RESUMEN

Background: The aim of the study was to determine the prevalence and factors associated with asymptomatic bacteriuria among pregnant women. Methods: A cross-sectional study was used to determine the prevalence of uro-pathogens among pregnant women attending Antenatal Clinic at Weija-Gbawe Municipal Hospital. The data was collected between May 2023 and October 2023. The data collection process involved the use of a structured questionnaire, while urine specimens were collected in sterile urine containers and cultured on CLED medium. The bacteria colonies were identified and evaluated for their susceptibility to antibacterial agents. Multiple logistics regression was used to determine the factors associated with bacteriuria among pregnant women. Results: The age range of the participants was 14 years to 45 years with mean age of 30 years (SD±6.5). About 13% of the participants had no formal education. Majority (77.4%) of study participants were obese while only 3.42% had normal weight. Isolates were Escherichia coli (27%), Klebsiella species (23%), Proteus species (18%) and Citrobacter koseri (18%). Gentamicin was sensitive to E. coli and cotrimoxazole was the most resistant antibiotics. Factors such as BMI, educational level, smoking status, recurrent UTI, parity were not found to have statistical influence on prevalence of bacteriuria among pregnant women. Conclusions: The study revealed 15.1% prevalence of ASB among pregnant women with E. coli being the predominate isolates. Gentamicin and cotrimoxazole were the most sensitive and resistant antibiotics respectively. Pregnant women should be advised to adhere to optimal hygiene practices and safe sexual behaviours, with a focus on the role of these practices in preventing uro-pathogen colonization.

The Lazarus syndrome, named after the biblical account of Lazarus, being raised from the dead, refers to the uncommon occurrence of spontaneous recovery of circulation following failed resuscitation attempts. This review article delves into the historical foundations, underlying mechanisms, prominent instances, disputes, ethical concerns, medical interventions, and psychological consequences of Lazarus syndrome. Despite being recorded in medical literature, the mechanisms underlying this condition are not well known. Possible explanations include medication-related delays, hyperkalemia, reperfusion damage, myocardial shock, and brainstem reflexes. While Lazarus syndrome calls into question our notion of life and death, it also raises ethical concerns about death verification, informed consent, quality of life, resource allocation, and cultural values therapies include diagnosing underlying reasons, enhanced cardiac life support, extracorporeal membrane oxygenation, therapeutic hypothermia, and ongoing monitoring. The psychological burden on healthcare staff and families is significant, necessitating assistance and coping measures. Understanding and managing the intricacies of Lazarus syndrome is critical to the well-being of everyone concerned.

Acute complicated diverticulitis presents a more severe form of diverticular illness frequently needing urgent medical intervention and possibly surgical therapy. The aim of this review is to gather conclusive evidence from the literature comparing laparoscopic techniques to open and conservative ones in order to determine the most effective treatment plan for complicated diverticulitis. Online databases like PubMed, Google Scholar, Cochrane, Elsevier and many others were systematically searched according to an inclusion criterion to obtain a total of 13 studies to be included in the review. 8/13 studies presented short term outcomes while 5/13 studies concluded with long term outcomes following index surgeries. Based on the end results, it can be concluded that laparoscopic surgery, in particular laparoscopic colon resection is superior to other techniques in treating complicated diverticulitis in terms of fewer short-term complications, low mortality rate better quality of life with few recurrence rates. However, other approaches have their own advantages and can be given priority based on the unique presentation of each case. The clinicians are advised to make informed decisions keeping in view all the patient and disease associated aspects.

