RESUMEN

The prognosis of patients with peritoneal metastasis from colorectal cancer is poor. At present, the comprehensive treatment system based on cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) has significantly improved the survival of these patients. However, CRS and HIPEC have strict indications, high procedural difficulty, and high morbidity and mortality. If CRS+HIPEC is performed in an inexperienced center, overall survival and quality of life of patients may bo compromised. The establishment of specialized diagnosis and treatment centers can provide a guarantee for standardized clinical diagnosis and treatment. In this review, we first introduced the necessity of establishing a colorectal cancer peritoneal metastasis treatment center and the construction situation of the diagnosis and treatment center for peritoneal surface malignancies at home and abroad. Then we focused on introducing our construction experience of the colorectal peritoneal metastasis treatment center, and emphasized that the construction of the center must be done well in two aspects: firstly, the clinical optimization should be realized and the specialization of the whole workflow should be strengthened; secondly, we should ensure the quality of patient care and the rights, well-being and health of every patient.

OBJECTIVE@#To observe the occurrence time of neuralgia and the expression of purinergic ligand-gated ion channel 7 receptor (P2X7R) in the dorsal horn of the spinal cord after intraperitoneal injection of streptozotocin (STZ) in diabetic rats, and to explore the effect of electroacupuncture (EA) and pretreatment of EA on the heat pain threshold and expression of P2X7R in the spinal dorsal horn in rats with diabetic neuropathic pain (DNP), and to explore the possible mechanism of EA for DNP.@*METHODS@#PartⅠ: Thirty male SD rats were randomly selected from 64 male SD rats as the control group; the remaining rats were given intraperitoneal injection of STZ (10 mg/mL) at a dose of 65 mg/kg to establish the diabetes model, and 30 rats were successfully modeled as the model group. The control group and the model group were divided into three subgroups respectively at 7, 14 and 21 days, with 10 rats in each subgroup. Body mass, fasting blood glucose (FBG) and thermal pain threshold were recorded at 7, 14 and 21 days after injection; the expression of P2X7R in spinal dorsal horn was detected by Western blot. PartⅡ: Eight SD rats were randomly selected from 35 male SD rats as the blank group, and the remaining 27 rats were given intraperitoneal injection of STZ (10 mg/mL) at a dose of 65 mg/kg to establish the diabetes model. The 24 rats with successful diabetes model were randomly divided into a DNP group, an EA group and a pre-EA group, 8 rats in each group. Fifteen to 21 days after STZ injection, the EA group received EA at "Zusanli" (ST 36) and "Kunlun" (BL 60), continuous wave, frequency of 2 Hz, 30 min each time, once a day; the intervention method in the pre-EA group was the same as that in the EA group. The intervention time was 8 to 14 days after STZ injection. The body mass, FBG and thermal pain threshold were recorded before STZ injection and 7, 14 and 21 days after STZ injection; the expression of P2X7R in spinal dorsal horn was detected by Western blot 21 days after injection.@*RESULTS@#PartⅠ: Compared with the control group, in the model group, the body mass was decreased and FBG was increased 7, 14 and 21 days after STZ injection (P<0.01), and the thermal pain threshold was decreased 14 and 21 days after STZ injection (P<0.05), and the expression of P2X7R in spinal dorsal horn was increased 7, 14 and 21 days after STZ injection (P<0.05, P<0.01). PartⅡ: Compared with the blank group, in the DNP group, the body mass was decreased and fasting blood glucose were increased 7, 14 and 21 days after STZ injection (P<0.01). Compared with the DNP group, in the pre-EA group, the heat pain threshold was increased 14 and 21 days after STZ injection (P<0.05), while in the EA group, the heat pain threshold was increased 21 days after STZ injection (P<0.01), and the expression of P2X7R in the dorsal horn in the EA group and the pre-EA group was decreased (P<0.01).@*CONCLUSION@#The diabetic neuropathic pain is observed 14 days after STZ injection. EA could not only treat but also prevent the occurrence of DNP, and its mechanism may be related to down-regulation of P2X7R expression in the dorsal horn of the spinal cord.

