RESUMEN

Objective:To analyze the relationship between transition shock of nursing interns and occupational cognition and coping styles, and explore the factors affecting the status quo of nursing interns transformation, so as to provide guidance for the development of nursing students to respond to clinical transition shock.Methods:A cross-sectional survey of nursing students in Hunan Province was conducted among nursing interns from 18 colleges and universities in Hunan Province by using the General Demographic Data Questionnaire and the Transition Shock of Newly Graduated Nurses Scale, the Nurses' Professional Cognitive Survey Scale, and the Simplified Coping Style Questionnaire. The collected data were processed by SPSS 18.0 software, and multivariate linear regression analysis was performed to analyze the influencing factors of the shock.Results:The overall scores of nursing students transition shock were (64.49±16.33) points, while the transition shock was negatively correlated with occupational cognition ( r=-0.433, P<0.01) and positive response ( r=-0.263, P<0.01). And the occupational cognition, proposed academic qualification, reason for choosing nursing profession and positive response were the main factors affecting the transition shock of nursing students ( P=0.00, R2=0.21). Conclusion:The transition shock of nursing students is at the middle level, and has a strong negative correlation with the nursing students' occupational cognition and positive response. School teachers and clinical instructors should pay attention to the confusion, suspicion and unclear feelings and experiences caused by the transition shock of nursing students, helping nursing students to improve their occupational cognition, and learning how to positively respond to the transition shock problem, thereby improving the quality of clinical internship teaching and training of nursing students.

Objective:To deeply study and explore the cognitive and barrier factors of clinical nurses' nutrition management in patients with pressure injury (PI), and provide evidence for developing quantifiable standardized management model and strengthening individualized nutrition management.Methods:A semi-structured in-depth interview was conducted with 11 nurses. The NVivo10.0 software and Colaizzi's 7-step analysis of phenomenological data were used to analyze interview data.Results:The study summarized four themes that hindered nutrition management: subjective judgment bias; education training lags behind and radiates one-sided; lack of policy and configuration; conflict between workload, roles and perceptions.Conclusions:There are many obstacles to the implementation of nutrition management in patients with PI. An objective, feasible and standardized nutrition management plan should be established, support should be provided by policy, configuration and information system, and relevant knowledge training and multi-team cooperation should be strengthened to improve patient life.

OBJECTIVE:To identify Amomum villosum from different habitats and its adulterants. METHODS :Through the identification methods of microscopic characteristics ,microscopic characteristics maps of 9 batches of A. villosum from genuine producing areas ,domestic commercially available A. villosum and its adulterants were obtained. The feature maps were extracted digitally and analyzed by SPSS 21.0 software. RESULTS :Commercially available A. villosum was mainly from Guangdong , Guangxi,Yunnan and Fujian ;the collected adulterants of A. villosum included A. villosum Lour. var. xanthioides T.L.Wu et Senjen , A. aurantiacum H. T. Tsai et S. W. Zhao and other A. species from Yunnan Xishuangbanna , Laos and Myanmar. Under the microscope,it was observed that microscopic characteristics of surface (such as exocarp color ,prickle,non-glandular hairs , endocarp color ,endocarp oil chamber ) of A. villosum from different habitats and its adulterants were different. There was statistically significant difference in fruit width values and endocarp oil point diameter among all samples (P<0.05). CONCLUSIONS:The microscopic characteristics maps of A. villosum from different habitats and its adulterants by the microscopic characteristics identification methods will make up for the deficiency of traditional experience identification. The quantitative analysis of micro-property and the establishment of micro-property database of A. villosum can provide reference for the property identification and quality control of this medicinal material.

