RESUMEN

<p><b>OBJECTIVE</b>To investigate the clinical effect of electrochemical therapy (ECT) combined with pingyangmycin (PYM) injection for the treatment of high-return flow venous malformation.</p><p><b>METHODS</b>A total of 68 cases of the high-return flow venous malformation were retrospectively analyzed, in which 32 cases received simplely ECT,and 36 cases received ECT combined with PYM injection. The patients were followed up from 6 to 30 months.</p><p><b>RESULTS</b>The effectiveness of ECT was 78% (25/32), and of ECT combined with PYM injection was 97% (35/36). The effectiveness of ECT combined with PYM injection was higher than that of ECT (P < 0.05). No serious adverse effects were encountered.</p><p><b>CONCLUSIONS</b>ECT combined with PYM injection is an effective method for the treatment of high-return flow venous malformation.</p>

RESUMEN

<p><b>OBJECTIVE</b>To explore the therapeutic effect of electrochemical therapy(ECT) for high-flow vascular malformation in maxillofacial region.</p><p><b>METHODS</b>38 patients with high-flow vascular malformation in maxillofacial region were treated with ECT. The voltage was 6-8 V, and the electric current was 80-100 mA. The electric quantity was 10-20 C/cm2.</p><p><b>RESULTS</b>Among the 38 cases, 32 cases received only one treatment, 5 cases received two treatments and one case received three treatments. The patients were followed up for 2 months to 3 years. The total effective rate was 100% with 55.26% of cure rate and 44.74% of partial cure rate. No hemolysis happened during follow-up period.</p><p><b>CONCLUSIONS</b>ECT is a simple and effective method for high-flow vascular malformation with less morbidity.</p>

<p><b>OBJECTIVE</b>To investigate the classification of alar base depression, so as to provide the reference for the surgical management of secondary nasal deformity of unilateral cleft lip.</p><p><b>METHODS</b>From Jul. 2008 to Feb. 2009, 26 cases with secondary deformity of unilateral cleft lip were treated. All the patients underwent 3-dimensional CT for maxillary measurement. The nasal soft tissue measurement was performed pre- and post-operatively. The relationship between the maxillary and soft tissue at alar base was analyzed. The nasal deformity was classified.</p><p><b>RESULTS</b>The location of alar base was not related to the form of piriform aperture, but the bony defect at the alar base was correlated to the patient satisfactory. The nasal deformity was graded as I when the depression at alar base was less than 4.5 mm in depth, as II when it was 4.5-5.0 mm in depth, and as III when it was more than 5 mm in depth. The deformity could be corrected with only soft tissue plasty for grade I, with soft tissue plasty or artificial implants for grade II, with combined bone autograft or alveolar cleft repair for grade III.</p><p><b>CONCLUSIONS</b>The depression at maxillary does not necessarily result in alar base depression. The alar base can be adjust to proper position through operation. The operation should be designed based on the preoperative nasal measurement.</p>

<p><b>OBJECTIVE</b>To study the therapeutic effect of one-staged repair of unilateral cleft lip an nasal deformity with modified Mohler.</p><p><b>METHODS</b>The unilateral cleft lip and nasal deformity were repaired in one stage with modified Mohler. The nasal retainers were kept for at least 6 months. The patients were followed up at 3 months and 12 months after operation.</p><p><b>RESULTS</b>From Sept. 1998 to Aug. 2008, 103 cases, aged from 2.5 m to 12 m, were treated. 3 months after operation, 97 of 103 patients had good appearance of both lips and noses, which included unobvious scars, symmetric Cupid's bows and nostrils, intermedial nasal columellae and eminentia nasal tips. 6 of them had good appearance of noses only with a little malposition of Cupid' s bows. 89 patients were followed up for 12 months with no deterioration of nasal deformity and delayed growth of greater alar cartilages. 3 patients who had malposition of Cupid's bows also got better.</p><p><b>CONCLUSIONS</b>The one-staged repair of unilateral cleft lip and nasal deformity with modified Mohler is very practical.</p>

RESUMEN

<p><b>OBJECTIVE</b>To investigate the expression and correlation of mast cell, Clusterin/apoJ and transforming growth factor-beta (TGF-beta) in the different stages of human dermal hemangioma.</p><p><b>METHODS</b>Immunohistochemical stain technique (SABC) and toluidine blue (TB) stain technique were respectively used to detect the expression level of Clusterin/apoJ and TGF-beta and the number of mast cells in the different stages of human dermal hemangioma.</p><p><b>RESULTS</b>There was remarkable statistical difference between the advanced stage of proliferative hemangioma and the other stages of proliferative hemangioma in the number of mast cell(P<0.01). There was also remarkable statistical difference between the early stage of involutional hemangioma and the other stages of involutional hemangioma in the number of mast cell (P<0.01). The expression of Clusterin/apoJ and TGF-beta in the advanced stage of proliferative hemangioma was significantly higher than the other stages in proliferative hemangioma (P<0.01). The expression of Clusterin/apoJ and TGF-beta in the early stage of involutional hemangioma was significantly higher than the other stages in involutional hemangioma (P<0.01). There was a significantly positive correlation between Clusterin/apoJ and TGF-beta in the different stages of human dermal hemangioma (P<0.01). The expression level of Clusterin/apoJ and TGF-beta was positively correlated with the number of mast cell in the different stages of human dermal hemangioma (P<0.01).</p><p><b>CONCLUSION</b>Mast cell may play a promotive role of apoptosis during the spontaneous regulate the expression of Clusterin/apoJ and promote the spontaneous involution of human dermal hemangioma.</p>