Objective:To systematically review the relationship between CYP3A4*1G gene polymorphism and the analgesic effect of fentanyl-related drugs.Methods:The related literature about the effect of gene polymorphism on analgesic effect of fentanyl-related drugs published at home and abroad was searched in PubMed, Embase, Cochrane Library, CNKI, Wanfang and China Biomedical Literature databases from database establishment to August 2019. According to the inclusion and exclusion criteria, two researchers screened the literature independently, extracted the data and evaluated the methodological quality. Data analysis was performed by Review Manager 5.1 software.Results:Six literature was enrolled with 1 050 cases, including 543 cases of wild-type homozygote (CC), 437 cases of heterozygote (CG) and 70 cases of mutant homozygote (GG). The results showed that there was no significant difference in pain score between the three groups in a pairwise comparison at 24 hours after operation (all P > 0.05), but the consumption of fentanyl-related drugs in GG group was lower than that in CC group and CG group, and the differences were statistically significant [GG group vs. CC group: standardized mean difference (SMD) = -0.78, 95% CI -1.03- -0.52, P<0.01; GG group vs. CG group: SMD = -0.61, 95% CI -0.87- -0.35, P<0.01]. Conclusions:CYP3A4*1G gene polymorphism can affect the postoperative analgesia effect of fentanyl-related drugs. With the same analgesic effect, the consumption of fentanyl-related drugs in GG patients is reduced.

Objective To explore the determinants of infants overweight and obesity using generalized estimation equation (GEE). Methods Data were extracted from 26 624 2-year old infants who completed required health examination from January 2017 to December 2019 in Minhang District of Shanghai.The weight for height at 6, 12, 18 and 24 months of age was calculated to evaluate nutrition status of infants.Ordinal multinomial GEE was fitted with malnutrition, normality, overweight and obesity as responding variables to explore the determinants of infants overweight and obesity. Results Incidence rate of overweight and obesity in infants of 6 to 24 months of age had a tendency of decline, and proportion of normality showed as increasing tendency along with the increase of months of age(P < 0.001).GEE showed that the following factors were more likely involved in infant overweight and obesity: male(OR=1.182), lower education level of mother (OR=1.399 for primary school, OR=1.124 for junior high school, and OR=1.083 for senior high school, respectively), higher body weight of father(OR=1.003), higher pre-pregnant body weight of mother(OR=1.003), longer gestational age(OR=1.058), longer sleeping time at 6 month of age(OR=1.032), lower month of age (OR=2.911 for 6 months, OR=1.952 for 12 months, and OR=1.232 for 18 months, respectively).Those with low birth weight (OR=0.205 for < 2 500 g, and OR=0.410 for 2 500~3 999 g, respectively)and exclusive breastfeeding at 6 months of age(OR=0.946)had low likelihood of overweight and obesity. Conclusion Comprehensive measures should be taken to control infants overweight and obesity, including encouraging expectant parents to maintain normal body weight before pregnancy, strengthening the education of scientific feeding knowledge, and extending exclusive breastfeeding.