Objective:To investigate whether nursing students face the transition shock problems in the stage of clinical practice, and analyze the performance in the dimensions of physical, emotional, intellectual, sociocultural and developmental, and provide reference for formulating clinical intern training programs for nursing students.Methods:A cross-sectional survey of nursing students on the impact of transition shock and its influencing factors were conducted using the General Survey Form and the Transition Shock of Newly Graduated Nurses Scale.Results:The transition shock score of nursing students was 64.49±16.33. The score of physical dimension was 15.73±5.22, intellectual dimension was 13.13±3.80, emotional dimension was 19.15±5.32, sociocultural and developmental dimension was 16.47±5.41. The proposed academic qualifications ( F value was 5.96, P<0.01), the reason for choosing nursing profession ( F value was 7.28, P<0.01), family support level ( t value was -2.75, P=0.01), first internship department ( F value was 5.22, P<0.01), and internship intention ( F value was 3.43, P=0.03) cause the differences in the transition shock of nursing students. Conclusions:Nursing students will face the transition shock problems in the stage of clinical practice, and the impact of physical dimension is the most obvious. In the development of clinical internship training programs, hospitals and institutions should take into account the transition shock of nursing students, as earlier as possible perceive and prevent the occurrence and intensification of nursing students' clinical transformation problems, and assist them to successfully pass the transitional impact stage.

OBJECTIVES@#To understand the psychological status of the staff in a general hospital during the coronavirus disease 2019 and its influential factors, and to provide references for the mental health services to hospital staff.@*METHODS@#Using star platform of questionnaire, the staff in the general hospital were investigated via Depression, Anxiety, Stress Scale (DASS-21), Social Support Rating Scale (SSRS) and Simplified Coping Style Questionnaire (SCSQ). The influential factors were discussed by descriptive analysis, rank sum test, single factor analysis, correlation analysis and multiple factors binary logistic regression analysis.@*RESULTS@#A total of 2 060 valid questionnaires were collected. The negative emotions of nurses and cleaners were the most obvious. The depression scores, anxiety scores and stress scores for nurses and cleaners were 5.06±7.47, 6.36±7.84, 9.75±8.65, and 6.72±8.84, 4.51±6.56, 9.69±9.56, respectively. Multivariate binary logistic regression analysis showed that staff types, education levels, job status, economic situation and concerns on the supplies of protective goods were the main influential factors for depression; staff types, contacting status with infected patients, economic situation, concerns on the supplies of protective goods, history of disease were the main influential factors for anxiety; contacting status with infected patients, economic situation, concerns on the supplies of protective goods were the main influential factors for stress.@*CONCLUSIONS@#There are differences in psychological characteristics among different groups of staff in the general hospital under the outbreak. Thus psychological protection and intervention measures should be formulated according to different groups and work status.

The authors pointed out the development trend of nursing management in medical alliances as informatization, the long-term training mechanism for management talent, and the last is achieving the homogenization of nursing management.

RESUMEN

Traditional mindset in anti-medical violence legislation tends to divert the responsibility to prevent medical violence to the party of the lowest costs, a design of obvious loopholes in reducing medical violence occurrence. By combining the legal liability and penalty adjustment to counter medical violence crimes, the US, Australia and South Korea are trying to match the private cost of victims and its social cost with the punishment of crime law. In this sense, China should highlight the nature of public safety of medical service, zero tolerance report and optimal punishment to medical violence crimes. This approach can take place of the existing incentive mechanism featuring violent conflicts, being conducive to building a safe and harmonious doctor-patient relationship.

From the perspective of law-and-economics, this paper analyzed the characteristics between medical workplace and other workplace in terms of public safety liability, risk identifiability and preventability. Then it went on to explore the costs structure incurred by medical violence, namely personal costs of victims, public safety cost, risk prevention cost and service supply cost. A law-and-economics optimal penalty model is then developed based on the benchmark of crime of violence free of social cost, and by incorporating the social cost of medical violence. And it is found that an upward deflection of the crime cost curve sharply drives up the optimal penalty corresponding to various levels of violence, and lowers the threshold violence constituting criminal offence, thus balancing the social costs borne by the public for medical violence.

