RESUMEN

OBJECTIVE To investigate the monitoring of tacrolimus blood concentration in patients with nephrotic syndrome (NS),and to establish a prediction model for tacrolimus blood concentration. METHODS Data from 509 concentration monitoring sessions of 166 NS patients using tacrolimus were collected from January 1, 2020 to August 31, 2023 in Zhongshan Hospital Affiliated to Xiamen University. The relationship of efficacy and adverse drug reaction(ADR) with blood concentration was analyzed. A multilayer perceptron (MLP) prediction model was established by using the blood concentration monitoring data of 302 times from 109 NS patients with genetic information, and then verified. RESULTS In terms of efficacy, the median blood concentration of tacrolimus in the non-remission group was 2.20 ng/mL, which was significantly lower than that in the partial remission group (4.00 ng/mL, P<0.001) and the complete remission group (3.60 ng/mL, P=0.002). In terms of ADR, the median blood concentration of tacrolimus in the ADR group was 5.01 ng/mL, which was significantly higher than that in the non-ADR group (3.37 ng/mL) (P=0.001). According to the subgroup analysis of the receiver operating characteristic curve, when the blood concentration of tacrolimus was ≥6.65 ng/mL, patients were more likely to develop elevated blood creatinine [area under the curve (AUC) was 0.764, P<0.001); when the blood concentration of tacrolimus was ≥6.55 ng/mL, patients were more likely to develop blood glucose (AUC=0.615, P= 0.005). The established MLP prediction model has a loss function of 0.9, with an average absolute error of 0.279 5 ng/mL between the predicted and measured values. The determination coefficient of the validation scatter plot was 0.984, indicating an excellent predictive performance of the model. CONCLUSION Tacrolimus blood concentration has an impact on both efficacy and ADR in NS patients. The use of the MLP model for predicting blood concentration exhibits high accuracy with minimal error between predicted and measured values. The model can be used as an important tool in clinical individualized medication regimens.

RESUMEN

The coupling effect of angiogenesis and osteogenesis is of great significance for the remodeling of bone microenvironment. As a bone-specific microvascular subtype with high expression of CD31 and Endomucin(CD31 hiEmcn hi),H-type vessels are mainly distributed in the metaphysis, surrounded by Osterix + and Runt-related transcription factor 2(Runx2 +)osteogenic progenitor cells. H-type vessels, regulated by vascular endothelial growth factor(VEGF),platelet-derived growth factor BB(PDGF-BB),slit guidance ligand 3(SLIT3),hypoxia inducible factor 1α(HIF-1α),Notch and other signals, participate in the regulation of cell proliferation and differentiation of bone marrow mesenchymal stem cells(BMSCs),osteoblasts, osteoclasts, endothelial cells, etc. The mechanism of H-type vessels in coupling osteogenesis with angiogenesis has become a hot spot in recent research. The authors explore the progress of the mechanism of H-type vessels in coupling osteogenesis with angiogenesis by reviewing the characteristics of H-type vessel, mechanism of H-type vessel regulating bone formation, regulation mechanism of H-type vessel formation and related influencing factors,to provide a reference for clinical bone injury repair and anti-osteoporosis treatment.