Objective::To explore the effect of Tongxie Yaofang on p38 mitogen activated protease(p38 MAPK), mitogen and stress protein kinase 1(MSK1), cyclic adenosine effector response element binding protein(CREB)mRNA and protein expression in colon tissue of diarrhea type irritable bowel syndrome (D-IBS) rat model with liver depression and spleen deficiency(GYPX), and interleukin-1β(IL-1β), interleukin-6(IL-6), tumor necrosis factor-α(TNF-α), and the content of total superoxide dismutase (T-SOD) and malondialdehyde (MDA) in serum. Method::The 60 SPF Wistar rats were randomly divided into 6 groups, with 10 rats in each group. Except the normal group, rats in the other groups were given by gavage with folium sennae and chronic bondage to establish D-IBS with GYPX for 14 days. The low, medium, and high doses Tongxie Yaofang were administered to Tongxie Yaofang(2.25, 4.5, 9 g·kg-1)gavage respectively. The piveronium bromide group was given piveronium bromide tablets suspension(0.02 g·kg-1)gavage.The normal group group and model group were given the same volume normal saline for 21 days. After the last gavage for 18 hours, the heart blood was collected and the colon tissue was dissected. Real-time PCR was used to observe the expression of p38 MAPK, MSK1 and CREB mRNA in rat colon. Western blot was used to observe the expression of p38 MAPK, MSK1 and CREB protein. ELISA was used to observe the contents of IL-1β, IL-6 and TNF-α in colon. Hydroxylamine was used to observe the T-SOD level in serum, thiobarbituric acid(TBA)was used to observe the MDA content in serum. hematoxyl in-eosin(HE) staining was used to observe the morphological changes of colon tissues. Result::Compared with normal group, the expression of p38 MAPK, MSK1, CREB mRNA and the protein content of p38 MAPK, MSK1 and CREB in the colon tissue of model group rats increased significantly, while the content of IL-1β, IL-6 and TNF-α increased significantly(P<0.05). The level of serum T-SOD decreased significantly, and the content of MDA increased significantly(P<0.05). Compared with the model group, the medium and high dose group of Tongxie Yaofang significantly decreased the expression of p38 MAPK mRNA, content of p38 MAPK, CREB protein and IL-1β, IL-6, TNF-α in colon tissue(P<0.05). The level of serum T-SOD increased significantly, and the content of MDA decreased significantly(P<0.05). High dose group of Tongxie Yaofang can significantly decreased the expression of MSK1, CREB mRNA, content of MSK1 protein(P<0.05). Histopathological observation showed that no significant organic lesions were observed in the colonic morphology of each group of rats, which was consistent with the morphological characteristics of IBS. Conclusion::Tongxie Yaofang has a significant dose-effect relationship in the treatment of D-IBS rats with GYPX in a certain range, which may be related to its increases antioxidant stress and inhibit activation of p38 MAPK signaling pathway and reducing the level of downstream inflammatory factors.

Objective To systematically evaluate the efficacy of the lienal polypeptide injection combined with chemotherapy or radiotherapy on the short-term efficacy, Karnofsky score, immune function and adverse reactions in treatment of various cancers. Methods The databases of PubMed, Embase, Cochrane Library, CNKI, Wanfang and VIP were retrieved from database establishing time to January 2017, and the randomized controlled trials (RCT) about lienal polypeptide injection combined with radiotherapy or chemotherapy in the treatment of advanced cancer were collected. The quality assessment was conducted and eligible trials were included in the Meta-analysis. Results A total of 23 articles were included, involving 1658 patients. The patients were divided into treatment group (lienal polypeptide injection combined with chemotherapy or radiotherapy, 842 cases) and control group (chemotherapy or radiotherapy alone, 816 cases). Results of Meta-analysis showed that the short-term effective rate (RR = 1.20, 95% CI 1.07-1.33, P = 0.001), the improvement rate of Karnofsky score (RR = 1.77, 95% CI 1.43-2.19, P < 0.05) and immune function related indicators: CD3+ (MD = 9.48, 95% CI 6.76-12.20, P < 0.01), CD4+ (MD = 7.54, 95% CI 5.38-9.71, P < 0.01), NK cells (MD = 4.47, 95%CI 3.45-5.48, P < 0.01) and CD4 +/CD8 + (MD = 0.33, 95% CI 0.25-0.42, P < 0.01) in the treatment group were significantly higher than those in the control group, and the differences were statistically significant. The incidence of nausea and vomiting (RR = 0.51, 95% CI 0.35-0.73, P = 0.0002) and bone marrow suppression (RR = 0.41, 95% CI 0.25, 0.68, P = 0.0006) in the treatment group were significantly lower than those in the control group, and the differences were statisticallysignificant. There was no statistical difference in CD8+ level and the incidence of neurotoxicity, diarrhea, oral mucositis and hepatic injury between the two groups (all P > 0.05). Conclusion Lienal polypeptide injection combined with radiotherapy or chemotherapy is superior to conventional radiotherapy or chemotherapy alone in the treatment of cancer, which can improve the quality of life of patients with tumors, reduce the incidence of nausea and vomiting and bone marrow suppression induced by the treatment.

