RESUMEN

Objective:To evaluate the role of heat shock transcription factor 1 (HSF1) in the endogenous protective mechanism underlying mechanical ventilator-induced lung injury (VILI) in mice and the relationship with high mobility group box 1 (HMGB1).Methods:Forty SPF healthy male C57BL/6 mice, aged 6-8 weeks, weighing 20-25 g, were divided into 4 groups ( n=10 each) by the random number table method: control group (group C), VILI group (group VILI), negative control siRNA + VILI group (group NV) and HSF1 siRNA + VILI group (group siRNA). At 48 h before mechanical ventilation, negative control siRNA 5 nmol and HSF1 siRNA 5 nmol were intratracheally injected in NV and siRNA groups respectively, and the solution was diluted to 50 μl with the sterile phosphate buffer in both groups. Group C kept spontaneous breathing for 4 h, and the rest animals were mechanically ventilated (tidal volume 35 ml/kg, respiratory rate 75 breaths/min, inspiratory/expiratory ratio 1∶2, fraction of inspired oxygen 21%) for 4 h. Blood samples from the femoral artery were collected for arterial blood gas analysis immediately after endotracheal intubation and at 4 h of ventilation, and PaO 2 was recorded. Then the mice were sacrificed under deep anesthesia to collect lung tissues and bronchoalveolar lavage fluid (BALF). The concentrations of interleukin-1beta (IL-1β), tumor necrosis factor-alpha (TNF-α) and HMGB1 in BALF were determined by enzyme-linked immunosorbent assay. The pathological results were observed by hematoxylin-eosin staining, and lung injury was assessed and scored. The wet/dry (W/D) weight ratio of lung tissues was calculated. The expression of HMGB1 and HSF1 mRNA in lung tissues (by quantitative real-time polymerase chain reaction) and expression of HMGB1 and HSF1 protein in lung tissues (by Western blot) were determined. Results:Compared with group C, PaO 2 was significantly decreased at 4 h of ventilation, the concentrations of TNF-α, IL-1β and HMGB1 in BALF, W/D ratio and lung injury score were increased, and the expression of HMGB1 protein and mRNA in lung tissues was up-regulated in group VILI, group NV and group siRNA ( P<0.05 or 0.01). Compared with group VILI and group NV, PaO 2 was significantly decreased at 4 h of ventilation, the concentrations of TNF-α, IL-1β and HMGB1 in BALF, W/D ratio and lung injury score were increased, and the expression of HMGB1 protein and mRNA in lung tissues was up-regulated, and the expression of HSF1 protein and mRNA was down-regulated in group siRNA ( P<0.05 or 0.01). There was no significant difference in the parameters mentioned above between group VILI and group NV ( P>0.05). Conclusions:HSF1 is involved in the endogenous protective mechanism underlying VILI in mice, which may be related to the down-regulation of HMGB1 expression and attenuation of inflammatory responses in lung tissues.

