RESUMEN

A systematic approach was developed to investigate the stability of gentamicin sulfate (GS) and GS/poly (lactic-co-glycolic acid) (PLGA) coatings on hydroxyapatite surfaces. The influence of environmental factors (light, humidity, oxidation and heat) upon degradation of the drug in the coatings was investigated using liquid chromatography with evaporative light scattering detection and mass spectrometry. GS coated rods were found to be stable across the range of environments assessed, with only an oxidizing atmosphere resulting in significant changes to the gentamicin composition. In contrast, rods coated with GS/PLGA were more sensitive to storage conditions with compositional changes being detected after storage at 60 °C, 75%relative humidity or exposure to light. The effect of γ-irradiation on the coated rods was also investigated and found to have no significant effect. Finally, liquid chromatography–mass spectrometry analysis revealed that known gentamines C1, C1a and C2 were the major degradants formed. Forced degradation of gentamicin coatings did not produce any unexpected degradants or impurities.

RESUMEN

Lemierre’s syndrome is a rare disease that affects young adults and is mainly caused by Fusobacterium necrophorum and occasionally by other anaerobic bacteria of the species. This syndrome is characterized by a throat infection complicated with septic thrombophlebitis of the internal jugular vein and septic emboli mainly to the lungs. In the pre-antibiotic era its evolution was often fatal. Since the 1960’s this syndrome has rarely been reported given the extensive use of penicillin en pharyngeal infections. Currently the incidence of Lemierre’s syndrome is about one in a million. Currently, since imaging has a key role in the early diagnosis of this syndrome, the radiologist should be aware of and recognize its manifestations. Three cases of Lemierre’s syndrome and a literature review are presented.


El síndrome de Lemierre es una enfermedad rara que afecta a adultos jóvenes y es causada principalmente por Fusobacterium necrophorum y ocasionalmente por otras bacterias anaerobias de la especie. Este síndrome se caracteriza por una infección faríngea complicada con tromboflebitis séptica de la vena yugular interna y embolias sépticas principalmente a los pulmones. En la era pre-antibióticos su evolución era frecuentemente de curso fatal. Desde los años 60 este síndrome ha sido rara vez reportado dado el extensivo uso de penicilina en infecciones faríngeas. Actualmente la incidencia del síndrome de Lemierre es de aproximadamente uno en un millón. Dado que hoy en día las imágenes tienen un rol fundamental en el diagnóstico temprano de este síndrome el radiólogo debe estar al tanto y reconocer sus manifestaciones. Se presentan tres casos de Síndrome de Lemierre y una revisión de la literaratura.

Automotive industry in Malaysia is one of the booming industries which encompass the design, development and manufacturing of motor vehicles. However, it has its own setback as the interaction between complex tools, machines, and instruments, coupled with humans as workers pose several health hazards. A cross-sectional study was conducted to determine the prevalence of musculoskeletal disorders (MSD) and the association with awkward posture among automotive assembly line workers. A simple random sampling method was adopted and data were collected based on Standardized Nordic Questionnaire (SNQ) and Rapid Upper Limb Assessment (RULA) method for analysing awkward posture. With a response rate of 83%, a total of 232 assembly line workers with at least one year job tenure participated in this study. The fi ndings revealed that 78.4% of workers reported MSD while the highest percentage of complaints concerned the lower back (50.9%). Three factors were found to be signifi cantly associated with MSD: age (χ2 =5.61, p=0.018), job tenure (χ2 = 8.26, p=0.004) and awkward posture (χ2 = 65.37, p < 0.001). Logistic regression analysis indicated that signifi cant risk factors for MSD symptoms were workers aged ≥ 25 years old (OR= 2.25, 95%CI 1.14-4.46) and those workers with equal and more than three years job tenure (OR= 2.44, 95%CI 1.04-5.63). In addition, workers in the very high and high RULA action level who were 69 times (OR = 69.38, 95%CI 14.51- 331.73) and 12 times (OR = 12.42, 95%CI 5.21-29.58), respectively, also had higher odds of complaints of MSD. The high prevalence of MSD shows that MSD symptoms is a signifi cant problem among automotive assembly line workers while age, job tenure and awkward posture based on RULA action level are the signifi cant factors for MSD. In particular, this study proves that the prevalence of MSD increases as the RULA action level and job tenure increases. Thus, this problem could be reduced by decreasing RULA action level through appropriate ergonomic workstation design and ergonomic training for workers.

Introduction: Health care workers (HCW) are constantly exposed to blood-borne illnesses through needle stick injuries (NSI). Despite the increasing trend of NSI, evidence regarding the actual practice of universal precautions among these HCWs is lacking. This study assessed the practice of universal precautions towards prevention of NSI among HCWs in a teaching hospital setting. Methods: This cross-sectional survey involved a newly-designed self-completed questionnaire assessing demographic data, exposure to NSI and practice of universal precautions. Questionnaires were distributed to every ward and completed questionnaires were collected after a period of 7 days. Results: A total of 215 HCWs responded to the survey. 35.8% were exposed to bodily fluid, with 22.3% had NSI in the last 12 months. Blood taking was the commonest procedure associated with NSI. Of practices of universal precautions, recapping needle and removing needle from syringe were still wrongly practiced by the HCWs assessed. Conclusion: NSI among HCW are still common despite the introduction of universal precautions in our hospital. Incorrect practices in handling sharps should be looked into in order to reduce the incidence of blood-borne illnesses through NSI in the hospital.

Lead is not an essential component of the human body, but it is always present due to extensive exposure and easy absorption in various forms. Lead affects virtually all systems of the body. Several metals can indirectly evoke the generation of Reactive Oxygen Species (ROS) by way of disruption of normal calcium homeostasis. This is true, especially at high levels of blood lead. This study was undertaken to see if lead at low levels causes oxidative stress and damage. The level of total antioxidant status in blood was measured to indirectly indicate the extent of oxidative damage, and this was correlated with the blood lead levels. The results reveal that even at blood lead levels as low as 14.15μg/dl ± 4.8μg/dl, there is a significant decrease in the levels of total antioxidant status. The total antioxidant levels were 0.78 ± 0.25mmol/l in the lead-exposed as against 1.5 ± 0.76 mmol/l in the unexposed. This study supports the introduction of antioxidants along with chelation, as a regular treatment for lead poisoning cases, as also as a preventive measure against lead poisoning in persons exposed to different sources of lead.
