RESUMEN

The epidemiological hip fracture leads to a high death rate in the elderly with osteoporosis worldwide. However, the appropriate surgical styles or anti-osteoporotic therapy could prevent these patients with hip fractures from suffering refracture, but the efficacy of such treatment remains elusive for first hip fractured patients. Our retrospective analysis was conducted on 508 hip fracture patients who were enrolled from Show Chwan Memorial Hospital from January 2005 through December 2011 and followed up until the end of 2012. However, bipolar hemiarthroplasty replacement and open reduction internal fixation [ORIF] are treatment options for femoral neck and intertrochanic hip fracture in our study population. Among these patients, 82 suffered 2nd hip fracture [refracture] with femoral neck or intertrochanteric fracture and 39 died after surgical intervention accompanied complications. Kaplan-Meier analysis revealed a better outcome in patients with bipolar hemiarthroplasty replacement or fosamax therapy of hip fractured patients than those with femoral neck/ ORIF and intertrochanteric/ ORIF or without fosamax therapy. Multivariate cox regression analysis revealed the lowest incidence of refracture and mortality in hip fracture patients with received bipolar hemiarthroplasty replacement surgical intervention [OR=0.732, €1=0.587-0.912; P=0.006]. It is therefore concluded that fosamax therapy may improve bone density and increase bone tissue repair to prevent patients with hip fracture from refracture, and bipolar hemiarthroplasty replacement may promote patients who undertake outdoor activities to produce more vitamin D than those who have received ORIF

Objective To explore the relationship between drinking water from farmland edge and digestive tract cancers in rural areas.Methods A total of 180 cases of digestive tract cancers from Longquan city,Jinyun county and Qingtian county were selected and 360 controls with the same age group and sex were enrolled.1∶2 matched design of case-control study was used.The data was analyzed by Wilcoxon matched -pairs signed -ranks test.Results Overall,individuals who drank water from farmland edge were more likely to have digestive tract cancer among 3 areas (OR=4.005 ,95%CI:2.228-7.199).In Qingtian county the risk of digestive tract cancers in individuals who drank water from farmland edge was 5.375 times of control group population (95%CI:2.287 -12.635 ),But there was not statistically significant in Longquan city and Jinyun county (P >0.05 ).For gastric cancer,the proportion of drinking farmland edge water was significantly higher in cases than that in control group (OR=4.875,95%CI:2.007-11.839),while the difference was not statistically significant for liver cancer,colorectal cancer and esophageal cancer (P>0.05 ).In Qingtian county ,for gastric cancer,the proportion of drinking water from farmland edge was 5.750 times of control group population (95%CI:1.683-19.649).But there was not statistically significant in other cancers(P>0.05).Conclusion Drinking water near farmland edge may be one of the risk factors of gastric cancer in rural areas.

<p><b>BACKGROUND</b>Bone morphogenetic protein 9 (BMP9) and Wnt/β-catenin signaling pathways are able to induce osteogenic differentiation of mesenchymal stem cells (MSCs), but the role of Wnt/β-catenin signaling pathway in BMP9-induced osteogenic differentiation is not well understood. Thus, our experiment was undertaken to investigate the interaction between BMP9 and Wnt/β-catenin pathway in inducing osteogenic differentiation of MSCs.</p><p><b>METHODS</b>C3H10T1/2 cells were infected with recombinant adenovirus expressing BMP9, Wnt3a, and BMP9+Wnt3a. ALP, the early osteogenic marker, was detected by quantitative and staining assay. Later osteogenic marker, mineral calcium deposition, was determined by Alizarin Red S staining. The expression of osteopotin (OPN), osteocalcin (OC), and Runx2 was analyzed by Real time PCR and Western blotting. In vivo animal experiment was carried out to further confirm the role of Wnt3a in ectopic bone formation induced by BMP9.</p><p><b>RESULTS</b>The results showed that Wnt3a enhanced the ALP activity induced by BMP9 and increased the expressions of OC and OPN, with increase of mineral calcium deposition in vitro and ectopic bone formation in vivo. Furthermore, we also found that Wnt3a increased the level of Runx2, an important nuclear transcription factor of BMP9.</p><p><b>CONCLUSION</b>Canonical Wnt/β-catenin signal pathway may play an important role in BMP9-induced osteogenic differentiation of MSCs, and Runx2 may be a linkage between the two signal pathways.</p>