Objective:To evaluate the effect of irisin on the alveolar macrophage polarization in a rat model of ventilator-induced lung injury (VILI).Methods:Thirty SPF healthy adult male Sprague-Dawley rats, aged 6-8 weeks, weighing 200-250 g, were divided into 3 groups ( n=10 each) using a random number table method: control group (group C), VILI group (group V) and irisin group (group I). The rats were mechanically ventilation (tidal volume 20 ml/kg, respiratory rate 80 times/min, inhaled oxygen concentration 21%, inspiratory/expiratory ratio 1∶2, positive end-expiratory pressure 0) for 4 h to develop VILI model.Group C kept spontaneous breathing for 4 h. Irisin 1 μg/kg was injected via the tail vein at 30 min before tracheal intubation in group I, while the equal volume of normal saline was given instead in the other groups.The rats were sacrificed at 4 h of mechanical ventilation, the lung tissues were removed for examination of pathological changes which were scored and for determination of wet to dry weight ratio (W/D ratio), and bronchoalveolar lavage fluid (BALF) was collected for determination of concentrations of interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α) and IL-10 (by enzyme-linked immunosorbent assay), expression of inducible nitric oxide synthase (iNOS), argininase 1 (Arg-1), and phosphorylated nuclear factor kappa B (p-NF-κB) p65 and p-NF-κB p50 in alveolar macrophages (by Western blot), and percentage of M1 and M2 alveolar macrophages and M1/M2 ratio (by flow cytometry). Results:Compared with group C, the W/D ratio, lung injury score, and concentrations of IL-6, TNF-α and IL-10 in BALF were significantly increased, the expression of iNOS, Arg-1, p-NF-κB p65 and p-NF-κB p50 was up-regulated, and the percentage of M1 and M2 alveolar macrophages and M1/M2 ratio were increased in group V and group I ( P<0.05). Compared with group V, the W/D ratio, lung injury score, and concentrations of IL-6 and TNF-α in BALF were significantly decreased, the expression of iNOS and p-NF-κB p65 was down-regulated, the percentage of M1 alveolar macrophages and M1/M2 ratio were decreased ( P<0.05), and no significant change was found in levels of IL-10 and Arg-1 in BALF, percentage of M2 alveolar macrophages and expression of p-NF-κB p50 in group I ( P>0.05). Conclusions:The mechanism by which irisin reduces VILI may be related to inhibition of NF-κB signaling pathway activation and reduction of alveolar macrophage polarization to M1 phenotype in rats.

OBJECTIVE:To establish the method for simultaneous determination of 11 active components in Yuhuai tablets , such as gardoside ,shanzhiside,gardenoside,genipin 1-gentiobioside,geniposide,ziyuglycoside Ⅰ ,ziyuglycoside Ⅱ ,narirutin, naringin,hesperidin and neohesperidin. METHODS :HPLC-QAMS method was adopted. The determination was performed on Agilent TC-C 18column(250 mm×4.6 mm,5 μm)with mobile phase consisted of acetonitrile (A)-0.1% phosphoric acid solution (B) (gradient elution )at the flow rate of 1.0 mL/min. The column temperature was set at 30 ℃. The detection wavelengths were set at 238 nm for gardoside ,shanzhiside,gardenoside,genipin 1-gentiobioside and geniposide ,203 nm for ziyuglycoside Ⅰ and ziyuglycoside Ⅱ,and 283 nm for narirutin ,naringin,hesperidin and neohesperidin. Using geniposide as an internal reference ,the relative correction factors of other 10 components relative to this component were calculated ,and the contents of each component in 10 batches of samples were calculated. The results obtained by HPLC-QAMS method were compared with those obtained by external standard method. RESULTS :The linear ranges of gardoside ,shanzhiside,gardenoside,genipin 1-gentiobioside,geniposide, ziyuglycoside Ⅰ,ziyuglycoside Ⅱ,narirutin,naringin,hesperidin and neohesperidin were 0.87-43.50,1.99-99.50,4.06-203.00, 7.35-367.50,12.97-648.50,28.98-1 449.00,3.79-189.50,1.57-78.50,18.05-902.50,0.66-33.00 and 14.38-719.00 μg/mL(all r>0.999 0). RSDs of precision ,repeatability and stability (24 h)tests were all less than 2%(n=6). The average recoveries were 96.90%-100.10%,and RSDs were 0.67%-1.74%(n=9). CONCLUSIONS :The HPLC-QMAS method established in this study is convenient and accurate. It can be used for the simultaneous determination of gardoside ,shanzhiside,gardenoside,genipin 1-gentiobioside,geniposide,ziyuglycoside Ⅰ,ziyuglycoside Ⅱ,narirutin,naringin,hesperidin and neohesperidin in Yuhuai tablets.