Objective@#To evaluate clinical outcomes of autologous and allogeneic peripheral blood stem cell transplantation (PBSCT) for aggressive peripheral T-cell lymphoma (PTCL).@*Methods@#From June 2007 to June 2017, clinical data of PTCL patients who underwent PBSCT were assessed retrospectively.@*Results@#Among 41 patients, 30 was male, 11 female, and median age was 38(13-57) years old. Seventeen patients with autologous PBSCT (auto-PBSCT) and 24 patients with allogeneic PBSCT (allo-PBSCT) were enrolled in this study. Eight patients (8/17, 47.1%) in auto-PBSCT group were ALK positive anaplastic large cell lymphoma (ALCL), 7 patients (7/24, 29.2%) with NK/T cell lymphoma and 9 patients (9/24, 37.5%) with PTCL-unspecified (PTCL-U) in allo-PBSCT group (P=0.035). There were 58.8% patients (10/17) in complete response (CR) status and 11.8% (2/17) in progression disease (PD) status before transplantation in auto-PBSCT group, and 8.3% (2/24) in CR status and 45.8% (11/24) in PD status before transplantation in allo-PBSCT group (P=0.026). The 2-years cumulative overall survival (OS) were (64.0±10.8)% and (53.5±9.7)% for auto-PBSCT and allo-PBSCT respectively (P=0.543). The 2-years cumulative disease-free survival (DFS) were (57.1±12.4)% and (53.5±10.6)% for auto-PBSCT and allo-PBSCT respectively (P=0.701). In patients with dead outcomes after PBSCT, 83.3% (5/6) of death cause was relapse in auto-PBSCT and 41.7% (5/12) of death cause was relapse in allo-PBSCT.@*Conclusion@#Both auto-PBSCT and allo-PBSCT were effective for PTCL. Allo-PBSCT maybe was better than auto-PBSCT for high-risk PTCL with poor prognosis.

Objective:To establish an HPLC method for the simultaneous determination of rutin, salvianolic acid B and ginsen-oside Rb1 in Xinning tablets. Methods:The samples were separated on a CAPCELL PAK MG(250 mm × 4. 6 mm,5μm)column with gradient elution using acetonitrile(A) and 0. 1% phosphoric acid(B) as the mobile phase at a flow rate of 1. 0 ml·min-1. The col-umn temperature was set at 35℃. The wavelengths were set on 354nm, 286nm and 203 nm (in a wavelength switching mode). Re-sults:The linear range of rutin was 0. 0273-1. 3600 mg·ml-1(r =1. 0000), that of salvianolic acid B was 0. 0244-1. 2200 mg· ml-1(r=1.0000),and that of ginsenoside Rb1 was 0.0186-0.9310 mg·ml-1(r=0.9999), and the average recovery (n=6) was 102.3% (RSD=1.1%),98.7% (RSD=0.8%) and 101.7% (RSD=1.8%)(n=6), respectively. Conclusion: A rapid, simple, accurate HPLC method is successfully established for the simultaneous determination of 3 effective components in Xinning tab-lets, which is helpful to the quality control of Xinning tablets.

Objective To explore the clinical efficacy and safety of intraoperative radiotherapy (IORT) for the patients with unresectable locally advanced pancreatic cancer. Methods Cochrane library, PubMed, EMbase, CBM, CNKI, VIP database and WANFANG database were used to retrieve, in addition to manual retrieval important literature references and conference papers. The randomised controlled trials, cohort study, controlled clinical trials of the IORT in unresectable locally advanced pancreatic cancer were searched. According to the inclusion criteria and exclusion criteria, after the extraction of literature and data, RevMan 5.3 software was used for meta-analysis. Results The total of 1 401 articles were retrieved, 7 met the inclusion criteria, including 1 for randomized controlled trials, 4 for cohort study, 2 for case-control study. 313 cases in the IORT group, 362 cases in the control group, and meta-analysis showed that IORT group was better than control group in overall survival (OS) (Z=4.15, P<0.000 1, RR=2, 95%CI 1.05-2.94). Because lack of complete data in local control rate, pain remission rate, complications, descriptive analysis was done, rather than meta-analysis. Conclusions IORT may improve the OS of patients with unresectable locally advanced pancreatic cancer and the local control rate, and effectively alleviate pain with favorale safety but it still needs high quality randomized controlled trials to confirm.