Background: Diabetes and depression are two major issues related to community health. Diabetes patients frequently co-occur with depression. Diabetes patients frequently co-occur with depression, which calls for serious attention because delayed diagnosis and treatment can worsen the patients' complications. Assessing the prevalence of depression in diabetic patients and identifying the various factors associated with it were the objectives of this research study. Methods: In this study 70 adult patients suffering from type 2 DM participated in this 6-month prospective study. Sociodemographic data and clinical features of the participants were collected. The presence and severity of depressive symptoms in patients have been assessed by a PHQ9 questionnaire. Ethical approval was taken before the commencement of the study. SPSS (Version 20) was used for data analysis. Results: The majority of the patients were from 41-50 years of age group (32.9%) with a female predominance (58.6%), with no symptoms of depression before type 2 DM (92.9%). Most of them had primary education (32.9%) and majority were homemakers (44.3%) residing in urban are (65.7%) living joint family setup (68.6%). Most of them had 11 to 20 years of type 2 DM duration (745.8%) with a high family history of type 2 DM (75.7%). Majority of them were on Oral therapy (47.1%) with ophthalmic complications (32.9%). The majority of them had mild depression (5-9) i.e. 67.1%. Conclusions: Due to patient-specific diabetes management and inappropriate diabetes treatment, the majority of cases were found to have depressive disorders. This article focused on a few common factors and their relationships that lead to depression in people with diabetes.

Background: Amidst the emerging COVID-19 variants worldwide, India started the COVID precautionary or booster dose vaccination by early 2022. Protecting the healthcare force is crucial, as they are critical drivers in increasing vaccine uptake among the public. Objective was to assess COVID-19 Booster dose vaccine coverage among Health Care Workers (HCWs) and to know the reasons for vaccine booster dose hesitancy. Methods: A cross-sectional study was conducted among health care workers in Vijayapura city, during April-May 2022. The questionnaire in google form was shared to the target population, wherever necessary phone in interview was done. Data was tabulated in MS office Excel and analyzed using appropriate statistical tests. Results: Out of 275 participants from different levels of health care system, 50% were males, 75% in age group <40 years and 67% were from urban area. 37% of the participants took precautionary dose of vaccine against COVID-19. Among those who did not take a booster shot, 22.5% were hesitant to take the vaccine and the reasons were: “Lack of faith in vaccination”, “two doses are sufficient”, and “Fear of side effects”. Socio demographic factors such as age, gender and residence were found to be statistically significant with the COVID Booster vaccination status. Conclusions: Majority health workers are aware of booster dose vaccination, reasons for not taking booster dose were mainly lack of need and fear of side effects.

Background: Medical education in transgender health care can empower physicians to identify and change the systemic barriers to care that cause transgender inequities as well as improve knowledge about transgender specific care. The objective of this study is to estimate the awareness among medical students about the terms related to transgender and their healthcare needs, to create an environment of inclusivity and educate the undergraduate MBBS students to provide highest level of care and advocacy for transgender population and to stress on the fact that the transgender education in the medical curriculum is an inevitable one in the modern medical practice. Methods: Knowledge questionnaire and TABS scale questionnaire was distributed to the 150 students through Google forms and their responses were collected. Knowledge questionnaire was given a score as 1 point for each (28 points). TABS questionnaire was assessed with Likert scale. Results: 52.6 percentage of the students have scored below the median and remaining 48.4 percentage of students have scored above the median score. The undergraduate students have a basic understanding about the healthcare needs of the transgender population and attitude towards transgender is on par with population of similar age group Conclusions: It is been observed that more knowledge regarding the terms about transgender population and their healthcare needs is needed for undergraduate students. A good curriculum to teach about basic health needs of transgender population has to be discussed.

Abstract Sarcopenia is a pathology resulting from a progressive and severe loss of muscle mass, strength, and function in the course of aging, which has deleterious consequences on quality of life. Among the most widespread studies on the issue are those focused on the effect of different types of physical exercise on patients with sarcopenia. This randomized controlled study aimed to compare the effects of a whole-body vibration exercise (WBV) session on the inflammatory parameters of non-sarcopenic (NSG, n=22) and sarcopenic elderly (SG, n=22). NSG and SG participants were randomly divided into two protocols: intervention (squat with WBV) and control (squat without WBV). After a one-week washout period, participants switched protocols, so that everyone performed both protocols. Body composition was assessed by dual-energy radiological absorptiometry (DXA) and function through the six-minute walk test (6MWD) and Short Physical Performance Battery (SPPB). Plasma soluble tumor necrosis factor receptors (sTNFR) were determined by enzyme-linked immunosorbent assay (ELISA) and measured before and immediately after each protocol. After exercise with WBV, there was an increase in sTNFR2 levels in the NSG (P<0.01; d=-0.69 (-1.30; -0.08) and SG (P<0.01, d=-0.95 (-1.57; -0.32) groups. In conclusion, an acute session of WBV influenced sTNFr2 levels, with sarcopenic individuals showing a greater effect. This suggested that WBV had a more pronounced impact on sTNFr2 in those with loss of muscle strength and/or physical performance. Additionally, WBV is gaining recognition as an efficient strategy for those with persistent health issues.

