RESUMEN

OBJECTIVE@#To explore clinical effect of early incision and decompression combined with screw fixation in treating Lisfranc injury and foot osteofascial compartment syndrome.@*METHODS@#Clinical data of 5 patients with Lisfranc injury and foot osteofascial compartment syndrome were retrospective analysized from January 2017 to December 2018, including 4 males and 1 female, aged from 19 to 62 years old. All patients were suffered from closed injuries. The time from injury to treatment ranged from 1 to 14 h. According to Myerson classification, 1 patient was type A, 1 patient was type B, and 3 patients were type C. All patients were performed early incision decompression and screw fixation. Maryland foot functional scoring standard at 12 months after opertaion was used to evaluate clinical effect.@*RESULTS@#All patients were followed up for 10 to 48 months. All fractures were achieved bone union, and healing time ranged from 3 to 9 months. All metatarsal and tarsal joints were reached to anatomical reduction. No infection, osteomyelitis, loosening or breaking of internal fixation occurred. Postopertaive Maryland foot function score at 12 months was from 44 to 97, and 2 patients got excellent result, 2 good, and 1 poor.@*CONCLUSION@#Early incision and decompression with screw fixation for the treatment of Lisfranc injury and foot osteofascial compartment syndrome, which has advantages of simple opertaion, thoroughly decompression, screw fixation does not occupy space, stable decompression and fixation, and could receive satisfied clinical effect.

Objective:To observe the effect of Yangxin Tongmaifang (YXTM) on endogenous metabolites in the myocardial tissue of rats with coronary heart disease due to blood stasis based on the metabolomics approach, and to explore its mechanism in the treatment of heart blood stasis syndrome. Method:A rat model of chronic myocardial ischemia due to heart blood stasis was established via the high-fat diet combined with intragastric administration of vitamin D<sub>3</sub> and subcutaneous injection of isoproterenol (ISO), followed by the intervention with YXTM. The metabolites in the myocardial tissues of rats in the normal group (<italic>n</italic>=8), model group (<italic>n</italic>=8), and YXTM group (<italic>n</italic>=8) were detected by ultra-high performance liquid chromatography coupled to high resolution mass spectrometry. The high-throughput metabolomics data were then subjected to multivariate statistical analysis using SIMCA 14.1, and the related metabolic pathways were analyzed with MetaboAnalyst. Result:The myocardial sample points of rats in the three groups were located in different areas of the elliptical confidence interval. The normal group and the model group were completely separated. There existed some crossovers and overlaps between the YXTM group and the normal group. The heart blood stasis syndrome model was proved successfully replicated from the perspective of metabolic profiling, and YXTM had the potential to promote the body to return to a normal state. After the intervention with YXTM, six differential metabolites changed significantly. Such metabolic pathways as valine, leucine, and isoleucine biosynthesis, pantothenate and coenzyme A (CoA) biosynthesis, valine, leucine, and isoleucine degradation, biosynthesis of aminoacyl-transfer RNA synthetases, and purine metabolism were involved. Conclusion:The therapeutic effect of YXTM on heart blood stasis syndrome in rats is related to the improved levels of myocardial endogenous metabolites, and its mechanism involves phospholipid metabolism, amino acid metabolism, energy metabolism, inflammatory response, and platelet activation and aggregation.

RESUMEN

Objective To summarize the clinical features and imaging findings of six coronavirus disease 2019 (COVID-19) patients, so as to provide evidences for early diagnosis and clinical intervention. Methods Six COVID-19 patients with positive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) were enrolled from the Seventh People's Hospital of Shanghai University of Traditional Chinese Medicine from Jan. 1 to Feb. 22, 2020. The epidemiological history, clinical manifestations, imaging data and laboratory indicators were retrospectively analyzed. Results All six patients had a clear travel or residence history in Wuhan. Four patients had fever, three had cough, two had upper respiratory tract symptoms such as runny nose and sore throat, and two had systemic symptoms such as headache and muscle ache. Chest computed tomography (CT) showed that all the six patients had abnormal manifestations in bilateral lungs, and the lower lung lesions were more common than the upper lung lesions. The main manifestations were multiple ground-glass opacities, consolidation shadows, crazy paving sign and different degrees of fibrosis in lateral field of bilateral lungs. Chest CT examination later after onset showed lung consolidation and severe fibrosis. Conclusion The imaging of COVID-19 has special characteristics. Combined with the epidemiological history, clinical manifestations and the detection of SARS-CoV-2 nucleic acid, COVID-19 can be effectively diagnosed in the early stage.

