RESUMEN

Background@#The purpose of the study was to examine associations between depressive symptoms and work-related musculoskeletal symptoms, and health-promoting behaviors in Korean Coast Guards who are exposed to a vulnerable working environment. @*Methods@#A cross-sectional correlational study was conducted with 271 Korean Coast Guards in Gangwon-do. Depressive symptoms were measured using the Korean version of Center for Epidemiologic Studies Depressive Scale-Revised. Work-related musculoskeletal symptoms were measured using the KOSHA (Korea Occupational Safety and Health Agency) GUIDE H-9-2012. Health-promoting behaviors were measured using the Health Promoting Lifestyle Profile-II. @*Results@#Of all the Korean Coast Guards, 17.3% represents the risk of clinical depression, and 43.5% reported work-related musculoskeletal symptoms in any body parts. The average score of health-promoting behaviors were 2.4 (range, 1-4 scores). Depressive symptoms and work-related musculoskeletal symptoms were significantly negatively associated with health-promoting behaviors, respectively. Depressive symptoms were significantly negatively associated with the spiritual growth, interpersonal relationship, physical activity, and stress management in subscales of the Health Promoting Lifestyle Profile-II. Work-related musculoskeletal symptoms were significantly negatively associated with the physical activity and stress management in subscales of the Health Promoting Lifestyle Profile-II. @*Conclusions@#Among Korean Coast Guards, health-promoting behaviors were significantly associated with depressive symptoms and work-related musculoskeletal symptoms. Therefore, nursing strategies to promote health-promoting behaviors may be enhanced psychological health and physical health outcome of Korean Coast Guards.

RESUMEN

Hemolytic uremic syndrome (HUS) is a rare disorder characterized by the triad of microangiopathic hemolytic anemia, thrombocytopenia, and acute renal failure. HUS arises from a wide spectrum of conditions, and chemotherapeutic agents have been reported to be associated with HUS, including Mitomycin, Cisplatin, Bleomycin, and Gemcitabine. A 76-year-old man treated with Gemcitabine due to non-small cell lung cancer developed clinical and laboratory findings compatible with HUS. Gemcitabine was ceased and hemodialysis and plasma exchange were utilized and he recovered. A high level of suspicion for HUS is necessary when cancer patients are treated with Gemcitabine, and prompt recognition and treatment are also essential.

RESUMEN

Pancreatic pseudocysts occur following acute pancreatitis, chronic pancreatitis or secondary to pancreatic trauma. Most pancreatic pseudocysts are located in or around the pancreas, but they can be found in all the potential spaces around viscera in and outside of the abdominal cavity. The complications of pancreatic pseudocyst are infection, rupture, obstruction, fistula, or hemorrhage. Hemorrhage is a rare but frequently fatal complication of pancreatic pseudocysts. We report a case of pancreatic pseudocyst presented as bleeding of duodenum, which was misidentified for duodenal submucosal tumor bleeding.

RESUMEN

This report describes a rare case of perioperative midazolam hypersensitivity in a patient without any history of allergy. A 39-year-old man was admitted for endoscopic pansinus surgery. During transportation to the operating room after injecting antibiotic and midazolam intravenously, the patient complained of shortness of breath. At 3 months after the event, an intradermal sensitivity test for midazolam proved positive indicating the incident was caused by midazolam hypersensitivity.

RESUMEN

We present the case of a 57-year-old man who developed retroperitoneal hemorrhage due to unintentional arterial puncture during femoral artery cannulation for Guglielmi detachable coil embolization. On emergence from anesthesia, he developed severe hypotension. Computed tomographic angiogram of the abdomen showed retroperitoneal hematomas around the urinary bladder, liver, and spleen. Because femoral artery cannulation is a common procedure for intravascular embolization in neuroradiologic procedures, Clinicians should be aware of the development of severe hematomas as a consequence of femoral artery puncture.

RESUMEN

Jejunal diverticulosis is a rare malady and it is often asymptomatic. It may lead to chronic non-specific or acute symptoms such as malabsorption, intussusception, obstruction, bleeding, perforation and abscess formation. It usually is seen as an incidental finding on computerized tomography, enteroclysis or during an emergency operation. Since the advent of double balloon enteroscopy and capsule endoscopy, several cases of small bowel diverticulosis with complications have recently been reported. Lipomas are the rare benign tumors of the small intestine with no malignant potential and they are mostly incidentally encountered during investigation of the gastrointestinal tract. We report here on a case of massive small bowel bleeding with jejunal diverticulosis, and a pedunculated elongated lipoma was incidentally found.