Objective To understand the intention to quit smoking and its influencing factors among current smokers in Gansu Province, so as to provide scientific basis for tobacco control. Methods A multi-stage sampling method was used to extract current smokers aged 15-69 years, and a face-to-face survey was conducted using the questionnaire on smoking among residents in China. Intention to quit smoking between current smokers with different characteristics was analyzed. Logistic regression was used to explore influencing factors of intention to quit smoking. Results The intention of current smokers to quit smoking in Gansu Province was 16.4% (95% CI:15.5%-17.3%). Multivariate logistic regression analysis showed that current smokers who were in rural areas (OR=1.199, 95% CI:1.022-1.408, P=0.026); family smoking prohibited (OR=1.767, 95% CI: 1.273-2.454, P=0.001), medical staff discouraged smoking within 12 months (OR=1.599, 95% CI:1.359-1.842, P<0.001), visited smoking clinics (OR=3.089, 95% CI:2.031-4.698, P<001), higher educational level of junior high school, senior high school and college or above (OR=1.383, 95% CI:1.101-1.736; OR=1.627, 95% CI:1.252-2.116; OR=1.374, 95% CI:1.009-1.873, all P<0.05), tobacco hazards knowledge with higher scores of 1-, 3- and 5-6 (OR=1.248, 95% CI:1.030-1.514; OR=1.574, 95% CI:1.289-1.922; OR=2.288, 95% CI:1.879-2.786, all P<0.05) were more likely to quit smoking; furthermore, smokers aged 20-, 30- years or smoking 20-, 30- years had a lower chance of quit smoking (all P<0.05). Conclusions The intention of current smokers to quit smoking in Gansu province is generally not high. In the future, knowledge of tobacco hazards should be further promoted, medical staff should provide more smoking cessation services during the treatment process, and more smoking cessation clinics should be established.

Objective To evaluate the effectiveness of general practitioner contracted service in management of patients with type 2 diabetes mellitus (T2DM) in the community. Methods From January 2012 to April 2017, 1625 patients of T2DM (766 males and 859 females with an average age of 62.9 years) were treated in Xinjiekou Community Health Service Center. They were managed by a contracted general practitioner team for one consecutive year, including initial assessment, patient education, disease follow-up and annual assessment. Four classes of diabetes and hypertension knowledge were given, patient self-assessment and nursing scoring were conducted before and after the education, including healthy life habits, diet and exercise. Family self-assessment, knowledge and skills in monitoring blood pressure, blood glucose, drug use, and risk prevention were carried out. Data collection and analysis were based on quarterly and annual follow-up tables. Results Before and after management, the mean BMI of 1625 patients was (25.0± 3.1) and (24.6 ± 3.1) kg/m2 (t=13.25, P=0.001), systolic blood pressure was (133.3 ± 10.0) and (122.7 ± 7.7) mmHg (1 mmHg=0.133 kPa, t=43.84, P=0.001), diastolic blood pressure was (82.7 ± 22.1) and (76.1 ± 13.5) mmHg (t=10.46, P=0.001), fast blood glucose was (7.2±2.5) and (6.2±0.9) mmol/L (t=18.50, P=0.001), 2 h-postprandial blood glucose was (9.1±2.1) and (7.8±1.9) mmol/L (t=23.39, P=0.001) and glycated hemoglobin (HbA1c) was (6.8 ± 1.1) and (6.3 ± 2.2) % (t=8.68, P=0.001), all were decreased significantly after management. After management the rates of HbA1c, fasting blood glucose and 2 h-postprandial blood glucose reached the standard were increased (χ2=345.57, P=0.001;χ2=235.13, P=0.001;χ2=354.28, P=0.001). The rates of aerobic exercise, the proportion of 3-5 times a week, exercise time 30-60 minutes/time increased (χ2=1245.58, P=0.001; χ2=158.38, P=0.001; χ2=264.26, P=0.001). The rates of smoking, drinking, eating fried food, saltiness food, sweetness food decreased (χ2=13.92, P=0.001; χ2=23.88, P=0.001; χ2=311.71, P=0.001; χ2=674.31, P=0.001; χ2=247.86, P=0.001). After management, the rates of blood pressure, blood glucose and waist circumference self-measurement; using food scales, staple measuring, salt spoon, oil spoon and kettle, pedometer were increased significantly. The scores of health, ability, knowledge and comprehensive evaluation after management were also increased significantly (40.3± 6.0 vs. 20.9±11.0, t=76.46, P<0.001;36.4±7.8 vs. 19.4±9.2, t=75.99, P<0.001;14.8±3.3 vs. 7.6±4.4, t=51.03, P<0.001;91.5±11.7 vs. 48.0±20.3, t=89.54, P<0.001). Conclusion The general practitioner team can effectively improve the self-management ability and achieve better clinical effect in T2DM patients.

