RESUMEN

Objective:To explore the utility of a robotic trunk support brace (a RoboBDsys) in evaluating seated trunk control after a spinal cord injury (SCI).Methods:Twenty wheelchair-dependent SCI patients were tested for trunk resistance while seated and wearing the RoboBDsys. In the test they were required to maintain an upright seated position for 1 minute while external forces were applied from the left, right, anterior and posterior directions. The system generated eight observations of swings of the center of pressure (COP), their mean velocity and their standard deviations. Sway area, sway path (SP), mean sway frequency (MF) and mean sway amplitude (MA) were also recorded along with the maximum voluntary contraction torque (MVC). The trunk control test (TCT), the Tinetti Balance Scale (Tinetti), the modified Functional Reach Test (mFRT) and Spinal Cord Independence Measure III (SCIM-III) were also administerd and their results were correlated with the RoboBDsys resistance test results.Results:All twenty of the patients completed the resistance test in the left and right directions, but only 11 did it in the anterior and posterior directions. In the left and right-direction resistance tests the average center of pressure deviation, sway area and MVC showed some significant correlation with the TCT, Tinetti, mFRT and SCIM-III results (| r| ranging from 0.46 to 0.74). The average MVC in the anterior-direction correlated moderately with the TCT, mFRT and SCIM-III results (| r|=0.63 to 0.67), while that in the posterior-direction had moderate correlation with the TCT and mFRT results only (| r|=0.63 to 0.67). Conclusion:The Robotic Brace for Dynamic Trunk Support system can be effective in assessing reactive postural control and functional independence after a spinal cord injury.

RESUMEN

Objective:To investigate the test-retest reliability and criterion validity of the Robotic Brace for Dynamic Trunk Support System(RoboBDsys)in evaluating seated trunk control function in patients with spinal cord in-jury(SCI). Method:From October to December,2022,20 patients with spinal cord injury who were unable to walk were recruited.All participants completed RoboBDsys trunk control test including static control test and dynam-ic control test on the seated platform.The evaluation indexes were the supporting torque regression slope exert-ed by the robot platform on the direction of x,y,α,and θ,recorded as Kx,Ky,Kθ,Kα;the average sup-porting torque recorded as Fx,Fy,Mθ,Mα in the static control test;and the the Range of Motion(ROM)of trunk in four directions in the dynamic control test.All the participants completed Trunk Control Test(TCT)as criterion assessment.The test interval was one week,intraclass correlation coefficient(ICC)values were calculated between two RoboBDsys trunk control test. Result:There were 13 participants completed two tests.In the static control test,the ICCs of four indexes were 0.418-0.742 between 2 tests;the ICCs of all indexes in dynamic control test were 0.633-0.848,which all showed moderate to high test-retest reliability.In regards of criterion validity,there was low to mod-erate correlation between Ky,Mθ,Mα in static control test and TCT(|r|=0.467-0.561,P<0.05);all indexes except right lateral flection ROM in dynamic control test showed moderate to high correlation with TCT(r=0.559-0.758,P<0.05). Conclusion:RoboBDsys is a reliable and valid tool for evaluating seated trunk control function in patients with SCI.

RESUMEN

Objective:To discuss the clinical efficacy of Shenghua Decoction combined with conventional western medicine therapy in treatment of polycystic ovarian syndrome with blood-insufficiency congealing-cold and blood-stasis syndrome. Methods:A total of 140 polycystic ovarian syndrome patients with blood-insufficiency congealing-cold and blood-stasis syndrome were selected from January 2018 to December 2019 in our hospitals, and were divided into observation group and control group according to the random indicator method, 70 cases in each group. The control group received ethinylestradiol and cyproterone acetate tablets treatment, and Shenghua Decoction was added based on the control group’s treatment in the observation group. Changes in the TCM syndrome score, modified Ferriman-Gallway (mF-G) of hirsutism, Rosenfield of acne, Testosterone (T), Follicle-Stimulating Hormone (FSH) and Luteinizing Hormone (LH) levels detected by ELISA of two groups were observed. The adverse events were recorded and the clinical efficacy was evaluated. Results:The effective rate of the observation group was 88.6% (62/70), which was significantly higher than control group of 60.0% (42/70) ( χ2=14.957, P<0.001). After treatment, mF-G, Rosenfield and TCM Syndrome Scores in the observation group were significantly lower than those in the control group ( t=6.524, 3.111, 2.196, P<0.01 or P<0.05). After treatment, the levels of FSH [(5.16 ± 1.76) IU/L vs. (7.19 ± 1.06) IU/L, t=2.530], LH [(6.12 ± 1.88) IU/L vs. (8.03 ± 1.78) IU/L, t=2.403], T [(1.34 ± 0.32) μg/L vs. (1.80 ± 0.36) μg/L, t=7.080] in the observation group were significantly lower than those in the control group ( P<0.05 or P<0.01) Conclusion:The Shenghua Decoction showed beneficial effect for the patients with polycystic ovarian syndrome with blood-insufficiency congealing-cold and blood-stasis syndrome.

