RESUMEN

The study was carried on 79 adenoidectomized patients suffering from secretory otitis media. Forty patients received medical treatment for 3 weeks consisting of decongestant nose drop, septazol suspension, phenadone syrup with one of three different types of mucolytic agents. The cure rate was 53.3%, 40% and 40% according to the mucolytic agent used as a part of the combination therapy. The average cure rate of medical treatment was 45%. In another 39 patients myringotomy and insertion of ventilation tubes were done. Four commonly used ventilation tubes were the Shepard Grommets, Collar Button, T tube and Paparella tube in 34, 8 and 4 ears, respectively, to compare the incidence to complications including otorrhea, obstruction and extrusion. The average follow up was 5 months

RESUMEN

Vidian neurectomy is a relatively recent procedure used by the etolaryngologist to treat vasomotor rhinitis and sphenopalatine neuralgia. The procedure also interrupts the visceral motor [parasympathetic] nerve supply to the lacrimal gland and therefore has been in the treatment of crocodile tears following a seventh nerve palsy [Where tympanic neurectomy is impracticable or has failed]. In this paper we propose an additional indication for this operation namely epiphora caused by absence or stenosis of the lacrimal punotum and canaliculi; following dacryocystectomy or due to chronic dacryocystitis

The problem of one stage surgical correction of congenital external ear canal atresia and auricular deformities is presented. A brief embriological review is made and a case of combined first and second branchial arch deformity is presented with a satisfactory surgical correction. Indications for surgery are discussed

RESUMEN

The caloric vestibular nystagmus standardwasobtainedwhile processing the electronystagmograms in otosclerotic patients without any apparent vestibular pathology before operation in Kasr El-Einy Hospital, Cairo University. The mean duration of otosclerosis was 12 years [5-24 years], 10 males, 14 females, careful neurootological examinationand pure tone air and bone conduction audiometry, optokinetic nystagmus [OKN], spontaneous and positional as well as caloric induced nystagmus was examined in the supine and both lateral positions Seven patients had spontaneous nystagmus,therewas depressed caloric reaction [canal paresis], in 7 patients too, i.e. more than 50% having vestibular disturbances.The mean age of patients who had spontaneous nystagmus was 30 years and the duration of otosclerosis 6-17 years [mean 11 years]. The mean age of canal paresis patients was 36 years and duration of otosclerosis 6-24 years [mean 13 years] twelve patients show normal ENGfindings mean36 years. They suffered from otosclerosis for 12 years on average. [OKN] were normal in all patients.Thereweremeasurablehearing disturbances both air and bone conduction. In spite of a lack of subjective vertigo our objective findings of vestibular dysfunction were increased with longer duration of otosclerosis. The vestibular dysfunction of otosclerotic patients was more common than could be expected,and vestibular function tests must be done as additional diagnositic criteria in otosclerosis and in selection of patients before surgical interference ot minimize post operative sensory neural hearing loss

RESUMEN

50 patients with allergic rhinitis were treated with intranasal beclomethasone dipropionate Aerosol [Kenalog] and Intranasal injection with triamciolone diacetate [Kenalog]. The beclomethasone was administered at 50 micro g in each nostril four times daily and the triamcinolone diacetate was injected in the middle third of inferior turbinate using Batman 5 ml. metal and glass syringe. 2 cc. were injected in each turbinate [40 mg]. Treatment was allocated on a randomized bases. Both groups received treatment for two weeks and in the second group the injection was repeated after one week. Follow up was continued for 1 months for both groups. Some case were followed up from 6 months to 1 year, utilizing patient daily symptoms, diary cards, clinical assessment an local examination of the nasal mycosa before and after treatment, nasal smeers for eosinophils and other cell's blood analysis for eosinophils. It was concluded that both intranasal beclomethasone dipropoinate aerosol and intranasal injection of triamcinolone diacetate caused reduction of the symptom associated with allergic rhinitis. No side effects were noted nor were there any adverse changes observed in the state of the nasal mucosa
