@#Stooped posture, a forward trunk flexion, is a common clinical feature in patients with Parkinson’s disease (PD). However, the exact etiology and effects on gait and balance are not fully understood. In the present study we evaluated the effects of stooping on gait and balance using three-dimensional motion capture and clarified the relationship between the trunk angle and impaired motor function in patients with PD. Thirty-nine patients diagnosed with PD were enrolled in our study. All participants were asked to walk a 6-m tract at their preferred speed, gait parameters and trunk flexion angle were measured using a three-dimensional motion capture system. We analyzed the correlation between trunk angle and gait parameters including gait speed, length, and center of pressure distance for postural sway. Significantly negative correlations were observed between the trunk flexion angle and gait speed (r = -0.407, p = 0.010) and step length (r = -0.561, p < 0.001). Conversely, no correlation was found between trunk flexion angle and postural sway in static standing. We found that stooped posture destabilized gait pattern and did not affect postural sway in PD. Our result showed that stooped posture may not be a compensatory action for stabilizing gait and posture, but rather a symptom of PD.

Epidermolysis bullosa aimplex is a dominantly inherited syndrome characterized by the dissolution of the basal epidermal cells with mechanical stress. A 23 year-old man developed recurrent bullous eruptions with pain on hands, feet and face since childhood, which healed spontaneously without scarring. Grandmother of his father's side, father, aunt of his father's side and two of his three brothers had suffered similar skin lesions. Light and electron microscopy showed degeneration and bulla formation in the basal cell layer of epidermis and intact basement membrane attached to dermis.

The CRST syndrome, first reported by Winterbauer, represents a benign variant of scleroderma and consists of calcinosis(C), Raynauds phenomenon(R), sclerodactyly(S) and telangiectasia(T). The authors report a case of CRST syndrome confirmed by both clinical and histopathological findings. A 62-year-old female developed subcutaneous nodules on both elbow and knee joints, Raynauds phenomenon, sclerodactyly with acrosclerosis and scleroderrnatous changes on both hands and forearms and telangiectasia on the face, neck, and hands of 15 years duration, but she had no difficulty in swallowing. The authors treated a case of CRST syndrome with weekly intra-arterial administration of reserpine and were impressed not only by the marked loosening of the skin but also by the striking effect on the Raynauds phenornena; the latter disappeared within a few weeks of treatment.

Malignant schwannoma is a rare spindle cell tumor derived from schwann cells. There is no significant age or sex distribution. The typical clinical presentation is that of a painless slowly enlarging soft tissue mass, occasionally associated with a peripheral neuropathy. Tumors tend to recur lacally and eventually metastasize, commonly to the lung. We report a case of solitary malignant schwannoma with metastsis to the regional axillary lymph nodes. The patient is a 50-year-old male who had the recurred tumor on right upper arm and child fist sized palpable painless mass on right axilla, which were confirmed to be malignant schwannoma and. lymph node metastasis. But we could not find any evidence of pulmonary metastasis. Tumor and metastatic lymph nodes were widely excised and skin graft was performed.

We have encountered many patients with dermatologic problems caused by animals, but there are not so many systematized literature about them in Korea. Since these causative animals are different from foreign animals in their distribution and habitat, there have been much difficulties in making diagnosis and in studying them based on foreign literature. We have undertaken this study to get the basic material for the possible causative animals in Korea by comparing with foreign literature, searching out the books of zoological claasification, and through personal contacts. One hundred and five species are found to be identical with the species reported in foreign literature, but 324 species are not, although they could be placed in the same genus.

