RESUMEN

OBJECTIVE: The aim of this study was to compare the performance of the 18-item Korean version of the World Health Organization adult attention-deficit/hyperactivity disorder self-report scale (ASRS) with the six-item ASRS Screener for predicting attention-deficit/hyperactivity disorder (ADHD) group. METHODS: The study sample included 51 adult patients with ADHD and 158 normal controls. All participants completed the ASRS and were interviewed individually using the Mini-International Neuropsychiatric Interview. Receiver operating characteristic (ROC) curves were used to compare the ASRS (ASRS-18) with the ASRS Screener (ASRS-6) in Korean samples. RESULTS: The ADHD group had higher ASRS and ASRS subscale scores than those of the control group. ROC curve analysis revealed the ASRS was more powerful to predict ADHD group than the ASRS Screener, but the ASRS Screener also had strong concordance with clinician diagnoses. CONCLUSION: This study shows that the 18-question ASRS outperforms the six-question ASRS Screener. However, the weighted Screener is also a valid and useful screening instrument both in epidemiological surveys and in clinical settings.

To investigate the seroprevalence of the Orientia tsutsugamushi infection of Apodemus peninsulae and genomic variations in O. tsutsugamushi isolates, 246 A. peninsulae were trapped in 14 mountainous areas approximately 500 meter above sea level in Korea during the period of 1997 and 2000. Seropositive rate of O. tsutsugamushi among A. peninsulae was 31.8% in Kyunggi, 8.2% in Chunbuk and 7.1% in Kangwon provinces by microimmunofluorescent test. The 56 kDa protein gene was amplified by PCR in the spleens of seropositive A. peninsulae. Two amplicons from seropositive A. peninsulae were sequenced and their phylogeny was analysed on the basis of sequence homology. The 56 kDa genes of A. peninsulae 98-12 strain and A. peninsulae 98-16 strain showed 98.7% nucleotide homology and 96.6% amino acid similarity. A. peninsulae 98-12 and A. peninsulae 98-16 strain were related to Kuroki, Boryong and Karp strains showing 93.3~92.2%, and 87.1~84.6% homologies in nucleotide and amino acids levels, respectively. In the phylogenetic analysis, A. peninsulae 98-12 and A. peninsulae 98-16 strain formed a distinct group with Boryong, Kuroki and Nishino strains and were clearly distinguished from other genetic groups. The results suggest that A. peninsulae might be an important reservoir of O. tsutsugamushi in Korea.

To investigate changes in the radiographic appearance during weight-bearing in hallux valgus and examine the significance of the weight-bearing view, a comparative study was performed. Both normal and hallux valgus patients, who were all women over forty years old were studied. We reviewed radiographs of 100 normal feet in 50 patients and 70 symptomatic hallux valgus feet in 40 patients. Dorsoplantar radiographs were taken during weight-bearing and nonweight-bearing. Quantifiable angles were identified on dorsoplantar films in both the weight-bearing and nonweight-bearing positions. The measured angles were the hallux valgus angle, intermetatarsal angles between the first and second metatarsals, the second and third metatarsals, the third and fourth metatarsals, the fourth and fifth metatarsals, the first and fifth metatarsals, and the second and fifth metatarsals, and the interphalangeal angle. The hallux valgus angles in both groups were not constant on weight-bearing, whereas the intermetatarsals angle between the first and second metatarsals in both groups increased significantly on weight-bearing. The results show that most of the changes thought to occur with weight-bearing cannot be consistently demonstrated. Obtaining a weight-bearing view may not be routinely indicated in all patients.

Legg-Calve-Perthes disease (LCP) has been reported to be associated with hypofibrinolysis and a deficiency in coagulation. The goal of this prospective study was to confirm or refute these findings and to establish a guide for a screening test. The coagulation systems of twenty-three patients with Legg-Calve-Perthes disease were evaluated by means of the tests which included prothrombin time, activated partial thromboplastin time, antithrombin-III (AT-III), protein C, protein S, lipoprotein(a), and anticardiolipin antibody (ACA). Study subjects were 21 males and 2 females. Mean age was 8.7 years. No abnormal results were found for Antithrombin-III and Protein S, but 2 patients had low Protein C level. Two of 23 patients had Lipoprotein(a) values greater than 30 mg/dl. Another two patients had positive finding for Anticardiolipin antibody. There were no statistical differences in all parameters between Catterall group stage in the patients with Legg-Calve-Perthes disease. We were unable to establish an association between thrombotic tendency and Legg-Calve-Perthes disease based on this prospective study. So, routine screening of patients with Legg-Calve-Perthes disease for abnormalities of antithrombotic factors would be not warranted.

Recent development and advances in arthroscopic surgical techniques for Anterior Cruciate Ligament(ACL) reconstruction have led to the single-incision techniques. Several potential pitfalls in the classic single-incision techniques were found and it has become necessary to develop a new, simpler method to overcome these problems. Seventy-five cases of arthroscope-assisted ACL reconstruction were prospectively studied. The radiologic results were compared between the group in which femoral tunnel preparation was performed by the tibia tunnel(classic technique, 35 cases) and the group in which the anteromedial portal was employed(modified technique, 40 cases). Significantly better results were obtained at the angle formed by the bone graft and the interference screw, the angle between the femoral tunnel axis and the anatomic axis of the femur, and the angle between the femoral tunnel axis and tibial tunnel axis in the modified technique(P<0.05). This study presents a modified arthroscope-assisted ACL reconstruction technique that uses the anteromedial portal for femoral tunnel preparation and insertion of interference screw. In the modified ACL reconstruction technique, the femoral tunnel preparation and femoral graft fixation were accomplished in the same direction and the femoral tunnel preparation was performed regardless of the tibial tunnel preparation, this allows the surgeon to handle the arthroscope freely and advance more posteriorly to get a better view of the posterior cortex.

Since Green et al has introduced a new technique of retrograde intramedullary nail, the use of retrograde interlocking intramedullary nails has been recommended as one of the treatment options for the distal femoral fracture. However there are some disadvantages that an arthrotomy is required for insertion and the knee joint could often be violated. Authors present a simple, arthroscopically assisted method using the retrograde intramedullary nailing for distal femoral fractures with minimal invasiveness to the knee. From March 1995 to March 1997, the retrograde intramedullary nail was used to treat 9 distal femoral fractures. Five of 9 patients were fractured at the distal shaft of the femur and others were fractured at the supracondylar region of the femur. Only one of the fractures was open injury(Gustilo-Anderson grade II). Significant concomitant knee joint injuries were revealed through the arthroscopy in 3 patients. Eight of 9 fractures healed by 5 months, but one fracture was not healed and required bone grafting. Average knee range of motion was 130. Complications included 1 nonunion and 1 hardware failure. There were no patellofemoral problems and no posttraumatic arthritis of the knee joint. This arthroscope-assisted method have some potential benefits that include decreased risk of damage to the knee joint, early evaluation and treatment of the associated knee joint injuries, and accurate placement of the nail.
