RESUMEN

Vascular endothelial growth factor [VEGF] plays a crucial role in preservation of renal functions and may also serve as a useful biomarker in monitoring the progression of lupus nephritis [LN]]. We thought to correlate VEGF expression in the kidney with renal histopathology in lupus nephritis to unveil its possible relation to disease activity and severity. We consecutively enrolled 15 patients with lupus nephritis and ten renal biopsy specimens from patients with cystic renal diseases as controls. The study measurements included SLEDAI, SLICC/ ACR damage index and BILAG renal score. Paraffin sections from renal biopsies were subjected to routine haematoxylin and eosin staining and Immunohistochemical staining for VEGF. Results: Among SLE patients, 7 [46.7%] showed mild expression of VEGF, 5 [33.3%] showed moderate while 3 [20%] had strong expression of the marker. On the contrary, the control samples [100%] revealed strong marker expression. All subjects with class IV and V lupus nephritis had mild renal expression of VEGF. Renal expression of VEGF had a significant positive correlation with serum creatinine and complement C3 levels. The 24 hours' excretion of urinary proteins had a significant negative correlation with the renal expression of the marker. On the other hand, the activity indices and therapeutic modalities did not correlate with VEGF expression. Conclusion: This pilot study among pediatric cases of SLE revealed mild to moderate VEGF expression in most cases of proliferative LN. Further longitudinal studies are needed to investigate the consequences of this finding on the prognosis of the disease

RESUMEN

To report experience with laparoscopic sleeve gastrectomy [LSG] in obese, morbidly obese, and super morbid obese patients, and to evaluate comparative efficacy of LSG among these patient groups. A total of 147 patients underwent LSG between March 2008 and December 2011 at the Department of Surgery, King Saud Medical City, Riyadh, Kingdom of Saudi Arabia. Patients were grouped according to the preoperative body mass index [BMI] into obese [35-39.9 kg/m[2]], morbidly obese [40-49.9 kg/m[2]], and super morbid obese [>50 kg/m[2]]. Patients who did not have a regular follow-up [n=38] were excluded, and 108 patients were included in this prospective study. The mean total weight loss [TWL] among the super morbid obese group [41.31 +/- 21.23 kg] was statistically significantly greater compared to the obese group [24.31 +/- 13.00 kg, p=0.009] and morbidly obese group [26.81 +/- 15.56 kg, p=0.001]. The mean percentage excess weight loss [EWL] was clinically significant among obese [57.8%], morbidly obese [42.5%], and super morbid obese patients [45.7%], however, it was not statistically significant between the groups [F[2,105] =2.132, p=0.124]. There was no mortality; however, 6 major complications occurred including intra-abdominal collection with suspected leak, staple line bleeding, bowel ischemia, and inferior vena cava injury. Laparoscopic sleeve gastrectomy resulted in satisfactory and effective EWL in all 3 groups of obesity patients at 30-months follow-up

RESUMEN

The main problem in the postoperative period is pain relief. Adequate postoperative analgesia not only leads to patient's comfort but also decreases morbidity, nursing care and time of hospitalization. Determination of the effect of intraperitoneal pethidine on postoperative pain in women scheduled for elective tubal ligation was undertaken. In a double blind clinical trial study of 60 women, ASA I, 25-45 years old, were enrolled for elective tubal ligation in Kosar hospital in Qazvin, IRAN. Patients were randomly divided in two equal groups [30 each]. One group received pethidine intraperitoneally and the other group received equal amount of placebo in the same region. The intensity of postoperative pain was evaluated by visual analogue scale [VAS] for about 8 hours. Incidence of nausea was also evaluated. Data was transformed to SPSS software. Then data analysis was performed by U-test. There was no significant statistical difference with regard to age, weight, and time of operation between the two groups. The mean score of pain was significantly lower in intraperitoneal pethidine group than placebo group but the incidence of nausea in the intraperitoneal pethidine group was more than in placebo group [P<0.05]. Thus, intraperitoneal pethidine decreases postoperative pain but increases postoperative nausea