Background: The prevalence of type 2 DM is alarmingly rising on a global scale. Improved treatments for type 2 DM are still needed, in order to slow the disease抯 development. A role in the pathophysiology of type 2 DM has been suggested by the correlation between vitamin D insufficiency and several non-skeletal illnesses, including DM. The goal of the study was to determine if vitamin D supplementation may help type 2 DM patients whose glycemic status was uncontrolled even after using oral antidiabetics. Methods: 60 individuals with type 2 DM and vitamin D insufficiency participated in this 12-week open-label, before-and-after study. For 12 weeks, in addition to oral anti-diabetic medications, these patients also received 60,000 IU of vitamin D3 orally every week. HbA1c, FBS, and 25(OH)D levels parameters were included. Results: The majority of the patients were from 41-50 years of age group (48.3%) with a male predominance (60%). Most of them were having >1 year of type 2 DM duration (78.3%) with a high family history of type 2 DM (70%). After 12 weeks, there was a substantial (p<0.001*) drop in FBG levels and a significant (p<0.001*) decrease in HbA1c. 25(OH)D levels showed a high rise (p<0.001*). None of the patients had any side effects. Conclusions: Vitamin D treatment improves glycemic status, which slows the development of type 2 DM and its associated effects. As such, vitamin D supplementation is a safe and promising adjuvant treatment for individuals with type 2 diabetes who are low in vitamin D.

Abstract This study evaluated the effect of the volatile oil of Alpinia zerumbet (VOAz) on caveolin-1 gene expression and muscular fibrosis. The rats were immobilized to induce fibrosis of the gastrocnemius muscle, and they were treated with VOAz. Collagen quality was assessed by histology and the expression of the caveolin-1 (CAV-1) gene was evaluated using qPCR. Histomorphological analysis indicated a significant reduction in the perimeter, width, and intensity of collagen in the treated groups, thus showing that the oil was effective in regulating the quality of collagen at the three concentrations. The results of expression levels suggested a decrease in the lesioned group and in two treatment groups (0.0115 µg/g and 0.009 µg/g). However, with the lowest concentration (0.0065 µg/g), no significant difference was observed, with levels similar to those found in healthy tissue. Therefore, the results showed that VOAz has the potential to be a non-invasive and low-cost alternative to aid in the treatment of muscular fibrosis.


Resumo Este estudo avaliou o efeito do óleo volátil de Alpinia zerumbet (OVAz) na expressão do gene da caveolina-1 e na fibrose muscular. Os ratos foram imobilizados para induzir a fibrose do músculo gastrocnêmio, e foram tratados com OVAz. A qualidade do colágeno foi avaliada com histologia e à expressão do gene caveolina-1 (CAV-1) foi avaliada usando qPCR. A análise histomorfológica indicou uma redução significativa no perímetro, largura e intensidade do colágeno nos grupos tratados. Os resultados dos níveis de expressão sugeriram diminuição nos grupos de lesão e em dois grupos de tratamento (0,0115 µg/g e 0,009 µg/g). No entanto, com a menor concentração (0,0065 µg/g), não foi observada diferença significativa, apresentando níveis semelhantes aos encontrados em tecido saudável. O uso do OVAz foi eficaz para reverter as alterações do colágeno causadas pela fibrose, e sua menor concentração apresentou uma possível tendência de aumento na expressão do CAV-1. Portanto, os resultados mostraram que o OVAz tem potencial para ser uma alternativa não invasiva e de baixo custo para auxiliar no tratamento da fibrose muscular.