BACKGROUND@#Weight gain during pregnancy reflects the mother's nutritional status. However, it may be affected by nutritional therapy and exercise interventions used to control blood sugar in gestational diabetes mellitus (GDM). This study aimed to evaluate weight gain during gestation and pregnancy outcomes among women with GDM.@*METHODS@#A retrospective study involving 1523 women with GDM was conducted between July 2013 and July 2016. Demographic data, gestational weight gain (GWG), blood glucose, glycated-hemoglobin level, and maternal and fetal outcomes were extracted from medical records. Relationships between GWG and pregnancy outcomes were investigated using multivariate logistic regression.@*RESULTS@#In total, 451 (29.6%) women showed insufficient GWG and 484 (31.8%) showed excessive GWG. Excessive GWG was independently associated with macrosomia (adjusted odds ratio [aOR] 2.20, 95% confidence interval [CI] 1.50-3.52, P < 0.001), large for gestational age (aOR 2.06, 95% CI 1.44-2.93, P < 0.001), small for gestational age (aOR 0.49, 95% CI 0.25-0.97, P = 0.040), neonatal hypoglycemia (aOR 3.80, 95% CI 1.20-12.00, P = 0.023), preterm birth (aOR 0.45, 95% CI 0.21-0.96, P = 0.040), and cesarean delivery (aOR 1.45, 95% CI 1.13-1.87, P = 0.004). Insufficient GWG increased the incidence of preterm birth (aOR 3.53, 95% CI 1.96-6.37, P < 0.001).@*CONCLUSIONS@#Both excessive and insufficient weight gain require attention in women with GDM. Nutritional therapy and exercise interventions to control blood glucose should also be used to control reasonable weight gain during pregnancy to decrease adverse pregnancy outcomes.

Objective To explore the influence factors of help-seeking behavior in people with subjective memory complaints (SMCs).Methods A total of 305 eligible SMCs people were recruited and investigated with illness perception questionnaire-memory (IPQ-M),GDS-15,Mini-mental State Examination (MMSE),auditory verbal learning test(AVLT).Subjects were comprised of two groups:help-seeking group(n=44) and non-help-seeking group(n=261).Results The ratio of SMCs people for help-seeking was very low,only accounted for 14.4%.The ratio for going to hospital to see a specialist memory clinic accounted for only 2.0%.Logistic regression analysis showed 6 related factors were prime motivators for help-seeking behavior,which were symptom (OR=1.455),treatment control (OR=2.456),emotional representation (OR=2.268),accepting dementia education (OR=1.576),lacking of blood supply to the brain(OR=1.115),not using brain(OR=2.079).Three causal attributions were obstructive factors for help-seeking behavior,which were loneliness (OR=0.253),aging (OR=0.450),and chance or bad luck (OR=0.374).Conclusion Illness perception-memory in subjects with SMCs had some prediction for help-seeking behavior.Community nurses should attach importance to illness perception-memory of SMCs people and help them promote help-seeking,so as to facilitate earlv screening,identification and prevention of dementia.

