RESUMEN

@#Objective    To analyze the surgical efficacy and influencing factors of myasthenia gravis (MG) patients with thymic atrophy after thymectomy. Methods    The clinical data of MG patients with thymic atrophy undergoing thymectomy between October 2014 and May 2018 in Daping Hospital of Army Medical University and Shijiazhuang People Hospital were retrospectively analyzed. Results    A total of 71 patients were collected, including 40 males and 31 females with a mean age of 45.17±12.42 years. All patients received the surgery successfully. After the surgery, 20 (28.17%) patients were stable remission, 12 (16.90%) patients were minimal manifestation status,19 (26.76%) patients were improved, 5 (7.04%) patients showed no change, 3 (4.23%) patients were worsened, 10 (14.08%) patients were exacerbated and 2 (2.82%) patients were dead. Multivariate logistic regression analysis showed that the preoperative illness duration (OR=4.61, 95%CI 1.13-18.85, P=0.03), and postoperative pyridostigmine combined with immunosuppressive (OR=0.12, 95%CI 0.03-0.45, P=0.00) were independent risk factors for long-term efficacy of thymectomy for MG patients with thymic atrophy. Conclusion    Early surgery after diagnosis of MG and postoperative pyridostigmine combined with immunosuppressive treatment is beneficial to the prognosis of MG patients with thymic atrophy.

RESUMEN

Objective To investigate the detection value of plasma brain natriuretic peptide(BNP) in patients with heart failure.Methods 90 patients with heart failure were selected as subjects of this experiment.These include left ventricular failure,left ventricular diastolic failure and normal cardiac function.The same period 100 cases of healthy people were selected as control group.Compared four groups of patients with plasma BNP.Results plasma BNP of Heart failure patients was higher than the healthy group( P < 0.05 ).The level of the serum BNP of failure patients with left ventricular systolic function was higher than those of other heart failure patients ( all P < 0.05 ).Conclusion Patientswith heart failure plasma BNP to aid medical diagnosis,it has high clinical value.

RESUMEN

Background and purpose:Bone marrow cytomorphology is the main approach for determination of bone marrow involvement in patients with malignant lymphomas. In recent years, with the development of detection of cellular surface markers and molecular biology, flow cytometry (FCM) and polymerase chain reaction (PCR) were gradually applied in the detection of bone marrow in malignant lymphomas. Because such systematic research has not been done domestically , we performed a study to compare diagnostic value and clinipathological signifi cance of cytomorphology of bone marrow aspirates, immunophenotype detected by flow cytometry and immunoglobulin heavy chain gene rearrangement detected by polymerase chain reaction in bone marrow of B cell lymphomas. Methods: Bone marrow cytomorphology, FCM and PCR were simultaneously carried out in 75 bone marrows of B cell lymphoma and compared with each other. Results:(1)16 were demonstrated by cytomorphology, 36 by FCM, 33 by PCR. The positive rates were 21.3%, 48% and 44% respectively. <p><b>OBJECTIVE</b>To study the genetic basis of aberrant crypt foci (ACF), which serve as a very early morphological alteration during the development of carcinogenesis by analyzing the loss of heterozygosity (LOH).</p><p><b>METHODS</b>DNA from 35 colorectal carcinomas (CRC) and 34 matched ACF were isolated by microdissection. LOH of microsatellite loci at 18q12, 18q21, 5q12, 5q21, 3p21, 2p16, 17q21, 17q11 and 11p13 was detected by means of ABI-SEQUENCER and GeneScan software was applied for analysis.</p><p><b>RESULTS</b>The rate of LOH in ACF (41.18%) was less than that in carcinoma (68.57%) (P < 0.05). The profile of LOH rates at loci 18q12, 5q12, 3p21, 17q21, 17q11, 11p13 and 2p16 in ACF was similar to that in carcinoma. The LOH frequencies on 18q12, 18q21, 5q12, 5q21, and 3p21 were higher than that on 17q11 and 11p13. However the rate at 18q21 and 5q21 in ACF was much lower than that in the carcinoma (P < 0.05). The co-existing carcinomas displayed more polypoid growth pattern and located more at the sigmoid colon and rectum. LOH in carcinomas did not correlate with the location, size, type of the carcinoma and Duke's stage.</p><p><b>CONCLUSIONS</b>ACF are putative preneoplastic lesions that might represent the earliest morphological lesion with the alteration at molecular genetic level. Our study provides further genetic evidence in the pathogenesis of colorectal carcinomas.</p>

<p><b>OBJECTIVE</b>To investigate the expression of NOEY2 gene in breast cancer tissue and its relation to clinicopathological and other molecular parameters.</p><p><b>METHODS</b>The mRNA expression of NOEY2 gene was monitored in benign and malignant breast lesions by RT-PCR and in situ hybridization (ISH) with transcripted antisense RNA probes. The protein expression of estrogen receptor (ER), Ki67, p27 and p21(WAF1) in the 60 breast cancer lesions was detected by immunohistochemical method.</p><p><b>RESULTS</b>All 6 benign lesions, and 13 (72.2%) of the 18 breast cancers were NOEY2 positive by RT-PCR. By ISH, positive NOEY2 was obtained in all 10 benign lesions but only in 31 (51.7%) of the 60 breast cancer lesions. The difference was statistically significant (P = 0.025). NOEY2 positive rate tended to decrease with the increase of histological grade. However, NOEY2 expression was negatively correlated with the status of axillary lymph nodes. The positive NOEY2 rate was 75% in those without lymph node metastasis but only 26.7% in those with metastasis (P < 0.001). No correlation with other clinicopathological parameters including ER, Ki67, p27 or p21(WAF1) were found.</p><p><b>CONCLUSION</b>NOEY2 gene may be related with the pathogenesis of breast cancer. A link between NOEY2 loss expression and the spreading mechanism of breast cancer may possibly exist.</p>
