.51%),brain surgery department (977 strains, 11.81%). The drug resistance rate to 18 kinds of antibiotics showed a wave-like decreasing trend (P<0.001). In 2018,drug resistance rate to ampicillin and aztreonam was more than 80%,and that to ampicillin/sulbactam ,ceftazidime,levofloxacin, Compound sulfamethoxazole ,gentamicin,amikacin,tobramycin and tegacyclin was less than 50% ,among which the drug resistance rate to amikacin and tegacyclin were 14.7% and 0,respectively. CONCLUSIONS :There is no significant change in the number of isolates and detection rate of A. baumannii in our hospital between 2015 and 2018. The bacteria mainly cause respiratory tract infection. Amikacin or tegacyclin are recommended for treatment.

Objective To study the effects of different hemodialysis modes on growth factor-15 (GDF-15) and left ventricular function in uremic patients undergoing maintenance hemodialysis (MHD). Methods One hundred and twenty uremic patients with chronic renal failure whose MHD > 3 months admitted to Guiyang Second People's Hospital from June 2017 to June 2018 were enrolled, and they were divided into a hemodialysis (HD)+ hemofiltration (HDF)+hemoperfusion (HP) group (HD 8 times per month, HDF 4 times per month, HP 1 time per month), a HD+HDF group (HD 8 times per month, HDF 1 time per month) and a HD group (HD 8 times a month) according to different dialysis modes, each group 40 cases. The patients' venous blood was collected before treatment and 6 months and 1 year after treatment, serum was separated, and the GDF-15 levels in the three groups were detected; the left ventricular end-diastolic dimension (LVDD), left ventricular end-systolic dimension (LVDS), left ventricular end-diastolic volume (LVVD), left ventricular end-systolic volume (LVVS), left ventricular posterior wall thickness (LVPWT), ventricular septal thickness (LVST), maximum blood flow ratio (E/A) of early to late diastole and left ventricular ejection fraction (LVEF) in three groups were detected by echocardiography. Results After treatment, the GDF-15 levels and LVDD, LVDS, LVVD, LVVS, LVPWT, LVST and E/A in the three groups were significantly lower than those before treatment, while LVEF was significantly higher than that before treatment (all P < 0.05); the changes after treatment in the HD+HDF+HP group were more significant than those in the HD+HDF group and HD group [GDF-15 (ng): 853.78±78.80 vs. 921.73±72.54, 971.07±72.05, LVDD (mm): 48.25±1.25 vs. 50.67±1.26, 51.69±1.33, LVDS (mm): 35.21±1.01 vs. 37.84±0.90, 38.91±0.83, LVVD (mL): 101.44±4.40 vs. 109.27±6.47, 115.11±5.46, LVVS (mL): 35.75±1.52 vs. 37.75±1.70, 39.48±1.48, LVPWT (mm): 8.26±0.77 vs. 10.24±0.98, 11.22±0.91, LVST (mm): 9.07±0.48 vs. 10.47±0.61, 11.60±0.58, E/A: 1.03±0.05 vs. 1.07±0.06, 1.15±0.08, LVEF: 0.64±0.03 vs. 0.59±0.03, 0.51±0.04, all P <0.05]. Conclusion The combined hemo- dialysis with different hemodialysis modes can effectively reduce the level of GDF-15 in uremic patients with chronic renal failure and MHD and improve their left ventricular function, thus the incidence of cardiovascular events and mortality in such patients can be decreased.

Transvaginal ultrasound is the preferred imaging method for the diagnosis of adenomyosis.The ultrasonographic features of adenomyosis are closely related to its histopathological features.The main sonographic features include spherical enlargement of uterus,asymmetric thickening of anterior and posterior walls,uneven echo of myometrium,linear streaks or island nodules under endometrium,unclear demarcation of endometrium-myometrium,thickening,irregular or interrupted junction zone(JZ),fence-like shadow,intramyometrial cysts,CDFI showing increased blood flow signals in the affected area of myometrium,translesional vascularity and so on.Advanced ultrasound technology,such as cline clips and three-dimensional ultrasound,can complement the shortcomings of conventional ultrasound and improve the diagnostic efficacy of ultrasound for adenomyosis.