Objective To analyze the diagnostic status of haemophilia in Chinese children in recent years,and to provide information for increasing the life quality of children with haemophilia in China.Methods The pediatric haemophilia cases registration data were collected and analyzed by using questionnaire,from January 1,2008 to March 30,2014 in 13 haemophilia treatment centers of haemophilia treatment center collaboration network of China pediatric group.These centers were from 12 provinces / municipalities.Results A total of 554 cases were collected;median age was 7.0 years old(0.1 to 17.9 years old);among them,481 cases(86.8%) were hemophilia A,and 73 cases were hemophilia B (13.2%);55 mild cases (9.9%),290 moderate cases (52.4%),and 209 severe cases (37.7%);162 cases(29.2%) had family history,the other 392 cases(70.8%) had no family history.The diagnosis was made at a median age of 12.0 month-old(0 to 180.0 months);the diagnosis time was 0.5 months(0 to 144.0 months) after the first bleeding;diagnosis timing with short interval was in 356 cases(64.3%),long interval was in 198 cases(35.7%).The diagnostic timing was not correlated with disease severity (P =0.812) or the family history (P =0.243);but correlated with the severity of first bleeding(P =0.027) and per capita gross domestic product (P < 0.01) in patients' residence.From 1996 to 2013,the annual number of newly diagnosed cases was increasing year by year,with a higher proportion of short interval of diagnose timing.Conclusions With development of hemophilia work in China,the number of diagnosis of haemophilia children is increasing year by year.Moderate and severe hemorrhage are both taken seriously and diagnosed timely.But the diagnosis is delayed in some children.Chinese haemophilia work still need to be strengthen and the propaganda and diagnostic technology are to be popularized.

Objective To investigate the expression of miRNA associated with hepatic fibrosis induced by Schistosoma ja-ponicum soluble egg antigen stimulation in mouse hepatocytes(AML12),so as to lay the foundation for clarifying the mecha-nism of schistosome infection leading to hepatic fibrosis. Methods The expressions of miR-122,miR-182,miR-23b,miR-27b and KSRP in AML12 cells treated with SEA were measured by q-PCR. KSRP protein in cell lyses was measured by Western blotting. AML12 cells were transfected with miR-27b precursor or anti-miR-27b for 24 h,then q-PCR was adopted to determine KSRP mRNA,and KSRP protein was detected by Western blotting. Results The expressions of miR-182,miR-23b and miR-27b were decreased and miR-122 was increased in AML12 cells following SEA treatment(all P<0.05). An increase of mRNA and protein of KSRP expression was also observed in AML12 cells after SEA stimulation(both P<0.05). In addition,KSRP mRNA expression was not changed significantly in AML12 cells transfected with anti-miR-27b or miR-27b precursor,and miR-27b precursor reduced KSRP protein expression as compared with the control. In contrast,the expression of KSRP protein was increased in the anti-miR-27b group and decreased in the miR-27b precursor group. Conclusions After the stimulation of SEA,the expressions of a variety of liver fibrosis-related miRNAs and KSRP change in murine hepatocytes,including miR-27b. And miR-27b can regulate the expression of KSRP. These findings might lay a foundation for further study on the molecular mechanism of fibrosis induced by schistosome infection.

Objective To investigate the optimal operative time interval from the combined application of endoscopic sphincteropapillotomy ( EST) and endoscopic nasobiliary drainage ( ENBD) to laparoscopic cholecystecto-my ( LC) in the treatment of concurrent cholecystolithiasis and choledocholith.Methods Retrospective analysis was made in the 180 patients who had received sequential treatment of concurrent cholecystolithiasis and choledocholith with the combined application of EFS+ENBD+LC.Patients in group A received LC 3 days after EST +ENBD;patients in group B received LC 7 days after EST +ENBD;patients in group C received LC 30 days after EST +ENBD.Comparative analysis was made in the duration of LC,the intraoperative blood loss,the number of conversion to laparotomy and the total hospital expenses among patients in the three groups.Results Operation time in group A,B and C was separately (34.70 ±2.24)min,(51.00 ±2.31)min and (34.43 ±2.51)min.Patients in group B suffered significantly-extended surgical duration ( F=6.15,P<0.01);Intraoperative blood loss in the three groups was separately (28.86 ±3.35)mL,(50.43 ±3.90)mL,(28.57 ±3.40)mL.Significant increase was observed in group B(F=7.23,P<0.01).There were 1,6 and 1 patients who underwent conversion to laparotomy in the three groups. Slight increase was found in group B(χ2 =6.54,P<0.05).Total hospitalization expenses was separately (10 093.29 ± 496.05)yuan,(10 845.86 ±744.27)yuan and (15 250.57 ±1 006.67)yuan.Patients in group C had incurred a significant increase(F=6.62,P<0.01).Statistical difference was equally found.Conclusion 3 days after EST+ENBD is the optimal operative time for LC.

