RESUMEN

2019 novel coronavirus (2019-nCoV) is one of the beta coronaviruses and was identified as the pathogen of the severe "coronavirus disease 2019 (COVID-19)" in 2019. China has formally included the 2019-nCoV in the statutory notification and control system for infectious diseases according to the Law of the People's Republic of China on the Prevention and Treatment of Infectious Diseases. Currently, the national defending actions on the 2019-nCoV in China is in a critical period. Burn Department is also confronted with risk of infection by the 2019-nCoV. According to the guidelines on the diagnosis and treatment of COVID-19 (6th trial edition), the latest relative literature at home and abroad, the features of the COVID-19, recommendations for the COVID-19 prevention and control issued by the National Health Commission of China, and management experience of diagnosis and treatment in the related disciplines, we put forward recommendations for the medical practices of burn treatment during the outbreak of the COVID-19 in outpatient and emergency treatment, inpatient treatment, operation and ward management, etc. We hope these recommendations could benefit the professionals of the same occupation as us and related hospital managers, improve the treatment of burn during the outbreak of the COVID-19, and avoid or reduce the risk of infection of medical staff .

RESUMEN

Fluid therapy is a crucial treatment for patients with extensive burn, which affects patients′prognosis directly. Accurate urine output measurement plays an irreplaceable role in guiding fluid resuscitation in clinic. As one of the best indexes in traditional burn resuscitation, urine output comprehensively reflects systemic circulation. However, it doesn′t fully reflect all the specific chapters of microcirculation and systemic circulation and deficient cellular oxygen metabolism exactly. We need to use urine output combined with other shock parameters to ensure adequate fluid replacement. Currently, the most common way of urine output monitoring is manual measurement. The article reviews the application of urine output monitoring in guiding fluid resuscitation of burn shock.

Objective To investigate the low hemoglobin density (low haemoglobin density,LHD) and red blood cell volume distribution width (red blood cell volume distribution width,RDW) in iron deficiency anemia (iron-deficiency anemia,IDA)in children with the value of diagnosis and treatment.Methods From February 2016 to May 2017,in the Second People's Hospital of Longgang District in Shenzhen City,86 cases of children with iron deficiency anemia for IDA group,and 120 cases of healthy children (as the control group) at the same time were confirmed.Blood routine of children with IDA were detected before and after treatment hemoglobin concentration and the results were analyzed statistically.Results 120 cases of healthy children in the peripheral blood LHD value was 2.74 % ± 0.90 % and the boy was 3.07 % ± 0.81%,higher than that of 2.26 % ± 0.69 % of the girls.Between them,there was statistically significant difference (t=3.815,P<0.05).The value of RDW was 12.37 % ± 2.84 %,12.09 % ± 2.80 % for the boys and 12.56 % ± 2.87 % for the girls,there was no statistically significant difference between the them (t=0.517,P>0.517).Before treatment,86 cases of children with IDA LHD and RDW values in peripheral blood were 30.67 % ± 20.12 % and 16.62 % ± 3.27 %,significantly higher than the control group,the difference was statistically significant between (t=4.025 ~ 4.025,P<0.05 ~ 0.01),and there was no statistically significant difference between male and female children (t =0.761 ~ 0.917,P> 0.05).After treatment L HD and RDW values were 10.65 % ± 8.92 % and 14.02 % ± 2.93 %,significantlylower than before the treatment,between them was statistically significant difference (t=2.912~9.675,P<0.05).Conclusion Children with iron deficiency anemia treatment before the LHD and RDW values significantly elevated in the blood,significantly decreased after treatment,the LHD and RDW values for diagnosis and treatment of children with iron deficiency anemia dynamic monitoring has certain application value,worthy of popularization and application.