RESUMEN

Objective The prevalence of obesity is constantly increasing. Multiple metabolic complications are related to obesity, including type 2 diabetes mellitus and non-alcholic fatty liver disease(NAFLD). Our study aimed to investigate the prevalence of obesity comorbidities and its association with BMI. Methods 765 individuals who visited the multidisciplinary clinic for obesity in Peking University First Hospital from 2015, Jun. to 2018, Sept. were enrolled in this study. The height, body weight, waist circumference, hip circumference were measured during the first visit. Body adipose percentage and basal metabolic rate were recorded. Questionnaires for daily food intake, comorbidity, and lifestyle were recorded. Fasting insulin, C peptide, glucose, HbA1C , uric acid, liver enzymes and lipid profile were measured. Statistical analysis was performed using SPSS 16. 0, and P<0. 05 was considered as statistical significant. Results Daily energy intake was higher in obesity group [ obese vs non-obese, (2136.6±739.4vs1905.7±468.4)kcal/d,P=0.046].Hypertension,NAFLDandgoutriskincreasedsignificantly in obesity group (obese vs non-obese, 36.0%vs 24.5%, P=0.02;76.5% vs 60.6%, P<0.01;6.9% vs 1.8%, P=0.04, respectively) . Family history of obesity and diabetes increased in obesity group ( obese vs non-obese, 64.5%vs 53.6%, P=0.03;47.4%vs 37.3%, P=0.048). Fasting insulin and C-peptide levels were higher in obesity group [obese vs non-obese, (24.8 ± 15.3 vs 13.6 ± 9.5)μIU/ml, P<0.01;(3.72 ± 1.40 vs 2.70 ± 1.16)μIU/ml, P<0.01). Liver enzymes increased significantly in obesity group [obese vs non-obese, (47.2±45.4 vs 23.3±21.4)U/L, P<0.01; ( 30. 4 ± 24. 0 vs 19. 9 ± 8. 5 ) U/L, P=0. 001 ] . Conclusions Obesity population had higher risk of hypertension, NAFLD and gout. Fasting insulin, C-peptide, liver enzymes, and UA also increased significantly in these patients. It is critically important to those obese individuals for regular screening of NAFLD and diabetes mellitus.

Objective: To compare the balance ability between normal people and non-specific low back pain (nsLBP) patients and explore the relationship between balance ability and muscle function in nsLBP patients. Method: Ten nsLBP patients as nsLBP group and 10 age and gender-matched healthy control subjects as control group were investigated. Posturography on balance platform and surface electromyography (sEMG) were performed to assess all the subjects' function of equilibrium and muscle activities of erector spinea(ES), muhifidus(MF), abdominal external oblique (EO), hamstring (HS) and maximal gluteus (MG) bilaterally. Result: The nsLBP subjects had greater sway on anterior-posterior direction (Y-speed and Y-extension, P=0.05) on feet-together posture and bigger main axis (P=0.023) on nature standing with eyes closed when compared with controls. The iEMG ratios of right MG in nature standing (eyes closed)/ nature standing (eyes open) and feet-together (eyes open)/nature standing (eyes open) in control group were significant higher than that in nsLBP group (P=0.03 and P=0.013). Conclusion: Balance evaluation combined with sEMG measurement on trank and lower limb muscles provided some quantitative information about functional deficits such as pestural control and muscle activities in nsLBP patients. This relationship should be emphasized in prevention and rehabilitation of nsLBP.

@# Objective To analyze the features of surface electromyography (sEMG) signal of the tibial anterior muscle and gastrocnemius muscle in the stroke patients when sitting and standing.Methods Fifteen stroke patients and fifteen normal subjects were involved in this study. All subjects were asked to stand up and sit down. It repeated five times continuously. The electromyographic signals were collected by surface electrode and then processed by linear time and frequency domain method.Results In sitting position, tibial anterior muscle had significant differences in mean power frequency and median frequency ( P<0.05) when the paretic and non-paretic lower limb, the non-paretic lower limb in stroke patients and in normal subjects were compared. The gastrocnemius muscle had notable differences ( P<0.05) in root mean square when the non-paretic lower limb in stroke patients and in normal subjects were compared. In standing position, tibial anterior muscle also had significant differences in root mean square and integrated electromyography ( P<0.05) as the same frequency domain comparing as above. The gastrocnemius muscle had notable differences on time domain, when the paretic and non-paretic lower limb, the paretic lower limb in stroke patients and the limb in normal subjects were compared ( P<0.05 or P<0.01).Conclusion sEMG is a brief, applied, feasible assessment method in rehabilitation.