To compare the effects of spinal anesthesia between pre-eclamptic patients and normal pregnant women during cesarean section. The participants of this hospital-based cohort study consisted of 60 women. Their age range was between 18-40. Thirty patients were pre-eclamptic and the others were normal pregnant women. Spinal anesthesia was administered with 2ml lidocaine 5%. Hemodynamic changes, dose of ephedrine requirement, incidence of nausea and Apgar score were compared between two groups. Data were transformed to the SPSS software. Then Chi-square, T-student and Fischer exact tests were performed for statistical analysis. There was no significant statistical difference of age and weight between two groups. In regard to basic, post spinal and post ephedrine systolic and diastolic blood pressure, significant statistical differences were noted. Similarly as regards the number of patients who developed hypotension and the dose of ephedrine injection, there were meaningful statistical differences between two groups. Development of hypotension and dosage of ephedrine injection in pre-eclamptic patients were less than healthy pregnant women during cesarean section under spinal anesthesia. As such spinal anesthesia is a safe method for cesarean section in pre-eclamptic patients

RESUMEN

Three new simple, different and sensitive spectrophotometric methods are suggested for the determination of Dropropizine [DRO]. The first is based on the reduction of ferric into ferrous in presence of 0-phenanthroline by the mentioned drug to form a highly stable orange-red ferroin chelate[Fe-[Phen][3]][2+], measured at [lambda][max] 510.5 nm for zero-order [D[degree sign]] while the peak amplitude was measured at 537.5 the nm for the first derivative [D[1]] using distilled deionized water as a solvent. Beer's law was obeyed in the range of 0.5-2.2 micro g/ml for both D[degree sign] and D[1] with mean percentage recoveries of 100.08 +/- 1.370 and 100.03 +/- 1.064, respectively. The second method is based on the reaction of DRO with 7,7,8,8- tetracyanoquinodimethane [TCNQ] in acetonitrile solvent to form a highly stable charge transfer colored complex that was measured at 734.5 nm. Beer's law was obeyed in the range of 10-55 micro g/ml with mean percentage recovery of 99.93 +/- 1.070. The last method is based on the reaction of DRO with cerium [IV] ammonium sulphate in acidic medium to form a highly stable colored oxidative product that was measured at 486.5 nm. Beer's law was obeyed in a concentration range 145 - 335 micro g/ml with mean percentage recovery of 99.67 +/- 1.077. The optimum assay conditions and their applicability for the determination of the cited drug in pharmaceutical formulation were described. The proposed methods were statistically compared with the reported method revealing good accuracy and precision

RESUMEN

To see the effect of oral paracetamol and clonidine on pediatric preoperative anxiety in adenotonsillectomy. The participants of this double blind clinical trial study consisted of 120 children. Their age range was between 3-12 years. They were in ASA class I of anesthesia and were chosen for elective adenotonsillectomy in Qods hospital in Qazvin, Iran. The patients were randomly divided in three equal groups. One group received paracetamol 20 mg/kg, another group received 4mcg/kg clonidine orally 60-90 minutes before operation and the third group received placebo. As the first step, in order to do the statistical analysis, intensity of preoperative anxiety had been evaluated by Modified-Yale Preoperative Anxiety Scale [m-Yale PAS] and data were transformed to the SPSS software. Then different statistical procedures like Chi-square, One way Annova and Tukey test were performed. There was no significant difference [statistical] of age, weight and sex among three groups. Mean score of anxiety was significantly lower in paracetamol and clonidine's groups than placebo's group, but there was no significant statistical difference between paracetamol and clonidine's groups in decreasing preoperative anxiety score. Oral paracetamol like oral clonidine decreases preoperative anxiety score. Thus one can use paracetamol instead of clonidine