OBJECTIVE@#To explore the clinical efficacy of acupuncture combined with granule for nerve-root type cervical spondylosis and its effects on serum interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-α), interleukin-1 beta (IL-1β) and hemorheological indexes.@*METHODS@#A total of 114 patients with nerve-root type cervical spondylosis were randomly divided into an observation group and a control group, 57 cases in each group. The patients in both groups were treated with traction. The patients in the control group were treated with oral administration of granule, 4 g each time, 3 times a day, while based on the treatment of control group, the patients in the observation group were treated with acupuncture at Dazhui (GV 14), Tianzhu (BL 10), Houxi (SI 3), cervical Jiaji (EX-B 2), Quchi (LI 11), Hegu (LI 4) and Waiguan (TE 5), once a day. Both groups were treated for 4 weeks. The simplified McGill pain questionnaire (MPQ), neck disability index (NDI), numbness score, levels of IL-6, TNF-α, IL-1β in serum and hemorheological indexes were observed before and after treatment, and the clinical efficacy was compared between the two groups.@*RESULTS@#The total effective rate was 91.2% (52/57) in the observation group, which was higher than 71.9% (41/57) in the control group (<0.05). Compared before treatment, the scores of MPQ, NDI and numbness in the two groups were reduced after treatment (<0.05). After treatment, the scores of MPQ, NDI and numbness in the observation group were lower than those in the control group (<0.05). After treatment, the serum levels of IL-6, TNF-α and IL-1β in the two groups were reduced (<0.05), and those in the observation group were lower than the control group (<0.05). After treatment, the plasma viscosity, fibrinogen, low shear rate of whole blood viscosity and high shear rate of whole blood viscosity in the two groups were lower than before treatment (<0.05), and those in the observation group were lower than the control group (<0.05).@*CONCLUSION@#Acupuncture combined with granule have significant clinical efficacy for nerve-root type cervical spondylosis, which could reduce the serum levels of IL-6, TNF-α and IL-1β and improve hemorheology.

AIM:To investigate the effect of over-expression of peroxisome proliferator-activated receptor γ co-activator-1α (PGC-1α) on mitochondrial morphology and cell apoptosis in the cortical neurons with oxygen glucose depriva-tion/reoxygenation(OGD/R). METHODS:The whole gene sequence of PGC-1α was obtained from the cerebral cortex of C57BL/6 mice by RT-PCR and cloned into the eukaryotic expression vector pEGFP-N1. The pEGFP-N1-PGC-1α was iden-tified by PCR,and transfected into cortical neurons. The level of PGC-1α expression was identified by Western blot. The cortical neurons transfected with pEGFP-N1 and pEGFP-N1-PGC-1α vectors were treated with OGD/R. The mitochondrial mass,reactive oxygen species (ROS) and ATP production,cell apoptosis and changes of cleaved caspase-3 were detected by MitoTracker Red staining,flow cytometry,ATP metabolic assay kit and TUNEL. RESULTS:Over-expression of PGC-1α inhibited the decrease in mitochondrial biogenesis capacity and the ROS formation of OGD/R neurons(P<0.05),en-hanced the ability of ATP synthesis (P<0.01),inhibited neuronal apoptosis (P<0.01) and decreased the activation of caspase-3 (P<0.01). CONCLUSION:PGC-1α over-expression inhibits neuronal apoptosis with OGD/R treatment by promoting mitochondrial biogenesis,inhibiting the production of ROS and maintaining mitochondrial function. PGC-1α may be used as a target for the development of cerebral ischemia/reperfusion injury drugs.

RESUMEN

Objective To analyze the NIH data sharing repository in order to provide reference for the related stud-ies in biomedical data field and for the development of medical data sharing repository in China. Methods Ten typi-cal data sharing repositories ( such as UniProt, Protein Data Bank, and GenBank) were compared. Their data ac-cess methods, data management and sharing model, and service forms were summarized. Results The data manage-ment links and their process standards were designed according to their characteristics. Conclusion Data service tools can be designed, semi-artificial and semi-automatic data check can be carried out, detail meta-data can be collected, and citation rules confirming to the characteristics of data sharing repository can be established in our country by learning the experiences of NIH in developing its data sharing repository.

This study was aimed to investigate the expression of osteopontin (OPN) in central nervous system leukemia (CNSL) and to understand its clinical significance. The expression level of OPN in serum of 62 pediatric patients (22 cases of CNSL, 20 cases of acute leukemia without extramedullary infiltration and 20 cases of nontumor patients) and 19 cases of CNSL with complete remission (CR)were assayed by ELISA; the expression changes of OPN mRNA in bone marrow of the CNSL patients were detected by RT-PCR. The results indicated that the serum OPN level was significantly higher in CNSL group (25.21 ± 6.87 ng/ml) than that in acute leukemia group (13.24 ± 2.73 ng/ml) (P < 0.001) and nontumorous group (3.14 ± 1.60 ng/ml) (P < 0.001); the serum OPN level (4.35 ± 1.50 ng/ml) in CNSL group with CR decreased obviously (P < 0.001) after therapy; RT-PCR analysis showed that the expression of OPN mRNA was higher in CNSL group as compared with other two groups (P < 0.01). It is concluded that the OPN expression may play a role in central nervous system infiltration of leukemia, the mechanism of which remains to need further clinical exploration.