<p><b>OBJECTIVE</b>To observe the effects of artificial dermis combined with basic fibroblast growth factor (bFGF) on the treatment of cicatrix and deep skin wounds.</p><p><b>METHODS</b>The clinical data of 72 patients with wounds repaired with artificial dermis, hospitalized in our unit from October 2010 to April 2015, conforming to the study criteria, were retrospectively analyzed. The types of wounds were wounds after resection of cicatrices, deep burn wounds without exposure of tendon or bone, and wounds with exposure of small area of tendon or bone, in a total number of 102. Wounds were divided into artificial dermis group (A, n=60) and artificial dermis+ bFGF group (B, n=42) according to whether or not artificial dermis combined with bFGF. In group A, after release and resection of cicatrices or thorough debridement of deep skin wounds, artificial dermis was directly grafted to wounds in the first stage operation. After complete vascularization of artificial dermis, wounds were repaired with autologous split-thickness skin grafts in the second stage operation. In group B, all the procedures were exactly the same as those in group A except that artificial dermis had been soaked in bFGF for 30 min before grafting. Operation area, complete vascularization time of artificial dermis, survival of skin grafts, and the follow-up condition of wounds in the two groups were recorded. Data were processed with t test and Fisher's exact test.</p><p><b>RESULTS</b>(1) Operation areas of wounds after resection of cicatrices, deep burn wounds without exposure of tendon or bone, and wounds with exposure of small area of tendon or bone in the two groups were about the same (with t values from -1.853 to -0.200, P values above 0.05). Complete vascularization time of artificial dermis in wounds after resection of cicatrices, deep burn wounds without exposure of tendon or bone, and wounds with exposure of small area of tendon or bone in group B were respectively (15.6 ± 2.9), (14.7 ± 2.7), and (20.3 ± 4.4) d, and they were shorter by an average time of 2.7, 4.0, 7.4 d, respectively, as compared with those in corresponding types of wounds in group A [respectively (18.3 ± 4.7), (18.7 ± 4.2), and (27.7 ± 8.8) d, with t values from -2.779 to -2.383, P values below 0.05]. (2) The ratio of skin grafts with excellent survival in the three types of wounds in group B were higher than those in corresponding types of wounds in group A, but there were no statistically significant differences (with P values above 0.05). (3) Patients were followed up for 1 to 48 months, and there were no obvious cicatrices in skin graft sites and the donor sites during the following time.</p><p><b>CONCLUSIONS</b>Artificial dermis combined with bFGF can effectively shorten the vascularization time of artificial dermis in wounds after resection of cicatrices and deep skin wounds.</p>

Objective:To explore in state of Chlamydia trachomatis persistent infection,the STAT3-TLR2 axis may be activated and mediating abnormal secretion of inflammatory cytokines.Methods: We established acute infection and IFN-γinduced persistent infection model of Ct in HeLa cell.Gene transcription, cytokine secretion and protein expression were detected by using qRT-PCR, ELISA and Western blot respectively in STAT3-TLR2 signaling axis in each Ct infection model.Results: Persistent Ct infections upregulated the transcription of TLR2,significantly increased both the secretion of inflammatory cytokine IL-1αand the expression of STAT3 and TLR2,moreover,enhanced the activation of STAT3 simultaneously.Conclusion: In the Ct persistent infection induced by IFN-γ,the STAT3-TLR2 signaling axis activated significantly in HeLa cell.

Objective To investigate the clinical application value of point-of-care combined detection of cTnI,MYO and CK-MB in the patients with acute myocardial infarction (AMI).Methods 74 cases of coronary syndrome were collected as the patients group,including 36 cases of AMI and 38 cases of unstable angina pectoris(UAP),and 40 individuals with healthy physical examina-tion were selected as the control group.The concentrations of cardiac marker cTnI,MYO and CK-MB in blood at different times were detected by using the point-of-care rapid immune quantitative method.Then the sensitivity and specificity for diagnosing AMI were compared for determining the best time of single detection and combined detection.Results The positive rates of cTnI,MYO and CK-MB within onset 2-12 h with the symptoms in the AMI group were higher than those in the control group and the UAP group,the difference had statistical significance (P <0.01).The combined detection of cTnI,MYO and CK-MB within onset 6-12 h with symptoms had the higher sensitivity and higher specificity for diagnosing AMI.The combined detection of cTnI and CK-MB within onset 12-24 h with symptoms could reach the optimal sensitivity and optimal specificity for the diagnosis.The sensitivity and specificity of cTnI within onset 24-72 h with symptoms for diagnosing AMI were 100.0% and 100.0% respectively.Conclu-sion The point-of-care combined detection of cTnI,MYO and CK-MB can conveniently and rapidly diagnose AMI.The sensitivty and specificity of various indexes in different time periods are different.Their combined detection can increase the diagnostic rate for AMI.