Objective At present, modern medicine cannot yet clarifythe mechanism of radiation-induceddamage (RID) to salivary glands and its treatment and protective measures remain in the exploration stage .This study was to explore the mechanism of RIDto the submandibular gland and observe the effect of Dark Plum Spray ( DPS) on the submandibular gland after RID in order to provide some evidence for its further application . Methods Using the random number table , 84 healthy Wistar male rats were divid-ed into a normal, an RID model, and an experimental group ,all fed normally.The rats of the RID model groupwere left untreated , while those of the experimental group were intervened with DPS , tid, after RID.On the 1st, 7th, 14th, and 28th day after irradiation, 7 rats were taken from each group for measurement of the body weight, collection of the saliva , and calculation of the salivary flow rate.The submandibular glands were harvested for determination of the mRNA and protein expressions of autophagy-related Atg5 by RT-PCR and Western blot . Results In the 7th day after irradiation , the average body weight in NG[(239.87±16.50)g] was significantly higher (P<0.05) than UG[(213.84±14.42)g] and EG[(222.71±11.14)g].In the 14th ,28th day after irradiation, the average body weight in UG and EG were significantly lower ( P<0.05) than NG;the average body weight in EG was significantly higher ( P<0.05) than UG.In the 1st , 14th day after irradiation , the salivary flow rate in UG and EG were lower than NG but there were no significantly difference(P>0.05).In the 7th day after irradiation, the salivary flow rate in NG[(49.29±16.90)μL/min] and EG[(50.99±6.79)μL/min] were significantly higher (P<0.05) than UG[(30.13±13.19)μL/min].In the 28th day after irradiation, the salivary flow rate in NG[(69.29±11.32)μL/min] were significantly higher (P<0.05) than UG[(49.26±14.13)μL/min] and EG[(46.56±13.60)μL/min] .In the 1st , 7th and 14th day after irradiation , RT-PCR showed that the expressions of Atg 5 in UG and EG were significantly higher(P<0.05) than NG.In the 1st , 7th and 14th day after irradiation, Western blot showed that the expressions of Atg 5 in UG and EG is on the upper trend than NG;In the 1st , 7th day after irradiation , there were no significantly difference between UG and EG .In the 28th day after irradiation , Western blot showed that the expressions of Atg 5 in UG is on the declining trend than NG . Conclusion The autophagic activity of submandibular gland cells may associated with early radiation -induced injury , and Dark Plum Spray may en-hance the action of theanti-apoptosis cytokine in repairingradiation-induceddamageto the submandibular gland .

BACKGROUND: There is little information available in the mechanism of radiation-induced salivary gland injury, and its treatment and prevention are still at the exploratory stage.OBJECTIVE: To establish a rat model of radiation-induced xerostomia with 18 Gy electron beam and to observe the pathological changes of the submandibular gland and changes in saliva ingredients.METHODS: Totally 115 Wistar rats were randomly divided into exposure and control groups: the rats in the exposure group were subjected to anesthesia, and the submandibular gland received 18 Gy electron beam radiation to establish the model of radiation-induced xerostomia. The rats in the control group were only anesthetized but not exposed to radiation. The water intake was recorded at 21 dys after modeling. The saliva was collected and the submandibular gland was removed at 1, 3, 7, 14, 21, 28, 35 and 42 days to detect the saliva volume and submandibular gland index,and the morphological changes of the submandibular gland were observed by hematoxylin-eosin staining.RESULTS AND CONCLUSION: At 1-21 days after modeling, the average daily water intake was (6.42±1.91) mL in the exposure group and (4.82±1.20) mL in the control group, respectively (P < 0.05). During 42 days after modeling, the saliva secretion volume in the exposure group was lower than that in the control group, which was the lowest on day 7,and the difference was significant at 7, 21, 28 and 42 days after modeling between two groups (P < 0.05). The submandibular gland index in the exposure group was significantly lower than that in the control group at 1 and 21-42 days after modeling (P < 0.05). Hematoxylin-eosin staining results showed that in the exposure group, the rat submandibular gland appeared with inflammatory infiltration, glandular atrophy and karyopyknosis that were aggravated with time until day 42. To conclude, the rat model of radiation-induced xerostomia is established successfully with 18 Gy beam, characterized as increased water intake, decreased saliva volume and progressive aggravation of pathological injury of the submandibular gland.