Acute febrile neutrophilic dermatosis, which was introduced to the dermatologic literature by Sweet in 1964, is characterized by persistent high fever and preceding upper respiratory infection like symptoms, polymorphonuclear leukocytosis in the peripheral blood, raised painful plaques on the limbs, face and neck, histologically a dense dermal infiltration with polymorphonuclear leukocyte, dramatic response to corticosteroids, and the absence of scarring. Case 1 was a 35 year-old female who had suffered from malaise, sore throat, and fever 3 to 7 days before the each episode of the cutaneous manifestation. Painful, red, raised, well-demarcated plaques appeared on the face with tendency of recurrences, and was accompanied by a few, small, shallow ulcerations on the lower Iip and tongue, and conjunctivitis. Polymorphonuclear leukocytosis (10400 with 65%, 11600 with 75%) and elevated ESR(23mm/hr, 19mm/hr) were found on two occasions. Histologically a dense inflammatory cell infiltration composed predominantly of polymorphonuclear neutrophils, small round cells, and a few eosinophils were seen in the dermis and subcutis, especially around the dermal capillaries. The nevua of Ota is a congenital pigmentary disorder of the skin and mucaus membrane, partieularly in the periorbital region, the temple, the forehead, the malar area and the nose. The nevus of Ita differs from the nevus of Ota in its distribution in the supraclavicular, scapular, and deltoid regions. It may occur alone or in association with ipsilateral or rarely with bilateral nevus of Ota. We present a 22 year-old female who has had pigmentary anomalies on the both malar areas and left sclera associated with pigmentation of the right shoulder.

A neurilemmoma or schwann cell tumor is a rare and distintive nerve sheath tumor. Neurilemmomas usually are solitary and affect woman more frequently than men withaut particular affinity for any age group. They occur commonly on the cranial nerve, with the most acoustic nerve. When peripheral neurilemmomas are found, they are seen to occur on nerves in the scalp, face, neck, extremities and also visceral organs. Clinically, they are rounded or ovoid with uniform texture and consistency varyingfrom soft to moderately firm in gray, pink or yellowish color and are accompanied with intermittent radiating pain along the course of a peripheral nerve. We present a case report in a 28 year old man who developed two coin size ovoid painful masses on the left extremity for 6 months and describe a brief review associated with the analysis of the statistics of 56 cases of neurilemmoma for the past 9 years' duration in our hospital.

Pyogenic granuloma is not an uncommon oral cavity lesion and is a benign soft tissue tumor arising from the connective tissue of the skin or mucous membrane. We have reported a pedunculated tumor on the junctional area of the left side of upper lip and buccal mucosa which was developed without any history of trauma and clinically it was composed with condyloma acuminatum. Histopathological findings showed many newly formed blood vessels with marked endothelial swelling and collarette formation of rete ridges of epidermis. The lesion was completely subsided after surgical excision.

A case of Dariers disease was presented and the literatures were reviewed. The patient was 20 year old male whose skin lesions revealed normal skin colored, dirty, scaly, keratotic papules with mild itching on face, anterior chest, back, abdomen, axillae, groins and flexural surfaces of extremities. They had a tendency to aggrevate in summer and improve in winter. Histopathologically, suprabasal lacunae, hyperkeratosis, papiIIomatosis, moderate degree of acanthosis and corps ronds in Iacuna were demonstrated in the epidermis. Dermal papillae surrounded with single layer of basal cells proliferated inta the lacunae. Upper dermis revealed mild nonspecific inflammatory cell infiltrates.

The 447 cases of skin tumors were studied at the departments of dermatology and surgical pathology, National Medical Center in Seoul during the period of 18 years and 8 months from January, 1959 to August, 1977. The results were as follows: 1) the total number of specimens surveyed in the study was 447, among which benign tumors accoungted for 234 cases and malignant tumors for 213. 2) Male to female ratio for benign tumors was 1.2:1 and for malignant tumors 2.2:1. 3) Among the 234 cases of benign tumors, 67 cases were epidermal cyst. 53 cases of hemangioma and 34 cases of pigmented neni. 4) Among the 2 cases of malignant melanoma. 5) The peak age incidence for benign tumors were below the 2nd decade, The peak ago incidence for malignant tumors were 50th decade. 6) the sites of predilection of the epidermal cysts were trunk, face and neck. The sites of predilection of the hemangiomas and pigmented nevi were face. 7) The sites of peredilection of the squamaous cell carcinomas were penis(27.1%), lower extremities(25.7%) and face(21.5%) The sites of predilection of the basal cell carcinomas were face(83.3%) 8) The ratio of squamous cell carcinoma ws basal cell careinoma was 4:1. 9) The accuracy of clinical dianosis were 42.7% for benign tumors and 79.3% for malignant tumors.

