RESUMEN
Introducción: la obesidad es una enfermedad crónica que se asocia a cambios fÃsicos, farmacológicos y fisiológicos que deben ser considerados en la valoración preanestésica. Objetivo: hacer una puesta al dÃa sobre la obesidad y sus implicaciones: una mirada desde la anestesiologÃa. MetodologÃa: revisión narrativa no sistemática de la literatura en los últimos 20 años, en base de datos Pubmed, Embase, Lilacs. Resultados: el anestesiólogo se enfrenta hoy a una de las morbilidades más frecuentes de pacientes que se programan a cirugÃas electivas y de urgencias: el sobrepeso y la obesidad. Debe estar preparado para prevenir complicaciones propias del aumento del tejido graso en el individuo como son los cambios en la vÃa aérea, los sistemas cardiovascular, respiratorio, gastrointestinal y del sistema nervioso. Debe conocer la farmacocinética y farmacodinamia en el metabolismo de estos pacientes y anticiparse a las condiciones postoperatorias que requieran tratamiento intensivo. La cirugÃa bariátrica es uno de los tratamientos propuesto para la conducta a seguir en pacientes con obesidad mórbida, y es otro reto para el anestesiólogo. Conclusiones: de su papel seguramente va a depender el éxito en la sobrevida del paciente.
Introduction: obesity is a chronic disease associated to physical, pharmacological and physiological changes which should be taken into account during the preanesthetic evaluation. Objective: update information about obesity and its implications from the perspective of anesthesiology. Method: non-systematic narrative review of the literature published in the last 20 years in the databases PubMed, Embase and LiLacs. Results: when dealing with patients scheduled for elective and emergency surgery, anesthesiologists are faced with one of the most common morbidities: overweight and obesity. They should be trained to avoid complications related to the increase in fatty tissue, such as changes in the patient's airways and in their cardiovascular, respiratory, gastrointestinal and nervous systems. They should know about the pharmacokinetics and pharmacodynamics of the metabolism of these patients, and anticipate postoperative situations requiring intensive treatment. One of the treatments proposed for the management of morbidly obese patients is bariatric surgery, an additional challenge for anesthesiologists. Conclusions: success in achieving high survival rates will surely depend on the work of anesthesiologists.