RESUMEN

Objective:To summarize the clinical characteristics of patients with coronavirus disease 2019 (COVID-19) infected with Delta variant, so as to provide further references for clinical diagnosis and treatment. Methods:A real-world study was conducted to analyze the characteristics of 166 COVID-19 patients infected with Delta variant at Guangzhou Eighth People’s Hospital, Guangzhou Medical University. Results:The study enrolled 5 asymptomatic cases, 123 non-severe cases (mild and moderate type), and 38 severe cases (severe and critical type). Among these patients, 69 (41.6%) were male and 97 (58.4%) were female, with a mean age of 47.0±23.5 years. Thirty-nine cases (23.5%) had received 1 or 2 doses of inactivated vaccine. The incidence of severe COVID-19 cases was 7.7% in 2-doses vaccinated patients, which was lower than that of 11.5% in 1-dose and 26.8% in unvaccinated patients. The proportion of severe cases in 2 dose-vaccinated patients was 7.7%, which was lower than that of 11.5% in 1-dose vaccinated patients and 26.8% in unvaccinated patients, but the difference was not significant ( P>0.05). The most common clinical symptom was fever (134 cases, 83.2%), and 39.1% of cases presented with high-grade fever (≥39 °C); other symptoms were cough, sputum, fatigue, and xerostomia. The proportion of fever in severe cases was significantly higher than that of non-severe cases (97.4% vs. 76.4%, P<0.01). Similarly, the proportion of severe cases with high peak temperature (≥39 ℃) () was also higher than that of non-severe cases (65.8% vs. 30.9%, P<0.01). The median minimal Cycle threshold (Ct) values of viral nucleic acid N gene and ORFlab gene were 20.3 and 21.5, respectively, and the minimum Ct values were 11.9 and 13.5, respectively. Within 48 h of admission, 9.0% of cases presented with decreased white blood cell counts, and 52.4% with decreased lymphocyte counts. The proportions of increased C-reactive protein, serum amyloid A, interleukin 6, and interleukin 10 were 32.5%, 57.4%, 65.3%, and 35.7%, respectively. The proportions of elevated C-reactive protein, serum amyloid A and interleukin-6 in severe cases were significantly higher than those in non-severe cases ( P<0.01). Logistic regression analysis showed that older age and higher peak temperature were associated with a higher likelihood of severe cases ( OR>3, 95% CI: 2-7, P<0.01). In terms of treatment, traditional Chinese medicine (TCM) was used in 97.6% of non-severe cases and 100% in severe cases. Other treatments included respiratory and nutritional support, immunotherapy (such as neutralizing antibodies and plasma of recovered patients). The median times from admission to progression to severe cases, of fever clearance, and of nucleic acid conversion were 5 days, 6 days and 19 days, respectively. No deaths were reported within 28 days. Conclusions:The symptoms of Delta variant infection in Guangzhou are characterized by a high proportion of fever, high peak temperature, long duration of fever, high viral load, a long time to nucleic acid conversion, and a high incidence of severe cases. The severe cases exhibit a higher percentage of elderly patients, a longer duration of fever and have a higher fever rate and a higher hyperthermia rate than non-severe cases. Age and hyperthermia are independent risk factors for progression to severe disease. The combination of TCM and Western medicine can control the progression of the disease effectively.

RESUMEN

The era of bioeconomy has ushered in a new wave of technological and industrial revolution for mankind. The strategic deployment for the bioindustry in China has achieved remarkable results. However, there are still problems such as unbalanced regional development in the process of bioindustry development. In order to comprehensively assess the current situation of the competitiveness of the bioindustry in various regions of China, the assessment indicator system of the overall competitiveness of the bioindustry is constructed from the perspective of the four sub-industries of biomedicine, bioenergy, bioagriculture and bio-based industry. The weight of each assessment indicator is determined by the analytic hierarchy process. According to geographical administrative division and regional economic relations, an empirical analysis of the comprehensive competitiveness score of the bioindustry in seven regions of China is carried out. The assessment results show that the competition of bioindustry in various regions of China presents a gradient distribution in space. In view of this, relevant policy recommendations are put forward from four aspects: (1) implementing the strategy of rural revitalization, (2) implementing the strategy of regional coordinated development, (3) deepening the supply-side structural reform for the bioindustry, and (4) establishing regional unified information collaboration network system.

RESUMEN

Objective:To explore the feasibility to establish a novel retinal ischemia reperfusion (RIR) injury model by applying pars plana vitrectomy (PPV) combined with retinal vascular counterpulsation in the central retinal artery occlusion (CRAO) model of New Zealand rabbit. Methods:Twenty New Zealand adult rabbits were randomly divided into two groups by random number table, 10 for each group.Rabbits in the laser group were treated with retinal laser photocoagulation only, while rabbits in the counterpulsation group were treated with PPV combined counterpulsation. The right eye of each New Zealand rabbit was used as the experimental eye and the left eye was used as the normal control (the normal control group). Fundus fluorescence angiography (FFA) , oxygen partial pressure (PO 2) in vitreous cavity was performed to assess the recovery status of perfusion.Scotopic 3.0 oscillatory potentials (OPs) in electroretinogram (ERG) were used to evaluate the retinal function, and retinal pathological sections were used to evaluate the structural changes in the retina. The use and care of the animals complied with the Statement of the Association for Research in Vision and Ophthalmology (ARVO), and the study was approved by the Animal Research Committee of Zhongshan Ophthalmic Center, Sun Yat-sen University. Results:In the counterpulsation group, retinal reperfusion was observed during counterpulsation processure.FFA examination at 2 hours after counterpulsation showed reperfusion of retinal blood stream in all the eyes.Retinal artery filling, followed by venous filling was observed in the early stage, with no delay in filling and no interruption of blood flow. The percentage of vitreous PO 2 was significantly different among the counterpulsation group, the laser group and the normal control group at different time points ( Fgroup=330.87, P<0.001; Ftime=985.70, P<0.001). The percentages of vitreous PO 2 in the counterpulsation group at different time points was (18.67±6.29)%, (38.82±1.48)%, (57.33±4.25)%, (84.51±3.91)% and (89.20±2.97)%, which were significantly higher than that in the laser group ([23.24±1.95]%, [31.44±3.29]%, [40.21±3.05]%, [43.65±3.82]% and [58.07±2.93]%) (all at P<0.05). The percentage of OPs was significantly different among the counterpulsation group, the laser group and the normal control group at different time points ( Fgroup=164.09, P<0.001; Ftime=447.91, P<0.001). The percentages of OPs of counterpulsation group at different time points after laser and after PPV were (47.23±2.73)%, (70.79±3.09)%, (78.39±3.63)%, (76.69±4.08)% and (82.18±1.78)%, which were significanthy higher than that in the laser group ([46.83±2.89]%, [55.32±1.58]%, [51.08±4.02]%, [52.32±6.59]% and [53.46±6.46]%) ( all at P<0.05). There was a lesser damage in inner retinal structure in pathological section in the counterpulsation group. The myelinated nerve fiber layer (MFL) was loose and a mass of vacuolar changes were observed in MFL.The structure of MFL, inner plexiform layer, inner and outer plexiform layer in the laser group were disordered, and the Müller cell nerve fibers were destroyed in the laser group. Conclusions:A novel type of RIR injury model can be established by applying PPV combined with counterpulsation in the CRAO model of New Zealand rabbit.