Acute Non Lymphoblastic Leukemia is one of the most common malignant tumors of haematology. With the recent progress in chemotherapy and supportive therapy, the remission and survival rate have been markedly improved. In this study, cyclin A2 and multidrug resistance expression was measured by flow cytometry and RT-PCR in 52 de novo AML patients with acute myeloid leukemia. Their expression was correlated with other prognostic criteria, response to treatment and to overall survival. The rate of CR and PR was significantly higher in the group of positive expression of cyclin A2, compared to that with negative expression. However a statistically significant difference was only reached by PCR [p=0.02]. By flow cytometry, the overall Survival [OS] in the group with positive cyclin A2 expression is significantly higher than that in the group of negative cyclin A2 expression, p=0.03. Regarding MDRI, it was expressed in 39% of our patients and the level of expression was slightly higher by RT-PCR. The rate of CR and PR in the group of negative MDR expression was significantly higher as compared to the group of positive MDR expression, by both flow cytometry and RTPCR [p= 0.005, 0.004, respectively]. The OS in the group with negative MDR1 expression was significantly higher than that in the group of positive MDR1 expression, p=O.04. There was a significant inverse relationship between Cyclin A2 and MDR expression in our AML cases by RTPCR technique [p= 0.005], while it showed no significance by Flow cytometry [p=0.12]. There was no agreement [Kappa=0.25] between Flow cytometry and RT-PCR in detection of cyclin A2. On the contrary, there was an agreement between Flow cytometry and RT-PCR in detection of MDR. In conclusion, the low expression of cyclin A2 and high expression of MDR1 are indicators for unfavorable prognosis for individuals with AML. The detection of cyclin A2 level would predict drug resistance. However, it is one of many other factors

RESUMEN

The synthesis of twenty nine novel derivatives of pyrazolol [3,4-d] pyrimidines as non acidic nonsteroidal anti-inflammatory drugs has been achieved via reaction of 4-chloro-l-phenylpyrazolo [3,4-d] pyrimidine with different substituents including 4-amino-3-methylphenol, 4-phenylene diamine and 4-acetamidophenol. The anti-inflammatory activity of thirteen representative compounds have been screened compared to indomethacin as a reference drug. The results revealed that all the tested compounds showed anti-inflammatory activity with the exception of 10a.

3-Amino-2-[substituted phenoxymethyl / propyl] quinazolin-4 [3 H] ones 4a-c have been prepared. Refluxing 4 with 5-nitro-2-fur- aldehyde / 4-nitrobenzaldehyde afforded the corresponding substituted methylidene-ammo derivatives 5 and 6 respectively. Reaction of 4 -with isatin yielded the indolylideneamino derivatives 7. Refluxing 4 with ofloxacin acid chloride furnished the corresponding carboxamides 8, Reaction of chloroacetyl chloride with 4c produced the 3-chloroacetylamino derivative 9 which upon further reaction with the potassium salts of some antibacterial acids gave the corresponding carboxylate derivatives 10. Sixteen compounds were screened for their antibacterial and antifungal activities. Thirteen compounds were found to possess [high to moderate] activity against Pseudomonas aerugenosa and some of them were also active against Escherichia coli. Only one compound was found to exhibit moderate antifungal activity against Candida albicans

RESUMEN

Four series of new acridine derivatives of anticipated antitumor activity have been designed and synthesized. The first series belongs to 4-substituted phenylhydrazinocarbonylmethyl 9-oxo-9,10-dihydroacridine-4-carboxylate 10a-h. The second series consists of phenylhydrazinocarbonylmethyl 9-[4-substituted phenyl]aminoacridine-4-carboxylate 12a-k, while the third series comprises 4-substituted phenylcarbonylmethyl 9-[4-substituted phenyl]aminoacridine-4-carboxylate 15a-k. The fourth one belongs to phenylcarbamoylmethyl 9-[4-substituted phenyl]aminoacridine-4- carboxylate 17a-j. The chemical structure of synthesized compounds was elucidated by spectral data and elemental analysis. Seventeen selected compounds [10a, 10g, 10h, 12at 12d, 12g, 12h, 12k, 15a, 15c, 15g, 15h, 15k, 17a, 17f, 17g and 17j] were tested against breast cancer cell line [MCF7] and eight compounds [12g, 12h, 12k, 15g, 15h, 17f, 17g, 17j] were found to exhibit significant antitumor activity