Objective To evaluate the localization of transplanted human menstrual-derived stem cells (MenSCs)in the premature ovarian failure (POF)mouse model.Methods Mice were intraperitoneally injected with cisplatin to create the POF mouse model.Ninety mice aged 8 weeks were randomly divided into MenSCs transplantation group,POF model group and blank control group.Green fluorescence protein (GFP)-expressing MenSCs were transplanted to MenSCs transplantation group by the tail vein;meanwhile the same volume of PBS was injected into the mice in POF model group and blank control group.Physiological characteristics of mice,serum concentration of FSH and E2,ovarian histology and distribution of green fluorescence signal in the ovarian tissues were assessed in the three groups at 10 days after transplantation.Results Following anorexia and lethargy,the average weight of mice in POF mouse model decreased compared with that in blank control group. The concentration of FSH increased but E2 level decreased significantly in POF model group.Meanwhile,the number of primordial,growing and mature follicles reduced significantly and interstitial fibrosis was observed in the ovary ofPOF model group.After MenSCs transplantation,the average weight of mice in MenSCs transplantation group increased gradually with the improvement of feeding and activities.Ten days after injection of MenSCs,sex hormone levels and the number of follicles returned to normal values.The level of E2 was significantly higher and the level of FSH was lower,while the number of primordial,growing as well as mature follicles in MenSCs transplantation group was significantly higher than that in POF model group.By observing the location of GFP-MenSCs,we found that GFP-positive cells were located in the interstitium but not in follicles,granulose cells,the liver,the kidney,or lung tissue.Conclusion MenSCs can home to the ovary and be located in the interstitium in the POF mouse model.MenSCs can repair ovarian injury and improve endocrine functions of the ovary.

OBJECTIVE: To evaluate the efficacy of 3, 5, 4'-trimethoxy-trans-stilbene(BTM) on knee articular cartilage in a rat model of iodoacetate induced knee osteoarthritis (OA), METHODS: OA was induced by injecting iodine acetate in SD rats right knee, for 15 d. OA rats were assigned randomly into 6 groups(n = 10): model group, CMC-Na solvent group, BTM 25, 50 and 100 mg · kg-1, diacerein 8.0 mg · kg-1 control group. Another set of normal group, sham group(n = 10). Animals were intragastrically administered once daily for 4 weeks. The day after the last administration, the rats were anesthetized and blood samples were collected from abdominal aorta. The serum levels of IL-1β, TNF-α and MMP-2 were measured. The rats were sacrificed after the blood collection, the right side of the knee was cut. The gross morphology of the joint cavity was observated and cartilage histopathological changes was examined. RESULTS: Four weeks later, there have a significant decrease of the cartilage damage of BTM 50 and 100 mg · kg-1 groups (P < 0.05, P < 0.01). The serum level of IL-1β and TNF-α of all treated groups were significantly lower than that of model group (P < 0.05, P < 0.01). CONCLUSION: Our results suggest that BTM may be considered as a possible therapeutical agent in the treatment of OA, and its effects may be related to inhibiting inflammatroy factors including IL-1β, TNF-α.