RESUMEN

<p><b>OBJECTIVE</b>To evaluate the significance of cardiac output (CO) response against exercise determined by IGR method and LVEF, 6 MWT distance in patients with chronic heart failure (CHF).</p><p><b>METHOD</b>To adopt 6 MWT, and before and after the test measuring the CO by the IGR method, furthermore, measure LVEF to 36 patients (heart failure group) with CHF, compare with the health groups (control group).</p><p><b>RESULTS</b>The 6MWT distance of heart failure group (333.00 +/- 49.64) m decrease compared with the control group (582.56 +/- 67.97) m (P < 0.01), moreover, the distance of NYHA class III (314.82 +/- 36.27) m is significantly shorter than II (361.57 +/- 55.42) m (P < 0.05). The LVEF of heart failure group (47.0 +/- 0.4)% reduce compared with the control group (66.9 +/- 5.2)% (P < 0.01), and the data of NYHA class III (43.3 +/- 10.3)% is significantly lower than II (52.8 +/- 7.6)% (P < 0.01). The increase in CO response against exercise of heart failure group (5.97 +/- 1.89) L/min decrease compared with control group (8.88 +/- 0.52) L/min (P < 0.01), furthermore, the value of NYHA class III (5.31 +/- 1.52) L/min, compared with II (7.01 +/- 1.98)L/min, is obviously lower (P < 0.01). The 6MWT distance correlates positively with the increase in CO response against exercise (r = 0.63, P < 0.01), but the correlation is not found between the increase CO response against exercise and the LVEF (r = 0.11, P > 0.05).</p><p><b>CONCLUSION</b>Our results show that CO response against exercise measured by IGR method, with the advantages of being noninvasive, safe, convenient and accurate, combining with the 6MWT can evaluate cardiac reserve in patient with CHF.</p>

RESUMEN

<p><b>OBJECTIVE</b>To study a new method for repair of facial depression and facial nerve defect after parotid carcinoma resection.</p><p><b>METHODS</b>12 patients with parotid carcinoma and peripheral bone invasion were treated using facial nerve canal dissection and radical resection of the tumor, the parotid gland and the involved facial nerve and bone, including the mastoid, stylomastoid foramen, styloid process and the rear part of the mandible. A sternocleidomastoid muscle flap was elevated and transferred to repair the facial depression. The great annular nerve in the flap was anastomosed with the severed end of the facial nerve in the canal.</p><p><b>RESULTS</b>The depressed deformity of the parotid area was well corrected in 9 patients. The aesthetic results were compromised in 2 patients because of tumor recurrence and reoperation. The depressed deformity was not corrected in 1 patient because of infection. Postoperatively, the function of the facial nerve recovered to a normal level. The recovery time ranged from 12 to 20 weeks ,with an average of 16.3 weeks. The local control rate of tumor was improved.</p><p><b>CONCLUSIONS</b>Immediate transplantation of the sternocleidomastoid muscle-great auricular nerve flap and facial nerve canal dissection in radical parotidectomy can repair the depressed deformity of the parotid area, restore facial nerve function,and decrease tumor recurrence. The method is an ideal operation with functional recovery.</p>

RESUMEN

Proliferation of vascular smooth muscle cells (VSMCs) is often accompanied by changes in intracellular actin distribution. The changes are controlled by the signal transduction pathways of protein kinase C/mitogenic activated protein kinase (PKC-MAPK), but the mechanism is unclear. In order to study the effect of insulin on the intracellular signal transduction (PKC-MAPK) probably involved in the modulation of proliferation and redistribution of actins in the VSMCs, the DNA synthesis, MAPK activities and its gene expression, and the redistribution of intracellular actins were investigated in the isolated VSMCs of SHR pretreated with PKC inhibitor and/or insulin, respectively. We found that insulin treatment resulted in proliferation of the VSMCs and an increase in [(3)H] TdR incorporation. Meanwhile, the activities and expression of MAPK increased significantly compared to the control group. These effects of insulin were blocked by PKC inhibitor. In addition, insulin caused a redistribution of the intracellular actins in VSMCs, which was also inhibited by PKC inhibitor. It is, therefore, suggested that these effects of insulin on VSMCs proliferation and distribution of the intracellular actins may be mediated by the MAPK signal transduction pathway.