<p><b>BACKGROUND</b>As a novel molecular markerof non-small cell lung cancer (NSCLC), PRDI-BF1 and RIZ homology domain containing protein 14 (PRDM14) is over-expressed in NSCLC tumor tissues. Extracellular matrix degradation mediated by the balance between matrix metalloproteinases (MMPs) and tissue inhibitor of metalloproteinases (TIMPs) is one of the most important mechanism in lung cancer metastasis. This study aimed to determine if PRDM14 promoted the migration of NSCLC cells through extracellular matrix degradation mediated by change of MMP/TIMP expression.</p><p><b>METHODS</b>The expression of PRDM14 was down-regulated in human cell line A 549 after transfection with lentiviral vector-mediated short-hairpin ribonucleic acids (shRNAs) which targeted the PRDM14 promoter. Cellular migration of shRNA-infected cells was detected by a scratch wound healing assay and transwell cell migration assay. Expression levels of MMP1, MMP2, TIMP1, and TIMP2 were measured by quantitative real-time polymerase chain reaction (RT-PCR).</p><p><b>RESULTS</b>Migration of PRDM14-shRNA-infected cells was significantly inhibited relative to control cells as measured by the scratch wound healing (P < 0.05) and transwell cell migration assays (P < 0.01). The expression of MMP1 in A549 cells infected by PRDM14-shRNA was down-regulated significantly (P < 0.01), whereas the expression of TIMP1 and TIMP2 was up-regulated significantly (P < 0.01).</p><p><b>CONCLUSIONS</b>PRDM14 accelerates A549 cells migration in vitro through extracellular matrix degradation. PRDM14 is considered as a potential therapeutic target in metastatic NSCLC.</p>

RESUMEN

The teaching status of Molecular Pharmacognosy in 28 institutions in China was investigated by questionnaire and the survey data was analyzed by SPSS. Research contents included course beginning years, majors, class hours, characteristics of the course, teaching ways, the theory and practice contents, evaluation modes, selection of teaching material, teaching achievements, teachers and so on for undergraduates and graduates. Research results showed that with 20 years' development, Molecular Pharmacognosy had been offered for both undergraduate and graduate students in at least 20 colleges and universities and Molecular Pharmacognosy education in China showed good development momentum. At the same time, to promote the development of Molecular Pharmacognosy further, investment for it should be increased and practical teaching condition should be improved.

Objective To study the biomechanical effect from different prosthetic materials used on unilateral maxillary defect repair by three dimensional finite element (FE) analysis method. Methods The maxillary unilateral defect model was reconstructed by medical image processing software MIMICS. The prosthesis was generated by mirroring technology. After processing, the FE unilateral maxillary defect model and healthy maxillary model were established to simulate stress distributions on the maxilla during occlusion. Results The maximum stress on maxilla using different repair materials was always found concentrated on the edge of maxilla connected with zygoma (the positions attaching zygolabialis and levator anguli oris). The minimum stress of from the maxilla edge on the repair side was 8.471 MPa by using tricalcium phosphate (TCP) composite. The maximum stress from the maxilla edge on the repair side was 17.55 MPa by using hydroxyapatite (HA) material. Conclusions Different repair materials had no significant effect on natural side of the maxilla after restoration, while a greater impact of force status on the repair side was found. The elastic modulus of different repair materials can be used as a basis for maxilla restoration in clinic.

Objective To explore any changes in surface electromyogram (sEMG) signals from the lumboabdominal muscles during exercise on stable and unstable surfaces. Methods sEMG signals from the lumbo-abdominal muscles of 33 healthy young persons [18 male and 12 female; average age (26.5 ± 4.3 ) years] were measured with the FlexComp Infiniti apparatus.Each subject performed 5 exercises on and off a Swiss ball:sit,bridge,bridge with both knees flexed,reverse bridge as well as press-up. Results ①In bridging there was a significant increase in the activation of the erector spinae during exercise on the ball compared with on the stable surface.②Bridging with both knees flexed gave a significant increase in activations of the erector spinae,the external obliques and the transverses abdominus/internal obliques during exercise on the unstable surface compared with the stable surface.③During reverse bridging there was a significant increase in activation of the erector spinae and rectus abdominus during exercise on the unstable surface compared with the stable surface.④During press-ups there was a significant increase in activation of the rectus abdominus,the external obliques and the transverses abdominus/internal obliques during exercise on the ball compared with the stable surface. Conclusion The unstable surface provides better training stimulus for the activation of the lumbo-abdominal muscles.

