RESUMEN

A rare case of intrasellar arachnoid cyst is reported. The patient was a 55 years old woman presented with 8 years history of blurred vision, headache and bitemporal visual field cuts aggravated for last 4 months. The pathophysiology and the CT and MRI findings being useful in the differentiation of cystic sellar masses were discussed.

RESUMEN

We report. herein a case of pruritic urticatial papules and plaques of pregnancy in a Z8-year-old primigravida. She had the urticarial papules and plaques on the abdomen and thighs, which developed at 30th week of pregnancy. She was treated with topical fluorinated steroid and chlorpheniramine, 6mg/ day. The itching sensation was relieved within 24 hours after the therapy and the skin lesions resolved after delivery.

RESUMEN

Adenocarcinoma metastatic to the skin usually originates in the lung, breast, large intestine or ovary. Cutaneous metastatic tumors from rectum or colon tend to appear in the abdominal or perineal area, but rarely appear in the perianal and genital skin. A 53-year-old female presented multiple nodules in and near the surgical scars of inguinal, pubie, vulvar, perianal areas and normal skin of left thigh two months after surgical removal of rectal adenocarcinoma. Biopsy specimens from the nodules of the perianal area and left thigh showed adenocarcinoma of well differentiated atruetures like the primary rectal carcinoma.

RESUMEN

During the 10-year period up to December 1984, 176 patients with anterior communicating aneurysn(ACOMA) among total 467 patients of intracranial aneurysms were admitted to this Catholic Medical Center. Of these, 135 cases of ACOMA were operated by direct intracranial procedures. To analyse the factors influencing the mortality involving in surgery of 135 patients with ACOMA, a classification of ACOMA was attempted. The origins and projections of aneurysms, anatomical variations were analysed with the aid of angiography, intraoperative findings and intraoperative photographs which permitted the establishment of a classification of ACOMA with their direction. Our classification of operated 135 cases of ACOMA are seven types : 32 anterior(23.7%), 30 anterior-rostral(22.2%), 42 antefior-caudal(31.1%), 3 posterior(2.2%), 18 posterior-rostral(13.4%), 4 posteriorcaudal(3.0%), caudal 6(4.4%), respectively. Most of ACOMA projecting anteriorly, anterio-rostrally were situated above or between the optic nerve, and the less frequent posterior-caudal and inferior aneurysms were in close proximity to hypothalamic branches of the anterior communicating artery and A2 segment. There was on overall operative mortality of 6%. The surgical morbidity and mortality were significantly higher in the posteriorly projecting group, garticulary in the posterior-caudal direction. In direct surgery of ACOMA, subpial resection of the gyrus rectus was effective for not only anteriorly, but also posteriorly directing aneurysms.

RESUMEN

We report herein a case of pityriasis lichenoides et variloiformis acuta in a 48- year-old man, who has wide spread erythematous macule, papule, vesicle, hemorrhagic vesicle and reddish-brown crust on the neck, trunk, axillae and both extremities. He was treated successfully with tetracycline 2. 0 gm a day for 4 weeks.

RESUMEN

This study was performed to investigate clinical and histopathologic features of steroid acne, which was induced by systemic administration and topical application of corticosteroids. Thirty five cases of steroid acne visited to Department of Dermatology, Hangang Sacred Heart Hospital from September, 1g79 to June, 1984 were analyzed, and the results obtained can be summarized as follows: 1. The peak age of the subjects was third decade(42.9%) with an average age of 30. 1 years, and male to female ratio was l.9: 1,2. The skin lesions had unique clinical features that showed many, uniform sized, erythematous papules and pust;ules. 3 The predilection sites of steroid acne induced by systemic steroid therapy were anterior chest(93.1%), back(44,8%), neck(31.0%), shoulder(31.0%) and face (20.7%) 4 Among thirty five cases of steroid acne, twenty cases were induced by parenteral adrninistration of dexamethasone disodium phosphate(group A), nine cases by oral administration of prednisolone(group B), and six cases by topical application of three kinds of steroid creams(group C). 5. The mean induction time after starting steroid in group A(ll. 3 days) was shorter than those in group B and C(18.9 days and l4.8 days respective)y). The mean total dosage of used steroid in group A was 191. 3mg of dexamethasone disodium phosphate and that in group B was 515. 7mg of prednisolone. On histopathologic findings of twenty two skin biopsy specimens of the three groups, perivascular inflammatory reaction was the most common finding followed by intra-and peri-follicular inflammatory reaction, dermal vascular dilatation, necrosis of follicular epithelium, comedo, intraand periollicular abscess and rupture of follicle.