Objective To evaluate the indoor effect of six human metabolic compounds for trapping adult Culex pipiens quinquefasciatus. Methods The effects of six human metabolic compounds alone (acetic acid, propionic acid, octanoic acid, lactic acid, 1-octene-3-alcohol and urea alone), liquid lactic acid, acetic acid, propionic acid, octanoic acid, lactic acid or 1-octene-3-alcohol in combination with urea at an equal mass ratio, and lactic acid-urea combinations at various mass ratios, for trapping Cx. p. quinquefasciatus were examined using the trapping method, while the dechlorinated water served as a control. Results The indoor mosquito-trapping efficacy of the six human metabolic compounds was all superior to the dechlorinated water. Acetic acid, propionic acid, octanoic acid, or 1-octene-3-alcohol combined with urea at a mass ratio of 1∶1 had a comparable mosquito-trapping efficacy with acetic acid, propionic acid, octanoic acid, or 1-octene-3-alcohol alone (all P values > 0.05). The lactic acidurea combination at a mass ratio of 1∶1 had a significantly higher mean cumulative trapping capacity [(35.60 ± 8.11) mosquitoes] than lactic acid [(20.80 ± 8.53) mosquitoes], urea [(17.00 ± 7.18) mosquitoes] or dechlorinated water alone (7.20 ± 2.68) (all P values < 0.05). In addition, the lactic acid-urea combinations at mass ratios of 1∶1, 1∶2, 1∶3, 1∶4 or 1∶5 all had significantly greater mosquito-trapping efficacies than lactic acid, urea or dechlorinated water alone (all P values < 0.05), and the optimal combination (lactic acid-urea at a 1∶4 mass ratio) had a mean cumulative trapping capacity of (56.20 ± 9.88) mosquitoes, which was significantly superior to lactic acid [(17.00 ± 3.94) mosquitoes], urea [(16.40 ± 3.78) mosquitoes] or dechlorinated water alone [(7.40 ± 3.44) mosquitoes] (all P values < 0.05). Conclusions The lactic acid-urea combination remarkably increases the indoor trapping capability of Cx. p. quinquefasciatus, and this combination has a weak smell, which is suitable to be used at home and office environments.

RESUMEN

Trimethylamine-N-oxide (TMAO) is a biologically active molecule which is metabolized by intestinal microflora and excreted by the kidney. The incidence and mortality of cardiovascular disease (CVD) in patients with chronic kidney disease (CKD) are significantly increased, which seriously affects the prognosis of patients with CKD. Recent studies have shown that TMAO can cause myocardial fibrosis and cardiovascular damage. Plasma TMAO levels are elevated in CKD patients, and TMAO was an independent predictor for all-cause mortality in patients with CKD, including CVD events. As a "uremic toxin", TMAO can promote the progression and the occurrence of CVD in CKD patients through its toxic effects, such as inflammatory response and oxidative stress. Taken together, it may suggest that reducing TMAO production and plasma TMAO levels may decrease the occurrence of CVD events in CKD and that TMAO may be a therapeutic target for ameliorating the prognosis of CKD.

