RESUMEN

Objective:To evaluate the efficacy of proximal humerus internal locking system (PHILOS) via the minimally invasive plate osteosynthesis (MIPO) in the treatment of proximal humeral fractures.Methods:This retrospective study analyzed 30 elderly patients with proximal humeral fracture who had been treated by PHILOS via MIPO from September 2016 to March 2020 at Department of Orthopedic Surgery, People’s Hospital of Zhuxi County. They were 19 females and 11 males with an average age of 60.96 years (from 45 to 80 years). All patients were treated by closed reduction. After fracture reduction was confirmed by fluoroscopy, a minimally invasive incision was made below the acromion, with a couple of suture wires reserved at the rotator cuff attachments. A PHILOS plate was inserted at 4 mm lateral to the intertubercular sulcus, with the suture wires passing through the proximal suture holes on the PHILOS. After a lag screw was first screwed up into the compression hole on the PHILOS plate, the crossing suture wires were tightened up to resist the rotator cuff stress and maintain the internal inclination of the humeral head. Kirschner wires were used to temporarily stabilize the reduction. After satisfactory reduction and fine plate positions were confirmed by fluoroscopy, locking nails were screwed up. The internal inclination of the affected humeral head was compared between preoperation and the last follow-up. The therapeutic efficacy was evaluated by the shoulder Neer scoring system, and the visual analogue scale (VAS) pain scores and complications were recorded at the last follow-up.Results:All the 30 patients were followed for 6 to 18 months (average, 12 months). There was no incision infection, neurovascular injury, or internal fixation failure. Anatomical reduction was achieved in 25 patients and functional reduction in 5. The inclination of the humeral head was significantly improved. The VAS scores at the last follow-up averaged 1.9. Follow-up X-ray examination showed that bony union was achieved after 6 to 18 months (average, 9 months) for all patients. At the last follow-up, the inclination angle of the affected humeral head was 130°±5°, significantly improved compared with the preoperative 90°±11.2° ( P<0.05). All patients had good functional recovery of the shoulder. The efficacy was, according to the Neer shoulder scores at the last follow-up, excellent in 22, good in 6 and fair in 2 cases. Conclusions:Treatment of proximal humeral fractures using PHILOS via MIPO technique is suitable for patients with osteoporotic fracture, and may lead to fine therapeutic efficacy.

OBJECTIVE@#To observe the effect of heat sensitive moxibustion based on the articular injection for knee joint synovitis.@*METHODS@#Sixty-seven patients with knee joint synovitis were randomly assigned into an observation group (34 cases) and a control group (33 cases). Articular injection of triamcinolone was used in the control group for 2 courses, 2 times as a course, once 5 days. Based on the treatment in the control group, heat sensitive moxibustion was used in the observation group for 2 courses, 2 weeks as a course, 5 times a week. The acupoints were Yanglingquan (GB 34), Yinlingquan (SP 9), Liangqiu (ST 34), Xuehai (SP 10), Heding (EX-LE 2) or the pain point, subcutaneous nodules around the patella. American Knee Society (AKS) score was measured before and after treatment and one month after treatment. At the same time, the knee joint synovium thickness was measured by MRI; the knee joint effusion was observed by ultrasound; the serum interleukin-1 (IL-1) and matrix metalloproteinase-3 (MMP-3) were measured.@*RESULTS@#After treatment, the AKS scores increased in the two groups (both <0.05); after treatment and at follow-up, the AKS scores in the observation group were higher than those in the control group (both <0.05); at follow-up, the AKS score in the control group was lower than that after treatment (<0.05). After treatment, the synovium thickness, effusion, IL-1 and MMP-3 were lower than those before treatment in the two groups (all <0.05); after treatment and at follow-up, the four indexes in the observation group were lower than those in the control group (all <0.05); at follow-up, the four indexes in the control group increased than those after treatment (all <0.05).@*CONCLUSION@#Heat sensitive moxibustion combined with medicine articular injection can improve synovium inflammatory reaction and the absorption of effusion in the articular cavity,with long-time effect, which are better than simple medicine articular injection.

