RESUMEN

ABSTRACT Objective: To evaluate the perception and self-confidence of Dental interns regarding endodontic treatment performed on patients. Material and Methods: A total of 111 dental interns from 5 different educational institutes of a state in South India participated in a questionnaire-based survey. The questionnaire consisted of 12 questions related to root canal procedures performed on patients. Analysis was done using SPSS version 17.0. Results: Sixty-five interns (59%) performed less than 10 root canal treatments on patients during the course of the internship. Twenty-eight interns (25% of the intern population) took up more than 10 root canal treatment cases, whereas only 18 interns (16%) did not perform the treatment on patients and answered the questionnaire based on their experience with extracted teeth model work. Only five interns out of 111 were 'Very confident' and 22 were 'Confident' in performing the root canal treatment. Conclusion: The present study indicates that the self-perceived confidence level among dental interns can affect their clinical performance. It also recommends additional hands-on workshops, problem-based learning and clinical guidance for the dental interns to become more confident with the root canal procedure.

RESUMEN

Purpose: The aim of this study is to evaluate and compare the effectiveness of five pre?treatment behaviour modification techniques in 4–7?year olds in reducing dental anxiety by evaluating pulse rate, partial pressure of oxygen, systolic blood pressure, diastolic blood pressure, salivary flow rate, salivary pH, and through modified facial anxiety scale. Material and Methods: Using simple random sampling technique (drawing of lots), 125 children were equally distributed into 5 groups of 25 each: Group 1: tell?show?do (control); Group 2: tell?show?play?doh; Group 3: Playmobil Dentist; Group 4: mobile dentist games; Group 5: role play as dentist. Samples in each group were treated in a single appointment after using the behaviour modification techniques. Class I or Class II cavities were prepared on carious primary molar and restored using glass ionomer cement. Patient’s anxiety level was assessed by recording blood pressure, pulse rate, oxygen saturation, salivary pH, salivary flow rate, and facial anxiety scale at different time intervals, that is before the treatment, during the treatment, and after the treatment. Results: All the intervention Groups (Groups 2–5) showed reduced anxiety scores in both physiological and facial anxiety compared to the control group, that is tell, show do. The modified distraction techniques aid in better modification. Conclusion: Tell?show?play?doh, Playmobil dentist games, mobile dentist games and role play as dentist are effective distraction techniques as compared to conventional tell?show?do techniques which can be incorporated in day?to?day clinical practice to reduce dental anxiety in paediatric patients

RESUMEN

Background: The global health burden from domestic violence against women in reproductive age group is about 9.5 million disability adjusted life years (DALYs). Women suffering from violence have more chances of suffering from physical, emotional, and mental problems such as anxiety, depression and post-traumatic stress disorder. The objectives of the present study are to find the prevalence and awareness of domestic violence in an urban slum of Gurugram and to elicit associated risk factors and the reasons for tolerance of domestic violence. Methods: A community based, cross sectional study was conducted among married women (18-45 years) using a semi structured questionnaire in a rural area of Gurugram by systematic random sampling. Sample size collected was 900. Study population was enquired about the awareness regarding domestic violence, self-experience about domestic violence and about the form of violence experienced and the reasons for their tolerance. Data was entered and analyzed using Epi Info ver 7.Results: Total 980 subjects were enquired about their awareness and self-experience of domestic violence. Overall prevalence of domestic violence in the study population was 28%. Prevalence of domestic violence was statistically significantly associated with education, employment, duration of marriage (p value <0.05).The prevalence was highest of emotional violence (40.5%), followed by physical (33.4%), economic (21.4%) and sexual violence (4.7%).Conclusions: Public health professionals should emphasis on measures to raise public awareness so that women can talk freely about domestic violence and its consequences and help in mitigating this medico-social problem.

RESUMEN

We examined interactions between the 83 kDa heat-shock protein (Hsp83) and hsrω long noncoding RNAs (lncRNAs) inhsrω66 Hsp90GFP homozygotes, which almost completely lack hsrω lncRNAs but over-express Hsp83. All +/+; hsrω66Hsp90GFP progeny died before the third instar. Rare Sp/CyO; hsrω66 Hsp90GFP reached the third instar stage butphenocopied l(2)gl mutants, becoming progressively bulbous and transparent with enlarged brain and died after prolongedlarval life. Additionally, ventral ganglia too were elongated. However, hsrω66 Hsp90GFP/TM6B heterozygotes, carrying +/+ or Sp/CyO second chromosomes, developed normally. Total RNA sequencing (+/+, +/+; hsrω66/hsrω66, Sp/CyO; hsrω66/hsrω66, +/+; Hsp90GFP/Hsp90GFP and Sp/CyO; hsrω66 Hsp90GFP/hsrω66 Hsp90GFP late third instar larvae) revealedsimilar effects on many genes in hsrω66 and Hsp90GFP homozygotes. Besides additive effect on many of them, numerousadditional genes were affected in Sp/CyO; hsrω66 Hsp90GFP larvae, with l(2)gl and several genes regulating the centralnervous system being highly down-regulated in surviving Sp/CyO; hsrω66 Hsp90GFP larvae, but not in hsrω66 orHsp90GFP single mutants. Hsp83 and several omega speckle-associated hnRNPs were bioinformatically found topotentially bind with these gene promoters and transcripts. Since Hsp83 and hnRNPs are also known to interact, elevatedHsp83 in an altered background of hnRNP distribution and dynamics, due to near absence of hsrω lncRNAs and omegaspeckles, can severely perturb regulatory circuits with unexpected consequences, including down-regulation of tumoursuppressor genes such as l(2)gl.