Objective To explore the molecular types of methicillin-resistant Staphylococcus aureus (MRSA) strains present in major hospitals in Qingdao area, using pulsed-field gel electrophoresis (PFGE) and multilocus sequence typing (MLST) methods, trying to find out the epidemiological characteristics of these MRSA isolates. Correlation of the PFGE types with microbiological phenotypes and clinical data was also studied. Methods 360 isolates of MRSA were procured during 2003 to 2007 from major hospitals in Qingdao. PFGE technology was applied to comparatively analyze the chromosomal DNA digested with endonuclease Sma Ⅰ . Comparison of DNA fragments patterns from each MRSA strain and cluster analysis were performed with the Bionumericus version ' 2.0' software. A dendogram was generated using PFGE macrorestriction fragments on gel images. Data was used to predict the possibility of each PFGE type via SPSS software version 11.0, using the variables as predictors including groups on patient's age, gender, source and the site where MRSA was isolated. Antibiotic sensitivity patterns of these MRSA isolates were determined by K-B tests, and a correlation between these patterns and PFGE types was investigated. Housekeeping genes were amplified with PCR and sequenced in representative strains of variant PFGE types to identify their allelic profile. Results 5 types of PFGE patterns (M0-M4) were identified with MI being the predominant and M2 next to it which was significantly correlated to the isolates from wounds. M3 type strains were mainly isolated from ICU wards and there were a few cases complied with M4 type with no correlated variant factors found in this study. A unique pattern of MRSA isolates with its M0 distinct from other types had not been reported. No significant association was found between PFGE individual types,gender or age groups. M1 and M2 types were the major proportional PFGE patterns among different hospitals. No vancomycin-resistant isolates were detected among 360 MRSA strains. No significant association was found between individual antibiotic resistance and specific PFGE types. Data from MLST analysis showed that the aUelic profiles of M1 and M3 type strain had the same ST239 linage which was commonly present in China. For M2 and M4 representative strains, the allelic profiles were ST5 and ST240, respectively. ST45 and ST398 were corresponding to two PFGE patterns clustered as M0 type. Conclusion Nosocomial infection due to MRSA was evenly distributed among different age groups and no gender bias was observed. The PFGE types of MRSA strains isolated in major hospitals in Qingdao were highly correlated with the sources of isolates and ST239 isolate seemed the prevalent and widespread one. Strategies should be designed to further monitor and prevent or minimize the spread of ST5 MRSA isolates and the like, in Qingdao area.

BACKGROUND: Biomechanical changes of the trunk might be an important factor contributing to the pathogenesis and poor recovery of lumbar disc herniation.OBJECTIVE: To study the biomechanical changes of the trunk of patients with lumbar disc herniation by isokinetic test of the protruded lumbar disc.DESIGN: Non-randomized controlled retrospective study of concurrent patients.SETTING: Department of Rehabilitation Medicine, Changhai Hospital, Second Military Medical University of Chinese PLA.PARTICIPANTS: Thirty patients with lumbar disc herniation, admitted in the clinic of Department of Rehabilitation, Changhai Hospital affiliated to Second Military Medical University between February 2001 and January 2002, were enrolled in this study, with another 30 concurrent patients without lumbar disc herniation serving as the control group. Informed consent was obtained from all patients involved.METHODS: A Biodex Multi-joint Testing System was employed for measuring the peak torque(PT), peak torque to body weight(PT/BW), time to peak torque(TPT), torque at 0. 2 s(T@ 0. 2), total work(TW), average power(AP) and flexion to extension(F/E) ratio of the trunk muscles of the patients. The results were analyzed by t test using SPSS 9.0 software.MAIN OUTCOME MEASURES: Main outcome: isokinetic evaluation of the lumbar and dorsal flexors and extensors; secondary outcome: F/E ratio.RESULTS: The strength of the trunk flexors and extensors decreased significantly in patients with lumbar disc herniation at each testing speed of retraction, and the bursting strength and indices for work efficiency of the muscles also exhibited obvious changes. The flexors showed greater reduction in muscle strength than the extensors. The F/E ratios during isokinetic concentric contraction at 60°/s and 180°/s were 57.99 ±5.68 and 65.74 ± 8.12, respectively, in patients with lumbar disc herniation, in comparison with the ratios of 95.25 ±5. 18 and 83.03 ±7.61 in the control patients, showing significant difference between the two patient groups( P < 0.01 ).CONCLUSION: Biomechanical changes of the trunk muscles of patients with lumbar disc herniation are definite, and proper rehabilitative treatment of these patients should consists of specific training protocols to restore the mechanical balance of the trunk and break the vicious cycle on the basis ofaccurate evaluation of such changes.