This study was made in order to investigate the present status of superficial dermatophytoses and some changes on the frequency of the diseases, sex distributions and varieties of the causative organisms. The survey was made by 1486 out-patients of dermatology clinic of National Medical Center from May to August in 1978. Following results are obtained: 1. The incidence of superficial fungal infections including Tinea versicolor is 22. 2% and Tinea versicolor alone is 2. 8% among total l486 out-patients. 2. The ratio of male to female patients is 190:140 and shows more prevalence in male. 3. The age group from 20 to 29 shows the highest incidence and 38. 3% of all patients with superficial fungal infection falls in this age group. 4. Tinea pedis reveals the commonest disease and others in decreasing frequency are Tinea corporis, Onychomycosis, Tinea manus, Tinea corporis, Candidiasis and Tinea capitis. 5. Coexisting fungal infection is found in 60 patients and is most prevalent in the age group from 40 to 49, related to the ratio of age specific coexisting cases to each age group patients. 6. Tinea pedis with Onychomycosis reveals the most prevalent coexisting superficial fungal infection and others in decreasing frequencies are Tinea pedis with Tinea manus, Tinea pedis with Tinea cruris, Tinea cruris witb Tinea corporis and so on. 7. Positive rate of KOH mount among 362 scrapings is 64. 7% and Tinea cruris shows the highest positive rate (86. O%.) and Tinea corporis the next (78. 6%). 8. 187 strains are isolated by Sabourauds dextrose agar, 140 from positive KOH scrapings and 47 from negative KOH scrapings. a. Trichophyton rubrum is the most common organism of superficial dermatophytosis (43 8%) b, Microsporum species reveal the most common organism of Tinea capitis (80%).

We studied in acne vulgaris of Korean adolescence by inspections and questionaires. We examined 4,766 of male and 4,178 of female, aged 10 to 21. Maximal prevalence rate was observed in the group of 20 years old male (84.4%) and 18 years old female (50.4%). Generally, prevalence rate and severity in male were higher than fema1e. Female, however, had higher prevalence rate than male before 14 years of age. The mean age of onset was 15.9 + l.4 in male and 14.9 + 1.5 in female. The earliest age with acne eruption was 11 in male and 10 in female. The extensive acne was observed to be more comon in male than in female. The highest prevalerice of more severe forrn tha Grade 2 (clinical acne) was observed in male of 16 years old group (67.9%) and in female of 14 (16.1%). The course of acne vulgaris was 1-2 year earlier in fcmale than in male. Acne vulgaris in Korean was milder, lesser common, and had later development than in Caucasoid.

One case of malignant acanthosis nigicans associated with gastric adenocarcinoma (scirrhous type) and papilloma on the oral mucosa is presented. The patient, 36 year old Korean male, visited S.N.U.H, complaining of sore-mouth and sore-tongue of 20 days duration. He had been affected with severe papillomatosis on the oral mucosa and with hyperkeratosis on the periorbital area, the perioral area, the neck, the axillae, the palms, the umbilical area, the inguinal area, the anogenital area and the soles which were moderately hyperpigmented with grayish to brown color. He had not been complaining of G-I troubles except sore-mouth and sore-tongue, but he was diagnosed as stomach carcinoma by gastroendoscopy, He got an operation of extended total gastrectomy with isoperistaltic jejunal interposition. Post-operative histopathologic examination rvealed well differentiated adenocarcinoma of the stomach which had already metastasized to the neighboring lymph nodes. After operation the papillomatosis on the oral mucosa and skin eruptions had been improved but becarne aggravated again one and half month after discharge.

The diagnosis of calcifying epithelioma is more easily made on the basis of the histopathological findings than the clinical manifestations. Calcifying epidermal cyst is only one that must be differentiated from calcifying epitheIioma because of its similarity in histopathological findings. 9 cases of calcifying epithelioma which have been collected at Seoul National University Hospital for the past ten years, frorn October 1960 to September 1970, are reported. Summary of the clinical and histopathological findings are as follows; 1) Range of age: one to sixty years old. 2) Ratio of sex: nearly equal in both sexes (5 male, 4 female.) 3) The final diagnosis of all of the cases was made not on the clinical basis but on the histopathological findings. 4) Only one case out of nine showed multiple skin lesions.