One of the main challenges of tissue engineering in dentistry is to replace bone and dental tissues with strategies or techniques that simulate physiological tissue repair conditions. This systematic review of in vitro studies aimed to evaluate the influence of the addition of nanohydroxyapatite (NHap) to scaffolds on cell proliferation and osteogenic and odontogenic differentiation of human mesenchymal stem cells. In vitro studies on human stem cells that proliferated and differentiated into odontogenic and osteogenic cells in scaffolds containing NHap were included in this study. Searches in PubMed/MEDLINE, Scopus, Web of Science, OpenGrey, ProQuest, and Cochrane Library electronic databases were performed. The total of 333 articles was found across all databases. After reading and analyzing titles and abstracts, 8 articles were selected for full reading and extraction of qualitative data. Results showed that despite the large variability in scaffold composition, NHap-containing scaffolds promoted high rates of cell proliferation, increased alkaline phosphatase (ALP) activity during short culture periods, and induced differentiation, as evidenced by the high expression of genes involved in osteogenesis and odontogenesis. However, further studies with greater standardization regarding NHap concentration, type of scaffolds, and evaluation period are needed to observe possible interference of these criteria in the action of NHap on the proliferation and differentiation of human stem cells.

Background: Epilepsy is the fourth most common neurological disorder in world. Managing an epilepsy with anti-epileptic drugs (AEDs) either as monotherapy or polytherapy is necessary to reduce the deleterious effect of the disease and to provide neuroprotection. AEDs exert their negative effects on cognition by suppressing neuronal excitability or enhancing inhibitory neurotransmission. These neuropsychological side effects are found to be modest when the drug level is within the therapeutic concentration and used as monotherapy. Objectives were to assess the prevalence of neuropsychological side effects among epilepsy patients who were on antiepileptic drug therapy. Methods: An open label, cross-sectional, clinical study was conducted at a tertiary care hospital, 126 participants were recruited. Participants demographic data, detailed medical and seizure history followed by neuropsychological tests was performed. The prevalence was assessed based on the number of participants scoring <15th percentile in one or more tests. Results: Out of 126 participants who were recruited, 82 participants were on monotherapy and 44 participants were on polytherapy. Levetiracetam was the most commonly prescribed drug as monotherapy, followed by phenytoin, carbamazepine and valproate; whereas in polytherapy levetiracetam, clobazam followed by phenytoin were the commonly prescribed AED. The most common adverse effect was drowsiness, followed by headache, hypersensitivity reaction, giddiness, tremors, anxiety etc. The prevalence of neuropsychological side effects was 77.8%. The prevalence of impairment between monotherapy and polytherapy was statistically insignificant (p=0.727). Conclusions: In this study, levetiracetam was the most commonly prescribed drug and the most common adverse effect was drowsiness due to AEDs. The subgroup analyses between monotherapy and polytherapy did not show any statistically significant neuropsychological impairment when compared based on the gender, age groups, duration of epilepsy with medication and duration of current therapy.

Calcinosis cutis is a rare condition characterized by the abnormal deposition of calcium salts in the skin and subcutaneous tissues. It usually occurs as an isolated entity, but is also associated with various systemic disorders. We had reported 8 cases in our department in the past one year. A striking finding was that all of them had clinical symptoms of at least 6 months and had normal serum calcium levels. The gross evaluation of the specimen showed areas of calcification which pointed out towards the diagnosis to be consistent with calcinosis cutis. The various histologic findings were presence of basophilic acellular material corresponding to calcification, eosinophilic hyalinization, foreign body giant cell reaction and an Inflammatory infiltrate. All the biopsies had conclusive evidence of the first finding. It is evident that all the cases encompass a variety of morphological findings and aim to convey a clear histopathological understanding of the lesion for a prompt diagnosis.