Following the wishes of volunteer patients,316 cases of type 2 diabetics of our community center in 2008 carried out a three-year intervention from 2008 to 2011 on the basis of rational drug therapy plus the interventions of health education,regular review and lifestyle strengthening.And regular monitoring and recording were performed on the parameters of body mass index (BMI),blood pressure,blood lipids,fasting glucose,2 h postprandial blood glucose,glycosylated hemoglobin (HbA1c) and liver & kidney function.After intervention,some indicators changed:BMI (25.2 ±3.5) vs.(25.0 ±3.3) kg/m2,systolic blood pressure (129.1 ± 11.8) vs.(126.2 ±7.9) mm Hg(1 mm Hg=0.133 kPa),fasting glucose (7.80 ±2.81) vs.(7.25 ± 1.96) mmol/L,2 h postprandial blood glucose (11.04 ±4.60) vs.(9.83 ±3.60) mmol/L,HbA1c (7.39 ± 1.61) vs.(7.17 ± 1.65)%,total cholesterol (5.08 ±1.21) vs.(4.74 ± 1.35) mmol/L,low density lipoprotein cholesterol(LDL) (3.09 ± 0.87) vs.(2.85 ±0.83) mmol/L,high density lipoprotein cholesterol (HDL) (1.27 ± 0.33) vs.(1.41 ± 0.32) mmol/L,serum creatinine 65 vs.72 μmol/L,uric acid 300 vs.317 μmol/L.And the differences were statistically significant (P < 0.05).After intervention,blood pressure compliance rate increased from 72.5% to 88.0%,LDL-C compliance rate improved from 27.2% to 38.6%,HDL-C compliance rate of 54.9% increased to 66.9%.And the differences were statistically significant (P < 0.05).The pre-intervention combined compliance rate of 11.4% (n =36) rose to 17.7% (n =56).And there was significant difference (P =0.024).

RESUMEN

<p><b>OBJECTIVE</b>To investigate the effects of the histone deacetylase inhibitor, MS-275, on the immune molecule content and categories in hepatocarcinoma exosomes.</p><p><b>METHODS</b>Exosomes were isolated from the human hepatocarcinoma cell lines, HepG2 and Hep3b, and purified by a combination technique of ultrafiltration centrifugation and sucrose gradient ultracentrifugation. The expressions of heat shock protein (HSP)70, human leukocyte antigen (HLA)-I, HLA-DR, cluster of differentiation (CD) 80 and NY-ESO-1 on exosomes were analyzed with immunoelectron microscopy and Western blotting before and after MS-275 treatment. Intergroup differences were statistically analyzed by the Student's paired t-test.</p><p><b>RESULTS</b>MS-275 treatment of both HepG2 and Hep3b cell types significantly increased the numbers of exosomes, their total protein content, and expression of HSP70, HLA-I and CD80 (per 100 exosomes), as compared to non-treated cells (all, P less than 0.01). MS-275 was also found to induce de novo expression of HLA-DR, but had no significant effect on NY-ESO-1 expression (P more than 0.05). The findings from immunoelectron microscopy confirmed those from Western blotting.</p><p><b>CONCLUSION</b>The histone deacetylase inhibitor, MS-275, can significantly alter the immune molecule content and categories in exosomes of hepatocarcinoma cells. The differential expression profile may reflect an anti-cancer immune response and represent molecular targets for novel anti-hepatoma therapeutic or preventative strategies.</p>

RESUMEN

<p><b>OBJECTIVE</b>To study the stress changes of maxillary central incisor restored with or without fiber post using three dimensional finite element method, and analysis the role of fiber post in determining the stress distribution in dentin.</p><p><b>METHODS</b>Three dimensional finite element models of maxillary central incisor with various remaining tooth structure were established by spiral CT, Mimics software and ANSYS software. Test samples were restored with all-ceramic crown and fiber post all-ceramic crown, respectively. The von Mises stress and maximal tensile stress of dentin were recorded.</p><p><b>RESULTS</b>The stress level in dentin of maxillary central incisor restored with fiber post all-ceramic crown was smaller than that restored with all-ceramic crown, the stress distribution of both were similar.</p><p><b>CONCLUSION</b>The apply of fiber post can reduce the stress level in dentin of maxillary central incisor and decrease the risk of tooth breakage, but not change the stress pattern.</p>