RESUMEN

Fungal endophthalmitis is a kind of severe ocular infectious disease with poor prognosis. The damage degree of fungal endophthalmitis to ocular tissues is due to the fungi's destructivity, delayed diagnosis and non-standardized therapy.Early and proper intervention is crucial for saving visual function. There are different features of risk factors, pathogens and clinical presentations in exogenous infection and endogenous infection.Different key points of theraputic approaches should be identified to achieve early diagnosis and effective treatment, such as reasonably selecting the right type of antifungal drugs, different ways of administration, appropriate drug dose or surgical method. In the treatment of fungal endophthalmitis, the application of glucocorticoid is controversial, and the use of local or systemic glucocorticoid should be avoided as far as possible.

RESUMEN

Objective: To investigate the effects of sodium valproate (VPA) on the expression of FAK and FAK-pY397 in hippo-campus of rats with seizure induced by pentylenetetrazole (PTZ). Methods: A total of 75 rats were randomly divided into 5 groups:the normal control group, the epilepsy model group (PTZ group) and the VPA groups(150,300 and 600 mg·kg-1·d-1)with 15 ones in each group. The model rats were continuously given PTZ (32 mg·kg-1·d-1) by intraperitoneal injection for 4 weeks and paid close attention to the behavioral changes, and then VPA was administrated orally for 2 weeks. The pathological changes of hippo-campus tissue were observed by HE staining. The expressions and distributions of FAK, FAK-pY397 and integrin in serum and hippo-campus were evaluated by immunohistochemical assay and enzyme-linked immunosorbent assay (ELISA). Results: Compared with the model group, the symptoms of epilepsy in VPA groups were significantly relieved and cell apoptosis was improved. Immunohistochemis-try showed that the expression of FAK-pY397 decreased significantly in VPA groups with the increase of sodium valproate dose, and there was no significant difference in the expression of ITGα3. The VPA groups significantly reduced the expression of FAK, FAK-pY397 and ITGβ1(P<0. 05), the expression of FAK and ITGβ1 protein in peripheral serum decreased significantly (P<0. 05), but the expression of FAK-pY397 did not change significantly. Conclusion: VPA can effectively participate in or affect the process of epi-lepsy by inhibiting the expressions of FAK-pY397 and ITGβ1 in hippocampal tissue of epileptic rats.