Objective To determine the fetal facial angles at 11 -38 weeks of gestation by three‐dimensional ultrasound ( 3DUS) and analyze the correlation between facial angles and gestational age( GA ) . Methods From 2013 April to 2014 February ,439 singleton fetuses ranged 11-38 weeks of gestation were enrolled in this study . T he details of mid‐sagittal plane of facial profile was confirmed with 3DUS . Four facial angels were measured in this plane ,including frontomaxillary facial angle ( FM F ) ,frontonasal angle ( FNA ) ,mandibulomaxillary facial angle( M M F) and maxilla‐nasion‐mandible angle( M NM ) . T he intra‐and interobserver reliability were calculated in first 30 cases ,intra‐class correlation coefficient( ICC) greater than 0 .75 indicated good reliability . Pearson′s correlation coefficient ( r ) ,curve estimation and polynomial regression models were used to evaluate the correlation of the fetal facial angles with GA . Results ICC of the same observer were 0 .968 ,0 .962 ,0 .974 and 0 .988 ,respectively . ICC of different observer were 0 .948 , 0 .905 ,0 .874 and 0 .889 ,respectively . T he fetal facial angles of FM F ,FNA ,M M F and M NM showed correlations with GA ( r = -0 .369 ,0 .447 ,-0 .470 ,0 .386 ; all P =0 .000) . Using GA as the independent variable and the facial angles as the dependent variables , the best fit regressing equation was cubic polynomial :FM F=135 .300-6 .473×GA+0 .235×GA2 -0 .003×GA3 ( R2 =0 .240 , P =0 .000 ) ;FNA=58 .920+7 .452×GA -0 .274×GA2 -0 .003×GA3 ( R2 =0 .297 , P =0 .000 ) ;M M F=132 .329 -5 .337× GA+0 .191× GA2 -0 .002× GA3 ( R2 = 0 .304 , P = 0 .000) ;M NM = -24 .592+ 4 .653× GA -0 .173× GA2 + 0 .002 × GA3 ( R2 = 0 .413 , P = 0 .000 ) . Conclusions The development of fetal facial angles are related to GA . T he growing patterns of fetal facial angles fit with a cubic polynomial function .

Objective@#To explore the value of prenatal ultrasound in diagnosis of congenital dacryocystocele.@*Methods@#The ultrasonographic features of 16 fetuses with congenital dacryocystocele were retrospectively reviewed and the outcome of pregnancy were followed up.@*Results@#The median gestational week detected with prenatal ultrasound was 30.29 weeks, the mean diameter was (8.96±1.96)mm. Congenital dacryocystoceles were unilateral in 12 fetuses and bilateral in 4 fetuses, 10 were female and 6 were male. The typical ultrasonic feature was anechoic cystic mass with clear boundary in relation to the medial and inferior aspects of the fetal orbit. The dacryocystocele resolved spontaneously prenatally in 5 fetuses, resolved spontaneously after delivery in 10 fetuses. One fetus died in caesarean section due to complete placenta previa.@*Conclusions@#Congenital dacryocystitis has its characteristic ultrasonographic features, and most cases can disappear naturally in prenatal or early newborns.

AIM:To investigate the effect of down-regulation of X-box binding protein 1(XBP1)expression on the viability and apoptosis of glioma cells.METHODS:The mRNA expression of XBP1 in the glioma tissues was de-tected by qPCR.Small interfering RNA(siRNA)interfering with XBP1 expression(XBP1-siRNA)was transfected into human brain glioma U251 cells.At the same time,control group(the cells without special treatment)and negative control (NC-siRNA)group(transfected with siRNA without any interference)were set up.The mRNA expression of XBP1 in the 3 groups 48 h after transfection was detected by qPCR.The protein levels of XBP1, proliferating cell nuclear antigen (PCNA),B-cell lymphoma/leukemia-2(Bcl-2),Bcl-2-associated X protein(Bax),cyclin D1(cyclin D1), phosphati-dylinositol 3-kinase(PI3K)and phosphorylated Akt(p-Akt)were determined by Western blot.The cell viability was measured by CCK-8 assay.The cell cycle distribution and apoptosis were analyzed by flow cytometry.RESULTS:The ex-pression level of XBP1 in the glioma tissues was significantly higher than that in the tumor adjacent tissues(P<0.05). The XBP1 expression at mRNA and protein levels was significantly decreased in the cells transfected with XBP1-siRNA(P<0.05).No statistically significant difference of the cell viability, cell cycle, apoptotic rate and the protein levels of PCNA,Bcl-2,Bax,cyclin D1,PI3K and p-Akt between NC-siRNA group and control group was observed.Compared with control group,the cell viability, S-phase cells and the protein levels of PCNA, Bcl-2, cyclin D1, PI3K, and p-Akt in XBP1-siRNA group were decreased significantly, and the apoptotic rate, G0/G1-phase cells and Bax protein expression were significantly increased(P<0.05).CONCLUSION:Down-regulation of XBP1 gene expression in brain glioma cells reduces the viability of cancer cells,blocks the cells in G1phase and promote apoptosis.The mechanism is related to the inhibition of PI3K/Akt signaling pathway.