Background: Genital herpes is caused predominantly by Herpes simplex virus type 2 (HSV-2) and less commonly by HSV-1. Genital HSV-1 infection results from oral sex, with fewer recurrences, mild symptoms, less asymptomatic shedding and poor genital transmission. The present study was undertaken to identify frequency of occurrence of HSV-1 and HSV-2 in genital herpes by microscopy, serology and Real Time Polymerase Chain Reaction (RT-PCR). Methods: Genital ulcer swabs and serum were collected at regional STI centre, Govt Medical College and Hospital, Nagpur. A total of 53 patients of Genital Ulcer Disease from December 2020 -22 were examined for etiology by microscopy, serum IgM and IgG against HSV-1 and 2 by ELISA and HSV-1 and 2 DNA by RT- PCR. Results: Out of 53 genital swabs processed, 6 (11.3%) and 28 (52.8%) samples were positive for HSV-1 and 2 DNA respectively. Of the 6 HSV-1 DNA positive samples, seropositivity for HSV-1 IgM was in 2 (33.3%) samples and for HSV-1 IgG in 4 (66.7%) samples. Of the 28 HSV-2 DNA samples, HSV-2 IgM was positive in 4 (14.3%) samples and HSV-2 IgG was positive in 7 (25%) samples, multi nucleated giant cells were seen in 2 (7.14%) samples. The remaining 15 (53.6%) HSV-2 DNA positive samples were seronegative. Conclusions: HSV-1 was detected in 6 (11.3%) samples. Though these genital ulcers may be mild, it is important to counsel the patients for abstinence or safe sex practices to prevent their partners from acquiring painful non-genital ulcers due to HSV-1.

An experiment was carried out at Faculty of Agriculture Department of Agricultural Meteorology Farm, Centre for Advanced Agricultural Meteorology, College of Agriculture, Pune during Kharif seasons of 2014 and 2015.The experiment was laid out in split plot design with three replications.The treatment comprised of three brinjal hybrids viz.,V1:Phule Arjun, V2: Krishna, V3: Panchganaga as main plot and four planting windows viz., P1: 31st MW (30 July-5 August), P2: 32ndMW (6-12August), P3:33rdMW (13-19 August) and P4: 34thMW (20-26 August) as sub plot treatments.Cumulative GDD, HTU and PTU at the end of each growth stages showed that numerically higher requirement was observed in hy.Phule Arjun over hy.Krishana and hy.Panchganaga hybrids during both year 2014 and 2015 experimentation period. Whereas, the lowest canopy temperature was found in hy.Phule Arjun (29.0 0C) than rest of the brinjal hybrids. Canopy reflected PAR and transmitted PAR was higher in (191.54 and 188.62 µ mol m-2s-1) Panchganaga hybrids among the brinjal hybrids. Heat unit requirement or GDD has been used for characterizing the thermal response in brinjal crop. GDD for entire crop growing period decreased with subsequent delay in planting. HTU and PTU were also decreased during later planting windows condition. GDD in different stages in that emergence (59.6 and 72.3), vegetative growth (481 and 478), 50% flowering (575 and 568), first harvesting (681 and 645), last harvesting (1178 and 1183) was observed in hybrid Phule Arjun during 2014 and 2015, respectively. Lower GDD was observed in hy.Panchaganaga during 2014 and 2015, respectively. The highest HTU observed in 31st MW planting windows in hybrids Phule Arjun (5376 and 9190.4).This was followed by hy.krishna and Panchganaga (5370 and 9086) during 2014 and 2015, respectively. Highest HTU was observed in 31st MW in hybrids Phule Arjun followed by hy.krishna and lower in panchganga.