RESUMEN

Epidermal nevus syndrome is now delineated as a congenitally acquired syndrome consisting of deformities of the skin (various linear epidermal or organoid nevi), central nervous system, skeleton, cardiovascular system, and renal system. A 4-year-old girl had linear epidermal nevus on the right lower extremity, right ear lobe and anterior neck, and multiple skeletal abnorrnalities since birth. And she had an adult fist-sied, soft, nontender mass on the right lower abdomen since 2 years of age. Radiologic examination showed scoliosis of the thoracic spines, deformities of the lumbar spines, focal gigantisms of the left hand and right foot, and other skeletal abnormalities. Histological examination of the linear, verrucous lesion of the right leg showed hyperkeratosis and papillomatosis. The surgically removed abdominal mass revealed the finding of hydrosalpinx.

RESUMEN

Cooper at al and Mills et al asserted that the essential pathologic finding of pyogenic granuloma (PG) is a unique lobular organization of capillaries. This pathologic pattern is present not only in conventional cutaneous PG, oral-nasal PG but also rarely in intradermal PG, subcutaneous PG and intravenous PG. A 32-year-old man presented with a two-day history of an asymptomatic, movable, pea-sized nodule on his left forearm. Excision biopsy revealed a pink, oval, cyst-like nodule within the vein of the subcutaneous tissue of the medio-lateral aspect of the forearm. Histopathologic finding of the nodule showed rnultiple lobules of capillaries and the lumens of the capillaries were lined by normal endothelial cells. The nodule was connected with the venous wall by a thin stalk. Each lobule was separated and surrounded by fibrous stroma. There was no evidence of thrombi.

RESUMEN

Cutaneous leishmaniasis (oriental sore) is usually a self-limited infection of the skin caused by the protozoan Leishmania tropica. The disease is endemic to the Mediterranean, Asia, Africa, and the Middle East. It has been seen in this country among many Korean technical experts and labourers working in the endemic areas of the disease. Our patient had acquired cutaneous leishmaniasis in Saudi Arabia and it had remained active for six months. He had been treated with antimony and metronidazole but failed because of severe side effects. And then we treated the patient witb cryosurgery and the skin lesions were followed by resolution with cosmetically acceptable scar in 4 months. The brief review of literature on the treatment of cutaneous leishmaniasis was undertaken.

RESUMEN

This is a report of 23 year old soldier who developed sudden onset of flaccid paraplegia and loss of all sensory modalited below the level of T4. The clinical entities of hematomyelia were sudden onset of pain, repidly developing pyramidal signs and paraplegia. The mainly involving sites of this disease were cervical and thoracic regions. The causative factors were hemorrhagic diasthesis, vascular malformation and inflammatory process as well as trauma and neoplasms affecting the spinal cord. We experienced a case of hematomyelia which did not combine with vascular anomaly, inflammatory process, hemophilia or trauma history.

RESUMEN

Authors experienced the myelotomy for the syringomyelia in 3 cases. Moreover a recent case who underwent the craniovertebral decompression with myelotomy was associated with Arnold-Chiari malformation. Post-operative courses were uneventful in two but a case with the thoracic myelotomy became paraparetic after 5 years of follow up due to the spinal arachnoiditis. It seems that the careful studies, including the ventriculography and/or C-T scan are necessary to find the hind-brain malformation and to give the appropriate management.
