RESUMEN

Objective:To investigate the influence of perioperative probiotics supplement on short-term clinical outcomes in gastric cancer patients receiving neoadjuvant chemotherapy combined with radical gastrectomy. Methods:The prospective randomized controlled study was conducted. The clinicopathological data of 80 patients who underwent neoadjuvant chemotherapy combined with radical gastrectomy in the Affiliated Hospital of Qingdao University from July 2020 to September 2021 were selected. Based on random number table, patients were allocated into two groups. Patients undergoing perioperative probiotics supplement were allocated into the experiment group, and patients undergoing perioperative conventional treatment were allocated into the control group, respectively. Observation indicators: (1) grouping situations of the enrolled patients; (2) intraoperative situations; (3) follow-up and postoperative situations; (4) inflammation related hematological indexes. Follow-up was conducted using telephone interview and outpatient examina-tion to detect postoperative complications and startup of adjuvant chemotherapy up to October 31,2021. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was conducted using the independent sample t test. Measurement data with skewed distribution were represented as M(range), and comparison between groups was conducted using the Mann-Whitney U test. Count data were described as absolute numbers, and comparison between groups was performed using the chi-square test or Fisher exact probability. Comparison of ordinal data was analyzed using the Mann-Whitney U test. Repeated measurement data were analyzed by the repeated ANOVA or generalized estimating equation. Results:(1) Grouping situations of the enrolled patients. A total of 80 patients were selected for eligibility. There were 51 males and 29 females, aged 64(42-80)years. Of the 80 patients, there were 40 patients in the experiment group and 40 patients in the control group, respectively. (2) Intraoperative situations. RESUMEN

Objective:To evaluate the presentation of small arteries in abdominal contrast-enhanced CT late-arterial images using the deep learning image reconstruction (DLIR) combined with low tube voltage (kV) technique relative to the adaptive statistical iterative reconstruction V (ASiR-V) algorithm. Methods:Patients who were admitted to Peking University People′s Hospital from December 2021 to January 2022 and needed to be screened for abdominal diseases and receive abdominal and pelvic contrast-enhanced CT scan were prospectively collected. The patients were divided into low-voltage (LV) with 80 kV and high-voltage (HV) with 120 kV groups. According to two different reconstruction algorithms, each group was further divided into DLIR-H (D) subgroup and ASiR-V 50% (A) subgroup. The automatic tube current adjustment technique was used for CT enhanced scanning of patients, and the noise index value was uniformly set to 9. Subjective and objective evaluations were performed on the late-arterial images with a constructed slice thickness of 0.625 mm, and the radiation doses were recorded. Results:A total of 168 patients were included, including 76 males and 92 females, aged 18-85 (53±15) years old, body mass index (24±3) kg/m 2; 91 patients in the LV group and 77 in the HV group. The CT values of the aorta and common hepatic artery in the LV group were significantly higher than those in the HV group ( t=-14.20, P<0.001; t=-0.95, P<0.001). When the tube voltage was the same, the late-arterial image noise in subgroup D was significantly lower than that in subgroup A, and the signal to noise ratio (SNR) and contrast to noise ratio (CNR) of the liver, aorta and common hepatic artery were significantly higher than those in subgroup A (all P<0.001). The SNR and CNR of the aorta and common hepatic artery in the LV-D subgroup were significantly better than those in the LV-A, HV-D, and HV-A subgroups (all P<0.001). In the subjective evaluation of abdominal vascular display, the special resolution of the common hepatic artery, inferior mesenteric artery and the edge of the ascending branch of the left colic artery, and the contrast of the ascending branch of the left colic artery in the LV-D subgroup were significantly better than those of the LV-A, HV-D, and HV-A subgroups ( P<0.05). Moreover, the presentation rate of margin artery of splenic region (54.9%, 50/91) in the LV-D subgroup was significantly higher than those in the HV-D subgroup (24.7%, 19/77) and HV-A subgroup (32.5%, 25/77) (adjusted P<0.05). There was no significant difference in the radiation doses between LV and HV groups [(4.91±1.97) mSv vs (5.43±1.78) mSv, P>0.05]. Conclusion:The contrast-enhanced CT scan of abdomen with low tube voltage combined with DLIR algorithm can effectively improve the display level of the ascending vessel of left colonic artery from the inferior mesenteric artery and the margin artery, which brings more possibilities for the evaluation of similar small blood vessels.