RESUMEN

An experiment “Effect of weather parameters on growth, yield and insect pest infestation on brinjal varieties under different planting windows” was carried out at Faculty of Agriculture Department of Agricultural Meteorology Farm, Centre for Advanced Agricultural Meteorology, College of Agriculture, Pune during Kharif seasons of 2014 and 2015. The experiment was laid out in split plot design with three replications. The treatment comprised of three brinjal hybrids viz., V1:Phule Arjun, V2: Krishna, V3: Panchganaga as main plot and four planting windows viz., P1: 31st MW (30 July-5 August), P2: 32ndMW (6-12 August), P3:33rdMW (13-19 August) and P4: 34thMW (20-26 August) as sub plot treatments.The correlation of meteorological parameters with incidence of brinjal shoot and fruit borer was studied in 2014. The correlation of shoot and fruit borer on brinjal (0.764*and 0.796*) and (0.784* and 0.704*) with the weather parameters are presented. Whereas morning relative humidity, rainfall and bright sunshine hours showed positive correlation with shoot and fruit borer population Average number of aphids/ jassid/ whiteflies plant showed highly significant negative correlation with minimum temperature(r= -0.702*,-0.710* and 0-.800*,) respectively. During 31stMW planting windows maximum temperature showed significant negative correlation with shoot and fruit borer population(r= -0.796*).It was observed that the infestation of shoot and fruit borer started increasing from October to November and its decreased onwards meteorological week. Amongst all the brinjal hybrids, Phule Arjun hybrids found significantly superior under extended planting windows followed by Krishna and Panchganga. Planting during 31st MW (1st week of August) was observed to be most suitable and optimum for brinjal considering the growth and yield attributes. This planting window was at par with 32nd MW planting window. Linear correlation analysis for brinjal fruit yield with weather parameters was significantly positively correlated with maximum temperature and minimum temperature.Timely planting during 31st MW (P1) and 32nd (P2) recorded lower incidence of shoot and fruit borer, aphids, jassid and whiteflies. Whereas, crop planted during 34thMW (P4) recorded maximum incidence of all pests. Among the brinjal hybrids, higher incidence was recorded with Panchganaga and minimum was recorded on Phule Arjun.Among the brinjal hybrids lower incidence of shoot and fruit borer, aphids, jassids and whiteflies was recorded on hy. Phule Arjun, which was found to be tolerant. This was followed by hy. Krishna. The higher incidence of all pests was recorded hy. Panchganga, which was found to be susceptible. Pest population on brinjal shoot/fruit/Aphid/Jassid/Whiteflies had significant negative correlation with minimum temperatures, whereas, morning and evening relative humidity, wind speed, pan evaporation and bright sunshine hours showed positive correlation pests population.

The dragon fruit (Hylocereus costaricencis) is a new emerging fruit crop, there is very less information available on the spacing and fertilizer dose requirements of this crop so the present study was carried out to find the effect of fertilizer doses, spacings, and interaction effect of both on plant growth of dragon fruit (Hylocereus costaricencis) at the Orchard, Horticultural College and Research Institute, Tamil Nadu Agricultural University, Coimbatore. during the year 2022. The design of the experiment was a split plot with four main plots viz. M1 = 3.5 x 2.0 m2, M2 = 3.0 x 3.0 m2, M3 = 3.0 x 2.5 m2, M4 = 2.5 x 2.5 m2 and six sub plots viz. S1 = N540 P420 K360, S2 = N495 P385 K330, S3 = N450 P350 K300, S4 = N340 P260 K225, S5 = N225 P175 K150, S6 = N0 P0 K0 g per pillar was applied and replicate four times. A significant difference was observed in terms of vine length (m), cladode girth (cm), cladode length (cm), and cladode number when different doses of fertilizers were applied. The highest vine length (2.96), cladode girth (24.52), cladode length (67.21), and number of cladodes (67.48) were observed in S3 (N450 P350 K300). Similarly, the highest vine length (2.503) and cladode girth (19.78) were observed in M4 (2.5 x 2.5 m2). The highest cladode length (67.47) was observed in treatment combination M2S3. Hence, the application of fertilizer dose S3 (N450 P350 K300) and adoption of spacing M4 (2.5 x 2.5 m2) is good to enhance the vegetative growth characters of dragon fruit under Tamil Nadu conditions.

In micrometeorological studies of the higher radiation absorptions and lower reflection was absorbed under hy. Phule Arjun as compared to hy. Krishana and hy Panchganga. The maximum Incident PAR (1270 u mol m-2s-1) Intercepted PAR (86.47u mol m-2s-1),Absorbed PAR (1094.4u mol m-2s-1) and Radiation use efficiency (2.43gmMJ m-2) was observed in hy .Phule Arjun. Cumulative GDD, HTU and PTU at the end of each growth stages showed that numerically higher requirement was observed in hy. Phule Arjun over hy. Krishana and hy. Panchganaga hybrids during both year 2014 and 2015 experimentation period. Whereas, the lowest canopy temperature was found in hy. Phule Arjun (29.0 0C) than rest of the brinjal hybrids. Canopy reflected PAR and transmitted PAR was higher in (191.54 and 188.62 µ mol m-2s-1) Panchganaga hybrids among the brinjal hybrids.
