RESUMEN

ObjectiveTo investigate the effect and mechanism of Dendrobium mixture (DMix)-containing serum on high glucose-induced podocyte injury in mice. MethodThe MPC5 mouse glomerular podocytes were cultured in vitro, and the optimal glucose concentration for modeling, modeling time, and concentration of DMix-containing serum for administration were determined. The cells were classified into normal (5.5 mmol·L-1 glucose+10% blank serum), model (30 mmol·L-1 glucose+10% blank serum), DMix-containing serum (30 mmol·L-1 glucose+10% DMix-containing serum), ferroptosis inhibitor (Fer-1, 30 mmol·L-1 glucose+10% blank serum+1 μmol·L-1 Fer-1) groups. The corresponding kits were used to measure the levels of Fe2+ and lactate dehydrogenase (LDH) in cells. Enzyme-linked immunosorbent assay was employed to determine the content of glutathione (GSH) and lipid peroxide (LPO) in cells. Fluorescence probe was used to measure the reactive oxygen species (ROS) level. Real-time fluorescence quantitative polymerase chain reaction and Western blotting were employed to determine the mRNA and protein levels, respectively, of Wilms' tumor-1 (WT-1), desmin, long chain acyl-CoA synthase 4 (ACSL4), and glutathione peroxidase 4 (GPX4) in podocytes. ResultCompared with the blank group, the intervention with 30 mmol·L-1 glucose for 48 h reduced podocyte viability (P<0.01), and the 10% DMix-containing serum showed the most significant improvement in podocyte viability (P<0.01). Compared with the normal group, the model group presented elevated levels of Fe2+, LDH, LPO, and ROS, lowered GSH level, up-regulated mRNA and protein levels of desmin and ACSL4, and down-regulated mRNA and protein levels of WT-1 and GPX4 (P<0.01). Compared with the model group, the DMix-containing serum lowered the Fe2+, LDH, LPO, and ROS levels, elevated the GSH level, down-regulated the mRNA and protein levels of desmin and ACSL4, and up-regulated the mRNA and protein levels of WT-1 and GPX4 in podocytes (P<0.05, P<0.01). ConclusionDMix-containing serum exerts a protective effect on high glucose-induced podocyte injury by inhibiting ferroptosis.

Objective:To explore the value of deep learning technology based on mammography in differentiating for breast imaging reporting and data system (BI-RADS) category 3 and 4 lesions.Methods:The clinical and imaging data of 305 patients with 314 lesions assessed as BI-RADS category 3 and 4 by mammography were analyzed retrospectively in Shenzhen People′s Hospital and Shenzhen Luohu People′s Hospital from January to December 2020. All 305 patients were female, aged 21 to 83 (47±12) years. Two general radiologists (general radiologist A and general radiologist B) with 5 and 6 years of work experience and two professional breast imaging diagnostic radiologists (professional radiologist A and professional radiologist B) with 21 years of work experience and specialized breast imaging training were randomly assigned to read the imaging independently at a 1∶1 ratio, and then to read the imaging again in combination with the deep learning system. Finally, breast lesions were reclassified into BI-RADS category 3 or 4. The receiver operating characteristic curve and area under the curve (AUC) were used to evaluate the diagnostic performance, and the differences of AUCs were compared by DeLong method.Results:The AUC of general radiologist A combined with deep learning system to reclassify BI-RADS category 3 and 4 breast lesions was significantly higher than that of general radiologist A alone (AUC=0.79, 0.63, Z=2.82, P=0.005, respectively). The AUC of general radiologist B combined with deep learning system to reclassify BI-RADS category 3 and 4 breast lesions was significantly higher than that of general radiologist B (AUC=0.83, 0.64, Z=3.32, P=0.001, respectively). There was no significant difference in the AUCs between professional radiologist A combined with deep learning system and professional radiologist A, and professional radiologist B combined with deep learning system and professional radiologist B in reclassifying BI-RADS category 3 and 4 breast lesions ( P>0.05). Conclusion:The deep learning system based on mammography is more effective in assisting general radiologists to differentiate between BI-RADS category 3 and 4 lesions.