RESUMEN

[Objective]To investigate the molecular mechanism of abnormal platelet activation induced by platelet O2 ·- and H2O2 levels in type 2 diabetes mellitus.[Methods]The platelet parameters in patients with type 2 diabetic patients and normal controls were measured;Immunofluorescence technique was used to observe the platelet morphology changing;Flow cytometry was used to detect platelet intracellular O2 ·- and H2O2 content in two groups, then with platelets in normal controls treated with NADH/PMS system and H2O2 respectively, platelet activation positive percentage was observed. Standard Western blot analysis protocols were used to detect expression difference of Catalase and type 2 super-oxide dismutase(Mn-SOD)in platelets.[Results]The MPV in the group of type 2 diabetic patients was significantly higher than in the normal control group(P < 0.001),but there was no statisticdifference in PLT,PDW,PCT between two groups. Immunofluorescence results showed that morphology of platelets in type 2 diabetic patients changed contrast to normal group. Through flowcytometry detection, the content of mitochondrial O2·-and H2O2 of platelet in type 2 diabetic patients were obviously higher than in normal group(P<0.05),whereas no significant difference in cytoplasmic O2·-. We adopted NADH/PMS system and H2O2 to treat platelets of normal group, heightened activated positive percentage were observed which described O2 ·- and H2O2 can significantly promote platelet activation(P<0.01). Western blot results showed that expression of Catalase in platelet of type 2diabetes patients decreased, while the expression and activity of Mn-SOD had no difference.[Conclusion]It is diabetic platelet Catalase expression decreased that may lead to Diabetic platelet mitochondrial O 2 ·- and H2O2 level increased ,thus regulating aberrant activation of diabetic platelet.

RESUMEN

Objective To discuss the therapeutic effect of transcatheter arterial embolization using hardening agent combined with oral propranolol in treating giant hemangioma at maxillofacial region in infants. Methods During the period from October 2013 to December 2014 at Imaging Center of Anhui Provincial Children's Hospital, transcatheter arterial embolization using hardening agent combined with oral propranolol was employed in a total of 27 infants with giant hemangioma at maxillofacial region. The age of the infants ranged from 2 months to 22 months (mean 5.9 months) and the body weight was 4.5-10 kg with a mean of 6.32 kg. Angiography via femoral artery was performed, which was followed by super-selective catheterization of hemangioma-feeding artery, and then pingyangmycin lipiodol emulsion was injected into the hemangiomas with subsequent injection of PVA particles to obstruct the hemangioma-feeding artery. After the embolization treatment, the patient received oral propranolol for 3-6 months. Results All the infant patients were followed up for 3-6 months. Clinical examination and ultrasonography indicated that the hemangioma was cured in 20 infants (75%) and the therapeutic result was effective in 7 infants (25%). Skin necrosis at hemangiomas site was observed in 2 infants (7.5%), which was cured after symptomatic treatment. No serious complications such as pulmonary embolism, cerebral embolism occurred, and no recurrence was observed. Conclusion For the treatment of giant hemangioma at maxillofacial region in infants, transcatheter arterial embolization using hardening agent combined with oral propranolol is minimally invasive, quickly effective and highly safe;and this treatment leaves no scar formation in most cases. Therefore, this technique is worthy of clinical application.

There were obvious correlation between the risk level and tumors' size and boundary which could be used to build the diagnostic equation of high-risk GISTs. Conclusion The equation ln [P/(1-P)]=-2.612X_3+0.371X_5 (X_3:tumors' boundary, X_5:tumors' size) is valuable in the diagnosis of high-risk GISTs.