Idiopathic thrombocytopenic purpura, a disorder characterized by autoantibody mediated platelets destruction causing decreased number of circulating platelets manifest as bleeding tendency. Since the discovery of Helicobacter pylon [H. pylon] several studies have been published concerning a hypothetical role of this bacteria in idiopathic thrombocytopenic purpura. Evaluate the pathogenic correlation between H. pylori infection and idiopathic thrombocytopenic purpura. A cross sectional study was done on 30 cases of idiopathic thrombocytopenic purpura admitted to the Pediatric Hemato-Oncology unit in ALKadhimiyia Teaching Hospital and 20 cases of sex and age matched healthy children asa control io determine the relation between H. pylon infection and idiopathic thrombocytopenic purpura. The peak age for idiopathic thrombocytopenic purpura was between [6-10 years] of age, [43.33%]. Male: female ratio equal to 2:1. ELIZA test was used to measure the serum IgG antibody titer against H. pylon, 5 cases of idiopathic thrombocytopenic purpura was found to be positive for H.pylori [16.67%]while the test was negative in all of the control group [100%] a result which is statistically highly significant, p - value < 0. 05. Of the positive cases 3 [60%] was chronic idiopathic thrombocytopenic purpura and two cases [40%] was acute idiopathic thrombocytopenic purpura. Patients with positive test were older than those with negative test. The mean platelet count was less in the positive than those with negative test. A weak inverse correlation was found between platelet count and H. pylori serum antibody titer [r- value equal to -0.13437]. The test was sensitive in [20%] and speicific in [100%] of cases H. pylori infection may play a role in the initiation of idiopathic thrombocytopenic purpura and eradication of H. pylori infection can cure idiopathic thrombocytopenic purpura specially in chronic and resistant cases. The ELIZA test can be used as a screening test to detect H.pylori infection in children with idiopathic thrombocytopenic purpura

Low bone mineral density is a major risk factor for development of osteoporosis. The estrogen receptor alpha gene is a candidate locus for genetic influence on bone mass. The possible association between two polymorphisms in the first intron of this gene, independently or in combination, and bone mineral density was widely studied in different population. In the present study, the relationship of TC [Pvull] and AG [Xbal] polymorphisms to variability in bone mineral density [BMD] was determined in 69 unrelated postmenopausal Egyptian women aged 43-75 years. All of the participants were non-smoker with no history of medication affecting bone loss or bone turnover. Hepatic and renal investigations were evaluated to ensure normal functions. BMD at the calcaneous bone was measured by peripheral computed tomography and expressed as t-values for the corresponding age. Analysis of the TC [Pvull] and AG [Xbal] polymorphisms revealed that BMD was significantly lower in women with homozygous absence of the restriction sites, in other words, women with the PP genotype showed significant reduction in their bone mass than in those with the pp genotype. And women with the XX genotype showed significant lowered BMD compared to those with Xx then xx genotypes. Analysis of combined genotypes in the same sample revealed that BMD was significantly lower in Egyptian women with the PPXX genotype [ANOVA, P= 0.002] than in those with the PpXx genotype [ANOVA, P= 0.022]. The xxpp genotype did not exhibit any significance association with the reduced BMD in either the osteoporotic or osteopenic groups. The overall distribution of ER Pvull and Xbal genotypes frequency identified in the present study: Pp 55%, pp 20%, PP 17%, Xx 55%, xx 30%, XX 10% is consistent with that found in the postmenopausal American white women, and postmenopausal Italian women, as the heterozygous [ +/- ] Pvull or Xbal genotype is the most prevalent one then the homozygous presence of restriction site, [+/+] genotype, followed by the homozygous absence of the restriction sites, [-/-] genotype is the least prevalent one. Identified allele frequencies [P= 0.52, p= 0.48 and X= 0.42, x= 0.58] at the two polymorphisms are in Hardy-Weinberg equilibrium. Results of the present analysis, which to the best of our knowledge is the first one in our population, suggest that testing for ER Pvull and Xbal restriction fragment length polymorphisms might be useful in screening and identifying postmenopausal Egyptian women at risk for developing osteoporosis, PPXX followed by PpXx genotypes were found to be associated with reduction in BMD in our population

To study the prevalence and demographic characteristics of mood disorders among Iranian adults. In this cross-sectional population-based epidemiological study [age 18] in Iran, 25180 individuals were selected through a randomized cluster sampling method for a diagnosis using the Schedule for Affective Disorders and Schizophrenia [SADS]. They were then interviewed at home by 250 trained clinical psychologists. The estimated lifetime prevalence of Major Depressive Disorder [MOD] and Minor Depressive Disorder [mDD] were 3.1% and 0.3% respectively. Also, the estimated lifetime prevalence of Bipolar Mood disorder [BMD] type I and type II were 0.1% and 0.7% respectively. The current prevalence of MDD, mDD, BMD-I and BMD-II were 1.8%. 0.2%, 0.04%, and 0.3% respectively. Mood disorders were associated with female gender, lower education, being married, being middle-aged, living in cities, and not being a homemaker. The prevalence of mood disorders was lower among Iranian adults than reported in Western studies, and a number of demographic associations differed from those reported in Western studies: Important cultural differences in the nature or manifestation of depression are implied by these results