RESUMEN

<p><b>OBJECTIVE</b>To probe into the method for effectively increasing complete abortion rate of medicine-induced abortion and observe pathological changes.</p><p><b>METHODS</b>Two hundred cases were randomly divided into an observation group and a control group, 100 cases in each group. The observation group were treated by acupuncture at Hegu (LI 4), Sanyinjiao (SP 6), Neiguan (PC 6) and Kunlun (BL 60), and the control group were not treated by acupuncture. The abortion rate, bleeding condition, adverse reaction and pathological changes were observed in the two groups.</p><p><b>RESULTS</b>The complete abortion rate was 96.0% in the observation group, better than 88.0% in the control group (P < 0.05). There were significant differences between the two groups in bleeding time and pathological changes (P < 0.05).</p><p><b>CONCLUSION</b>Acupuncture can increase complete abortion rate of medicine-induced abortion, with shorter bleeding time and less residual villus.</p>

RESUMEN

Objective To investigate the effects of continuous passive motion (CPM) protocols on outcomes of total knee arthroplasty. Methods Seventy-two patients who underwent total knee arthroplasty were divided into 2 groups and treated with different CPM protocols: group 1 with CPM from 0 degree to 40 degrees and increased by 10 degrees per day, while group 2 with CPM from 90 degrees to 50 degrees (early flexion) and gradually progressed to full extension over a 4-day period. The CPM was administered twice a day for 1 hour over a 7-day period. All the patients participated in the same postoperative physiotherapy program. Results Group 2 (the early flexion group) had significantly more range of motion than group 1. Conclusion The early flexion can help achieve favorable functional gain after total knee arthroplasty.

RESUMEN

<p><b>OBJECTIVE</b>To establish a new method of multiplex semi-nested polymerase chain reaction (PCR) to detect pathogens in cerebrospinal fluid (CSF).</p><p><b>METHODS</b>According to the analysis of the conservative and variable regions in bacterial 16S rRNA genes, we designed universal primers for all bacteria and specific primers for most gram-positive and gram-negative bacteria. All primers were added into the same reaction systems successively of a two-step PCR assay to amplify the different bacterial DNA in CSF, and the results were compared with common culture method with sensitivity and the specificity both detected at the same time.</p><p><b>RESULTS</b>Both gram-positive and gram-negative bacteria amplified DNA fragment about 1,032 bp after first-step amplification with universal primers. In the second step, specific fragments of 336 bp and 127 bp were amplified in gram-positive and gram-negative bacteria respectively besides fragments of 1,032 bp; The detection limit for E. coli was 8 cfu/ml. The comparison of 62 CSF samples detected by both multiplex semi-PCR and conventional culture method revealed sensitivity, specificity, positive and negative values of 93.8%, 95.7%, 88.2%, and 97.8% respectively for PCR.</p><p><b>CONCLUSION</b>The result suggested that the multiplex semi-nested PCR we established was sensitive, specific and rapid method for clinical laboratory to detect pathogens in CSF.</p>

RESUMEN

20 normal males were examined by BIODEX isokinetic test and rehabilitation system,and data collected were used to observe the influence of angle and time on the isokinetic concentric constriction of knee joint. It was found that angle and time, especially angle factor obviously affected the testing results.