Objective To evaluate the clinical outcome of Osteoset artificial bone mixed Rifampicin for injection after radical debridement in the treatment of sacroiliac joint tuberculosis,and compare with debridement alone.Methods From May 2005 to August 2010,48 patients with sacroiliac joint tuberculosis patients underwent two different surgical methods were selected as analysis subjects in the Orthopedics Department of Chest Hospital of Hebei Province.Among them,27 cases underwent radical debridement and fusion with Osteoset artificial bone combined with Rifampicin for injection (Grafting group),and 21 case were treated with radical debridement only(Control group).The local wound healing,adverse reactions and regular imaging examination (pelvic X-ray and CT scan) were observed,and the surgical time,the intraoperative blood loss,postoperative 6 months and 12 months,18 months bone graft fusion rate,and Majeed scoring results of the patients were recorded and statistically analyzed.Results All patients were followed up from 20 to 30 months with an average of 24 months.There was no significant difference in terms of surgical time and operative bleeding between the two groups(P>0.05).The rate of bone fusion in grafting group was 33.33% (9/27) of 6 months after operation,88.9%(24/27) of 12 months after operation,and 96.3% (26/27) of 18 months after operation.While that in control group was 4.76% (1/21) of 6 months after operation,42.85 % (9/21) of 12 months after operation,and 42.85% (9/21) of 18 months after operation.The differences of these parameters between the two groups were significant(x2 =5.85,20.92,15.90;P<0.05).According to Majeed scoring system,the excellent and good rates were 100% (27/27) in grafting group,76.19% (16/21) in control group,the difference was statistically significant(x2=7.18 P<0.05).Conclusion With Rifampicin loaded Osteoset artificial bone can recruit bone graft mass during the bone fusion for sacroiliac joint tuberculosis,and reduce the adverse reactions,achieve bone fusion earlier than the control group.

Objective:To develop a method for the quality control of Cuochuang powder .Methods:Rhei Radix et Rhizoma, An-gelicae Dahuricae Dadix and Saposhnikoviae Radix were identified by TLC .GC was used for the determination of patchouli alcohol , menthol and borneol .Results:The TLC spots were clear without any interference from the negative control .The linear range of pat-chouli alcohol was 0.020 1-0.805 6 mg· ml-1 , that of borneol was 0.010 0-0.401 6 mg· ml-1 , and that of menthol was 0.005 1-0.202 8 mg· ml-1.The average recovery (n=6) was 102.03% (RSD=0.91%), 100.10% (RSD =1.94%) and 103.15%(RSD=1.68%),respectively.Conclusion:The method is simple, accurate and repeatable, which can be used for the quality control of Cuochuang powder .

Objective To compare the effect of endoscopic retrograde cholangiopancreatography sphincterot-omy plus laparoscopic cholecystectomy(ERCP/S +LC)and laparoscopic common bile duct exploration(LCBDE)in the treatment of cholecystolithiasis patients with concurrent choledocholith.Methods Retrospective analysis was made of information of 280 cholecystolithiasis patients with concurrent choledocholith,who were randomly divided into group A (n =60)and group B (n =220).group A was arranged with LCBDE and group B was treated ERCP/S +LC. The operation time,hospital stay after operation,hospitalization fee,size of common bile duct calculi,stone clearance rate and the incidence of postoperative complications were compared.Results The operation time,hospital stay after operation,hospitalization fee,size of the common bile duct calculi,stone clearance rate and the incidence of postopera-tive complications of group A &B were (95.58 ±22.66)m vs.(87.46 ±40.25)m,(15.18 ±5.46)d vs.(14.32 ± 4.21)d,(2.45 ±0.32)ten thousand yuan vs.(2.89 ±0.64)ten thousand yuan,(1.12 ±0.34)cm vs.(1.39 ± 0.38)cm,96.6% vs.91% and 1.6% vs.3.2%.There was no significant difference in calculus clearance rate between the two treatments(χ2 =1.44,P >0.05 ).Patients in LCBDE group spent slightly less on hospitalization expenses than patients in ERCP/S +LC group,but there was no significant difference shown(t =0.923,P >0.05). Patients in ERCP/S +LC group had suffered a relatively high incidence of complications than patients in LCBDE group,and significant difference was found(χ2 =4.17,P <0.05).Conclusion Both LCBDE and ERCP/S +LC are effective minimally invasive therapies for concurrent cholecystolithiasis and choledocholith,and should be clinically applied in accordance with their specific characteristics.