A retrospective study was done in the Pediatric ward /AL-Kadhimiyia Teaching Hospital on cases diagnosed as idiopathic thrombocytopenic Purpura admitted since the first of January 1992 - the end of December 2004. The objective of this paper is to review the natural history of idiopathic thrombocytopenic Purpura, presenting feature and response to treatment. The review included age, sex, clinical presentation, physical finding, complications, investigation treatment and course of the diseases. Total number of the patients were 65, peak age was between [2 -5] years, 40 cases [61.53%]. Females were affected more than Males with male: female ratio of I: 2.09 .There was no seasonal variation and all of the cases were preceded by viral upper respiratory tract infection. The main presentation was petechiae and ecchymosis, were found in all of them [100%] followed by epistaxis, 43 cases [66.15%]. In the majority the platelet count was < 20.000 X 10[9] /L, 32 cases [49.23%] and the hemoglobin level was between 10-11 gm /dl in most of them, 43 cases [66.15%]. Steroid was the first line treatment, response occur within 2-3 weeks in the majority, 51 cases [78.46%]. Chronic idiopathic thrombocytopenic Purpura occurred in 3 cases only [4.61%]. Splenectomy was done in 2 of them [66.66%]. No mortality detected. Idiopathic thrombocytopenic purpura is a mild disease with complete recovery in the majority of patients

Seventy eight patients with beta - thalassemia major were Collected randomly from the thalassemia center in Ibn-Al-Baldy Hospital [all of them were transfusion dependent], together with fifty six age and sex matched healthy children were collected as a control. To determine the incidence ofthromboembolism among them and to evaluate the precipitating factors. History was taken and physical examination was done. EDTA Anticoagulated blood samples were taken prior to the next transfusion, platelet parameters were estimated for both groups using the MS - 9 coulter counter. Thromboembolic events was detected in five patients [6.41%]. The main site was the central nervous system in 4 of them [5.2%] followed by the lung J case [1.2%]. Associated predisposing factors were found in 3 of them [60%]. The patients had a statistically significant higher platelet counts, platelet crit, mean platelet volume and platelet distribution width than the control group. The splenectombed had higher platelet counts. platelet crit than the non splenectomizedwith nearly equal mean platelet volume and platelet distribution width, a results that is in favour of thrombosis. This results suggest that beta- thalassemia patients had a hypercoagulable state which makes them susceptible to thrombosis and pulmonary embolism even in the young age group and even before splenectomy is performed

Glucose 6 phosphate deosphate dehydnogenase [G6PD] deficiency is a common problem in Iraqi as well as in tropical and subtropical region.To study the epidemiological aspect of G6PD defiaiencv among Iraqi children.This cross sectional study was conducted in the Pediatric ward/AL Kadhiymia Teaching Hospital during the season of Fava beans ingestion. Patients who were presented with acute attack of pallor were collected, history was taken and physical examination was done.Ninety seven cases were studied, males were affected more than female with a ratio of 3.85:1, the peak of age was between [1-5years], 76 cases ,[78.35%].Previous history of neonatal jaundice was found in 24 cases [24.74%], the acute hemolytic attack was precipitated by Fava beans ingestion in all of the cases [100%] and pallor was the main presenting feature in all of them. The attack was very severe in 43 cases [44.32%] with arrange of PCV between [1 - 9%]. History of recurrence was encountered in 11 cases [11.34%], family history of G6PD deficiency was found in 22 cases [22.68%] .G6PD level was estimated two months later in 31 cases [31.95%] only and were found to be deficient

This prospective study was done to know the complications of TURP as a treatment modality for BPH. 1200 patients having TURP at Jinnah and Mayo Hospital, Lahore from 1996 to 2002 were included. All the patients were evaluated by history, examination and investigations. Age range included from 45 to 115 years. Retention of urine was commonest indication for the procedure followed by raised prostatic symptom score. Seventy-two% patients had prostate size between 31 to 50 gram on DRE and Ultrasonography. Hypertension, DM and pulmonary diseases were common comorbid conditions in these patients. In early complication clot retention in 8% and ITTI in 7.6%, and failure to void in 4.7%. In the late complication stricture 6%, impotence 2.3% and incontinence 0.5%. Histopathology detected carcinoma of the prostate in 6% patients