Objective:To establish the predictive models for the prognosis of ductal carcinoma in situ (DCIS) at different pathological stages, and to evaluate the predictive performance of the models.Methods:Complete data of 273 patients with confirmed DCIS at different pathological stages who underwent mammography examination in Shenzhen People′s Hospital, Peking University Shenzhen Hospital and Shenzhen Luohu People′s Hospital from November 2014 to December 2020 were retrospectively collected, including 110 cases in the DCIS+ductal carcinoma in situ with microinvasion (DCIS-MI) group and 163 cases in the invasive ductal carcinoma (IDC)-DCIS group. The clinical, imaging and pathological features were analyzed. Mammary Mammo AI fusion model and deep learning-based natural language processing (NLP) structured diagnostic report model were used for image feature extraction. Patients in each group were randomly divided into training set and validation set with a ratio of 6∶4, and the predictors were screened by univariate and multivariate logistic regression analysis. The lowest Akaike information criterion value of each group was selected to construct the final predictive model. The receiver operating characteristic (ROC) curve was drawn to evaluate the performance of each model.Results:Taking estrogen receptor (-) or human epidermal growth factor receptor 2 (3+) as the poor prognostic reference, there were 62 cases considered with poor prognosis and 48 cases with good prognosis in DCIS+DCIS-MI group; while in the IDC-DCIS group, taking the Nottingham prognostic index as the reference, 33 cases were considered with poor prognosis, 73 cases with moderate prognosis, and 57 cases with good prognosis. Four predictive factors were screened to construct the DCIS+DCIS-MI-group predictive model, including DCIS nuclear grade, calcification with suspicious morphology in mammography, DCIS pathologic subtype and DCIS with microinvasion. Five predictive factors were screened to construct the IDC-DCIS-group predictive model, including neural or vascular invasion, Ki67 level, DCIS subtype, DCIS component proportion and associated features in mammography. The area under curve (AUC) for predicting poor prognosis of DCIS+DCIS-MI was 0.92 (95%CI 0.84-1.00) in the training set and 0.90 (95%CI 0.82-0.99) in the validation set; while the AUC for predicting poor prognosis of IDC-DCIS was 0.84 (95%CI 0.76-0.93) in the training set and 0.78 (95%CI 0.64-0.91) in the validation set.Conclusion:The developed models based on deep learning combined with NLP can effectively predict the prognosis of DCIS at different pathological stages, which are beneficial to the risk stratification of patients with DCIS, providing a reference for clinical decision.

BACKGROUND:The therapeutic effectiveness on tendon injury is closely related to the material of tendon suture. OBJECTIVE: To review the progress of tendon suture materials and tendon biomechnics in recent years. METHODS:A computer-based search of CNKI (January 1999 to December 2014), and PubMed (January 1950 to December 2014) was performed for relevant articles using the keywords of “tendon, suture materials, biomechanics” in Chinese and English, respectively. RESULTS AND CONCLUSION:Ideal tendon repair refers to the restoration of the continuity of its anatomical structure, tensile strength and sliding function in physiology, which is influenced by many factors. Suture technique and choice of suture materials are two steps that cannot be ignored. With the development of surgical tendon suture technique, in order to improve the quality of tendon healing, ideal tendon suture is first to have sufficient strength to avoid an early tensile fracture; secondly, the elasticity cannot be too large that can cause a gap between suturing ends and affect tendon healing, and time for protecting the tendon strength is as long as possible. Therefore, the optimal choice of tendon suture materials should be based on suture methods and biomechanical characteristics of suture lines, thereby to promote tendon healing.

RESUMEN

<p><b>OBJECTIVE</b>To observe the changes in serum transforming growth factor-β1 (TGF-β1) in patients with early-stage nasopharyngeal carcinoma (NPC) after radiotherapy and explore the correlation of serum TGF-β1 with radiation injury and disease-free survival.</p><p><b>METHODS</b>The average serum TGF-β1 level (50.2∓3.2 ng/ml) determined from 32 healthy volunteers was used as the standard value for NPC patients in this trial. Fifty-seven patients with early-stage (T1-2N0-1M0) NPC without prior treatment were divided into two groups with serum TGF-β1 level before treatment lower than or equal to the standard value (group A, 29 cases) and a level beyond the standard value (group B, 28 cases). Serum TGF-β1 level was determined in all the patients before, during and after the radiotherapy to evaluate the radiation injury and therapeutic effect.</p><p><b>RESULTS</b>The serum TGF-β1 level before radiotherapy was significantly lower in group A than in group B (35.4∓1.4 vs 58.8∓1.0 ng/ml, P<0.05). After radiotherapy, acute radiation mucositis and skin reaction was significantly severer in group B (P<0.05). The serum TGF-β1 level before radiotherapy was significantly higher in patients with grade 3 acute radiation mucositis and skin reaction than in those with injuries below grade 3 (54.0∓2.2 vs 42.0∓2.3 ng/ml and 54.3∓2.4 vs 43.4∓2.2 ng/ml, P<0.05). The two groups showed no significant differences in the locoregional failure rate (3.4% vs 7.1%), distant metastasis rate (3.4% vs 10.8%) or disease-free survival (P>0.05).</p><p><b>CONCLUSIONS</b>Radiotherapy can significantly decrease serum TGF-β1 level in early NPC patients. Serum TGF-β1 level before radiotherapy can help predict the degree of acute radiation mucositis and skin reaction, but shows no correlation with disease-free survival of early-stage NPC patients.</p>