RESUMEN

Determining effective traditional Chinese medicine (TCM) treatments for specific disease conditions or particular patient groups is a difficult issue that necessitates investigation because of the complicated personalized manifestations in real-world patients and the individualized combination therapies prescribed in clinical settings. In this study, a multistage analysis method that integrates propensity case matching, complex network analysis, and herb set enrichment analysis was proposed to identify effective herb prescriptions for particular diseases (e.g., insomnia). First, propensity case matching was applied to match clinical cases. Then, core network extraction and herb set enrichment were combined to detect core effective herb prescriptions. Effectiveness-based mutual information was used to detect strong herb-symptom relationships. This method was applied on a TCM clinical data set with 955 patients collected from well-designed observational studies. Results revealed that groups of herb prescriptions with higher effectiveness rates (76.9% vs. 42.8% for matched samples; 94.2% vs. 84.9% for all samples) compared with the original prescriptions were found. Particular patient groups with symptom manifestations were also identified to help investigate the indications of the effective herb prescriptions.

Objective To explore the effect of flexible ureteroscope holmium laser treatment on segment kid-ney stones and ureter.Methods 100 cases with stone were randomly divided into two groups according to the digital table method.The control group received puncture percutaneous nephrolithotomy lithotomy,the observation group was treated with flexible ureteroscope holmium laser surgery.The surgical time,hospital stay,surgical removal rates and adverse events were observed.Results The operation time[(46.12 ±11.34) min],hospital stay[(5.34 ± 1.67)d],surgical removal rate(92.00%) and adverse reaction rate (14.00%) of the observation group were better than the control group (t=4.21,2.98,4.53,χ2 =3.97,all P<0.05).Conclusion Flexible ureteroscope holmium laser surgery in the treatment of renal and ureteral calculi can get better treatment.

Ningbo City has established a third-party mediation mechanism for medical disputes,attempting to resolve medical disputes by a third party independent of doctors and patients in a fair manner.Such a third party refers to the people's mediation committees for medical disputes and medical disputes compensation centers at various levels.This mechanism features advantages more than one,and has its challenges to address as well.Tasks to perform include study of the existing laws and regulations on compensation,appropriate increase of present compensation level for medical damages,improvement of the medical responsibility insurance mechanism,clarification of the nature of the people's mediation committee,and promotions for an ideal social environment.

RESUMEN

AIM: To investigate the nasal absorption of ligustrazine and promotion action of synthetic borneol. METHODS: In situ nasal recirculation method was used to study the dose effect relationship of nasal absorption of ligustrazine and the effect of synthetic borneol on nasal absorption of ligustrazine in rats. RESULTS: When the concentrations of ligustrazine were 0.25 , 0.75 ,and 1.25 g?L -1 in circulation liquid, respectively, the corresponding values of the absorption rate constant k were 0.0195 , 0.0227 , and 0.0241 min -1 . However, when the concentrations were 2.5 and 5.0 g?L -1 in circulation liquid, the absorption rate constant k lowered with the concentration being added. CONCLUSION: Ligustrazine can be absorbed through the nasal mucous and synthetic borneol can promote the absorption.

AIM: To investigate the effect of l menthol on percutaneous absorption of metronidazole through changing the skin character in rabbits. METHODS: Using rabbit skin with different character as transdermal barrier, and l menthol as transdermal enhancer, the absorbance of metronidazole was determined with a two cells diffusion apparatus in vitro, and its permeability was figured out. RESULTS: The transdermal action of l menthol on percutaneous absorption of metronidazole was very evident when the corneum was eliminated; l menthol and azone could increase the desposit function of metronidazole in the intact skin, but had no effect on the skin without corneum. The result also showed that the percutaneous absorption and intact skins desposit function of metronidazole had no difference between l menthol and azone. CONCLUSION: L menthol can increase the percutaneous absorption of metronidazole. The site of the action is mainly the corneum which has desposit function.

AIM: To compare the permeation enhancing characters of menthol and azone on the model drug of indomethacin and to study the enhancing effect when the two agents were used in combination. METHODS: The transdermal test was conduced on the penetration experiment apparatus of isolated skin. The cumulate penetrated amount, penetrating rate, stable flux and lag time were calculated upon the concentrations. RESULTS: After adding the penetration enhancer, the penetrating rate of indomethacin increased remarkably. It was 6.21 , 4.91 and 6.92 times of that of the controls respectively. RESUMEN

OBJECTIVE:To study the anti-asthmatic effects of artesunate on the durg-induced asthmat.METHOD:Drug-induced asthmatic model of guinea-pig. RESULTS:Using nubilous inhalation or intragastric administration,artesunate significantly inhibited the guinea-pig model asthma in a dose-dependent manner.It's ED50 were (99.95±7.69)mg.L-1(n=4) and (15.64±1.25)mg.kg-1.d-1(n=4) respectively. CONCLUSION:artesunate showed anti-athmatic effect in guinea-pig asthmatic model.

