RESUMEN

To determine the fibrinoid necrosis and hyalinization extent in placenta observed in normal, diabetic and hypertensive pregnancies. Comparative cross-sectional study. Institute of Basic Medical Sciences, Dow University of Health Sciences, Karachi, from 2008-2010. One hundred and fifty placentae were divided in three groups on the basis of their histories and clinical examination. Group A [control], Group B [Diabetic] and Group C [Hypertensive], each consisted of 50 samples. The samples were transferred to Dow Diagnostic Reference and Research Laboratory for histopathology and gross examination. The tissue samples were taken from different sites, processed and routine staining done. The slides were then examined under light microscope for hyalinization and fibrinoid necrosis. The data was analyzed by applying ANOVA and post-hoc Tukey at 95% confidence interval. Mean +/- standard deviations [SD] were computed. The mean number of hyalinized villi in control group was 0.54 +/- 0.908, 1.18 +/- 1.9540 in the diabetic group and 2.14 +/- 1.863 in the hypertensive group. The difference in their average turned out to be statistically significant [p-value < 0.001]. Mean number of villi having fibrinoid necrosis was statistically significant in both the diabetic and hypertensive groups as compared to the control group i.e. 13.98 vs. 4.02 and 10.08 vs. 4.02 respectively [p-value < 0.001]. There was significantly greater fibrinoid necrosis and hyalinization in placentae from mothers having diabetes and hypertension. The fibrinoid necrosis was seen more in diabetic group as compared to hypertensive and control, while hyalinization was observed more frequently in hypertensive group as compared to the other groups. Placental changes as seen in examination of delivered placentae will be helpful in preventing the adverse effects in successive pregnancies

RESUMEN

The Norplant is a long term sub-dermal implantable contraceptive devise consisting of six capsule each containing 36mg of Levonorgestel [Progestrone]. It is implanted on the inner surface of the arm of the women for the period of five years, and is slowly released from the Norplant capsule into women's blood stream and acts as a contraceptive. The aim of the study was to find out the effect of Norplant on the weight of the uterus of the female albino rat on gross examination. SETTING: The study was conducted in BMSI, JPMC Karachi over a period of 65 days. MATERIAL AND METHODS: In the present study, the total number of 60 adult female albino rats were selected, divided into 6 groups A,B,C,D,E and F. Each group except A was comprising of 6 animals. Group A, acted as an age matched control, A was subdivided into AI, A2, A3, A4 AND A5. all comprising of six animals to act as age matched control for group B, C, D, E and F. All the animals in the respective groups received subdermal insertion of single rod of Norplant. The animals were sacrificed at the interval of 13 days and tissue [uterus] was dissected out under aseptic conditions and their weight was done on sartorius balance. RESULTS AND OBSERVATIONS: The statistical analysis of comparison between different groups were carried out by applying student's test. The weight of the Uterus revealed a highly significant increase with the length of the treatment. The conclusion drawn from this study is that the Norplant on one hand is no doubt acting as a contraceptive but on the other hand, leading to adverse effects such as increase in weight of the tissue [Uterus]due to fibrosis and distortion and the structure of the uterus which may lead to evil ailment as cancer

RESUMEN

To study noticeable changes in the status of endometrium of uterus in female albino rats with long term contraceptive Norplant. Design: Light microscopic study of uterus of female albino rats with Norplant therapy. Setting: Department of Anatomy, Basic Medical Sciences Institute, Jinnah Post Graduate Medical Centre, Karachi. Subjects: Sixty Six adult female albino rats were divided into six group A, B, C, D, E and F, and each group comprising of six animals, group 'A' served as age-match control, group 'B', 'C', 'D', 'E' and 'F' received subdermal insertion of single rod of Norplant at the interval of 13 days respectively. All the animals were sacrificed at their schedule time and uterus was removed, processed for paraffin embedding and stained with H and E and also with special stains. All the observations were statistically analysed. In Norplant treated animals the thickness of endometrium demonstrated a highly significant increase in groups 'B', 'C', 'D', 'E' and 'F' when compared with age-matched controls. The height of epithelial cells showed a highly significant decrease, the connective tissue fibres also showed a marked increase. On the basis of present study, it is concluded that Norplant initiate cellular injury which causes damage to the tissue and produces fibrosis