RESUMEN

Aspirin and non-steroidal anti-inflammatory drugs (NSAIDs) are widely prescribed drugs, and they are known to be responsible for many cases of gastroduodenal ulcerations by inhibiting prostaglandin synthesis via blocking the cyclo-oxygenase production. Colonic side effects of these drugs are rare, but they are increasingly being reported to be due to the popularization of colonoscopy and the new formulations of drugs such as enteric coated pills or slow release pills. There is currently no consensus for making the clinical diagnosis and administering the proper therapy for drug-induced colonopathy in Korea. We experienced the patient who had multiple colonic ulcers after ingesting large amounts of aspirin and NSAIDs. In near future, we hope to determine the clinical and endoscopic features of drug associated colon injury.

RESUMEN

Retrobulbar block is commonly performed under monitored anesthesia prior to cataract surgery. Known complications associated with retrobulbar block include cranial nerve palsies, seizures, and cardiorespiratory arrest. We report a case of severe shivering following a retrobulbar block. Two minutes after the block was administered, the patient experienced severe shivering, which subsided after injection of pethidine 25 mg. The likely etiology of the shivering was inadvertent dural puncture of the optic nerve sheath and local anesthetic spread into the cerebrospinal fluid space. Shivering may be a warning sign of brain stem anesthesia, and in such a scenario the clinician should direct special attention to possible life-threatening complications.

RESUMEN

Ultrasound-guided catheterization of the central vein is a precise, safe, and easy compared with the conventional landmark method. We incidentally detected thrombosis in the right internal jugular vein (IJV) during the ultrasound-guided cannulation (on) in a 22-month-old male patient. Cardiac operations had been performed 5 times previously and the right IJV was cannulated lastly 2 months ago. The thrombosis occluded right IJV near totally, so we cannulated the left IJV instead using the ultrasound. This case demonstrates that ultrasound-guided central vein catheterization approach is helpful not only in precise puncture but also in selection of appropriate vessel.

BACKGROUND: Influenza is a highly contagious respiratory disease. Influenza virus, which causes epidemics every winter season, has the high possibility of appearing with new virus types every year due to antigen variation. Therefore, we intended to analyze the data on the epidemiology of influenza that had been acquired by laboratory surveillance in Incheon during the 2003/2004 and 2004/ 2005 seasons and to apply the knowledge to the control and prevention of influenza in Korea. METHODS: Specimens were inoculated into Madin-Darby canine kidney (MDCK) cells and, when cytopathic effect (CPE) was seen, culture supernatants were tested by mutiplex RT-PCR for typing and subtyping of influenza viruses. RESULTS: The first virus of the season was isolated at week 47 (3rd week on November) in 2003 during 2003/2004 and at week 43 (4th week on October) in 2004 during 2004/2005, which was about 4 weeks earlier than in the 2003/2004 season. From 532 specimens cultured for influenza virus during the 2003/2004 season. 330 (62.0%) viruses were isolated: 161 (48.8%) A/H3N2, 1 (0.3%) A/H1N1, and 168 (50.9%) B. During 2004/2005 season, 457 specimens were tested and 278 (60.8 %) were positive for influenza virus: 232 (83.5%) A/H3N2, 5 (1.8%) A/H1N1, and 38 (13.7%) B. The incidence of influenza was the highest in the school-age children and young adults of 7 to 19 years age group in both seasons. CONCLUSION: Influenza virus was isolated at a high rate (more than 60%) by the laboratory influenza surveillance system in Incheon during the 2003/2004 and 2004/2005 seasons: the predominant strain was influenza A/H3N2 subtype.

PURPOSE: As the use of hormone replacement therapy for the menopausal women increases, some caution is advised, since there is an increased risk of breast cancer. Accordingly, the importance of regular mammography has been addressed. This cross-sectional study analyzed the effects of different hormone therapies on mammographic density. MATERIALS AND METHODS: Sixty-seven postemenopausal women who had completed one year of hormone therapy and had undergone follow-up mammography, were divided into two groups : Group I : continuous conjugated equine estrogen, 0.625 mg, plus continuous medroxyprogesterone acetate, 2.5 mg (n=48), Group II : continuous conjugated equine estrogen 0.625 mg (n=19). The mammograms were read by two radiologists. RESULTS: With regard to the radiologists involved, interobserver reliability (kappa) was 0.70 and intraobserver reliability (kappa) was 0.51 and 0.67. Before hormone therapy, factors related to decreased mammographic density were age and number of full term pregnancies (p<0.05). After one year of hormone therapy, body fat showed a significant increase (p<0.05), but in spite of this, increased mammographic density induced by hormone therapy remained significantly high (p<0.05). Compared with Group II, Group I showed a significant increase in mammographic density (p<0.05). In Group I, mammographic density increased from P2 to DY pattern in two cases, but there was no such change in Group II. CONCLUSION: The increase of mammographic density seen in Group II was much more significant statistically than that seen in Group I. The mammograms of women who have undergone continuous combined hormone therapy should therefore be interpreted very cautiously.