Objective To study on the relationship between the levels of homocysteine(HCY) ,folic acid and vitamin B12 and pregnancy-induced hypertension(PIH) in different pregnancies .Methods 539 pregnant women who registered for prenatal exami-nation of pregnant in the hospital were selected as research subjects .And there were 87 cases of PIH(PIH group) and 452 cases of normal pregnancy(normal pregnancy group) among them .The fasting blood samples were collected respectively in early pregnancy (8-10 weeks and 12-14 weeks of pregnancy) ,mid pregnancy(18 pregnancy weeks and 24 pregnancy weeks) ,and late pregnancy (30 pregnancy weeks and 36 pregnancy weeks) ,and the levels of HCY ,folic acid and vitamin B12 were measured .At the same time ,the supplements of folic acid and vitamin B12 and the incidence of PIH and birth defects were asked ,registered and checked . Results Compared with normal pregnancy group ,the serum HCY level of PIH group significantly increased in medium and late pregnancy periods (P0 .05) .In mid and late pregnancy periods , the serum HCY levels of PIH group and normal pregnant group negatively correlated with serum folic acid levels (r0 .05) .Conclusion In middle and late pregnancy periods ,if the serum HCY level of pregnant women increased ,the risk of PIH increased significantly .

<p><b>OBJECTIVE</b>To compare the dissolution properties of Ginseng micropowder and common powder in vitro and investigate the effect of micronization on dissolution of Ginseng.</p><p><b>METHODS</b>Five ginsenosides including Rg1, Re, Rf, Rb1, and Rb2 were determined by high performance liquid chromatography, and the dissolution curves of Ginseng micropowder and common powder were drawn.</p><p><b>RESULTS</b>The dissolution rates of ginsenosides from micropowder exceeded 90% within 5 min, significantly greater than that from common powder. But when the powders were packed in tea bags, the dissolution rates of ginsenosides were higher from common powder than from micropowder.</p><p><b>CONCLUSION</b>Micronization treatments can promote dissolution of ginsenosides from Ginseng.</p>

Objective To study on the characterization of MICA/B genetic polymorphism in a northern Chinese Hunan Han population.Methods Ninty-five unrelated individuals were involved in this study and MICA/B genotypes were determined by two methods:PCR-sequence-specific primers (PCR-SSP) and PCR-sequence-based typing (PCR-SBT).Results In northern Hunan Han population,eleven MICA alleles were found,among which MICA * 010 (28.95%),MICA * 008 ∶ 01 (20.53%) and MICA * 002 ∶ 01 (15.79%) were the common alleles.Five MICA-STR(short tandem repeat) alleles were found,among which MICA * A5 (37.89%) and MICA * A5.1 (21.05%) predominated.In this population,ten MICB alleles were found.The common alleles were MICB * 005 ∶ 02/* 010 (58.42%),MICB * 002 ∶ 01 (10.00%),and MICB * 008 (7.89%).Two kinds of MICA-MICB haplotypes were MICA * 004-MICB * 004 ∶ 01 and MICA * 010-MICB * 005 ∶ 02/010 in significant linkage disequilibrium.This study also showed MICA/B gene with high polymorphism in different populations.Conclusion MICA/B alleles distribution in northern Hunan Han population with its unique characteristics.

RESUMEN

Poly(epsilon-caprolactone) microspheres were fabricated with an average particle size of 5.08 +/- 0.23 microns. The effect of poly(epsilon-caprolactone) microspheres on apoptosis and cell cycle of fibroblast was studied with flow cytometry. The data obtained clearly indicated that poly(epsilon-caprolactone) microspheres purified in different ways showed different cytocompatibility. The well purified microspheres have good cytocompatibility.