AIM: To study the effects of l-menthol and its binary systems on percutaneous penetration and deposit effects of 5-fluorouracil (5-FU). METHODS: Percutaneous absorption experiment was performed on the apparatus of isolated skin, penetrated amounts of different times of 5-FU were determined, the cumulate penetrated amounts and permeation rates and increased permeation times were calculated, and the deposit effect of 5-FU was investigated. RESULTS: L-menthol of different concentrations could significantly enhance the percutaneous penetration of 5-FU, its increased permeation times were 1.25 and 1.45 and 1.37 times respectively,and the action of 5-FU with 2% l-menthol was the strongest. 5-FU with 2% and 4% l-menthol had a marked deposit effect. When some kinds of enhancers were used alone or combined with l-menthol, the percutaneous penetration of 5-FU had a significantly increase. CONCLUSION: L-menthol and its binary systems can markedly promote the percutaneous penetration and the deposit effect of 5-FU.

A thin layer chromatograph-fluorescent method was used to assay content of scopoletin in Erycibe obtusi folia Benth.. The method showed that it not only can minimize analytical errors in thin layer scan and column contamination in HPLC,but also eliminated mutual interference of components in the preparation. It provides a good assay technique for quality control of hebal preparations. The method was carried out on thin layer plates of silica gel H(40?)with benzene-ether (1. 1: 1 )as the developer,at an excitation spectrum of 347nm,fluorescence spectrum of 420nm and a temperture of 25??1℃.The method showed good linearity(r= 0. 9998, n = 5)in the range of 0.08~0. 40?g/?l of scopoletin and the average recovery was 98. 52 % ~ 102. 67 % with RSD

Objective:To investigate the effect of synthetic borneol on penetration mechanism of ligustrazine hydrochloride. Methods: Permeation tests in vitro through rabbit skin in two compartment diffusion cells were performed to study the effect of synthetic borneol on transdermal absorption of ligustrazine hydrochloride by changing concentrations of synthetic borneol and rabbit skin condition. Results: Ligustrazine hydrochloride permeation coefficient increased linearly with incerease of concentrations of synthetic borneol, while those through strippde stratum corneum skin were not varied. Synthetic borneol enhanced the deposit functions of whole skin and stripped stratum corneum skin. Conclusions: Synthetic borneol could enhance the percutaneous absorption of ligustrazine hydrochloride, mainly through the stratum corneum, and increase the deposit function.

OBJECTIVE:Indomethacinum(Ind) and diclofenac sodium(Dcs) were taken as model drugs to study the enhancing effect of l-menthol on percutaneous absorption of two compounds with different characteristics,and to probe into the effect of combined use of l-menthol with azone.METHODS:Percutaneous penetration experiment was performed on in vitro device and deposit effect was observed with vivoperception.RESULTS:l-menthol could significantly enhance the percutaneous absorption and shorten the lag times(T1ag) of both Ind and Dcs.The enhancing effects of l-menthol,used either alone or in combination with azone,were several times stronger on Ind than on Dcs.Combined use of two enhances was more effective in enhancing penetration and longer in T1ag than single use of l-menthol,especially for Dcs.The study on deposit effect showed that l-menthol could increase the penetration rate than control agent(P

AIM To study the effect l-menthol pretreated nasal cavity on insulin pharmacological bioavailability(PBA). METHODS To set up diabetic rats model,nasal cavities were pretreated with l-menthol for two hours,insulin was dripped, blood glucose concentrations of different time were determined,and the area over the curve (AOC) and pharmacological bioavailability(PBA) were calculated. RESULTS After rats nasal cavity was pretreated with different concentrations l-menthol, nasal cavity absorption of insulin markedly increased between 2 h and 3 h,and PBA of insulin nasal cavity absorption increased respectively 5.77,7.00 and 3.12 times than control group. CONCLUSION l-Menthol promotes the nasal cavity absorption of insulin.

RESUMEN

For studyine the disturbance of energy metabolism in erythrocyte membrane of Down's syndrome, G3PD was analysed. It was found that G3PD activity and the ATPpromoted releasing rate of G3PD were increased significantly, but no significant increase was fonud in protein of G3PD by SDS-polyacrylamide gel electrophoresis. These results suggested Down's syndrome showed abnormality of energy metabolism in erythrocyte membrane.