RESUMEN

Toxic epidermal necrolysis (TEN) and Stevens-Johnson syndrome (SJS) are rare but potentially life threatening cutaneous adverse drug reactions. Drugs commonly implicated are anti-microbials, anti-epileptics and non-steroidal anti-inflammatory drugs (NSAIDs). Amongst anti-epileptics, carbamazepine and phenytoin are the most common offending drugs. We report here a case of SJS due to phenytoin.

RESUMEN

Uterine inversion is an unusual, rare entity in which the uterus descends in reverse direction exposing endometrial surface through the cervix and classified as puerperal or obstetric and non-puerperal or gynecological inversion. Non-puerperal uterine inversion is very rare and often clinically misdiagnosed by the clinician. Cervix remains drawn up thus making the vaginal part of cervix difficult to palpate. Acute inversion usually occurs in the 3rd stage of the labor and is an obstetric emergency. Diagnosis is based on the clinical and imaging techniques. We report a case of a postmenopausal non-puerperal uterine inversion secondary to submucous fibroid and its successful management

RESUMEN

Background & objectives: There is limited information available about the drug resistance patterns in extrapulmonary tuberculosis (EPTB), especially from high burden countries. This may be due to difficulty in obtaining extrapulmonary specimens and limited facilities for drug susceptibility testing. This study was undertaken to review and report the first and second-line anti-TB drug susceptibility patterns in extrapulmonary specimens received at the National Institute for Research in Tuberculosis (NIRT), Chennai, India, between 2005 and 2012. Methods: Extrapulmonary specimens received from referring hospitals were decontaminated and cultured using standard procedures. Drug susceptibility testing (DST) for Mycobacterium tuberculosis was done by absolute concentration or resistance ratio methods for the first and the second line anti-TB drugs. Results: Between 2005 and 2012, of the 1295 extrapulmonary specimens, 189 grew M. tuberculosis, 37 (19%) cases were multidrug resistant (MDR) while one was extensively drug resistant (XDR). Specimen-wise MDR prevalence was found to be: CSF-10 per cent, urine-6 per cent, fluids and aspirates-27 per cent, pus-23 per cent, lymph nodes-19 per cent. Resistance to isoniazid and ethionamide was found to be high (31 and 38%, respectively). Interpretation & conclusions: Drug resistance including MDR-TB was observed in a significant proportion of extrapulmonary specimens referred for DST. Access to culture and DST for extrapulmonary specimens should be expanded. Guidelines for MDR-TB management should have explicit sections on extra-pulmonary tuberculosis and training on laboratory techniques is urgently required.

RESUMEN

Expression of fimbriation was studied in Escherichia coli K-12 CA8000 HfrH, and its cya, crp and MS2 resistant mutants. The cells of cya+ crp+ parent strain were observed to be flagellated bacilli, lacking fimbriae, unable to agglutinate erythrocytes and deficient in ability to produce surface pellicle during growth in stationary culture. The cells of cya and crp mutants were observed to be cocci or coccobacilli devoid of flagella, having haemagglutinating activity, fimbriated and capable of producing surface pellicle in stationary cultures. The fimbriation and haemagglutinating activities were lower in cya mutants grown with cAMP supplementation. The cya and crp mutants produced relatively small, smooth and compact colonies consisting mostly of fimbriated cells, like those of earlier described Fim σ mutants. The cya+ crp+ MS2 resistant mutant produced large sized colonies like those of parent but was deficient in conjugal donor ability. It resembled cya and crp mutants in haemagglutinating and fimbriation properties. The cya and crp mutants have been earlier shown to be deficient in several Tra functions including conjugal donor ability. It is concluded that Escherichia coli K-12 cells express fimbriation when Tra functions of F-plasmid carried in them are not expressed either due to deficiency of active cAMPreceptor protein complex or mutation in F-plasmid or when F-plasmid is absent.