<p><b>OBJECTIVE</b>To investigate the effectiveness and the safety of clinical use of rhGH in the management of severe burn patients.</p><p><b>METHODS</b>Two hundred burn patients aged 20 - 25 yrs with TBSA from 20% to 50% and admitted within 5 postburn days (PBD) were enrolled in this study. The patients were divided into operation and non-operation groups. Furthermore, each group was randomly divided into control (N), treatment with rhGH in dosage 0.2 IU/Kg/d (A) and treatment with rhGH in dosage 0.4 IU/kg/d (B) groups. The general data, the metabolism of protein and glucose, the immune function, the urine biochemistry, the wound healing rate and hospital stay days recorded and compared among all groups.</p><p><b>RESULTS</b>Plasma protein was increased, the immune function recovered early, the wound healing time was shortened obviously, the wound healing rate was increased evidently and the hospital stay days were decreased significantly in the treatment groups (A and B groups) compared with the N group. The body weight of burn patients remained unchanged in N group. Nevertheless, the plasma levels of glucose and insulin were increased and the urine output of potassium, sodium and chloride decreased in N group compared with those in A and B groups. Moreover, the levels of all these indices in B group were higher than those in A group, whereas side effects were more common in B group than in A group. In addition, the incidence of adverse reaction was higher in operation group than that in non-operation group.</p><p><b>CONCLUSION</b>rhGH could effectively promote protein synthesis, enhance systemic immune function, accelerate wound healing and shorten hospital stay days in severely burned patients. But attention should be paid to appropriate dosage and duration of use.</p>

RESUMEN

Objective To explore the feasibility and safety of massive lung lavage to dogs in the early stage after acute serious smoke inhalation. Methods A total of 16 dogs were inflicted with smoke inhaled injury and then divided into group A (n=5) without lung lavage, group B (n=6) in which left lung lavage was followed by lavage on the right 30 min later, and group C (n=5) in which the interval of the lavages on the left and right lung was 4 h. Massive lung lavage was carried out in 1 h after the injury under the guiding fiberoptic bronchoscopy and a special ventilation jet was served to supply enough oxygen to the dogs. The stability of pulmonary function and blood circulation were observed at different time points in every group. Results The average operating time of unilateral lung lavage was about 15 minuted and the liquid volume left in every lung after operation was from 100 ml to 140 ml when the total lavaging volume was kept in 250 ml. The changes of hemodynamic parameters were identical in Group B and Group C. Inhalation injury had certain negative effect while lung lavage almost had no effect on the blood circulation of the animals in 2 lavage groups. No serious cardiac arrythemia was found in them during the lavaging. Pulmonary function in Group C was better than that in Group B but worse than that in Group A. In the 2 lavaging groups, PaO2 remained higher than 9.33 kPa(FiO2=0.4)during and after the lavaging. Conclusion The ventilation mode and the lavage method introduced here ensure the safety of massive lung lavage in dogs with acute smoke serious inhalation injury.

Objective To review the experience in the management of burned patients inflicted by inhalation injury in our institute in the past 42 years. Methods Patients with inhalation injury were analyzed chronically in three different periods as 1958 to 1980, 1981 to 1990, and 1991 to 2000. Results The mortality rates in general and in terms of different degrees decreased obviously with the elapse of time. In other words, the rates were 48.93%, 26.60% and 8.53 for the above three periods. Conclusion The mortality of inhalation injury was decreased dramatically during the last decade in our institute. Five main measures consisting of inhalation of high concentration or pure oxygen, preventive tracheotomy, frequent tracheal lavage with small amount of lavage fluid at early stage of postburn, mechanical ventilation with PEEP and humidification as early as possible, and application of exogenous pulmonary surfactant contribute greatly to the decrement of death rate in the patients with inhalation injury.

