OBJECTIVE: This study was designed to evaluate the effect of growth hormone on bone mineral density of corticosteoid-induced osteoporosis in male rat. METHOD: Twenty Sprague-Dwaley male rats was studied, divided into four group, each group has 5 rats. The group 1 was treated with saline. The group 2 was treated with corticosteroid (Methylprednisolone 10 mg/kg). The group 3 was treated with corticosteroid and growth hormone (recombinant human growth hormone 0.5 IU/kg). The group 4 was treated with growth hormone after corticosteroid treatment. The treatment duration was 6 weeks for each group. After six weeks of hormone administration, the animals were sacrificed, the bilateral femur were removed and tested for bone mineral density using dual energy X-ray absorptiometry and examined histomorphometrically. RESULTS: Administration of growth hormone after corticosteroid therapy, the growth hormone could reverse the decrease in body weight and bone mineral density induced by corticosteroid therapy (p<0.05). CONCLUSION: When growth hormone is administrated after corticosteroid therapy, the growth hormone can protect the osteoporosis in male rats induced by a high dose of corticosteroid.

Radionuclide cerebral angiography is a indirect method of measuring cerebral blood flow in ischmic cerebrovascular disease, but conventional radionuclide cerebral angiogra phy has been interpretated by simple inspection without quantification. We designed this study to obtain the quantitative data via computerized time-activity analysis of the radionuclide cerebral angiography in region of interest. In 30 persons without evidence of cerebrovascular disease right/left ratios were 0.993+0.252(mean+ 2SD) in antenor cerebral artery territories of the cerebral hemisphere and 0.947 0.186(mean+2SD) in middle cerebral artery territories. Thirteen patients of 33 patients(39.4%) with ischemic cerebrovascular disease had right / left ratios that were beyond 2 standard deviation of the normal mean. We suggest this study is valuable in patients suspected Gf cerebrovascular disease while this method will require extensive study.

In order to evaluate the influence of soft contact lens(SCL) on the tear turnover rate in the conjunctival sac with SCL, this study was performed with lacrimal scintillography. Two determinations were carried out in the same eye: the first determination was carried out in both eyes without SCL(control group) and the second determination was carried out in both eyes after adaptation to the SCL from 4 hours to 8 hours only in the left eye(study group). In the results, there was no statistical difference of the fractional turnover rate of the tear between both eyes without SCL. In left eyes, the fractional tunover rate of the study group was significantly lower than that of the control group, because the accumulation of the Technetium 99m sodium pertechnetate in the SCL. In right eyes, there was no statistical difference of the fractional turnover rate of the tear between the control group and the study group. This result may be derived from 'fatigue block' and/or the decreased sensitivity of the cornea fitted with SCL.

Angiography has been playing important roles in diagnosis and treatment in the field of orthopaedic surgery. Conventional angiography is the most reliable and widely used method in diagnosis of peripheral arterial disease. But the clinical use of the conventional angiography has been limited by the risk of possible complications and time-consuming procedures. Radionuclide sngiography is rapidly performed, rapidly interpretable and time-saving procedure for the visualization of arterial tree prior to vascular intervention in the critically ill patient. We have analyzed and compared the 22 cases who had taken radionuclide angiography and conventional angiography simultaneously from November, 1986 to August, 1987 in Department of Orthopaedic Surgery, Hanysng University Hospital. The results were as follow 1. Radionuclide angiography is simple, non-invasive, accurste, reproducible method. It eliminstes the discomfort and morbidity of conventional angiography and can be done on an outpatient basis at a much lower cost. It is useful in the patients who may be allergic to the contrast medium. 2. In cases of arterial occlusion, radionuclide angiogrpahy is also useful in the evsluation of obstruction of major artery and can be substituted for conventionsl angiography. 3. For the purpose of evaluation of success in the cases of vascular reconstruction, radionuclide angiogrphy also demonstrate the vasculsr blood flow without any risk. 4. Although rsdionuclide angiography has limitstion in snatomic detsil, it could be psrtially substituted for conventionsl angiography in the diagnosis of vascular anomalies or tumors.

Ventricular performance was evaluated in 21 patients with uncomplicated acute transmural myocardial infarction (10 anterior and 11 inferior). Left ventricular global and regional ejection fraction, ejection rate and right ventricular ejection fraction were assessed using radionuclide ventriculography. The results were as follows : 1) In anterior infarction, left ventricular global ejection fraction was more significantly depressed than in inferior infarction(mean+/-SD : 33. 0+/-15.4% versus 48.6+/-7.0%, P<0.01). 2) In inferior infarction, right ventricular global ejection fraction was more depressed than in anterior infarction(33.3+/-9.8% versus 45.0+/-13.2%, P<0.05). 3) Regional ejection fraction of septal, apico-inferior and posterolateral wall were found to be depressed in anterior infarction than in inferior infarction, but no significant differences were present. It is concluded that the magnitude of right and left vnetricular dysfunction was affected by the location of acute transmural infarction.

It has been well known that dilated cardiomyopathy (D-CMP) has characterized by systolic dysfunction of left ventricle (LV). But there are few studies about LV diastolic and right ventricular (RV) dysfunction in D-CMP. The purpose of this study is to assess the LV and RV systolic function as well as diastolic function in D-CMP. The purpose of this tudy is to assess the LV and RV systolic function as well as diastolic function in D-CMP. The gated blood pool scan (GBPS) was undertaken in 14 patients with D-CMP and 14 normal controls. The results are as follows : 1) Compared to normal controls, the global and regional ejection fraction of LV were significantly reduced (P<0.001) in D-CMP. But, there was no significant difference in LV ejection time between the two groups. 2) Peak filling rate and peak filling time were significantly reduced (P<0.001, P<0.05) in D-CMP 3) Global ejection fraction of RV was also significantly reduced in D-CMP compared to normal controls (P<0.001) But there was no significant difference in ejection rate of right ventricle between the two groups. 4) There was close correlation (R=0.802) between ejection fraction and filling rate of LV : filling rate was also reduced as ejection fraction decreased. And LV filling rate was also reduced with reduction of ejection rate. It is concluded that D-CMP shows diastolic impairment as well as systolic pumping failure and this systolic dysfunction is accounted for the diastolic impairment in D-CMP. And D-CMP shows also right ventricular dysfunction in almost all cases, as well as left ventricular dysfunction.

RESUMEN

CSF rhinorrhea can be divided into two groups according to its origin: the more common traumatic and the less frequent nontraumatic group. CSF is not only an obvious portal of entry but also a good culture media for bacteria. So meningitis is likely to be developed when CSF leaks. With reviewing some of concerned literatures, We present a case of the recurrent bacterial meningitis due to defect of the cribriform plate resulted from traumatic accident.