RESUMEN

OBJECTIVE: To evaluate a quantitative method for individually adjusting the tube current to obtain images with consistent noise in electrocardiogram (ECG)-gated CT cardiac scans. MATERIALS AND METHODS: The image noise from timing bolus and cardiac CT scans of 80 patients (Group A) who underwent a 64-row multidetector (MD) CT cardiac examination with patient-independent scan parameters were analyzed. A formula was established using the noise correlation between the timing bolus and cardiac scans. This formula was used to predict the required tube current to obtain the desired cardiac CT image noise based on the timing bolus noise measurement. Subsequently, 80 additional cardiac patients (Group B) were scanned with individually adjusted tube currents using an established formula to evaluate its ability to obtain accurate and consistent image noise across the patient population. Image quality was evaluated using score scale of 1 to 5 with a score of 3 or higher being clinically acceptable. RESULTS: Using the formula, we obtained an average CT image noise of 28.55 Hounsfield unit (HU), with a standard deviation of only 1.7 HU, as opposed to a target value of 28 HU. Image quality scores were 4.03 and 4.27 for images in Groups A and B, respectively, and there was no statistical difference between the image quality scores between the two groups. However, the average CT dose index (CTDIvol) was 30% lower for Group B. CONCLUSION: Adjusting the tube current based on timing bolus scans may provide a consistent image quality and dose optimization for cardiac patients of various body mass index values.

RESUMEN

Objective To establish and clinically evaluate a method to adjust tube current(mA)individually for obtaining images with consistent noise and optimized radiation dose in 64-slice spiral helical CT coronary angiography. Methods (1)80 patients,as a control group, were underwent CT coronary angiography(CTCA)using 64-muhislice spiral helical CT with fixed mA(650 mA).Two radiologists first assessed image quality of the fixed mA group by scoring from 1 to 5.A minimal dose requirement for meeting clinical diagnosis with the desired noise level(standard deviation SD0)were decided.By analyzing the images of the control group,we also established correlation between the image noises of timing bolus(TB)scans and cardiac(CA)scans and set up a formula to predict the required mA value for obtaining the desired cardiac image noise for individual patient.(2)Subsequently,another group of 80 patients(study group)wero scanned with individually adjusted mA using the established formula and the practicability of this method was then evaluated. The independent sample t test was used to compare the values of the two groups. Results (1)The noise of TB and CTCA scans was linearly correlated with r2=0.994. (2)A cardiac image noise level of 28 HU was found to be adequate for clinical diagnostic purpose based on the image quality analysis.(3)The average noise for the acquired cardiac images was found to be 28.97 HU for the study group with individually adapted mA using the established formula. The standard deviation of the image noisein study group was 1.93,which was much smaller than 5.14 in the fixed mA group, but the average image scores for the study group(4.27±0.68)and fixed group mA(4.03 4-0.93)were not statistically different.(4)In the study group,63 out of 80 patients required tube currents below the standard 650 mA,yielding an average CTDIvol of58.32 mGy,which was about 32.1%dose reduction than that of fixed mA(85.94 mGy).The lowest CTDIvol was 19.27 mGy with 160 mA which was 22.4%of the average CTDIvol in control group. Conclusion Adjusting mA individually based on timing bolus scan noise analysis is an accurate and practical method to obtain consistent image quality for different cardiac pailents and to minimize the radiation dose for individuals compared to the fixed mA.

RESUMEN

Objective To control the image quality and to optimize the scan protocols of 64 multi-slice CT coronary imaging by analyzing factors that influence the image quality. Methods Seventy-eight patients with known or suspected CAD underwent CT coronary angiography using 64 multi-slice CT.The image quality in those cases was analyzed retrospectively to derive the optimized scan protocols. The application value of the override function was discussed. The dependency of cardiac reconstruction algorithms and corresponding image quality on patient heart rates was analyzed. In addition, the relationship between heart rate and the contrast-enhancement effect was studied.Results(1)Correct use of override function could improve the quality of cardiac images.(2)For patients with heart rates below 70 beats per minute, the image quality of single-segment reconstruction was satisfactory. For patients with heart rates between 70 and 79 beats per minute, the scores of images reconstructed with two-segments were better than that of single-segment reconstruction.(3)In the group with averaged heart rate of 60.7 beats per minute, the forecast contrast quantity of 70 ml was judged insufficient in 65.7%(23/35)cases;while for the group with averaged heart of 71.1 beats per minute,52.9%(18/34)cases were enhanced well. Conclusion The use of override function, correct segment mode and appropriate contrast quantity could be selected according to the heart rate, which could improve the image quality and reliability.