RESUMEN

Myogenesis of skeletal muscles were viewed by Electron microscope. They were taken from the abdominal wall of fetal [both sexes] and neonatal male rabbits. revealed that myoblasts and young myotubes appeared at day 15 of gestation. By day 19 the myofibrils of maturing myotube have clear Z - lines, while at day 24 of gestation the myofibrils develop a full banding pattern with very clear Z-lines. Newborn rabbit muscles display very well developed and uniform banding pattern. The observations showed that male muscle development preceed that of the female at the same age. The distinctive results of this work is the proliferation of maturing muscle nucleus, which are so far not documented by any other researcher

Although many methods of surgical and non-surgical approaches for treatment of pilonidal sinus [PNS] have been proposed, an optimal treatment modality has not been achieved yet. In the case of pilonidal sinus treated with primary intention surgery, the uneventful healing is still difficult to obtain as indirectly proven by the number of different procedures. The aim of this study was to determine the advantages of adding an antibiotic carrier [Septocoll, septomycin] in the closed method technique after primary excision of the PNS. This randomized controlled prospective study included one hundred sixty-four consecutive patients who had primary or recurrent pilonidal sinus. These patients were treated by excision of the pilonidal sinus and primary closure. They were divided into 2 groups each included 82 patients: For Group I an antibiotic carrier [Septocolll] was introduced into the resulted cavity after excision of the PNS before skin closure. For Group II no antibiotic carrier was used and the cavity was closed directly. For group I: There was no operative mortality. Seventy-two patients [88%] had complete healing by the end of the study and were asymptomatic. Sixty six patients [80%] had healing by primary intention and in 16 patients [20%], the wounds healed by secondary intention. Fourteen patients [17%] developed postoperative prolonged pain. Ten patients [12%] developed recurrence. Mean length of hospital stay was 2.2 days [range Ito 3 days]. For group II: There was no operative mortality. Forty-nine patients [60%] had complete healing by the end of the study and were asymptomatic. Forty-six patients [56%] had healing by primary intention and in 36 patients [44%], the wounds healed by secondary intention. Thirty-three patients [40%] developed recurrence. Eighteen patients [21%] developed postoperative prolonged pain. Mean length of hospital stay was 3.1 days [range hto4 days]. Wide excision and primary closure by tension sutures after application of antibiotic carrier in the cavity is associated with short postoperative hospital stay, low morbidity, and low recurrence rate. It can be performed for managing primary or recurrent, clean or infected pilonidal sinuses with good long-term results

RESUMEN

The effects of varying dietary protein and vitamin C levels on several nutritional parameters were calculated for young Oreochromis niloticus. Live body weight of Nile tilapia [O. niloticus] increased significantly with increasing the dietary protein level or vitamin C supplementation in fish diet. The average daily gain weight increased with 37.31% in fish fed the high protein diet [31.1% crude protein] than those fed the normal protein diet [25.4%]. Also, the average daily weight gain increased with 5.41% and 14.86%, respectively, in fish fed diets supplemented with 50 and 100 mg vitamin C/kg diet than those fed diets without supplementation. Increasing the dietary protein level improved the feed conversion. Feed conversion was improved by 18.85% in fish group fed high protein diet when compared with those fed the normal protein diet. Feed conversion improved by 1.37 and 7.88%, respectively, in fish groups fed diets supplemented with 50 and 100 mg vitamin C when compared with those fed diet without vitamin C supplementation. Serum total protein, albumin, creatinine and aspartate aminotransferase [AST] significantly increased with dietary protein level; while, alanine aminotransferase [ALT] was insignificantly affected. Also, vitamin C supplementation in fish diets significantly increased the concentrations of serum total protein, albumin and AST; while, the concentration of ALT was insignificantly decreased. Fish body composition was not significantly affected with dietary protein level or dietary vitamin C supplementation. The interaction between dietary protein level and vitamin C supplementation did not show any significant differences in body weight, daily gain, daily feed intake, feed conversion, blood components or body composition