Objective To study the effect of acute serious smoke inhalation injury to unilateral lung on contralateral lung and the therapeutic efficacy of massive bronchoscopic lavage with saline to the primary injured bronchoalveolus at early stage after smoke inhalation on mitigating the secondary damage of the other lung and so as to confirm the therapeutic validity of lung lavage to smoke inhalation injury. Methods Fifteen mongrel dogs were insufflated with sawdust smoke into left lung and then randomized into 2 groups. The dogs in group A were maintained intravenous glucose saline transfusion and breathed air freely for 24 h after injury. Those in group B received a massive bronchoscopic lavage with 250 ml of saline to injured lung at 1 hour after injury and then were administrated similarly to those in group A. Gas exchange function of bilateral lungs in the process were observed and pathologic and phyiopathologic changes of the lung specimen were examined after the process. Results While the total lavaging volume was kept to 250 ml, the liquid volume remained in left lung after each operating was from 90 ml to 140 ml. All animals had a significant decline in pulmonary function after smoke inhalation injury. PaO2 in group B decreased significantly soon after the lavage and then increased gradually to exceeding that in group A. The left lung showed serious edema similarly in two groups but in right lung the water content was less, dynamic and static compliance was more in group B than in group A. Conclusion Bronchoalveolar massive lavage at early post-injury stage to injured lung after unilateral lung smoke inhalation injury can mitigate secondary damage of the non-injured lung. Lung lavage can decrease the intensity of secondary systemic inflammatory reaction and show a therapeutic validity to smoke inhalation injury.

Objective To investigate effects of the complex therapy on severe inhalation injury. Methods The cases with inhalation injury and acute respiratory failure (ARF) of 4172 consecutive burn patients from Jan. 2000 to Feb. 2003 in our institute were studied retrospectively. Results 30 of the 128 patients with inhalation injury occurred acute ARF. 48 hours after the exogenous pulmonary surfactant (PS) was used to treat 6 cases with uncomplicated ARF, pulmonary function obviously ameliorated as compared with routine therapy. Objective: To evaluate the effect of partial liquid ventilation with perfluorocarbon on the static lung compliance in rat with smoke inhalation injury. Method: Partial liquid ventilation (PLV)and mechanical ventilation (MV)were set up on rat's model,total static lung compliance (C_(2.94))and low volume static lung compliance (C_(0.49))were measured with hydraulic pressometer,the expansion index(EI)of lung was calculated. Object To study the HPCE fingerprinting of Gegen Qinlian Decoction (GQD). Methods A buffer was composed of 30 mmol/L sodium phosphate and 40 mmol/L borate solution. Capillary electrophoresis was performed using a 65 cm (43 cm to detector) ?50 ?m fused-silica capillary tube. Separation voltage was 22 kV, sampling time was 1 s, detected wavelength was 254 nm, and the temperature was maintained at 30 ℃. Results The 27 components in GQD were successfully separated. The observation of methodology was in keeping with quantitatine determination and qualitative study. Conclusion This method can be used for the quality control of the preparation of GQD with good precision.

Object To develop a high-performance capillary electrophoresis method to determine ferulic acid concentrations in serum of rats treated with Buyang Huanwu Decoction (BHD). Methods Capillary zone electrophoresis was applied for ferulic acid assay, quantitative determination was based on internal standard and detection was carried on by direct UV. The electrolyte buffer was composed of 25 mmol/L borax-methanol (85∶15). Capillary electrophoresis was performed using a 52 cm (30 cm to detector)?50 ?m fused-silica capillary tube. Separation voltage was 20 kV, sampling time was 3 s, detection wavelength was 320 nm, and the temperature was 30 ℃. Results Ferulic acid was successfully separated within 6 min, the recoveries were 97.2% in serum and 103.28% in BHD, respectively. RSD were 3.73% and 0.91% (n=3), respectively. Conclusion This method can supply reference for the determination of ferulic acid in serum samples and BHD.

AIM: To analyse the main chemical constituents of Shuanghuanglian Injection Power(Flos lonicerae japanicae,Radix scutellariae and Fructus forsythiae) by high performance liquid chromatography-electrospray ionization-mass/mass spectrometry. METHODS: The sample solution was analysed by a Zorbax C_18 column with a gradient mobile phase comprised of acetonitrile and water.Electrospray ionization mass spectrometry detector was used,full-scan detection mode was evaluated for the identification of the liquid chromatography peaks. RESULTS: To analyse the mass spectrum of every liquid chromatography peaks,we identified 18 molecular mass from the ion chromatogram of Shuanghuanglian Injection Powder,9 compounds were identified,including quinic acid,caffeic acid,three monocaffeoylquinic acids,dicaffeoylquinic acid,forsythin,forsythoside and baicalin. CONCLUSION: Quinic acid is identified from Shuanghuanglian Injection Power for the first time.High performance liquid chromatography-mass/mass spectrometry is emerging as an important tool in traditional Chinese medicine.