RESUMEN

Objective To study the efficiency of booster immunization with different recombinant hepatitis B vaccines.Methods 2789 children aged over 10 years who had completed the basic immunization of hepatitis B vaccine under 1 year old were selected.All the sampled children were classified into four groups (A,B,C and D) and immunized with different hepatitis B vaccines produced by different campanies respectively.Before booster immunization,their blood plasma specimens were detected for hepatitis B virus (HBV) surface antigen (HBsAg),antibodies to HBV surface antigen (anti-HBs) and antibodies to HBV core antigen (anti-HBc) by chemiluminescence.In each group,the anti-HBs positive children were immunized with one dosage and anti-HBs negative children were immunized three dosages of the same vaccine.Their blood specimens were collected again after 1 month,and detected for anti-HBs.Results The anti-HBs positive rates of A,B,C and D group were 36.43%,37.59%,42.91% and 46.46% respectively before immunization while 89.20%,91.52%,90.96% and 85.45% respectively after immunization with one dosage,99.12%,99.47%,98.87% and 98.85% respectively after immunization with three dosages.The differences of anti-HBs positive rates in the four respective groups showed statistical significances between any two rates of pre-immunization,post-immunization with one dosage and post- immunization with three dosages (all P<0.05).The anti-HBs positive conversion rates of four groups were 83.01%,86.41%,84.16% and 72.82% respectively after immunization with one dosage.The anti-HBs positive conversion rate of four groups were 98.62%,99.16%,98.03% and 97.84% respectively after immunization with three dosages and the difference of positive conversion rates in each group showed statistical significances between booster immunization with one dosage and booster immunization with three dosages.The average GMTs in anti-HBs positive children in the four groups were 2853.21,6254.23,3581.40 and 3021.32 mIU/ml respectively after immunization with one dosage.The average GMTs of anti-HBs negative children in the four groups were 273.08,648.52,387.87 and 245.36 mIU/ml respectively after immunization with one dosage,and were 632.30,2341.14,563.97 and 394.08 mIU/ml respectively after immunization with three dosages.Conclusion Our data showed that it would be suitable to anyone to use the four vaccines for anti-HBs positive children aged over 10 years with one dosage and for anti-HBs negative children aged over 10 years with three dosage booster immunization.

Objective To study the integrated monitoring program regarding mouse and plague, hemorrhagic fever of renal syndrome(HFRS)and leptospirosis. Methods Integrated monitoring plan was used. A designated office coordinated 5 departments' actions within the Zhejiang Provincial Center for Disease Control and Prevention(CDC). Cage-trapping method was conducted to monitor the density of mice from June to October, respectively. Results Lishui municipal CDC had finished the integrated monitoring program on mouse and mouse-borne disease while the Longyou CDC had finished the field investigation, using the integrated monitoring program.Specimens were sent to provincial CDC. The integrated monitoring program needed more number of personnel and better coordination. Lishui reported 3 leptospirosis cases and 58 HFRS cases in 2009,with the incidence rates as 0.13 and 2.44 per 100 000, respectively. Longyou reported 2 leptospirosis case and 1 HFRS cases in 2009, with the incidence rates as 0.49 and 0.25 per 100 000, respectively.Lishui and Longyou had no plague case. Lishui caught 91 mice in 2009 and the density was 4.17%.Longyou caught 37 mice in 2009, with the density as 1.18 percent. Most mice caught from Lishui were Apodemus agrarius and the next was Mus musculus. In Longyou the Rattus tanezumi ranked the first, followed by Apodemus agrarius. The positive rate of HFRS antigen in Lishui and Longyou were 10.42% and 4.59% respectively. The positive rate of HFRS antibody in Longyou was 3.70%. The culture positive rate of leptospirosis in mouse renal of Lishui and Longyou were 0 and 0.98%respectively. The culture positive rate of leptospirosis in pig renal, duck renal, frog renal and cattle urine of Longyou was 0. The culture positive rate of leptospirosis in duck blood of Longyou was 80%.Conclusion The integrated monitoring program on mouse and mouse-borne disease seemed to be feasible and could promote the integrated surveillance and control program on mouse and mouse-borne diseases in China.