RESUMEN

BACKGROUND:Polyethylene terephthalate (PEA) holds good mechanical properties, but its biocompatibility needs to be improved. OBJECTIVE:To investigate the effect of bioactive glass modified PEA on bone healing. METHODS:In vitro experiment:PEA (control group) and bioactive glass modified PEA (experimental group) were respectively co-cultured with MC3T3-E1 cells. The cell morphology was observed at 3 days of culture, and the cell proliferation and alkaline phosphatase activity were detected at 1, 3 and 5 days of culture. In vivo experiment:24 New Zealand white rabbits were enrolled, modeled into unilateral knee ligament rupture, and then randomly allotted to two groups. Bioactive glass modified PEA and PEA were implanted into experimental and control groups, respectively. Biomechanics and histological changes were compared between groups at 6 and 12 weeks after transplantation. RESULTS AND CONCLUSION:In vitro experiment:the cell density in the experimental group was significantly higher than that in the control group after 3-day culture (P<0.05);the cell proliferation and alkaline phosphatase activity were significantly higher than those in the control group at 3 and 5 days of culture (P<0.05). In vivo experiment:the maximum axial pull-out strength in the experimental group was significantly greater than that in the control group at 6 and 12 weeks after implantation (P<0.05). In the control group, the defect was obvious and there were a large number of inflammatory cells at 6 weeks after implantation;the defect region became fuzzy, abundant inflammatory cells existed and new bone formed on the surface at 12 weeks after implantation. In the experimental group, there were newly formed bones and trabeculae formed on the interface, the scar tissue became less, and the interface width was narrow at 6 weeks after implantation;numerous new tissues were observed, and only a few inflammatory cells observed in the defect region at 12 weeks after implantation. These results indicate that the modified PEA with bioactive glass possesses good biocompatibility and biomechanics, which can promote bone healing.

RESUMEN

BACKGROUND: Ligament Advanced Reinforcement Sytstm (LARS) artificial ligament can avoid the interfibrillar friction compared wiht traditional ligaments, which is conductive for autogenous tissue growth. OBJECTIVE: To investigate the biocompatibility of LARS artificial ligament used for cruciate ligament reconstruction and bone healing. METHODS: Twenty goats were randomly divided into two groups: the anterior cruciate ligament insertion at the femur was cut as stump group; the anterior cruciate ligament insertions at the femur and tibia, and all the ligaments and synovial tissues between them were cut as stump resection group. LARS artificial ligament was then implanted into both two groups. At 20 months after implantation, the ligament healing was observed using CT and MRI examinations, the fibrous tissues were observed under scanning electron microscope, and the growth of newborn ligaments was observed by histological staining. RESULTS AND CONCLUSION: CT findings showed that in the stump group, the number of femoral tunnel enlargement was significantly more than that in the stump resection group (P < 0.05), while the number of tibial tunnel enlargement in the stump group was significantly less than that in the stump resection group (P < 0.05). LARS artificial ligament in the femoral and tibial tunnel and free part of the joint presented with hypointensity on T1- and T2-weighted images, which showed no significant difference between two groups. Under the scanning electron microscope there were obvious dense fibrous tissues between LARS artificial ligaments in both two groups; and the autologous fibrous tissues arranged closely and evenly under high magnification. Hematoxylin-eosin staining results showed that in the two groups, the LARS artificial ligaments in the free part of the articular cavity and bone tunnel were surrounded by a large number of autologous tissues in a fusiform arrangement; autologous fibrous tissues and artificial ligament were wrapped well but arranged irregularly; mature fibrous tissues and few capillary infiltration were found in the bone tunnel and free part of the joint cavity, but without necrotic tissues. Masson staining results showed that the inner and outer parts of the LARS in the bone tunnel were wrapped by blue collagen, the red-stained bones located outside the collagen, and no obvious transitional zone appeared. These results showed that the LARS artificial ligament holds good biocompatibility and fibrous tissue induction, which can promote bone healing.

RESUMEN

Objective To investigate the difference in clinical features and chest HRCT findings between idiopathic nonspecific interstitial pneumonia(INSIP)and connective tissue disease-associated nonspecific interstitial pneumonia(CTD-NISP). Methods Totally 73 cases of NISP from 2011 to 2016 were retrospectively reviewed ,whose final diagnosis all were made after clinico-radiologic-pathologic discussion and 52 cases of them were diag-nosed as INSIP and 21 cases as CTD-NSIP. Clinical features ,lung function test results and chest HRCT findings of INSIP and CTD-NSIP were compared. Results Common underlying diseases of CTD-NSIP were poly-/dermato-myositis(PM/DM),rheumatoid arthritis(RA)and Sjogren syndrome(SS). The mean age of CTD-NSIP[(47.14 ± 9.24)y]was younger than that of INSIP[(59.09 ± 11.20)y](P<0.05). Compared to CTD-NSIP,expectoration was more common in patients with INSIP,while dry mouth/eyes,arthralgia and erythra were less common in INSIP (P < 0.05). Lung function test1 showed restrictive ventilatory dysfunction with dispersion function decline was found in both groups. There were no significant differences in lung function test results between INSIP and CTD-NSIP. In HRCT,the subpleural vertical line was more common in INSIP than that in CTD-NSIP,while patchy consolidation, subpleural curvilinear shadow, pleural effusion and esophageal dilation were less common in INSIP(P<0.05). Conclusions Specific difference of clinical and HRCT features between CTD-NSIP and INSIP are conducive to differentiating the two from each other.