<p><b>OBJECTIVE</b>To analyse the infection of high-risk human papiliomavirus (HR-HPV) in cervical lesion wome, and evaluate the significance of high-risk human pappilomavirus detection by hybrid capture II (HV-II) in screening and diagnosing cervical lesion, especially high grade cervical intraepithelial neoplasia (CIN).</p><p><b>METHODS</b>A series of 1130 patients of cervical lesion were preliminarily diagnosed by cervical cytological examination, HR-HPV detection by HC-II , colposcopy and biopsy under the colposcopy between June 2009 and December 2008, including 212 CIN I and (or) condyloma (CIN I/HPV I), 442 CIN II/III, 28 invasive cervical cancer. cervical cytological examination is by thin prep liquid-based cytology test(TCT),and HR-HPV detection is by HC-II.</p><p><b>RESULTS</b>In 1130 cases the positive of HR-HPV was 65.84% (744/1130). Unusual cytology result were 862 cases, with 356 ASCUS, 84 ASCH, 216 LSIL, 184HSIL and 22 cancer. The number of biopsy > or = CINI/HPVI was 682, positive rate of HR-HPV was 78.59% (536/682). In screening CIN II or above, sensitivity, specificity, PPV and NPV of TCT were 88.94%, 32.73%, 48.49%, 80.60%, of HR-HPV DNA detectiort by HC-II were 90.21%, 51.82%, 57.14%, 88.14%, and of HR-HPV detection combined with cytology were 97.45%, 22.42%, 47.22%, 92.50%.</p><p><b>CONCLUSION</b>The infection rate of HR-HPV in cervical lesions is higher in each age group. Infection rate of HR-HPV is ascending with serious degree of cervical lesion. HR-HPV detection by HC- II is an important method in screening cervical lesion. HR-HPV detection is a viable option in the management of women with ASCUS and LSIL of TCT, with higher sensitivity and NPV.</p>

Objective To investigate the prevalence of adolescent scoliosis in Guangzhou for development of effective prevention and treatment program to the disease. Methods From November2007 to July 2009, 30 142 students between 7 to 20 years old in primary and junior middle schools received physical check-up for detection of scoliosis through physical and radiographic examination.Results 211 cases were diagnosed as scoliosis (Cobb angle≥10°) , with the prevalence rate as 0.70%. 192 patients with idiopathic scoliosis (IS) were detected, accounting for 91.00%. There were 19 cases of congenital scoliosis, accounting for 9.00%. Sex ratio of scoliosis was 180/31. The prevalence of scoliosis was significantly lower in boys than that in girls (x2= 112.332, P<0.001 ).Conclusion The crude prevalence of adolescent scoliosis was 0.70% in Guangzhou with majority of idiopathic scoliosis. Investigation on scoliosis among school-age population seemed to be important for the purposes of early diagnosis, selection of effective prevention and treatment.

Objective:To study the expression and clinical significance of hypoxia-inducible factor-1? (HIF-1?) in squamous cell carcinoma of buccal mucosa.Methods:45 cases of paraffin embedded samples of squamous cell carcinoma of buccal mucosa were examined with HIF-1? antibody by immunohistochemistrical method. The relationship between HIF-1? expression and clinico-pathological factors such as sex, age, duration of tumour, T classification, differentiation, lymph node metastasis in the neck, recurrence, clinic classification, tumour thickness, the number of mitosis and the degree of lymphocyte invasion was analyzed.Results:The positive expression of HIF-1? was found in 31 out of the 45 cases(69%). HIF-1? positive reaction was predominately located in the center of tumour nest. While analyzing clinico-pathological factors, only T classification (between T1+T2 and T3+T4, P=0.008), lymph node metastasis of the neck (P=0.037) and clinic classification (between stage I+II and III+IV, P=0.011) showed a significant correlation with HIF-1? expression. HIF-1? possitive expression is higher in T3+T4(d=4.87 mm) than that in T1+T2(3.81 mm) (P=0.046). Logistic analysis showed that the longer the patients lived after operation, the less the HIF-1? positive expression.Conclusions:HIF-1? positive expression in squamous cell carcinoma of the buccal mucosa correlates to T classification, lymph node metastasis of neck, clinical classification and survival