To study the quality of Lonicerae iaponicae flos in compound Yuxingcao tablets. Methods: HPLC-ELSD and LC-MS/MS were used to determine Lonicerae flos using macranthoidin A, macranthoside B and dipsacoside B as the control. Re-sults:HPLC-ELSD was suitable for the detection of Lonicerae flos in compound Yuxingcao tablets. LC-MS/MS was used to verify the results, which could identify the certified or counterfeit Lonicerae iaponicae flos in the samples. Conclusion:The two methods can be used to control the quality of Lonicerae iaponicae flos in compound Yuxingcao tablets.

RESUMEN

OBJECTIVE@#To evaluate the condition of cancer-related fatigue (CRF) in breast cancer patients with chemotherapy and to explore the effect of Yoga on it.@*METHODS@#After the completion of Yoga, 100 breast cancer patients with CRF (CFS>0) were selected and were randomly divided into the Yoga group and the control group (n=50). Patients in the control group only received routine cure and care while patients in the Yoga group received extra Yoga exercise, lasting for 4 months. Cancer fatigue scale (CFS) was evaluated in the 2nd, 4th and 6th round of chemotherapy.@*RESULTS@#At the end, 82 cases qualified for the study, 42 cases for the control group and 40 for the Yoga group. The mean score of body fatigue was 12.67±3.46. There was no significant difference in CRF between the Yago group and the control group before the Yoga intervention (P>0.05). After the 4th round of chemotherapy, the mean scores of CFS and body fatigue in the Yoga group were significantly lower than that in the control group (P<0.05). After the 6th round of chemotherapy, the mean scores of CFS, body fatigue and cognitive fatigue in the Yoga group were lower than that in the control group (P<0.05). Repeated analysis of variance showed that the difference in the overall fatigue, body fatigue and cognitive fatigue between the Yoga group and the control group was significant (P<0.05); the time influence on the overall fatigue, body fatigue and emotional fatigue was significant difference between the 2 groups (P<0.05); there were interactions between the effect of Yago and time on the overall fatigue, body fatigue and cognitive fatigue (P<0.05).@*CONCLUSION@#The body fatigue was more serious in breast cancer patients with chemotherapy. Yoga intervention could significantly reduce body fatigue, cognitive fatigue, thus reduce the overall fatigue in breast cancer patients with chemotherapy.

RESUMEN

<p><b>OBJECTIVE</b>To detect the expressions of CXCR4 and Nrf2 in non-small cell lung cancer (NSCLC) tissues and analyze their association with the clinicopathological features of NSCLC.</p><p><b>METHODS</b>We investigated the expressions of CXCR4 and Nrf2 in 66 NSCLC and corresponding distant normal tissue specimens using immunohistochemistry and real-time PCR.</p><p><b>RESULTS</b>The expressions of CXCR4 protein and mRNA were significantly higher in NSCLC tissue specimens than in the distant normal tissues, while the expression of Nrf2 protein and mRNA increased significantly in NSCLC tissues compared to those in the distant normal tissues (P<0.01). A high expression level of CXCR4 was positively correlated with a large tumor size (P=0.048), poor differentiation (P=0.024), advanced TNM stage (P=0.018), lymph node metastasis (P=0.004), and distant metastasis (P=0.016). The expression of Nrf2 protein was positively correlated with a large tumor size (P=0.008), advanced TNM stage (P=0.028), lymph node metastasis (P=0.038), and distant metastasis (P=0.023). A strong correlation was found between CXCR4 and Nrf2 expressions in NSCLC tissues (r=0.324, P<0.01), and the co-expression of CXCR4 and Nrf2 was strongly correlated with lymph node metastasis and distant metastasis.</p><p><b>CONCLUSION</b>Abnormal expressions of CXCR4 and Nrf2 may contribute to the progression and malignant biological behavior of NSCLC.</p>