RESUMEN

Objective To investigate the clinical and imaging features of the reversible posterior leukoencephalopathy syndrome(RPLS).Methods The clinical and imagig data of 9 patients with RPLS were analyzed retrospectively.Results Among the patients,4(44%)were secondary to pregnancy-induced hypertension syndrome,1(11%)was second to short bowel syndrome,1(11% )was second to acute lymphoblastic leukemia,1( 11% )was second to IgA nephropathy,and 2(22%)were second to hypertensive encephalopathy.Their mean age of symptom onset was 23.4 ± 9.9 years.Their main clinical manifestations were headaches,seizures,blurred vision,and consciousness disturbance.Magnetic resonance imaging(MRI)showed symmetrical lesions of bilateral white matter in posterior brain.T1 weighted imaging and diffusion-weighted imaging showed low or isointense signal.T2 weighted imaging,fluid attenuated inversion recovery(FLAIR),and apparent diffusion coefficient(ADC)map showed high-intensity signal.The symptoms of the patients disappeared after the treatment.MRI reexamination showed that the lesions disappeared completely or almost completely.Conclusions The causes of RPLS were different.The imaging mainly showed the signal of vasogenic brain edema on the posterior white matter of the brain.After timely treatment,the symptorms and imaging features were improved rapidly.The patients with young-onset RPLS are their important clinical feature.[Key wonds]Posterior leukoencephalopathy syndrome;Magnetic resonance imaaging;Hypertension;Age factors were improved rapidly.The patients with young-onset RPLS are their important clinical feature.

RESUMEN

Objective To investigate the value of MSCTA in diagnosing the origin of abdominal ectopic ovarian tumor. Methods Twenty-eight patients with 30 ectopic ovarian tumors that mainly manifested as abdominal lumps were evaluated with MSCTA retrospectively. Vascular three-dimensional reconstruction by VR based on add vessel (AV) and MIP was performed for all these patients after MSCT scanning.The origin of these tumors was judged by the feeding artery and ovarian vascular pedicle (OVP) sign.The result was compared with surgical findings.Results The maximum diameter of the 30 ectopic ovarian tumors ranged from 5.2 to 19.5 cm with an average of 9.3 cm.Of the 30 ectopic ovarian tumors, the ramus ovaricus arteriae uterinae (ROAU) and ovarian artery (OA) were demonstrated in 96.7% (29/30) and 56.7% (17/30) respectively.The feeding artery of the tumor was ROAU (29 cases) or OA (1 case), and OA participated in feeding the tumor in 16 cases.The accuracy rate of diagnosis of ectopic ovarian tumors according to the feeding artery was 100% (30/30).According to OVP the accuracy rate was 85.2% (23/27) with an incidence of 76.7% (23/30).Conclusion By showing ROAU and OA as the feeding artery and ovarian vein (OV) as the draining vein, MSCTA can accurately diagnose ectopic ovary tumors in the abdomen.

Objective To evaluate the clinical value of preserving the intercostobrachial nerve after radical operation for breast carcinoma.Methods The clinical data of patients with breast cancer were analyzed retrospectively,all cases were divided into the observation group and the control group.all cases were followed up after operation from six months to six years in order to observe the abnormal sensation of the skin and the recurrence rate.Results The abnormal sensation of the skin:1 case in the observation group(4.5%),17cases in the control group(73.9%),Two groups have significant difference(x2=20.79 P<0.01),both groups had not found partial recurrence.Conclusion Preseving the intereostobmchial nerve in modified radical mastectomy for patients with breast cancer,dichat increase the partial recurrence,can increase the life quality of patients after operation,decrease the abnormal sensation of the skin.promote the health both body and mind.

Objective To study the effect of tanshinone ⅡA on the expression of P-selectin and ICAM-1 after cerebral ischemia reperfusion (I/R)injury in rats. Methods Rats were randomly divided into 4 groups: Sham operated group, I/R group, low dose Tan ⅡA treated group and high dose Tan ⅡA treated group. The focal middle cerebral artery occlusion (MCAO) model was made by suture-occluded method. Rats were pretreated with Tan ⅡA, ig for 3d,respectively before MCAO. After 90min MCAO following 24 hours of reperfusion, the expression of P-selectin and ICAM-1 was detected with using immunohistochemistry method. Result Compared with sham operated group, the expression of P-selectin and ICAM-1 increased after reperfusion for 24 hours in the ischemic territory(all P<0.01).Compared with I/R group, the expression of P-selectin and ICAM-1 decreased in a dose dependent manner in low and high dose Tan ⅡA treated group(P<0.01).Compared with that of I/R group, cerebral infarction volume was decreased in a dose dependent manner in low dose Tan ⅡA treated group and high dose Tan ⅡA treated group(all P<0.01).The change of ischemic impairment in low or high dose Tan ⅡA treated group was less than that in IR group, and the change of ischemic impairment in high dose Tan ⅡA treated group was less than that in low dose Tan ⅡA treated group. Conclusion Tan ⅡA may reduce cerebral ischemia-reperfusion inflammation injure by decreasing the expression of p-selectin and ICAM-1.Tan ⅡA plays protective effect on cerebral ischemia injury, especially when high dose of Tan ⅡA(30mg/kg)was used.
